Weighted zero-inflated Poisson mixed model with an application to Medicaid utilization data

  • In medical or public health research, it is common to encounter clustered or longitudinal count data that exhibit excess zeros. For example, health care utilization data often have a multi-modal distribution with excess zeroes as well as a multilevel structure where patients are nested within physicians and hospitals. To analyze this type of data, zero-inflated count models with mixed effects have been developed where a count response variable is assumed to be distributed as a mixture of a Poisson or negative binomial and a distribution with a point mass of zeros that include random effects. However, no study has considered a situation where data are also censored due to the finite nature of the observation period or follow-up. In this paper, we present a weighted version of zero-inflated Poisson model with random effects accounting for variable individual follow-up times. We suggested two different types of weight function. The performance of the proposed model is evaluated and compared to a standard zero-inflated mixed model through simulation studies. This approach is then applied to Medicaid data analysis.

Studies on the Arterial Supply to Inner and Basal Surface of the Brain in the Chick (닭(鷄)의 뇌내(腦內) 및 뇌저면(腦底面)의 동맥분포상(動脈分布狀)에 관(關)한 연구(硏究))

  • The arteries in and under the brain of the fowl were invesitigated. Arteries of the brain were injected with neoplen latex into the arteria carotis communis. Those were fixed in 10--- formalin. After fixation, those brain were carefully removed from the cranial cavity doing not wound the arteries of the brain. The results of observation on the arterial supply under the brain surface were essentially the same as those obtained by Hofmann Shiina and Mjyata, Kaku, and Kithoh. The nomenclature of the arteries used by other authers were written in the discussion. In this observation, several conclusions were obtained. 1. In the arteries under the brain of the fowl, their front anastomoses forming the circle of Willis under the brain surface of the mammalia were not seen in all brain. 2. It is very difficult to indicate so called A. cerebri anterior as only one artery ramus. 3. Artery distribution of the two-third in the fissura longitudinal cerebri surface originate from on1y one side in the left or right A. cerebri posterior, and origins of the right side were man number than left. 4. The origin of the A. basilaris originate from only one side R. posterior, therefore their origin were more left than right. 5. There is Rami arteries that branch from A. ethmoidalis to Chiasma opticum in all cases.

Clinical and scanning electron microscopic analysis of fractured dental implants: a retrospective clinical analysis

  • Many longitudinal studies have reported the successful osseointegration of dental implants, with survival rates approaching 90-95%. However, implants regarded as a "success" may have also failed to undergo osseointegration. A variety of complications and failures have been observed, including implant fracture - a rare and delayed biomechanical complication with serious clinical outcomes. Given the increasing popularity of dental implants, an increase in the number of failures due to late fractures is expected. This study sought to determine the rate of implant fractures and factors associated with its development. This retrospective evaluation analyzed implants placed at Wonkwang Dental Hospital (from 1996 to the present). In our study we found that the frequency of dental implant fractures was very low (0.23%, 8 implant fractures out of 3,500 implants placed). All observed fractures were associated with hybrid-surface threaded implants (with diameter of 4.0 or 3.75 mm). Prosthetic or abutment screw loosening preceded implant fracture in a majority of these cases.

The Language and Culture of the Kumkang Region (금강유역의 언어와 문화)

  • This paper centers around the etymological meanings of place names of the Kumkang region, restricting the discussion to a selected group of place names which make it possible to infer their cultural characteristics in the light of the latitudinal or longitudinal isoglosses. The legends of Komnaru and Choryongdae, for instance, derive directly from place names. Dialectal distribution is examined on the basis of the phonological features that are not very different between upper and lower reaches of the Kumkang river. The dialects get much more similar as they go further down to the lower region of the river. The original "Komanara(熊津)", also called "Northern Headland(北津)", underwent the sound change of Komannaru > Komkang > Kumkang(錦江), the last being alternatively abbreviated as Kum(錦). During the last half of the 14th century-around the close of the Koryo dynasty, that is-a literary work gave the beautified name "Nakhwa-am"(落花巖), meaning 'rock-bluff of falling flowers', to what was originally called Tasa-am(墮死岩), so-called because, in the year of 660 when the Paekje dynasty was being ravaged by the invading enemy, a number of court-ladies ended their lives by plunging into the river from there. It was 'a number of court-ladies'(諸後宮) who died; but by tradition of popular songs, the number has become "three thousand" - a case of hyperbole of course.

