  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. The maximum adsorbed amount was obtained in the neighborhood of pH 4.5. Isoelectric point of bovine albumin is pH 5.0, so experimental result shows that it shifted to acid area. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made. Comparisons between the valves were made on the transvalvular pressure drop, regurgitation volume and maximum valve opening area. The polymer valve showed smaller regurgitation volume and transvalvular pressure drop compared to the mechanical valve at higher heart rate. Because the point of maximun temperature of laser-irradiated gelatin exists below the surface due to surface cooling, evaporation at the boiling temperature is made explosively from below the surface. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. We obtained a two-dimensional current density distribution within a phantom by processing the real and imaginary MR images from a 4.77 NMR machine. We implemented a resistivity mage reconstruction algorithm using the finite element method and sensitivity matrix. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. We use combination of MLP and fuzzy filter for pattern recognition to estimate the direction of a muscle and Hogan's method for the force estimation. The main unit includes a 12.1" TFT color LCD, a main CPU board, and peripherals such as a module controller, Ethernet LAN card, video card, rotate/push button controller, etc. The main unit can connect at maximum three module cases each of which can accommodate up to 7 parameter modules. They include the modules for electrocardiograph, respiration, invasive blood pressure, noninvasive blood pressure, temperature, and SpO2 with Plethysmograph.SpO2 with Plethysmograph.

CBCT와 Simulation CT를 이용한 치료계획의 선량비교 (Comparison of using CBCT with CT simulator for radiation dose of treatment planning)

  • 최근의 방사선치료용 선형가속기에 부착된 진단용 kV 에너지 영역의 X선 선원과 아모퍼스 실리콘(a-Si)의 검출기로 구성된 온보드영상장치(OBI)를 이용하여 콘빔 전산화단층촬영 영상(CBCT)획득이 가능하다. CBCT영상을 이용하여 치료계획을 세우게 되면 치료실에서 CT영상 촬영이 가능해짐으로써 고식적 치료 환자들의 부담이 많이 감소 될 수 있고 더 나아가 선량을 재계산하여 치료과정 중 치료계획 재수립도 가능하다. 본 연구에서는 CBCT를 이용한 치료계획과 기존의 모의치료용 CT를 이용한 치료계획을 비교 연구 함으로서 CBCT영상만으로 광자선 선량계산이 정확한지를 평가하고 임상에서 고식적방사선치료를 목적으로 하는 환자들을 대상으로 온라인 방사선치료계획의 가능성을 연구하였다. 선량계산에 필요한 CT수와 밀도간의 상호관계 확인을 위하여 Catphan 600 팬텀을 이용하여 교정곡선을 산출하였고 팬텀과 환자들의 모의치료용 CT영상과 CBCT영상을 획득하여 치료계획 및 선량계산 된 결과를 비교하였다. CBCT 영상을 이용한 치료계획에서의 MU차이는 중심점에 100cGy 처방하였을 때 Phantom에서의 경우 3~4MU로 약 2.7%, 환자에서의 경우 1~3MU로 약 2.5% 이하로 차이가 났다. 팬텀과 환자에서의 Monitor unit(MU)차이는 2.7%, 2.5% 이내였으나, CBCT영상의 경우 검출기의 크기의 제약 및 환자의 불수의적인 움직임에 의하여 전자밀도가 큰 물질에서 산란선과 artifact의 발생이 크게 증가한다. 따라서 뇌 및 폐 영역의 치료계획시 선량의 오차가 더 커질 수 있어 이에 대한 주의가 요구된다. 치료시작 전 CBCT 영상을 획득하여 환자의 자세와 내부 장기의 위치를 보정하고 선량을 재계산하여 치료 계획을 재수립하는 적응방사선치료(ART)를 시행하기 위해서는 산란선과 움직임에 의한 artifact의 감소방안이 마련되어야 할 것으로 사료된다.

