Potts Shunt in Patients with Primary Pulmonary Hypertension

  • Idiopathic pulmonary arterial hypertension eventually leads to right-sided heart failure and sudden death. Its mortality rate in children is still high, despite improvements in pharmacological therapy, and therefore novel treatments are necessary. The Potts shunt, which creates an anastomosis between the left pulmonary artery and the descending aorta, has been proposed as a theoretically promising palliative surgical technique to decompress the right ventricle. We report the case of a 12-year-old girl with suprasystemic idiopathic pulmonary hypertension and right ventricular failure who underwent a Potts shunt for palliation with good short-term results.

Cor Triatriatum Sinistrum with an Ostium Primum Atrial Septal Defect in a Siamese Cat

  • An approximately 8-month-old, 2.61 kg, male Siamese kitten was referred with primary complaints of a 1-week history of respiratory distress, exercise intolerance and dyspnea. Diagnostic studies identified III/VI systolic murmur in the cardiac auscultation, right ventricular enlargement patterns in the electrocardiogram, pleural effusion and right-sided cardiomegaly in the thoracic radiography, and right marked ventricular dilatation, right atrial enlargement, atrial septal defect and abnormal left atrium divided by fibromuscular membrane. Based on these findings, the case was diagnosed as cor triatriatum sinistrum complicated with an ostium primum atrial septal defect. The cat was rescued with furosemide, nitroglycerine, oxygen supplement and fluid removal from pleural cavity.

Multi-Parametric Quantitative MRI for Measuring Myelin Loss in Hyperglycemia-Induced Hemichorea

  • Hyperglycemia-induced hemichorea (HGHC) is a rare but characteristic hyperkinetic movement disorder involving limbs on one side of the body. In a 75-year-old woman with a left-sided HGHC, conventional brain MR imaging showed very subtle T1-hyperintensity and unique gadolinium enhancement in the basal ganglia contralateral to movements. Multi-parametric MRI was acquired using pulse sequence with quantification of relaxation times and proton density by multi-echo acquisition. Myelin map was reconstructed based on new tissue classification modeling. In this case report of multi-parametric MRI, quantitative measurement of myelin change related to HGHC in brain structures and its possible explanations are presented. This is the first study to demonstrate myelin loss related to hyperglycemic insult in multi-parametric quantitative MR imaging.

Transitional Atrioventricular Septal Defect in a Dog

  • A 4-year-old castrated male Maltese was referred to our hospital with heart murmur. The client did not recognize any symptoms. Auscultation revealed systolic murmur located at the left heart base. Radiographs showed right-sided cardiomegaly with the main pulmonary artery bulging and enlargement of pulmonary vessels. Echocardiogram revealed a small ventricular septal defect and a large atrial septal defect. There was also a common atrioventricular valve. Based on diagnostic imaging studies, the dog was diagnosed with transitional atrioventricular septal defect.

  • In the present paper, we find the sharp bound for the fourth coefficient of starlike functions f which are normalized by f(0) = 0 = f'(0) - 1 and satisfy the following two-sided inequality: $$1+{\frac{{\gamma}-{\pi}}{2\;{\sin}\;{\gamma}}}\;<\;{\Re}\{{\frac{zf^{\prime}(z)}{f(z)}}\}\;<\;1+{\frac{{\gamma}}{2\;{\sin}\;{\gamma}}},\;z{\in}{\mathbb{D}}$$, where ${\mathbb{D}}:=\{z{\in}{\mathbb{C}}:{\left|z\right|}<1\}$ is the unit disk and ${\gamma}$ is a real number such that ${\pi}/2{\leq}{\gamma}<{\pi}$. Moreover, the sharp bound for the fifth coefficient of f defined above with ${\gamma}$ in a subset of [${\pi}/2,{\pi}$) also will be found.

A Case Report of Resection of a Mediastinal Paraganglioma: Why All the Fuss?

  • Mediastinal paragangliomas are rare tumors that have only been reported in individual cases or limited case series. Surgical resection of these tumors can be challenging, as they are highly vascular and intimately related to the great vessels. Surgery is usually performed via median sternotomy with or without cardiopulmonary bypass. We present the case of a mediastinal paraganglioma that was resected via a left-sided posterolateral thoracotomy. Histopathology revealed a completely resected 38-mm paraganglioma with a positive station 5 lymph node, indicative of locally aggressive disease. Hereditary paragangliomas are associated with malignant transformation; therefore, genetic testing is important. These tumors do not respond well to chemoradiotherapy, and consequently lifelong surveillance for early detection of recurrence is recommended.

