Association between Metabolic Syndrome and Depression among Korean Community-Dwelling Older Adults (지역사회 재가노인의 대사증후군과 우울의 관계)

  • This study evaluated the association between metabolic syndrome(MetS), its components and depression among Korean community-dwelling older adults. We analyzed data from the fifth Korea National Health and Nutrition Examination Survey(KNHANES-V) 2010-2012. This study was a descriptive study of 3,755 Korean elderly, aged 65 years and older. A modified version of the criteria of the National Cholesterol Education Program, Adult Treatment Panel III was used to define MetS. Depressive symptoms were assessed using the questionnaire of the KNHANES-V. Multiple regression analyses performed the association between MetS, its components and depression. MetS was not statistically associated depressive symptoms, but low high-density lipoprotein cholesterol(HDL-C) was statistically associated depressive symptoms(Odds raio[OR]=1.26, 95%Confidence Interval [CI]=1.002-1.584) in older adults. Among elderly women, MetS(OR=1.36, 95%CI=1.029-1.792), elevated fasting glucose(OR=1.41, 95%CI=1.070-1.851), low HDL-C(OR=1.21, 95%CI=1.078-1.746) were statistically associated depressive symptoms. In conclusion, present study showed an association between MetS and depressive symptoms in Korean community dwelling elderly women but not in men.

Association between physical activity and periodontitis as stratified by obesity in Koreans (한국인에서 비만에 따른 신체활동와 치주질환과의 연관성)

  • Background: The aim of this study was to examine the association between physical activity and periodontitis in the Korean population. Methods: This study utilized data from 9,191 participants of the Korea National Health and Nutrition Examination Survey. Periodontitis was defined as a CPI score of 3 or 4. Physical activity was assessed by the trained interviewer using the International Physical Activity Questionnaire-Short Form (IPAQ-SF). IPAQ-SF is composed of vigorous physical activity, moderate physical activity, and walking activity. Multivariable logistic regression analyses and stratified analyses by obesity were performed. All analyses were conducted separately for males and females. Results: Females who engaged in vigorous and moderate physical activity had a 28% (OR = 0.72, 95% CI = 0.54 - 0.95) and 34% (OR = 0.66, 95% CI = 0.44 - 0.98) lower risk of periodontitis, respectively. In obese females, moderate physical activity in obese had a 65% lower risk of periodontitis (OR = 0.35, 95% CI = 0.18 - 0.67). Conclusions: Our findings suggest that moderate-intensity physical activity was inversely associated with a lower risk of periodontitis. In obese females, moderate physical activity had an independent inverse association with periodontitis.

The Association Between Cancer and Network Structure of Depressive Symptoms (암과 우울증상 네트워크 구조의 연관성)

  • Objectives : The characteristics of depressive symptoms in patients with cancer is different from those in control group. However, few research has focused on the association between depressive symptoms in cancer patients. The aim of this study was to compare the network structure of depressive symptoms between patients with cancer and normal control. Methods : This study was based on cohort data from Korea National Health and Nutrition Examination Survey in 2016-2018. The Patient health Quetionnaire-9 (PHQ-9) was used to assess depressive symptoms in 599 patients with cancer and 599 age-sex matched controls. We estimated network structure of depressive symptom using Isingfit model. Results : There was no significant difference of each PHQ-9 item score. There were strong associations between symptoms were concentration problem-psychomotor activity, anhedonia-depressed mood, and depressed mood-suicidal ideation in both groups. Strength centrality of worthlessness was significantly higher in patients with cancer. Conclusions : These results suggest that worthless is associated with other depressive symptoms more tightly in patients with cancer. Worthless can serve as important treatment targets for intervention of depression in patients with cancer.

Prevalence of Activity Limitation in Korea's Older Adults Population (한국 노인의 활동제한 유병율)

  • This study was to identify the prevalence rate of the limitation of activity, the reasons for activity limitation, association with activity limitation and general characteristics among older adults in Korea's population. Data were extracted from the $7^{th}$ Korea National Health and Nutrition Examination Survey. The number of participants were 1,578 older adults who aged 65 years over. SPSS program (version 18.0) was used for analyzing on frequency, Chi-square test, and logistic regression analysis of the data. The total prevalence rate of activity limitation among Korea's population was 19.6% in 2016. The prevalence rate of the reasons for activity limitation in Korea was highest for neck and back problems(19.8%), followed by arthritis and rheumatism(17.1%). The prevalence rate of activity limitation was statistically significant associated with age and education level, respectively. In conclusion, Age, educational level, and musculoskeletal diseases such as problems related to back and neck, arthritis and rheumatism could be significant risk factors for activity limitations in older adults who aged 65 years over. These findings could be useful as evidence-based data for relieving the prevalence rate of activity limitation and the prevention and management of activity limitations in Korea population.

