A voxel based morphometry study in Alzheimer's disease

  • Several MRI studies have reported reductions in temporal lobe volumes in Alzheimer´s disease (AD). Measures have been usually obtained with regions of interest (ROI) drawn manually on selected medial and lateral portions of the temporal lobes, with variable choices of anatomical borders across different studies. We used the automated voxel based morphometry (VBM) approach to investigate gray matter abnormalities over the entire extension of the temporal lobe in 11 AD patients (MMSE 14 - 25) and 11 healthy controls. Foci of significantly reduced gray matter volume in AD patients were detected in both medial and lateral temporal regions, most significantly in the right and left posterior parahippocampal gyri. At a more flexible statistical threshold (P<0.001, uncorrected for multiple comparisons), circumscribed foci of significant gray matter reduction were also detected in the right amygdala/enthorinal cortex, the anterior and posterior borders of the superior temporal gyrus bilaterally, and the anterior portion of the left middle temporal gyrus. These VBM results confirm previous findings of temporal lobe atrophic changes in AD, and suggest that these abnormalities may be confined to specific sites within that lobe, rather than showing a widespread distribution.

The Effectiveness of the Use of Custom-Made Foot Orthotics on Temporal-Spatial Gait Parameters in Children With Spastic Cerebral Palsy

  • This study examined the effects of custom-made foot orthotics on the temporal-spatial gait parameters in children with cerebral palsy. Twenty spastic bilateral cerebral palsy (spastic CP) children (11 boys and 9 girls) participated in this study. GAITRite was used to examine the velocity, cadence, step length differential, step length, stride length, stance time, single support time, double support time, base of support, and toe angle while walking with and without foot orthotics. The differences in temporal-spatial parameters were analyzed using paired t-test. The significance level was set at .05. The velocity, cadence, both step lengths, both stride lengths, both bases of support and right toe angle significantly increased when the children with spastic CP with foot orthotics compared to without foot orthotics (p<.05). The step length differential between the two extremities, left stance time and left single support time, significantly decreased with foot orthotics (p<.05). Right stance time, right single support time, both double support times and left toe angle showed little change (p>.05). This study demonstrated that foot orthotics were beneficial for children with spastic CP as a gait assistance tool.

측두하악장애환자에 있어서 수의적 악물기시의 치아접촉점 및 저작근 활성에 관한 연구 (A Study on the Effects of Maximum Voluntary Clenching on the Tooth Contact Points and Masticatory Muscle Activities in Patients with Temporomandibular Disorders)

  • The purpose of the study was to evaluate the occlusal stability at the moment of dynamic occlusal tooth contact and to investigate the correlation between the occlusal stability and the masticatory muscle activities. It also evaluated the effect of short-term use of occlusal splints on the occlusal stability and the masticatory muscle activities in patients with temporomandibular disorders during maximum voluntary clenching by synchondronized with temporomandibular disorders during maximum voluntary clenching by synchronized use of the T-Scan system(Tekscan, Inc, USA) and K6-Diagnostic system(Myo-tronics Research, Inc, USA) The author measured its distance from retruded contact position(RCP) to intercuspal position(IP), average of contact intervals(ACI), total left-right statistics(TLR), average muscle activities of masseter and anterior temporal muscles during maximum voluntary clenching in 20 patients with temporomandibular disorders and 22 dental students as a control group. The data were compared between two groups and investigated for any correlations between the parameters. The results were as follows : 1. Both of the mean average of contact intervals and the mean absolute value of total left-right statistics during maximum voluntary clenching were increased in the patient group when compared with the control group. 2. Muscular disharmony of anterior temporal muscles of patient group is significantly greater than that of control group. However, muscular disharmony of masseter muscles of patient group is not significantly greater than that of control group. 3. There were significant correlations between muscular disharmony of anterior temporal muscles and average of contact intervals as well as total left-right statistics, and also between muscular disharmony of masseter muscles and total left-right statistics. 4. There were not any significant correlations between distance from RCP to IP and any other parameters. 5. There were a significant decrease in total left-right statistics and muscular disharmony of anterior temporal muscles during maximum voluntary clenching after a 1week use of occlusal splint in the patient group.

Neural Switching Mechanism in the late Korean-English bilinguals by Event-Related fMRI

  • Functional MRI technique was used in this study for examining the language switching mechanisms between the first language (L1) and the second language (L2). Language switching mechanism is regarded as a complex task that involves an interaction between L1 and L2. The aim of study is to find out the brain activation patterns during the phonological process of reading real English words and English words written in Korean characters in a bilingual person. Korean-English bilingual subjects were examined while they covertly read four types of words native Korean words, Korean words of a foreign origin, English words written in Korean characters, and English words. The fMRI results reveal that the left hemispheric language-related regions at the brain, such as the left inferior frontal, superior temporal, and parietal cortices, have a greater response to the presentation of English words written in Korean characters than for the other types of words, in addition, a slight difference was observed in the occipital-temporal lobe. These results suggest that a change in the brain circuitry underlying the relational processes of language switching is mainly associated with general executive processing system in the left prefrontal cortex rather than with a similarity-based processing system in the occipital-temporal lobes.

