Improving the Role of the Teaching Hospital in the Clinical Clerkship (의과대학 학생 임상실습을 위한 교육병원의 역할 제고 방안)

  • The mission of the modern medical school includes education, research, and patient care. The clinical clerkship is an important part of the core curriculum, and hospital facilities are needed for the clinical clerkship. However, unfortunately, education has moved to the periphery during the past several decades because of the dominance of research and patient care. This may lead to obstacles in the education of future physicians in the long term. To promote their education mission, teaching hospitals need to recognize and share the importance of this mission. In addition to the certification of teaching hospitals, a new paradigm for teaching hospitals should be introduced to produce a high quality clinical clerkship and postgraduate medical education. The relevant government departments need to allocate and expand financial support to medical schools and teaching hospitals, and to unify supervision of basic and postgraduate medical education.

A potential user' Need of Utilization and Development Direction of Day Care Center (노인주간보호시설에 대한 잠재이용자의 이용욕구 및 발전방향)

  • The purpose of this study is to identify a potential user's need for use and operational of the elderly day care center and to identify the problems and suggest development directions. The data collection was conducted on 320 ordinary people living in J-do through a questionnaire, and sought to understand the operational status of in-depth interview institutions for the heads and employees of six center. As a result of the analysis, a potential user's ware not aware of the functions and specific roles of day care center, the cost of facilities was covered by the government, and that facilities and the environment were important factors when using the center. In-depth interviews show that the current policy or system is far from reality and does not fit the status or role of the workers and the operational status of the agency, requiring the re-establishment of the on-site system. Based on the research results, the development direction of day care facilities requires active promotion and strengthening of family support for improving and enhancing awareness of day care facilities to citizens at the national level. Second, the quality of nursing services should be enhanced through improvements in facilities and the environment that reflect users' needs. Third, The government should improve the quality of nursing services by improving the poor employment conditions and treatment of workers to fit the reality.

Relationship Between Social Support and Cognitive Impairment of the Elderlies in Care Facilities (요양시설노인의 사회적 지지와 인지기능장애와의 관련성)

  • The purpose of this study was to establish the relationship between social support(MOS-SSS) and cognitive impairment(MMSE-K) to the elderly in long-term care facilities. The study subjects were 372 aged 65 and older who were admitted to care facilities located in D metropolitan city. Data were collected through a personal interview conducted by interviewers who visited each care facility in October 2018. As a result, the levels of cognitive impairment(MMSE-K) was significantly higher in the group with lower social support(MOS-SSS) scores. In correlation to the cognitive impairment and social support, the cognitive impairment score showed a significant positive correlation between the social support score (r=0.250, p<0.001). Therefore, the lower the social support level, the higher the level of cognitive impairment. In result of the logistic regression, the risk ratio of critical cognitive impairment increased significantly from 2.8 times (ORs=2.75, 95% CI=1.40 to 5.79) in the low group(Q1) than in the very high group(Q4) of social support scores. A hierarchical multiple regression analysis shows that the lower the social support, the higher the cognitive impairment. As such research suggests that the cognitive impairment of the elderly in care facilities increases with lower social support, it is necessary to seek concrete measures to improve social support as part of the measures to enhance the recognition capability of the elderly in care facilities.

A Study on the Radioactive Products of Components in Proton Accelerator on Short Term Usage Using Computed Simulation (몬테칼로 시뮬레이션을 활용한 양성자가속기 단기사용 시 구성품의 방사화 평가)

  • The evaluation of radioactivated components of heavy-ion accelerator facilities affects the safety of radiation management and the exposure dose for workers. and this is an important issue when predicting the disposal cost of waste during maintenance and dismantling of accelerator facilities. In this study, the FLUKA code was used to simulate the proton treatment device nozzle and classify the radio-nuclides and total radioactivity generated by each component over a short period of time. The source term was evaluated using NIST reference beam data, and the neutron flux generated for each component was calculated using the evaluated beam data. Radioactive isotopes caused by generated neutrons were compared and evaluated using nuclide information from the International Radiation Protection Association and the Korea Radioisotope association. Most of the nuclides produced form of beta rays and electron capture, and short-lived nuclides dominated. However, In the case of 54Mn, which is a radioactive product of iron, the effect of gamma rays should be considered. In the case of tritium generated from a material with a low atomic number, it is considered that handling care should be taken due to its long half-life.