Obesity Prevalence in Pediatric Headaches (소아청소년 두통 환자에서 비만 유병률)

  • Purpose: Obesity and headache are two highly prevalent diseases both in childhood and adolescent. In this study, we assessed the prevalence of obesity in pediatric headaches patients in a single institution in Korea, and differences according to age, sex, headache type, frequency, intensity, and disability. Methods: We retrospectively reviewed the medical records of 340 subjects (6-18 years of age) who visited the Pediatric Headache Clinic of Bucheon St. Mary's Hospital during the period from January 2015 through March 2018. Data on age, sex, height and weight, as well as headache type, frequency, intensity and disability, were collected. Body Mass Index (BMI) percentile was calculated based on the 2017 Korean Children Adolescence Growth Chart. Results: 17.6% of the pediatric headache patients were obese. The prevalence of obesity in male patients was higher than females (Male 23.8% VS Female 11.6%, P=0.002). There were no significant differences in obesity rate according to age, headache type, frequency, intensity, and disability. Conclusion: The prevalence of obesity in the pediatric headache population was 17.6% which is higher than 10.1% in general population (Korea National Health and Nutrition Examination Survey, KNHANES, 2013).

Analysis and Evaluation of Glycemic Indices and Glycemic Loads of Frequently Consumed Carbohydrate-Rich Snacks according to Variety and Cooking Method (탄수화물 간식류 식품 및 조리방법에 따른 혈당지수 및 혈당부하지수)

  • This study examined the glycemic indices (GIs) and glycemic loads of carbohydrate-rich snacks in Korea according to variety and cooking method. The most popular carbohydrate snacks (corn, potatoes, sweet potatoes, chestnuts, and red beans) from the Korean National Health and Nutrition Examination Survey nutrient database were cooked using a variety of conventional cooking methods (steaming, baking, porridge, puffing, and frying). The GIs of foods were measured in 60 healthy males after receiving permission from the University Hospital institutional review board (KMC IRB 1306-01). Blood glucose and insulin levels were then measured at 0, 15, 30, 60, 90, and 120 min after consuming glucose, and each test food contained 50 g of carbohydrates (corn: 170.0 g, potatoes: 359.7 g, sweet potatoes: 160.3 g, chestnuts: 134.8 g, red beans: 73.1 g). GI values for test foods were calculated based on the increase in the area under the blood glucose response curve for each subject. Steamed potatoes ($93.6{\pm}11.6$), corn porridge ($91.8{\pm}19.5$), baked sweet potatoes ($90.9{\pm}9.6$), baked potatoes ($78.2{\pm}14.5$), steamed corn ($73.4{\pm}9.9$), and steamed sweet potatoes ($70.8{\pm}6.1$) were shown to be considered high GI foods, whereas baked chestnuts ($54.3{\pm}6.3$), red bean porridge ($33.1{\pm}5.5$), steamed red beans ($22.1{\pm}3.2$), fried potatoes ($41.5{\pm}7.8$), and ground and pan-fried potatoes ($28.0{\pm}5.1$) were considered as low GI foods. The results suggest that the cooking method of carbohydrate-rich snacks is an important determinant of GI values.

Effect of coffee and green tea consumption on liver enzyme and metabolic syndrome in Korean (한국인에서 커피 및 녹차의 섭취빈도가 간염증 수치 및 대사증후군에 미치는 영향)

  • We investigated whether coffee and green tea consumption reduced the risk of elevated alanine aminotransferase(ALT)/aspartate aminotransferase(AST) activity and the prevalence of metabolic syndrome. Participants were 5,283 adults, aged 19-79 years, in the Third Korean National Health and Nutrition Examination Survey, with excessive alcohol consumption, overweight, viral hepatitis, metabolic syndrome. Increased coffee and green tea consumption was not associated with decreased serum ALT. However, amount of coffee consumption had negative correlation with serum AST activity. Moreover, coffee consumption reduced the prevalence of metabolic syndrome (p for trend <0.001). Hypertension, impaired glucose metabolism and dyslipidemia was involved as subgroup of metabolic syndrome. Comparing persons who drank more than 2 cups per day with less than 1 cup per day, the prevalence of all subgroups was declined significantly. In this large, national, population-based study, consumption of coffee was associated with lower the risk of metabolic syndrome.