Pseudoaneurysm of the superficial temporal artery after blunt trauma: case report and literature review

  • An 88-year-old man presented with a left temporal pulsatile mass that developed after blunt trauma. Based on suspicion of hematoma, needle aspiration was performed with the removal of approximately 15 mL of blood. No evident improvement was noted, and active arterial bleeding was observed at the needle puncture site. Doppler ultrasonography revealed a "yin-yang" sign, and the mass was diagnosed as a pseudoaneurysm of the left superficial temporal artery. Under general anesthesia, the superficial temporal artery was ligated and the pseudoaneurysm was removed. Superficial temporal artery pseudoaneurysm is a rare facial tumor that generally occurs after blunt trauma. Due to its rarity, pseudoaneurysms are often misdiagnosed as hematoma. The treatment of choice is excision, although endovascular intervention is a potential treatment option. However, when a pseudoaneurysm is small, conservative treatment can be used.

경기 승패에 따른 야구팬들의 정서경험: fMRI연구 (Emotional experiences of baseball fans at winning and losing games: An fMRI approach)

  • 본 연구는 경기장에서 스포츠 팬들이 자신이 응원하는 팀의 경기 승패에 따라 경험하는 긍정, 부정 정서가 정서의 편측화를 보이는 지를 fMRI 실험을 통하여 살펴보는 것이다. 이를 위해 S 프로야구 구단 팬 12명을 대상으로 S구단이 승리한 장면과 패배한 장면을 보여주었다. 측정결과, 승리장면을 지켜본 피험자들의 좌우 쐐기소엽, 우하후두이랑, 우하전두이랑, 좌편도체, 우해마옆이랑, 좌갈고리, 좌대상이랑, 좌하측두이랑, 우중측두이랑, 좌소뇌비탈, 좌소뇌정상 영역에서 활성화 반응이 나타났다. 반면에 패배장면을 지켜본 피험자들의 우중간전두이랑, 좌전대상회, 좌소이랑, 좌조가비핵, 좌창백핵, 좌배쪽전핵, 좌시상, 좌전장, 좌뇌섬엽 영역에서 활성화 반응이 나타났다. 본 연구결과 정서가의 반구비대칭 증거가 뚜렷하게 나타나지 않았으며 승리자극에서는 나타난 편도체의 활성이 패배자극에서는 나타나지 않아 패배정서 유발에 보다 심층적인 연구가 필요할 것으로 보아졌다.

유치 교환기의 교근 및 측두근의 근전도 연구 (Electromyographic studies on the masseter and temporal muscles during exchange of the deciduous teeth)

  • Electoromyographic studies were performed on the action of the muscles of the temporomandibular joints following exfoliation of the deciduous teeth. The subjects examined, being 50 children. between the age of 6 and 13 years, divided into 5 groups. They were; 1) Deciduous dentition were complete in the first group. 2) Deciduous incisors were missing in either upper or lower jaw in the second group. 3) Deciduous canine and molars were missing in the left side of either upper or lower jaw in the third group. 4) Deciduous canine and molars were missing in the right side of either upper or lower jaw in the fourth group. 5) Permanent dentition completed in the fifth group(except third molars). Electromyogram was recorded with 4 channel polygraph (Grass model VII modified for 7P3). Electrodes which were the cup-typed gold discs, 9 millimeters in the diameter, were located on the anterior, middle and posterior lobes of the temporal muscles, and also on the superficial and deep layers of the masseter muscles. Paired electrodes were held by electrode cream so that they were pressed on the skin surface at right angle, adhesive tape being used to anchor them. The distance of the pair electrodes was about 5 millimeters. The results obtained were as follow: 1) In rest position of mandible; All groups showed slight, electrical activities in the muscles involved, but in the middle lobe of temporal muscle they were slightly higher. 2) In molar occlusion of mandible; High activity-anterior lobe of temporal muscle and superficial layer of masseter muscle. Moderate activity-deep layer of masseter muscle. Low activity-middle and posterior lobes of masseter muscle. There were no differences among the first, the second and the fifth groups. In the third group the muscle activity was weaker than that of the right, and in the fourth group opposite characteristics was revealed. 3) In incisal bite of mandreble; Hight activity-superficial layer of masseter muscle. Modertae activity-deep layer of masseter muscle. Low activity-anterior, middle and posterior lobes of temporal muscle. The first, the third, the fourth and the fifth groups showed no differences but the second group showed less activity than those of others. 4) In protrusion of mandible; High activity-deep layer of masseter muscle Moderate activity-superficial layer of masseter muscle. Low activity-anterior, middle and posterior lobes of temporal muscle. In the first, the fourth and the fifth groups, there were no differences in the activities, but the second group showed less activity than the others. 5) In retrusion of mandible; High activity-deep layer of masseter muscle. Moderate activity-superficial layer of masseter muscle. Low activity-anterior, middle and posterior lobes of temporal muscle. In the first, the third, the fourth and the fifth groups, there were no differences but the second group showed less activity than the others. 6) In lateral excursion of the mandible (either direction); High activity-posterior lobe of temporal muscle. Moderate activity-anterior and middle lobes of temporal muscle. Low activity-superficial and deep layers of masseter muscle. The muscle action potentials were weaker than those of the right side in the third group and vice ver'sa in the fourth group. 7) In chewing movement; Temporal muscle activities were higher than those of masseter, especially in the middle lobe of temporal muscle the activity was highest. Right side muscle activities were higher than those of the left in the third group and, on the contrary, the left side was dominant over the right in the fourth group.