  • 본 실험은 왕복 회전 운동을 하는 단일 파일 시스템으로서 Reciproc과 WaveOne을 이용하였으며 glide path를 형성했을 때와 안 했을 때의 근관의 중심 변위율과 만곡도의 감소를 비교하여 새로운 단일 파일 시스템에서 glide path가 근관 성형 후 근관의 형태에 미치는 영향을 연구하였다. 총 40개의 모형 근관에 메틸렌블루 용액을 주입한 후 근관성형 전 이미지를 스캔 하였다. 네 개의 그룹으로 나누어졌으며 그룹 1은 PathFile (Dentsply Maillefer) #1, 2, 3 사용 후 R25 크기의 Reciproc 파일(VDW)을 사용하여 근관 성형하였다. 그룹 2는 PathFile 사용 없이 R25 크기의 Reciproc 파일을 사용하여 근관 성형하였다. 그룹 3은 PathFile #1, 2, 3 사용 후 Primary 크기의 WaveOne 파일 (Dentsply Maillerfer)을 이용하여 근관 성형하였다. 그룹 4는 PathFile 사용 없이 WaveOne 파일을 이용하여 근관 성형하였다. 파일은 모두 21 mm 길이를 사용했다. 각각의 파일을 위한 전기 모터로 Reciproc 파일은 VDW.SILVER motor (VDW), WaveOne 파일은 Waveone endo motor (Dentsply Maillefer)와 PathFile은 X-SMART (Dentsply Maillefer)를 사용하였다. 근관 성형을 마친 모형 근관을 다시 이미지 스캔 하였다. 성형 전, 후의 이미지를 Adobe Photoshop CS 3 (Adobe Systems Incorporated)을 이용하여 중첩하여 평가하였다. 중심 변위율은 Calhoun과 Montgomery에 의해 제안된 다음 공식을 사용하여 계산되었다:CR=|X1-X2|/Y. 실험 결과 근관의 만곡도가 감소하는 치근단 1, 2 mm level에서는 glide path의 형성에 따른 근관의 중심 변화율의 통계학적 유의성이 보이지 않았다. 근관 만곡 부위인 치근단 3, 4, 5 mm level에서는 glide path를 부여한 것이 통계학적으로 유의성 있게 낮은 중심 변위율을 보였다. 치근단 1 mm level에서는 Reciproc 파일이 유의성 있게 낮은 중심 변위율을 보였고, 치근단 1, 6 mm level을 제외한 모든 level에서 WaveOne 파일이 유의성 있게 낮은 중심 변위율을 보였다. 치근단 6 mm level에서는 Reciproc과 WaveOne의 차이는 관찰할 수 없었고 glide path를 형성한 그룹이 형성하지 않은 그룹에 비해 유의성 있게 낮은 중심 변위율을 보였다. 본 실험을 통해 만곡된 근관에서 근관 성형 전 glide path의 형성은 근관의 원래 형태를 유지하는데 필수적이며, 만곡된 근관에서는 WaveOne 파일 시스템의 선택이 근관의 원래 형태를 유지하는데 더 적절할 것으로 생각된다.

타이타늄 임플랜트 시편 내부에 설치한 자석의 자성강도에 따른 골형성 변화 (THE CHANGE OF BONE FORMATION ACCORDING TO MAGNETIC INTENSITY OF MAGNET PLACID INTO TITANIUM IMPLANT SPECIMENS)