Comparison of Oral Health Status according to Glycated Hemoglobin A1c (당화혈색소에 따른 구강건강상태 비교)

  • The purpose of this study was to compare dental health status of South Korean adults, according to the control protocol of glycated hemoglobin A1c (HbA1c). From the measurements of HbA1c levels of 4,991 individuals over the age of 19 who participated in the sixth Korea National Health and Nutrition Examination Survey, the following conclusions were obtained. Glycated hemoglobin was 7.2% in the HbA1c ${\geq}6.6$ diabetic group and 5.9% in women and 8.5% in men. The ratio in the glucose non-control group was higher in the older age group, the lower average monthly household income group, and the lower education level group. Both the tissue health index and functioning teeth index indices were slightly higher in both men and women in the glucose control group. The tissue health index was higher in the younger age group, in the higher average monthly household income group, and in the higher education level group. The missing teeth (MT) index was greater among women (3.775 pieces) than among men (2.317 pieces) in the glucose non-control group. Higher age correlated with a greater number of MT, while higher income and education levels correlated with a lower number of MT. In conclusion, we will continue to provide counseling and education on oral health problems, thereby widening the awareness of the importance of oral health care and providing a variety of media and education methods related to medical and oral health that can manage diabetic patients according to the level of HbA1c It should be developed.

Analysis and Risk Assessment of Benzo(a)pyrene in Edible Oils (식용유지류 중 벤조피렌의 함량 분석 및 안전성 평가)

  • To assess the health risk for benzo(a)pyrene by the intake of edible oils, 288 cases of edible oils collected from food markets were analysed using the high performance liquid chromatography with fluorescence detector. The levels of benzo(a)pyrene were from non-detection to $4.78{\mu}g/kg$, and the average was $0.11{\mu}g/kg$. The chronic daily exposures of benzo(a)pyrene for total population group and consumer-only group were estimated using the food consumption data in the fifth Korea National Health and Nutrition Examination Survey in 2011. The estimated daily intake of benzo(a)pyrene was $4.26{\times}10^{-3}ng/kg$ b.w./day for total population group and $7.64{\times}10^{-3}ng/kg$ b.w./day for consumer-only group. The MOE (margin of exposure) of benzo(a)pyrene for total population group and consumer-only group was $7.28{\times}10^7{\sim}1.74{\times}10^8$ and $3.95{\times}10^7{\sim}9.42{\times}10^7$, respectively. Accordingly, the health risk from benzo(a)pyrene caused by the intake of edible oils was considered as a very low level.

Association between the usage of dental floss and interdental brushes and the prevalence of systemic diseases (치실 및 치간칫솔 사용과 전신질환 유병률의 연관성)

  • Background: Oral health has a significant impact on systemic health, and the close association between oral and systemic diseases has been continuously reported. To prevent oral diseases, the role of oral hygiene products such as dental floss and interdental brushes, in addition to tooth brushing, is becoming increasingly important. This study aims to analyze the effect of using oral hygiene products on the lifetime prevalence of systemic diseases among Korean adults. Methods: This study utilized data from the 7th Korea National Health and Nutrition Examination Survey (2016-2018). The study population consisted of 13,199 adults aged 19 years and older. The independent variable was the use of oral hygiene products, and the dependent variable was the prevalence of systemic diseases diagnosed by a physician. Demographic variables, health status, and behavioral variables were included as covariates, and multivariable logistic regression analysis was performed. Results: The use of dental floss showed no significant association with the prevalence of systemic diseases. However, those who did not use interdental brushes had a 22% lower likelihood of dyslipidemia (OR 0.777, 95% CI 0.660-0.913). Among participants with periodontal disease, those who did not use dental floss had a significantly higher risk of myocardial infarction (OR 11.488, 95% CI 1.438-91.772). Conversely, those who did not use interdental brushes had lower risks of dyslipidemia, myocardial infarction, and angina, particularly among women and individuals under 65 years of age. Conclusion: This study found a low overall association between the use of oral hygiene products and the prevalence of systemic diseases, but there was a notable association with cardiovascular diseases. To reduce the risk of myocardial infarction, the prevention and treatment of periodontal disease, along with proper oral hygiene management, are crucial. Future prospective studies are needed to clearly establish the causal relationship between oral hygiene and systemic diseases.