뇌전증 환자의 MEG 데이터에 대한 분류를 위한 인공신경망 적용 연구 (Artificial neural network for classifying with epilepsy MEG data)

  • 본 연구는 좌측 해마 경화를 보인 내측두엽 뇌전증(left mTLE, mesial temporal lobe epilepsy with left hippocampal sclerosis) 환자군과 우측 해마 경화를 보인 내측두엽 뇌전증(right mTLE, mesial temporal lobe epilepsy with right hippocampal sclerosis) 환자군 그리고 건강한 대조군(healthy controls; HC)으로부터 측정한 뇌자도(magnetoencephalography; MEG) 데이터로 각 그룹을 분류하는 다중 분류 작업에 다양한 인공신경망을 적용하고 그 결과를 비교해 보고자 하였다. 합성곱 신경망, 순환 신경망 그리고 그래프 신경망으로 모델링한 결과, k-fold 정확도 평균은 합성곱 신경망 기반 모델, 그래프 신경망 기반 모델, 순환 신경망 기반 모델 순으로 우수하였다. 또한, 수행 시간은 순환 신경망 기반 모델, 그래프 신경망 기반 모델, 합성곱 신경망 기반 모델 순으로 우수하였다. 정확도 성능과 시간 면에서 모두 좋은 수치를 보이며, 네트워크 데이터의 확장성이 뛰어난 그래프 신경망이 앞으로 뇌 연구에 활용되기 적합한 모델임을 강조하고자 한다.

Arteriovenous Malformation of the Scalp : Efficacy of Computed Tomography Angiography

  • We report a rare case of scalp arteriovenous malformation[AVM]. A 55-year-old woman presented with a pulsatile palpable mass on her left temporo-parietal scalp. She complained of insomnia because of bruit, which was audible when she lay on her left side. Computed tomography angiography[CTA] for the scalp vessel showed AVM on the left temporo-parietal region. Multiple enlarged arteries, such as the superficial temporal artery, posterior auricular artery, and occipital artery, were directly connected to the elongated dilated superficial temporal vein. Digital subtraction angiography also showed similar results. Fistulous portions were clearly delineated on both modalities. Surgical excision of the malformations, including feeding arteries and the draining vein, resulted in immediate relief of the symptoms. Usefulness of CTA in the diagnosis of vascular lesions on the scalp was emphasized.

합곡(合谷) 침자(鍼刺)가 뇌혈류에 미치는 영향에 대한 핵의학적 고찰 (The nuclear medical study on the effect of Hap-Kok(LI4) Acupuncture on cerebral blood flow)

  • Objective : To localize and compare the cerebral regions- activated by the the stimulation of traditional and burning acupunctures in right Hap-Kok (LI4) acupoints. Methods : Thirty-four healthy normal volunteers (19 males, 15 females, age 31${\pm}$11 years) were studies by rest/acupuncture Tc-99m HMPAO SPECT using same-dose sequential injection method using right Hap-Kok(LI4), traditional and burning acupunctures. All images were spatially normalized and the differences between rest and acupuncture activation state were statistically analyzed using SPM 96. Results : Statistical analysis of the effect by the stimulation using traditional acupuncture in right L14 showed regional cerebral perfusion increase in right inferior frontal lobe, right straight gyrus, left anterolateral frontal lobe, left anteroinferior temporal lobe, left posterior temporal lobe, and left cerebellum. In the stimulation using burning acupuncture in right LI4, regional cerebral perfusion increased in right posterior prefrontal lobe, right precental gyrus, right postcentral gyrus, right poteroinferior temporal lobe, left precentral gyrus, left Broca's area, left anterior parietal lobe, left posterior prefrontal lobe, and left cerebellum. In right LA, diffuse perfusion increase were noted in the both inferior frontal lobe by traditional acupuncture compared to burning acupuncture. Conclusion : The results localized the cerebral areas showed the effect of the acupuncture on cerebral blood flow. The effects of traditional and burning acupunctures on cerebral blood flow were similar in right Hap-Kok (LI4) acupoints. But the effects of traditional acupunctures on cerebral blood flow are stronger than those of burning acupunctures on cerebral blood flow.
