Acrodysostosis Associated with Symptomatic Cervical Spine Stenosis

  • Acrodysostosis is an extremely rare disorder characterized by short fingers and toes with peripheral dysostosis, nasal hypoplasia, and mental retardation. We report a 16-year-old Korean boy with acrodysostosis who had characteristic clinical features and cervical spine stenosis manifested by neurologic symptoms. On presentation, he complained of difficulty in raising his arms, and suffered from intermittent pain and weakness in both upper extremities. He had short stature and dysmorphic facial features, including a broad, depressed nasal bridge, small, upturned nose, bilateral epicanthal folds, and mild hypertelorism. Moderate mental retardation and sensorineural hearing loss in both ears were also present. Radiological findings included broad, short metacarpals and phalanges with cone-shaped epiphyses, bilateral Madelung deformities, hypertrophied first metatarsals, and thickening of the calvarium. Magnetic resonance imaging findings included stenosis of the cervical spine, platybasia with compression into the cervicomedullary junction, and downward displacement of the cerebellar tonsils. Here, we report a case of acrodysostosis with symptoms and signs of cervical spinal stenosis first in Korea. If it is diagnosed in the early stages, possible life-threatening complications, including spinal canal stenosis, can be managed properly and permanent neurologic sequelae might be avoided. Therefore, it is important to consider acrodysostosis in the differential diagnosis of peripheral dysostosis.

The Problem of Property Portrayed in Baktaryeong and Shin Jae?hyo (<박타령>에 나타난 재화(財貨)의 문제와 신재효)

  • This study attempts to examine the problem of properties mentioned in Shin Jae hyo's adapted version of Baktaryeong through the property related behavior of the characters, and discuss Shin Jae hyo's view of properties and his contemporary perception problems. As a result, in Baktaryeong, Nolbu takes the shape of a rich farmer in the existing text with grain centered accumulated properties, and of a wealthy man who is skilled in money management and growth as an economically well-informed person. In contrast, Heungbu is a poor peasant isolated from his own farm without enough property to minimally survive, representing the alienated poor who can not adapt to the currency economy led by the Nolbu people. This adaptation could have been a product of Shin Jae hyo's own view of property. Through the detailed description of Baktaryeong, it can be seen that he found it difficult to observe too much, but he thought that interest in property and money seemed basically to be affirmed in human life. In addition, in terms of issues of the poor, he found that the economic efforts of the lower classes and the care of the rich should be needed for the poor. However, he was forced to put the sense of crisis and the self defense consciousness as a wealthy family of middle class in the local society under the rapidly changing circumstances of the time into the text. That is, Baktaryeong included his own diagnosis of the present reality by drawing the issue of wealth and poverty in existing Heungboga(jeon), which is perhaps more appropriate for reality based on Shin Jae hyo's own view of properties.

Convergent Factors Related to Depression of Wage Workers in Korea: Focusing on Gender Differences (한국 임금근로자의 우울과 관련된 융복합적 요인: 성별 차이를 중심으로)

  • This research was conducted to investigate the prevalence of depression and convergent factors related to depression in male and female wage workers. Using data from the 2014 and 2016 Korea National Health and Nutrition Examination Survey, 3,763 adults (1,888 males, 1,875 females) between the ages of 19 and under 65 were analyzed. Depression was measured by the Patient Health Questionnaire-9 (PHQ-9), and the depression (PHQ-9≥10) of all workers was 4.1% (3.2% for men, 5.0% for women). Multiple logistic regression analysis was performed to evaluate the depressive factors of male and female wage workers. In male workers, work-related characteristics such as employment type and working hours per week were found to be significantly related to depression even after adjusting for socio-demographic characteristics, health-related characteristics, and life style characteristics. However, in female workers, work-related characteristics did not expose a significant relationship, showing a difference from male workers. Therefore it is necessary to develop and implement workplace counselling or mental health promotion programs that take into account the gender characteristics of depression risk factors. Also, as health-related characteristics (diagnosis of depression, suicide plan, perceived stress level, unmet medical services, self-rated health status) were found to be related to depression for both male and female workers, continuous management of these factors is required.