Nutritional Support, Gastric Residual Volume and Nutritional Status during Enteral Nutrition in Intensive Care Unit Patients (중환자실 경장영양 환자의 영양지원, 위 잔여량 및 영양상태)

  • Purpose: The purpose of this study was to investigate the nutritional support, gastric residual volume, and nutritional status of the intensive care unit (ICU) patients on enteral feeding. Methods: A descriptive longitudinal design was used to collect 5 day data on enteral nutrition of 52 ICU patients in an university hospital. Nutritional support was calculated with actual caloric intake compared to individual caloric requirement. Residual volumes were measured prior to routine feedings, and the serum albumin levels and the total lymphocyte counts were checked to evaluate nutritional status. The data were analyzed using one group repeated measures ANOVA, paired t-test, and Spearman's bivariate correlation analysis. Results: The subjects received their first enteral feeding on the $5.75^{th}$ day of ICU admission. The mean nutritional support rate was 49.1% of the requirement, however prescription rate and support rate were increased as time goes by. Gastric residual volumes were less than 10 cc in 95% cases. A significant negative correlation was found between nutritional support and nutritional status. Conclusion: The nutritional support for ICU patient was low compared to the requirement, and their nutritional status was worse than at the time of ICU admission. Further studies are necessary to develop nursing interventions for improving nutritional support for ICU patients.

Analysis of External Peak Pressure Coefficients for Cladding in Elliptical Retractable Dome Roof by Wind Tunnel Test (풍동 실험을 통한 타원형 개폐식 돔 지붕의 외장재용 풍압 계수 분석)

  • This study investigates the wind pressure characteristics of elliptical plan retractable dome roof. Wind tunnel experiments were performed on spherical dome roofs with varying wall height-span ratios (0.1~0.5) and opening ratios (0%, 10%, 30% and 50%), similar to previous studies of cirular dome roofs. In previous study, wind pressure coefficients for open dome roofs have been proposed since there are no wind load criteria for open roofs. However, in the case of Eeliptical plan retractable dome roof, the wind pressure coefficient may be largely different due to the presence of the longitudinal direction and transverse direction. The analysis results leads to the exceeding of maximum and minimum wind pressure coefficients KBC2016 code.

Comparison of Effects among Tai-Chi Exercise, Aquatic Exercise, and a Self-help Program for Patients with Knee Osteoarthritis (슬골관절염 환자를 위한 타이치운동, 수중운동과 자조관리프로그램의 효과비교)

  • Purpose: This study was to compare the effects among Tai-Chi exercise, aquatic Exercise, and a self-help program for knee osteoarthritis patients on symptoms of arthritis, muscle strength, balance, and difficulty of performing activities. Method: There were 50 final subjects50. A non-equivalent pretest-posttest design was used. The collected data was analyzed using SPSS for Window. One-way ANOVA and Scheffe's multiple comparison test were used 8weeks after each program. Result: There were significant differences in joint pain(p=.000), stillness (p=.001), knee extensor peak torque(p=.006), knee flexor(p=.002), and difficult of performing activity (p=.000), but there was no significant difference in balance(p=.648). The Tai-Chi group was significantly different from the self-help group for knee extensor peak torque, knee flexor and stiffness on Scheffe's multiple comparison tests. In addition, the Tai Chi group or aquatic group were significantly different from the self-help group for difficulty of performing activities(p<0.05). Conclusion: There are significant differences in the effects of the nursing intervention among the three groups. The Tai Chi group and aquatic group were significantly different from the self-help group. However, it seems that Tai-chi exercise may be more suitable than aquatic exercise in osteoarthritis exercise programs. Further studies with a longitudinal study are necessary to confirm the longer exercise period.

