The kinematic variables for swimming and fin-swimming start motions were analyzed and compared using 3-dimensional cinematography. For the swimming start, the arm segment moved towards the upper rear and trunk towards the upper front followed by a descent towards the lower front, while the fin-swimming start motion showed movement towards the lower front for all segments. The total body center of gravity for the swimming start showed horizontal movement far to the front followed by a rapid descent while the fin-swimming start showed close movement towards the lower front in a short period of time. Upon entering the water, the center of gravity for swimming showed high vertical velocities while fin swimming had high horizontal velocities. For both swimming and fin swimming, the upper extremity velocity had more influence on the total center of gravity velocity than the lower extremities. Flexion of the hip joint was observed before the jump for the fin swimming start while the swimming start showed two flexions in mid-air succeeding the jump. The flexion and extension movements at the knee joint during the fin-swimming start motion were shown to be larger and more rapid than those of fin-swimming.

In this study, joint angles of the lower extremity and tibial acceleration and angular velocity were measured during a snowboard simulator exercises in order to evaluate the skill of snowboarders. Ten unskilled and ten expert snowboarders were recruited for the study. A three-dimensional motion capture system and two inertial sensor modules were used to acquire joint movements, acceleration and angular velocity of the lower extremities during snowboard simulator exercises. Pattern variations were calculated to assess variations in the snowboard simulator motion of unskilled and expert snowboarders. Results showed that expert snowboarders showed greater range of motion in joint angles and greater peak to peak amplitude in acceleration and angular velocity for tibia than unskilled snowboarders. The unskilled snowboarders did not show symmetrical shape(same magnitude but opposite direction) in tibial angular velocity during two edge turns in snowboard simulator exercises. The expert snowboarders showed smaller pattern variations for joint angle of lower extremity, tibial acceleration and tibial angular velocity than unskilled snowboarders. Inertial sensor data and pattern variations during the snowboard simulator exercises could be useful to evaluate the skill of snowboarders.

Purpose: The deep inferior epigastric perforator(DIEP) free flap is well known as an ideal donor site for the breast reconstruction. The flap can provide huge amount of fat tissue for breast and buttock contour, while it is also very useful as a thin skin flap to reconstruct the upper and lower extremities. We used a DIEP free flap in various site reconstructions besides the breast and would like to reinsure the usefulness of this flap. Methods: Twenty nine consecutive patients who underwent DIEP free flap surgery from 2001 January to 2007 December were reviewed. The case constituted seven male patients and twenty two female patients. There were sixteen breast reconstructions, five face reconstructions, five lower extremity reconstructions, two upper extremity reconstructions, and one buttock contour reconstruction. All clinical data were based on the patient's medical records. Results: All DIEP free flaps survived without major complications. There was no hematoma, seroma, or partial necrosis. The donor sites were closed primarily with linear scar on lower abdomen. The thinnest part of flap was 0.7 cm in thickness. The size of the largest flap was $38{\times}13cm$. The flaps were used in various types of skin and adipose tissue, adipose tissue only, and skin only according to the requirement of recipient site. Conclusion: The DIEP free flap was enough to provide a thin and huge flap for both breast and extremity reconstructions. It was able to provide versatile designs with sufficient adipose tissue. So we use it for 3 - dimentional face and buttock contour reconstructions. The DIEP free flap is a valuable reconstructive donor for face, upper and lower extremity in addition to breast without compromising the integrity of abdominal wall.
The purpose of this study is to analyzed the coordination of lower limb of elderly women who experienced a fall to present basic information for sports science and to deal with the factors that make elderly women fall more effectively. Twenty elderly women were divided into two groups of 10. The mechanisms of balancing lower limb during walk and differences were compared and analyzed using motion analysis and electromyography. The findings of this study are as follows. The first, walking patterns of these women were unstable as their hip joints did not provide sufficient support because of aging. Second, the left and right knee joints showed different walking patterns. The third, the motions of ankle joints became abnormal with increased age. As for the activation of major lower limb muscles, rectus fermois muscle and biceps fermois muscle contracted more to prevent the bending of knees and moved forward while anterior tibial muscle and inner gastrocnemius muscle were demanded highly during walk and the rate of plantar flexion was reduced.