  • In this paper, we define a g-regular semigroup which is a generalization of a regular semigroup. And we want to find some properties of g-regular semigroup. G-regular semigroups contains the variety of all regular semigroup and the variety of all periodic semigroup. If a is an element of a semigroup S, the smallest left ideal containing a is Sa.cup.{a}, which we may conveniently write as $S^{I}$a, and which we shall call the principal left ideal generated by a. An equivalence relation l on S is then defined by the rule alb if and only if a and b generate the same principal left ideal, i.e. if and only if $S^{I}$a= $S^{I}$b. Similarly, we can define the relation R. The equivalence relation D is R.L and the principal two sided ideal generated by an element a of S is $S^{1}$a $S^{1}$. 선천성 횡격막 탈장의 지연수술 결과 (The Result of Delayed Operation in Congenital Diaphragmatic Hernia)

  • Congenital diaphragmatic hernia (CDH) in the past was considered a surgical emergency requiring immediate operation. Several groups now advocate preoperative stabilization and delayed surgery. The treatment strategy for CDH in this institution is delayed surgery after preoperative stabilization. The aim of this study was to evaluate the results of delayed surgery. A retrospective review of 16 neonates with CDH was performed. Surfactant. conventional mechanical ventilation. high frequency oscillation. and nitric oxide were utilized for preoperative stabilization as necessary. The difference in outcome between two groups differentiated by the duration of the preoperative stabilization periods with mechanical ventilation (${\leq}$ 8 hours and > 8 hours) was determined. Chi-square test was used to analyze the data. There were 7 right-sided hernias and 9 left. The average duration of stabilization was 32.4 hours. Hepatic herniation through the defect was found in 6 cases and all died. The most common postoperative complication was pneumothorax. The mortality rate of the right side hernia was higher than the left (85.7% vs. 33.3%. p=0.036). Mortality rate of the group (N=8) whose preoperative stabilization period was 8 hours or less was better than that (N=6) whose preoperative stabilization period was more than 8 hours (25.0% vs. 83.3%. p=0.031). The overall mortality rate was 56.3%. The better prognosis was noticed in left side hernia. no liver herniation, or shorter preoperative stabilization period.

노인의 단기집중 균형훈련이 하지수행능력에 미치는 영향 (Effects of Short-Term Intensive Balance Training for the Performance Ability of Lower Extremities in the Elderly)

  • The objective of this study was to discern the effects of a balance training program on the performance of lower extremities in order to improve strength (muscle power, flexibility, walking power), balance control and walking ability in the elderly. The subjects selected were aged 65 years and over, with no known relevant medical history that may disturb their balance, and have also been visiting the Gimhae Senior Welfare Center. The variable group consisted of 30 subjects, of whom were people who had been participating in balance training programs (One Leg Standing; OLS, Functional Reach; FR, Timed Get Up and Go; TUG) as an intervention for 8 weeks 3 times per a week. They were examined in order to identify their balance control before and after. The control group consisted of thirty subjects who preferred to exercise without any intervention relating to balance training program. The subjects were measured before and after balance training in order to determine the effectiveness of exercise and the effectiveness of exercise combined with the aid of a Balance Performance Monitor. 1. Mean time on OLS test with left and right sided extremities in the experimental group was 35.44 sec, 42.10 sec longer than control group respectively. In FR tests applied to the left and right side, mean reaching distance was increased up to 5.56 cm, 6.73 cm in experimental group respectively. Mean time on TUG test from a chair in experimental group was decreased to 2.33 sec. 2. Mean value of decline in the level balance control, both left and right side, decreased to 2.24% as examined by the Balance Performance Monitor. Mean scores for sway level after balance training decrease to .98% and for balance control both anterior and posterior directions decreased to mean 1.07% and 1.44%, respectively.

Acute Type A Aortic Dissection in a Patient with Situs Inversus Totalis

  • We describe the occurrence of acute type A aortic dissection in a patient with situs inversus totalis. A 37-year-old man presented to the emergency department with acute chest pain. Initial chest X-ray findings showed a right-sided heart and a left-sided liver. Contrast-enhanced computed tomography revealed a Stanford type A acute aortic dissection, aortic root dilatation, and situs inversus totalis. All of the thoracic structures were mirror-image reversed and an abnormal coronary artery was observed. The Bentall operation was performed. This report demonstrates that computed tomography and echocardiography were useful for understanding the anatomy and the presence or absence of concurrent anomalies in a patient with situs inversus totalis. The patient's postoperative course was uneventful.
