A Study for Safe Storage Time for In-house Sterilized Products in a Korean Hospital (멸균품의 유효기간에 관한 연구 - 포장재와 보관환경 중심으로 -)

  • This study was conducted to justify the "prescribed shelf life" currently used in Samsung Medical Center and give recommendations based on scientific data to adopt for safe and cost-effective storage times for sterile goods Since the early 1970's, predetermined expiration dating policy has been used in Korea on all in-house sterilized products like any other countries; six months for the supplies wrapped in polyethylene peel pouches and two weeks for the rest. Recently, however, the studies suggest that current standards for preserving the sterility can be changing. It looks like we unnecessarily have too-short expiration date of sterilized supplies, that is far from being cost effective. Certainly, this research is an exciting project, especially in Korea, and will be leading and contributing to the paradigm shift of the conservative concept of shelf life for sterile supplies in Korean healthcare facilities. The major goal of this study was placed on the measurement of the recontamination time in packs wrapped in all kinds of wrapping materials used in S.M.C and stored in various conditions for periods ranging from three months to six months, and then develop a practical standard of "shelf life" based on the analyzed data. Objectives: To measure and compare the recontamination time of sterile packs among wrapppers and storage conditions, and develop a practical standard for the shelf life and storage conditions of sterile products. Samples and Method: The sterile integrity was investigated of the total 1,440 sterile sample packs containing gauze strip($1.5cm{\times}0.5cm$) we prepared and wrapped in nine wrapping materials (randomized reprocessed linens, linens reprocessed 99 times, linens reprocessed 120 times, new kraft paper, used kraft paper, new nonwoven fabrics, 6-time reprocessed nonwoven fabrics, new polyethylene peel pouches, used polyethylene peel pouches), and stored in four locations(closed cabinet of one of the typical wards, open shelf of one of ICUs, open shelf of separate storage room in ER, open shelf in OR), for three to six months, respectively in 1998. Each type of packs were opened weekly and the gauze strips inoculated into Brewer's thioglycollate broth at $35^{\circ}C$ for seven days. Results: There was no growth found for any types of the pack and storage conditions studied. Discussion: This study was conducted in an attempt to identify the possibility of extending the current shelf life for sterile goods we use in a sense of cost effectiveness. The findings suggest that we could extend our expiration dates, that means we can save a lot of materials and personnel time meeded for resterilizing, outdating, recleaning, wrapping, and reshelving. Then it will initiate a great reform in the field of healthcare in Korea, and now we are looking forward to this valuable revolution. We are going to continue this study and take all steps need to apply this new concept to our practice, and then trying to extend to other hospitals.

  • Our studies diagnose insulation degradation using the method of PD system. In analysis method of degradation, we make use of degradation information that analyzed the phase angle - PD pulse amplitude - degradation time( $\Phi$- PDA - t) pattern and phase angle- PD pulse number - degradation time( $\Phi$- PDN - t) pattern using statistical operators such as skewness(S), kurtosis(K). Secondly using statically operator such as the center of gravity (G), the gradient of the discharge distribution(C), we have analyzed for the prediction of life which we can be obtained the time, occurred of many pulse of small discharge amplitude.

  • When The temperature of the joining of SCR rises major changes of characteristics are increase of leakage current, decrease of break-down voltage, and increase of turn off time. Because these changes promote the inferiority of SCR it(SCR) will get out of control and its life time will be shortened and finally it will be destroyed. In this paper, we will measure the changes of the characteristics in accordance with a rise in temperature. On the basis of the result we will find out the appropriate time to change SCR and we will use the result for the maintenance of SCR and increase of reliability of SCR.

Development and pilot study of a cancer rehabilitation smartphone application for cancer survivors

  • The purpose of this study is to develop a cancer rehabilitation program for cancer survivors in the form of a smartphone application and to confirm the effectiveness through Pilot study. The contents of the application consisted of health records, lab-test records, and health information, and the information recorded by the patient was graphically checked for changes over time on my page. 7 subjects who ended acute treatment and were undergoing follow-up were asked to use the application for 4 weeks, and then changes in variables (uncertainty, e-health literacy, self-efficacy, and cancer rehabilitation) were confirmed. It was confirmed that e-health literacy and self-efficiency increased significantly over time after using the application. In addition, the level of cancer rehabilitation was found to increase significantly over time. The application for cancer rehabilitation developed in this study needs to be expanded to improve the quality of life of cancer survivors.

Wheat Blast: A New Fungal Inhabitant to Bangladesh Threatening World Wheat Production

  • World wheat production is now under threat due to the wheat blast outbreak in Bangladesh in early March 2016. This is a new disease in this area, indicating the higher possibility of this pathogen spreading throughout the Asia, the world's largest wheat producing area. Occurrence of this disease caused ~3.5% reduction of the total wheat fields in Bangladesh. Its economic effect on the Bangladesh wheat market was little because wheat contributes to 3% of total cereal consumption, among which ~70% have been imported from other countries. However, as a long-term perspective, much greater losses will occur once this disease spreads to other major wheat producing areas of Bangladesh, India, and Pakistan due to the existing favorable condition for the blast pathogen. The wheat blast pathogen belongs to the Magnaporthe oryzae species complex causing blast disease on multiple hosts in the Poaceae family. Phylogenetic analysis revealed that the Bangladesh outbreak strains and the Brazil outbreak strains were the same phylogenetic lineage, suggesting that they might be migrated from Brazil to Bangladesh during the seed import. To protect wheat production of Bangladesh and its neighbors, several measures including rigorous testing of seed health, use of chemicals, crop rotation, reinforcement of quarantine procedures, and increased field monitoring should be implemented. Development of blast resistant wheat varieties should be a long-term solution and combination of different methods with partial resistant lines may suppress this disease for some time.