Background: The purpose of this study was to investigate the relationship between the spine and the flat-foot, the stability and the alignment of the posture of the neck to prevent the alignment of the ankle joint operation and the lower back flexibility of the lumbar region according to the type of treatment using active stretching of the triceps, back pain, and to see how they affect weight bearing differences. Methods: The subjects of this study were 24 chronic low back pain patients. They were randomly divided into experimental group and control group. In the experimental group, ankle joint mobilization and active scraping of triceps were performed three times a week for a total of 6 weeks. The control group was performed in the same way without articulation. The range of flexion and extension motion of the lumbar spine and pain degree and difference of weight-bearing were measured before and after the experiment. Results: The model of ankle joint mobilization and calf muscle elongation of flat foot significantly improved the range of flexion and extension motion of the vertebrae (p<.05) and the VAS and distribution of weight-bearing were decreased in both of two groups (p<.05). In other words, the exercise and mobilization help to recover of the balance of the whole musculoskeletal, the vertebrae. Conclusions: The active exercise of the triceps muscle of the lower leg in this study It affects the flexibility of the lumbar spine, the pain and the difference in the weight support of the lower extremities, when we performed ankle joint mobilization for exercise and cramping, pain and the difference in weight support between the two lower limbs.
Purpose: Patients with tibial fractures can have functional problems with balance and gait, as well as lower extremity muscle weakness. This case report aimed to describe the effect of hip joint strengthening exercises using proprioceptive neuromuscular facilitation (PNF) on balance and gait and lower limb function in a patient with tibia fracture. Methods: One patient diagnosed with tibial fracture was treated for seven weeks with the basic procedure, pattern, and technique of PNF for a hip joint strengthening exercise. Results: The results of pre- and post-intervention treatment showed improvements in physical function and structure in the clinical tests, including the manual strength test; the modified Ashworth scale; sensory evaluation; balance, sit-to-stand, and gait performance; and evaluation of lower limb function. Conclusion: Based on the results of this study, it is suggested that the use of theory-based proprioceptive neuromuscular stimulation for hip joint strengthening exercises positively affects patients' functional improvement in tibial fracture patients, and this may be used as a therapeutic exercise method for those with orthopedic problems in the lower extremities. One limitation of this study was that it was performed on only one tibia fracture patient, which makes it difficult to extend the treatment effects to all patients with this condition.
Purpose: This study verifies the muscle activity around the amputation site during proprioceptive neuromuscular facilitation (PNF) pattern exercise for the upper extremities on the non-amputated part in upper extremity amputees and provides basic data on effective exercise around an amputation site. Methods: Manual resistance was applied to the PNF upper extremity pattern of the non-amputated part to generate muscle activity around the amputation site. The resistance was adjusted to an intensity that could cause maximal isometric contraction. The muscle activity of the amputation site and the non-amputated part was measured using a surface electromyogram for the upper trapezius, middle trapezius, infraspinatus, serratus anterior, and pectoralis major. Results: During the scapular exercise in the painless range, the amputated side showed significantly lower muscle activity and a lower muscle contraction ratio compared with the non-amputated side. During the PNF pattern exercise in the painless range, the amputated side showed lower muscle activity and a lower muscle contraction ratio compared with the non-amputated side. When the direct scapular exercise of the amputated side was compared with the PNF pattern exercise of the non-amputated side, their muscle contraction ratios were similar. Conclusion: This study confirmed the effectiveness of the PNF pattern exercise of the non-amputated part as a way to indirectly train the injured site with no pain for rehabilitation of patients with serious body injuries, such as amputation. It is necessary to develop effective exercise programs for the rehabilitation of the amputation site based on the results of this study.