Interventions Using Technologies for Older Adults in Long-term Care Facilities: A Systematic Review (장기요양시설 거주 노인 대상 테크놀로지 활용 중재에 관한 체계적 문헌고찰)

  • Purpose: Although innovative interventions using technologies have been introduced in long-term care settings, available evidence is still anecdotal. The purpose of this study is to investigate and synthesize the outcomes of interventions using technologies delivered to nursing home residents. Methods: Published clinical trials were identified through PubMed, CINHAL, Cochrane and PsycINFO databases and manually hand-searching. Eligible studies were articles published between 1997-2016 in English or Korean with a randomized controlled trial or quasi-experimental design in which interventions using technologies were delivered to nursing home residents. Results: A total of 20 studies were selected for this review. Types of interventions using technologies were classified into the electronic documentation technology (n=1), the clinical decision support system (n=1), the safety technology (n=1), the health and wellness technology (n=10), and the social connectedness technology (n=7). Overall resident outcomes indicated that interventions using technologies improved behavioral symptoms and psycho-social outcomes, but mixed results were shown in the aspects of physical function, cognitive function, social relationship and quality of service. Conclusion: This review demonstrates that incorporating technologies into nursing home care have positive effects on residents' psycho-social outcomes and behavioral symptoms. To disseminate the effectiveness of interventions using technologies, further research is needed to determine what mechanisms underlying such relationships exist.

Current status of long-term care facility workers' physical function improvement activities for the elderly

  • Objective: This study was conducted to provide basic data on physical function improvement activities of elderly care facilities by identifying the degree of physical improvement activities of elderly caregivers. Design: Descriptive correlational study. Methods: The instrument of this study consisted of 12 questions on the general characteristics of the subject. For the assessment of improvement in physical function activities, the measuring tool used for the elderly consisted of a total of 20 questions. For data analysis, Pearson's correlation coefficient & Spearman's rho and multiple regression were used. Results: The higher the age of the subjects, the lower their educational background (r=-0.273, p<0.05), and the higher the probability of having a religion (r=-0.258, p<0.05), the more stable the employment type (r=-0.333, p<0.05). The higher the level of education, the higher the monthly income (r=0.187, p<0.01), and the shorter the career (r=-0.204, p<0.05). The more stable employment, the more unstable duty (r=-0.245, p<0.05), and the more unstable work, the higher the monthly income (r=-0.206, p<0.05) and the longer the career, the higher the monthly income (r=0.247, p<0.05). The more stable the employment, the more activities to improve physical function were found (r=0.341, p<0.05), and the more unstable the duty, the more activities to improve physical function were found (r=0.321, p<0.05), and the higher monthly income, the less physical function improvement activities (r=-0.196, p<0.05). Conclusions: It is necessary to provide regular services by a dedicated physical therapist for physical function improvement activities in order to improve the quality of life of the elderly in the future.

An analysis of domestic research trends on elderly environment planing (국내 노인주거환경계획 분야 연구의 흐름 분석 연구)

  • Korean society expects to be changing into aged society more rapidly than any other countries due to low birthrate and increase in life expectancy. Increasing number of elderly and social problems of aging society have provoked increase in research on elderly environment. Elderly housing facilities and living conditions are significantly related to the quality of life for older persons. The purpose of this study is to systematically analyze empirical studies on elderly physical environments in Korea, find out research streaming and understand social backgrounds and to suggest future research problems. For this study, contents analysis was conducted. Articles of four academic peer reviewed journals published from 1986 to 2005 were units of analysis. Using a keyword through library database systems, the articles were systematically selected. As results, trends of research according to 4 periods were defined. Among them as major trends, expansion of the quantity, expansion to interior design features for older persons, more facility types for dependent elderly(assisted living facilities, facilities for elderly with dementia, long term care facilities) have appeared. This result showed some directions and implications on elderly facility planning and development.