  • Purpose. The purposes of this investigation were to discover the possibility of clinical application in the areas of dental implants and bone grafts by investigating the bone formation histologically around specimen which was depending on the intensity of magnetic field of neodymium magnet inside of the specimens. Material and method. 1. Measurement of magnetic intensity - placed the magnet inside of the specimen, and measured the intensity of magnetic field around the 1st thread and 3rd thread of specimen 20 times by using a Gaussmeter(Kanetec Co., Japan). 2. Surgical Procedure - Male rabbit was anesthetised by constant amount of Ketamine (0.25ml/kg) and Rompun (0.25ml/kg). After incising the flat part of tibia, and planted the specimens of titanium implant, control group was stitched without magnet, while experimental groups were placed a magnedisc 500(Aichi Steel Co., Japan) or magnedisc 800(Aichi Steel Co., Japan) into it, fixed by pattern resin and stitched. 3. Management after the surgery - In order to prevent it from the infection of bacteria and for antiinflammation, Gentamycin and Ketopro were injected during 1 week from operation day, and dressed with potadine. 4. Preparation of histomorphometric analysis - At 2, 4 and 8 weeks after the surgery, the animals were sacrificed by excessed Ketamine, and then, specimens were obtained including the operated part and some parts of tibia, and fixed it to 10% of PBS buffer solution. After embedding specimens in Technovit 1200 and B.P solution, made a H-E stain. Samples width was 75$\mu$m . In histological findings through the optical microscope and using Kappa image base program(Olympus Co. Japan), the bone contact ratio and bone area ratio of each parts of specimens were measured and analyzed. 5. Statistical analysis - Statistical analysis was accomplished with Mann Whitney U-test. Results and conclusion. 1. In histomorphometric findings, increased new bone formation was shown in both control & experimental groups through the experiment performed for 2, 4 & 8 weeks. After 4 weeks, more osteoblasts and osteoclasts with significant bone remodeling were shown in experimental groups. 2. In histomorphometric analysis, the bone contact ratios were 38.5% for experimental group 1, 29.5% for experimental group 2 and 11.9% for control group. Experimental groups were higher than control group(p<0.05) (Fig. 6, Table IV). The bone area ratios were 60.9% for experimental group 2, 46.4% for experimental group 1 and 36.0% for control group. There was no significantly statistical difference between experimental groups and control group(p<0.05) (Fig. 8, Table VII) 3. In comparision of the bone contact ratios at each measurement sites according to magnetic intensity, experimental group 2(5.6mT) was higher than control group at the 1st thread (p<0.05) and experimental group 1 (1.8mT) was higher than control group at the 3rd thread(p<0.05) (Fig. 7, Table V, VI). 4. In comparision of the bone area ratios at each measurement sites according to magnetic intensity, experimental group 2(5.6mT) was higher than control group and experimental group 1 (4.0mT) at the 1st thread(p<0.1) and experimental group 2(4.4mT) was higher than experimental group 1 (1.8mT) at the 3rd thread(p<0.1) (Fig. 9, Table IX, X). Experiment group 2 was largest, followed by experiment group l and control group at the 3rd thread of implant. There was a significant difference at the 1st thread of control group & experiment group 2, and at 1st thread & 3rd thread of experiment group 1 & 2, and not at control group experiment group 1.(p<0.1)

간암환자의 토모치료시 Body-fix 사용유무에 따른 유용성 평가 (Evaluation on Usefulness of Applying Body-fix to Liver Cancer Patient in Tomotherapy)