Purpose: The purpose of this study was to determine the asymmetrical difference between the use of leg muscles on the surgical and non-surgical sides during initial lower extremity ergometer exercise after unilateral knee arthroplasty. Methods: Twelve elderly patients diagnosed with degenerative arthritis of the knee and who underwent unilateral arthroplasty were included in this study. The leg length of each subject was taken into account when setting the application distance of the lower extremity ergometer. The same pedal resistance, strength, and speed were used for all the subjects. The total angle of use of the ergometer (360°) was analyzed by dividing it into an extension section and a flexion section. Using a surface electromyography system, the activities of the muscles of the surgical and non-surgical sides were converted into maximal voluntary isometric contraction (MVIC) and analyzed using the paired t-test. Results: When the activities of the muscles on the surgical and non-surgical sides were compared, it was found that the rectus femoris and biceps femoris had significant differences in the flexion and extension sections (p < .05), and that the tibialis anterior significantly differed in the flexion section (p < .05). There was no significant difference in the extension section of the tibialis anterior muscle, or in the flexion and extension sections of the gastrocnemius (p >.05). Conclusion: The results of this study confirm that the rectus femoris, tibialis anterior, biceps femoris, and gastrocnemius on the surgical side act in an opposite manner to those on the non-surgical side during pedaling in the same section.
Purpose: The purpose of this study is to compare lower extremity muscle activities and ankle joint angles between different foot strike patterns (forefoot strike, heelfoot strike) during stair ascent walking. Methods: The subjects of this study were 22 males who walked in each foot strike pattern on ascent stairs at a speed of 85 beats/min. During stair walking with the two types of foot strike patterns, the muscle activities of the rectus femoris, tibialis anterior, medial gastrocnemius, hamstring, and gluteus medius were measured. Additionally, ankle joint angles for inversion, eversion, dorsi flexion, and plantar flexion were recorded. Each participant underwent the experiment three times, with the foot strike pattern randomized. Results were averaged according to the foot strike pattern. Results: Significant differences in ankle angles were observed across all phases according to foot strike pattern. Muscle activities in the lower extremities showed significant differences in all phases except the swing 1 phase. Moreover, differences in foot movement trajectory were noted depending on the foot strike pattern. Conclusion: Walking on ascent stairs elicited differences in lower extremity muscle activities and ankle joint angles based on foot strike pattern. These findings can serve as foundational data for selecting a suitable foot strike pattern tailored to individual patient conditions when training patients in walking on ascent stairs.
Background: Recently transilluminated powered phlebectomy was introduced and used as a method of surgical treatment for varicose vein in lower extremities. The advantage of transilluminated powered phlebectomy are minimal scar and good cosmetic effect. However, the disadvantages of transilluminated powered phlebectomy is that a high priced Trivex system must be used which increases the patient's expenses. Therefore, we performed a transilluminated powered phlebectomy using an existing arthroscopic equipment instead of Trivex system and observed the effect of treatment and efficiency of the treatment. Material and Method: From March, 2000 to February, 2003, 78 patients (113 limbs) underwent transilluminated powered phlebectomy with an arthroscopic equipment. Patient's disease history, the number of operative scars and complications were reviewed. Result: The operation was performed in 133 limbs of the 78 patients (34 men, 44 women) and the age of patients were ranged from 16 to 72 years with mean age of 41.8 years. Operative time ranged from 20 to 65 minutes (average 48.7 minutes) per limb. The number of operative scar per limb from 2 to 7 (average 4.9). Postoperative complications are transient ecchymosis (78 cases) that desappeared spontaneously, edema persisting longer than 3 weeks (6 cases), remnant varicose vein (4 cases), skin perforation during operative procedure (2 cases), and contact dermatitis due to compression stocking (4 cases) The mean hospitalization day was 3.09 days. Subjective mean satisfaction degree of operation by the patients using a visual analogue scale was 92.6%. Conclusion: Our findings demonstrated that transilluminated powered phleectomy using arthroscopic equipment was possible and had good cosmetic results with acceptable complications.