Anger Expression Style and Risk Factors for Stroke (성인의 분노표현 방식과 뇌졸중 위험 요인)

  • Purpose: As anger may be strongly related to stroke, this study examined the relationship between anger expression style (anger-in, anger-out, anger-control) and risk factors for stroke to investigate the possible attribution of anger expression style to stroke incidence. Methods: The cross sectional study design was employed. The 291 participants signed consent forms and completed questionnaires including the anger expression style scales and risk factors for stroke. Data were analyzed using descriptive statistics, t-test and One-way ANOVA. Results: The highest score among anger expression styles of the participants was from anger-control. Participants with higher mean value of anger-out were current non-smokers, doing regular exercise, having scores above 23 in Body Mass Index, having scores above 90 cm (male) or 80 cm (female) in abdominal circumference, and having scores below 85 mmHg in diastolic blood pressure. Participants with more than 5 risk factors to stroke had the highest score of anger-in. Conclusion: The results of this study implied anger expression style was psychological risk factor for stroke. Further studies to differentiate the anger expression style contributing to the risk of stroke are needed with a longitudinal study design.

Precise Vehicle Localization Using 3D LIDAR and GPS/DR in Urban Environment

  • GPS provides the positioning solution in most areas of the world. However, the position error largely occurs in the urban area due to signal attenuation, signal blockage, and multipath. Although many studies have been carried out to solve this problem, a definite solution has not yet been proposed. Therefore, research is being conducted to solve the vehicle localization problem in the urban environment by converging sensors such as cameras and Light Detection and Ranging (LIDAR). In this paper, the precise vehicle localization using 3D LIDAR (Velodyne HDL-32E) is performed in the urban area. As there are many tall buildings in the urban area and the outer walls of urban buildings consist of planes generally perpendicular to the earth's surface, the outer wall of the building meets at a vertical corner and this vertical corner can be accurately extracted using 3D LIDAR. In this paper, we describe the vertical corner extraction method using 3D LIDAR and perform the precise localization by combining the extracted corner position and GPS/DR information. The driving test was carried out in an about 4.5 km-long section near Teheran-ro, Gangnam. The lateral and longitudinal RMS position errors were 0.146 m and 0.286 m, respectively and showed very accurate localization performance.

Anatomical Variations of the Hymen and Congenital Anomalies of the External Genitalia in Female Newborns (신생 여아에서 처녀막의 해부학적 구조와 외성기에 발생하는 기형)

  • The anatomical variations of the external genitalia including the hymen were observed prospectively in 1,500 female infants born between March, 1992 and July, 1992 at the Taegu Fatima Hospital. Careful inspection of the external genitalia was done within 24 hours after birth, and abnormal findings photographed. Almost all hymenal tissue appeared to be redundant. The hymen was annulus in 89.1 % of patients. Ninteen point nine percent had a central orifice and the remainder a ventral orifice. External ridges, intravaginal ridges, and clefts were present in 71.5 %, 50.7 % and 40.5 %, respectively. Congenital anomalies of the external genitalia were found in 20 patients(1.4 %). There were sixteen cases of hymenal cysts, two hymenal polyps, one imperforate hymen, and one Skene's duct cyst. Seven of the 16 hymenal cysts with stalks were excised and those without stalk were aspirated. Hymenal polyps were excised. Imperforate hymen was incised and drained, and Skene's duct cyst was aspirated successfully. The surgical procedure were done without anesthesia, and the results have been good. In conclusion, routine physical examination of the female newborn infant detects frequent developmental anomalies. The significance of those anomalies will be clarified after longitudinal long-term followup studies.