Development of Tissue-Tool Interaction Simulation Algorithms for Rotator Cuff Surgery Scenario in Arthroscopic Surgery Training Simulator

  • Various simulator systems for surgery training have been developed and recently become more widely utilized with technology advancement and change in medical education adopting actively simulation-based training. The authors have developed tissue-instrument interaction modeling and graphical simulation algorithms for an arthroscopic surgery training simulator system. In this paper, we propose algorithms for basic surgical techniques, such as cutting, shaving, drilling, grasping, suturing and knot tying for rotator cuff surgery. The proposed method constructs a virtual 3-dimensional model from actual patient data and implements a real-time deformation of the surgical object model through interaction between ten types of arthroscopic surgical tools and a surgical object model. The implementation is based on the Simulation Open Framework Architecture (SOFA, Inria Foundation, France) and custom algorithms were implemented as pulg-in codes. Qualitative review of the developed results by physicians showed both feasibility and limitations of the system for actual use in surgery training.

The Effect of Headaches and Quality of Life with the Combined Western-Oriental Medicine Massage Program for Chronic Headache Patients (양.한방 혼합 마사지요법이 만성두통 환자의 두통 및 삶의 질에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effect of combined Western and Oriental medicine headache massage program for chronic headache patients. Methods: This study was designed using the Nonequivalent control group pretest-posttest design. The forty nine patients were divided into an experimental group (25 patients) and a control group (24 patients). Collected data were analyzed by Chi-test, t-test, Fisher's exact test, Repeated measures ANOVA using SPSS/WIN 14.0 program. Results: After treatment in this program, the experimental group had no significan difference in scores for headache intensity and frequency as the length of time in treatment increased compared to the control group. But there were significant decreases in the use of medication (F=4.209, p=.046) and disturbances in the quality of life related to headaches (F=13.097, p=.001) after providing the combined Western-Oriental massage program. Conclusion: This program could have a positive effect on using medication for pain controllers and headache related quality of life. Based on the results of this study, further research focusing on the effectiveness of unique nursing intervention for chronic headache patients is suggested.

Pharmacological studies on roots of Achyranthes aspera Linn

  • The crude methanol extract of the roots of Achyranthes aspera Linn. was investigated for its possible antinociceptive, diuretic and neuropharmacological activities in animal models. At the dose of 250 and 500 mg/kg body weight, the extract showed a significant antinociceptive effect in acetic acid induced-writhing in mice comparable to that produced by diclofenac sodium, used as standard drug. The crude extract produced significant diuretic effect at the dose of 500 mg/kg of body weight comparable to that produced by furosemide, used as standard drug. The extract also potentiated significantly the pentobarbital induced sleeping time in mice; decreased the open field score in open field test, decreased the number of hole crossed from one chamber in the hole cross test and decreased the head dip responses. The obtained results provide a support for the use of this plant in traditional medicine and its further investigation.

  • The purpose of this study is to analyze potential adoption rates and reusing patterns of the new rice direct seeding technology. The model constructed and employed in this study is a system dynamics model of farmer adoption and reusing patterns for this new technology over time. The model incorporates a causal loop diagram that explains interactions among rice cultivation subsystems with feedback loops and further attempts to build a causal loop model with stock-flow diagram for computer simulation. As one example of how the model can be used to provide insight to rice farmers, simulations are run over varying levels on the cultivation process of rice. The major finding is to demonstrate the utility of system dynamics simulation methodology in aiding the rice wet direct seeding farmers' decision making.

Optimal Cutting Condition of Tool Life in the High Speed Machining by Taguchi Design of Experiments (다구찌 실험 계획법을 이용한 고속가공에서 공구수명 조건의 최적화)

  • High Speed Machining(HSM) reduces machining time and improves surface accuracy because of the high cutting speed and feedrate. Development of HSM makes it allowable to machine difficult-to-cut material and use small-size-endmill. It is however limited to cutting condition and tool material. In the machining operation, it is important to check main parameter of tool life and select optimal cutting condition because tool breakage can interrupt progression of operation. In this study, cutting parameters are determined to 3 factors and 3 levels which are a spindle speed, a feedrate and a width of cut. Experiment is designed to orthogonal array table for L9 with 3 outer array using Taguchi method. Also, it is proposed to inspect significance of the optimal factors and levels by ANOVA using average of SN ratio for tool life. Finally, estimated value of SN ratio in the optimal cutting condition is compared with measured one in the floor shop and reduction of loss is predicted.