Comparison of Sizes of Anatomical Structures according to Scan Position Changes in Patients with Interstitial Lung Disease Using High-Resolution Thoracic CT (고해상도 흉부 전산화단층촬영을 이용한 간질성 폐질환을 가진 환자의 자세에 따른 해부학적 구조물 크기 비교)

  • High-Resolution thoracic CT (HRCT) is a scanning protocol in which thin slice thickness and sharpness algorithm are utilized to enhance image resolution for diagnosis and assessment of interstitial lung disease (ILD). This examination is sometimes performed in both supine and prone position to improve sensitivity to early changes of these conditions. Anatomical structures (the size of lung field and heart and descending aorta) of 150 patients who underwent HRCT were retrospectively compared. HRCT had been conducted in two positions (supine and prone). Data were divided into five groups according to patient body weights (from 40 to more than 80kg, 10kg intervals, 60 patients/each group). Quantitative analysis was utilized in Image J program. In the supine position defined as the control group, the average values of lung fields and heart size and aorta were compared with the prone position defined as the experimental group. The size of the lungs was found to be higher in the supine position, and it was confirmed that there was a statistically significant difference in patients over 70 kg (p<0.05). In addition, both sizes of the heart and descending aorta were larger in prone position, but in the case of the heart, there was no correlation with the presence or absence of ILD disease (p>0.05). Also, the area of prone in the descending aorta was higher than supine position, but there was no statistically significant difference between supine and prone position (p>0.05). In conclusion, when the severity of ILD disease was severe, there was no statistically significant difference in the area difference between supine and prone position, so it is considered that it will be helpful in diagnostic decision.

Quality Control Tests and Acceptance Criteria of Diagnostic Radiopharmaceuticals (진단용 방사성의약품의 품질관리시험 및 기준)

  • Radiopharmaceuticals are drugs that contain radioisotopes and are used in the diagnosis, treatment, or investigation of diseases. Radiopharmaceuticals must be manufactured in compliance with good manufacturing practice regulations and subjected to quality control before they are administered to patients to ensure the safety of the drug. Radiopharmaceuticals for administration to humans need to be sterile and pyrogen-free. Hence, sterility tests and membrane filter integrity tests are carried out to confirm the asepticity of the finished drug product, and a bacterial endotoxin test conducted to assess contamination, if any, by pyrogens. The physical appearance and the absence of foreign insoluble substances should be confirmed by a visual inspection. The chemical purity, residual solvents, and pH should be evaluated because residual by-products and impurities in the finished product can be harmful to patients. The half-life, radiochemical purity, radionuclidic purity, and strength need to be assessed by analyzing the radiation emitted from radiopharmaceuticals to verify that the radioisotope contents are properly labeled on pharmaceuticals. Radiopharmaceuticals always carry the risk of radiation exposure. Therefore, the time taken for quality control tests should be minimized and care should be taken to prevent radiation exposure during handling. This review discusses the quality control procedures and acceptance criteria for a diagnostic radiopharmaceutical.

Vocational Rehabilitation for Improvement of Life Care in Patients with Schizophrenia : A Systematic Review (조현병 환자의 라이프케어 증진을 위한 직업재활 : 체계적 고찰)

  • This study selected a total of 19 studies using databases of PubMed, SAGE Journal, and Wiley Online Library from 2000 to 2019 to find out about vocational rehabilitation of patients with schizophrenia in the area of overseas occupational therapy. First, the quality level of the study was all Level I. Second, the diagnosis for participants of the studies other than schizophrenia had the most schizoaffective disorder. Third, by types of interventions, 11 papers, the largest, were about social skills and cognitive rehabilitation training plus supported employment, followed by 4 about supported employment and 3 about social skills and cognitive rehabilitation training, 1 about other vocational approaches. Regarding studies showing effects in the employment rate by interventions type following three types of employment, as the most effective of studies, they were social skills and cognitive rehabilitation training plus support employment in competitive employment, social skills and cognitive rehabilitation training in non-competitive employment, and support employment in paid employment. Fifth, looking at the studies trend in the last 5 years, the most studies were social skills and cognitive rehabilitation training plus supported employment. Since this study classified the types and effects of vocational rehabilitation for patients with schizophrenia based on previous overseas studies, it is hoped that this study will be provided as basic data for understanding vocational rehabilitation of schizophrenic patients and applying them in domestic clinical settings.