Development and Effects of an Educational Program for Caregivers on Hygienic Care for Diapers (요양보호사의 기저귀위생간호 교육프로그램 개발 및 효과)

  • The purpose of this study is to develop a diaper hygienic care education program for caregivers and to investigate the effects of the diaper hygienic care knowledge and performance ability on diaper hygienic care. A quasi-experimental research with nonequivalent control group pretest-post test design was conducted. The subjects for the study 50 caregivers working in long-term care facilities in Y-gun, Chungcheongnam-do were selected from the 25 experimental group and the 25 control group. The collected data were analyzed using SPSS Window 22 version. diaper hygienic care education program consisted of 4 sessions in total. According to results of the education program, the diaper hygienic care knowledge increased after the post-test($M{\pm}SD=4.88{\pm}0.23$) compared to the pre-test score($M{\pm}SD=4.64{\pm}0.23$), but not be statistically significant between the two groups(t = 1.253, p = .216). The performance ability of diaper hygienic care was significantly increased ($M{\pm}SD=2.89{\pm}0.15$) compared to the pre - score($M{\pm}SD=1.86{\pm}0.18$) and there was a statistically significant difference(t = 12.694, p =. 001). Thus, the developed diaper hygienic care education program is expected to contribute to improve nursing hygienic care ability of nursing caregivers.

Effect of Communication with a Guardian of a Recipient of a Long-term Care Program on Job Satisfaction of Care Helpers (장기요양수급 대상 보호자와의 의사소통이 요양보호사의 직무만족에 미치는 영향)

  • In this study, the effect of communication with a guardian of a recipient of a Long-term Care Program on job satisfaction of care helpers of in-home care facilities for the elderly is analyzed. First, most of the care helpers are content with their job despite low pay and short work-hour. Especially, they are aware of the need of communication with a guardian but skeptical about the need of having intimacy with a guardian, so the relation between a care helper and a guardian is usually formal only. Second, in terms of characteristics of a guardian, care helpers have higher job satisfaction when a guardian is a male and old, and does not have an unreasonable demand. Third, they have higher job satisfaction when they have better communicate with a guardian rather than higher intimacy. Considering this result, we should make programs such as education for a guardian to develop intimacy with a care helper and one for a care helper to develop intimacy and communicate effectively with a guardian. It is also needed to have guidelines which set up relationship between a helper and a guardian.

Current Status and Barriers to Health Care Services for Nursing Home Residents: Perspectives of Staffs in Korean Nursing Homes (노인요양시설 건강지원서비스 현황과 장애요인: 포커스 집단 면접)

  • Purpose: Focus group interviews were conducted to explore the current status and barriers of health care services in nursing home from the viewpoint of staffs taking care of nursing home residents. Methods: A qualitative thematic analysis using the focus group interview method was used. A total of 32 health care professionals (19 nurses and 13 social workers) from thirteen nursing homes in South Korea attended (5 focus groups) in 2014. Results: The two main themes were identified: 'minimal health care services that left personal care needs unmet' which has three subthemes of 'stereotyped and fragmented care by types of care providers', 'medically-oriented health care services' and 'health care services mixed with social or recreational programs'; and 'barriers to proper and timely care in nursing homes' with four subthemes including 'unmet care needs due to cognitive dysfunction or lack of expression', 'care guides or tools not suitable for long-term care facilities', 'health care needs that are beyond the facility's care boundary', and 'care delay due to lack of understanding on the older adult's status'. Conclusion: The findings from this study should help health care policy makers to recognize the factors that influence health care services and provide direction for nurses and other staffs involved in supporting health care services for nursing home residents.