  • 목 적: 방사선 치료에서 매번 치료를 시행할 때마다 호흡에 의한 해부학적인 내부 장기의 움직임과 환자자세의 변화에 따라 치료부위 오차가 발생한다. 이런 치료부위 오차의 발생은 치료부위와 정상 조직 간의 선량분포의 변화에 영향을 줄 수 있는 원인이 된다. 간암 환자 치료 시 Body-fix의 사용 유무 차이에 따른 임상치료에서의 유용성을 알아보고자 한다. 대상 및 방법: 본원에서 2009년 10월부터 2010년 7월까지 Hi-Art Tomotherapy를 시행 받은 55~60세 사이의 남자환자 중에서 간의 Couinaud 분류 중 V~VI 구역에서 간세포성 암(Hepatocellular carcinoma)이 발생한 환자 10명을 대상으로 Body-fix 사용 유무에 따라 2 그룹으로 분류하여 조사하였다. Body-Fix를 사용 시 진공펌프(Vacuum pump, Medical intelligence, Germany)를 이용하여 80 mbar의 압력으로 진공상태를 유지하였다. 환자는 정상호흡(free breathing)을 유지한 상태에서 바로누운자세(supine position)로 치료 시작부터 5회 치료 시 획득한 MV-CT (Megavoltage computed tomograpy, MVCT)와 KV-CT(Kilovoltage computed tomograpy, KVCT)을 융합하여 일치시키는 작업 후 X축의 좌우방향(Right to Left, RL), Y축의 상하방향(Craniocaudal, CC), Z축의 전후방향(Anterioposterior, AP)에서의 발생하는 방사선 치료 준비오차를 측정하여 비교 분석하였다. 결 과: 영상융합을 통한 평균 방사선 치료 준비오차는 A 그룹에서 $0.3{\pm}1.1\;mm$ (상하), $-1.1{\pm}0.7\;mm$ (좌우), $-0.2{\pm}0.7\;mm$ (전후)이었다. B 그룹에서는 $0.62{\pm}1.94\;mm$ (상하), $-3.62{\pm}1.5\;mm$ (좌우), $-0.22{\pm}1.2\;mm$ (전후)로 이동하였다. 두 그룹간의 Body-fix의 사용유무에 따른 X축, Y축, Z축 방향의 편차는 최대 X축(좌우) 5.5 mm, Y축(상하) 19.8 mm, Z축(전후) 3.2 mm로 측정되었다. 또한 오차방향의 분석과 관련하여 모든 환자에 일관성이 존재하지는 않지만 Body-Fix를 사용 시 매일 환자자세 변화와 호흡변화가 발생함에도 불구하고 방사선 치료 준비오차에 대한 위치 변화가 안정적인 양상의 결과를 보여준다. 결 론: 간암 환자 치료 시 Body-Fix의 사용은 방사선 치료 준비오차가 환자자세와 호흡변화에도 불구하고 각 그룹간의 편차가 발생하는 가운데에 안정적이며 규칙적이므로 Tomotherapy와 같은 고 정밀 치료에 효과적으로 사용되리라 사료된다.

5 Mega 화소 진단용 전자표시장치 인수검사 및 국내 실정에 적합한 정도관리 가이드라인 연구 (The Acceptance Testing of 5 Mega Pixels Primary Electronic Display Devices and the Study of Quality Control Guideline Suitable for Domestic Circumstance)

  • 2005년 6월 연세의료원 세브란스병원에서는 진단목적으로 20대의 5 mesa 화소 Totoku ME511L 평판 LCD 표시장치를 추가하여 PACS를 확장하였다. 20대의 Totoku ME511L 표시장치를 대상으로 반사, 휘도 반응, 휘도 공간 의존, 분해능, 잡음, 베일링 그레어, 그리고 색도 항목에 대하여 AAPM TG 18보고서에 기준하여 정량적인(또는 시각적인) 인수검사를 실시하였다. 측정에 사용된 장치는 색도를 측정할 수 있는 만원경식 휘도계, 조도계, 반사 측정에 필요한 빛 선원, 빛 차단 장치, 그리고 TG18 테스트 패턴들이었다. 선택된 8대의 표시장치에 대한 평균 확산 반사계수($R_d$)는 $0.019{\pm}0.02sr^{-1}$이었다. 휘도 반응 검사에서, 휘도비(LR), 최대 휘도 변이($L_{max}$), 그리고 최대 대조도 반응 변이는 각각 $550{\pm}100,\;2.0{\pm}1.9%$, 그리고 $5.8{\pm}1.8%$이었다. 휘도 균일도 검사에서, 최대 휘도 변이는 TG18-UNL10 테스트 패턴의 10% 휘도에 대하여 $14.3{\pm}5.5%$이었다. 분해능 측정은 휘도 측정법으로 수행하였으며 중심에서 퍼센트 휘도(${\Dalta}L$)는 $0.94{\pm}0.64%$이었다. 시각적인 방법의 잡음측정의 모든 경우에서, 중앙 및 주위 4사분면에서 가장 작은 것을 제외하고 15개의 모든 정사각형의 표적을 인식할 수 있었다. 글레어 비(GR)은 $12,346{\pm}1,995$이었다. 최대 색도 균일도 지표(D)는 $0.0025{\pm}0.0008$이었다. 또한 국내 실정에 적합한 진단용 표시장치의 정도관리 가이드라인 연구 결과를 제시하였다. 모든 인수검사 결과는 AAPM TG18 보고서 기준에 포함되어 진단 목적으로 사용하기에 적합하였다. 결론으로 인수검사는 표시장치가 진단에 사용하기에 적합한 성능임을 알려줄 뿐만 아니라 정도관리의 가이드라인, 최적 판독 환경, 그리고 표시장치의 교체시기를 결정하여주는 중요한 역할을 할 수 있다.