A Review on the Cause of Fever During Clozapine Treatment (클로자핀 투약시 나타나는 발열의 원인)

  • Clozapine is accepted as the "gold standard" antipsychotics for treatment-resistant schizophrenia. Clozapine rarely causes extrapyramidal syndrome and tardive dyskinesia, which are common with other antipsychotics, and only a transient elevation of hyperprolactinemia has been reported. Despite such clinical usefulness, there are limitations to the use of clozapine due to adverse drug reactions (ADR). Fever is a common in adverse drug reactions associated with clozapine. At initiation of clozapine most fatal ADR such as agranulocytosis and neuroleptic malignant syndrome associated with fever, in which case clozapine should be discontinued immediately. However, as benign causes of fever are much more frequent than life-threatening ADR, clozapine should not be discontinued unconditionally in the event of fever during clozapine initiation. In addition, fever may occur at any time during the maintenance of clozapine treatment. In particular, since the risk of pneumonia does not decrease over time, and clozapine has a higher risk of pneumonia than other antipsychotic drugs, it is recommended to adjust clozapine dosage through therapeutic drug monitoring.

Semantic Segmentation for Multiple Concrete Damage Based on Hierarchical Learning (계층적 학습 기반 다중 콘크리트 손상에 대한 의미론적 분할)

  • The condition of infrastructure deteriorates as the service life increases. Since most infrastructure in South Korea were intensively built during the period of economic growth, the proportion of outdated infrastructure is rapidly increasing now. Aging of such infrastructure can lead to safety accidents and even human casualties. To prevent these issues in advance, periodic and accurate inspection is essential. For this reason, the need for research to detect various types of damage using computer vision and deep learning is increasingly required in the field of remotely controlled or autonomous inspection. To this end, this study proposed a neural network structure that can detect concrete damage by classifying it into three types. In particular, the proposed neural network can detect them more accurately through a hierarchical learning technique. This neural network was trained with 2,026 damage images and tested with 508 damage images. As a result, we completed an algorithm with average mean intersection over union of 67.04% and F1 score of 52.65%. It is expected that the proposed damage detection algorithm could apply to accurate facility condition diagnosis in the near future.

Clinical characterization of 3-month-old pigs infected with African swine fever virus from Vietnam

  • African swine fever (ASF) is a fatal viral disease in pigs, with a short incubation period and causing immediate death. Few studies exist on the Asian epidemic ASF virus (ASFV) challenge in older pigs, including growing and fattening pigs and sows. We aimed to investigate clinical outcomes, pathomorphological lesions, and viral distribution in organs of 3-month-old growing pigs that were inoculated with the ASFV isolated in Vietnam. The clinical outcomes were recorded daily, and the dead or euthanized pigs immediately underwent necropsy. Viral loads were determined in 10 major organs using quantitative polymerase chain reaction. The average incubation period in growing pigs was more delayed (5.2±0.9 dpi) than that in weaned pigs, and the clinical signs were milder in growing pigs than in weaned pigs. The digestive and respiratory clinical signs in growing pigs showed at the end period of life, but these were observed at an early stage of infection in weaned pigs. The pathomorphological features were severe and nonspecific with hemorrhagic lesions in various organs. The viral loads in organs from growing pigs were higher than those from piglets, and the number of viral copies was related to gross lesions in the tonsil and intestine. In the absence of vaccines against ASF, early clinical detection is important for preventing the spread of the virus. Our findings elucidated that the clinical signs and gross lesions in growing pigs differed from those in weaned pigs, which provide valuable information for diagnosis of pigs with suspected ASF infection.

Influencing Factors of Nursing Performance for Life Care of Delirium Patients among Nursing Students (섬망환자의 라이프케어를 위한 간호학생의 섬망간호 수행 영향요인)

  • This study was conducted to identify factors affecting nursing performance of delirium among nursing students. A total of 252 fourth year students were recruited from nursing department in Gwangju. Structured questionnaire was self-administrated from April to September, 2017. The used statistical analysis were t-test, ANOVA, Pearson's coefficient and multiple regression analysis. Knowledge of delirium 29.0±7.24, self-confidence in the care for delirium 71.65±28.55 and nursing performance level for patients with delirium was 41.16±8.97. Nursing performance of delirium had significant positive correlations with delirium knowledge, self-confidence of delirium care. In multiple regression analysis, nursing experience for delirium patients, self-confidence of delirium care, practice experience in intensive care unit, use of nursing diagnosis related to delirium, and satisfaction of clinical practice were significant factors of nursing performance of delirium explaining 29.8% of the variables. In conclusion, to enhance nursing performance of delirium, it is necessary to develop educational program for increasing nursing experience for delirium patients during clinical practice and self-confidence of delirium care.