NEMA NU2-2001을 이용한 Siemens CTI ECAT EXACT 47 스캐너의 표준 성능 평가 (Performance Evaluation of Siemens CTI ECAT EXACT 47 Scanner Using NEMA NU2-2001)

  • 목적: 전신용 PET에 대한 표준 성능 평가 방법으로 NEMA NU2-2001이 확립되어 제안되었다. 따라서 새로이 설치되는 PET 스캐너뿐 아니라 기존에 사용 중인 스캐너에 대한 성능 평가가 이 표준 방법에 따라서 새로이 이루어 져야 한다. 이 연구에서는 NEMA NU2-2001 방법을 이용하여 CTI ECAT EXACT 47 PET 스캐너의 공간해상도, 민감도, 산란분획, NECR 등을 측정하였다. 대상 및 방법: 공간해상도를 평가하기 위하여 축 방향 시야의 정 가운데와 축 방향 시야 길이의 1/4을 벗어난 횡단면에 F-10을 채운 유리관(내경 1.1 mm)을 횡단면의 중심에서 1, 10 cm 떨어진 지점에 축 방향과 평행하게 위치시킨 후 PET 영상을 얻었다. 민감도를 측정하기 위하여 폴리에틸렌 및 알루미늄 관에 F-18을 채운 후 불응시간 손실이 1%를 넘지 않는 것을 확인한 후 영상을 획득하였다. 산란분획 및 최적 영상 획득 조건을 얻기 위하여 NECR을 NEMA 산란 팬텀을 이용하여 측정하였다. 결과: FBP재구성 방법(화소 크기: $0.515{\times}0.515mm^2$)으로 영상을 재 구성했을 때 스캐너의 중심에서 1cm 벗어난 지점에서 축방향, 횡축방향 공간 분해능은 0.62, 0.66 cm (FBP, 2D와 3D), 0.67, 0.69 cm (FBP, 2D와 3D)이었고 중심에서 10 cm 벗어난 지점에서 축방향, 횡축반경방향, 횡축접선방향 공간 분해능은 0.72, 0.68 mm (FBP, 2D와 3D), 0.63, 0.66 mm (FBP, 2D와 3D), 0.72, 0.66 mm (FBP, 2D와 3D)이었다. 민감도는 스캐너의 횡축방향 708.6 (2D), 2931.3 (3D) counts/sec/MBq, 횡축방향 중심에서 10cm 벗어난 지점에서 728.7 (2D), 3398.2 (3D) counts/sec/MBq 이었다. 산란 분획은 0.19 (2D), 0.49 (3D)이었고 최고 참 계수율과 NECR은 2차원 영상 획득 모드에서 40.1 kBq/mL 일 때 64.0 kcps, 40.1 kBq/mL 일 때 49.6 kcps, 3차원 영상 획득 모드에서 4.76 kBq/mL 일 때 53.7 kcps, 4.47 kBq/mL 일 때 26.4 kcps이었다. 결론: 이 실험에서 NEMA NU2-2001로 측정한 PET스캐너의 물리적 특성은 PET스캐너에 대한 객관적 평가 및 최적화 된 영상 획득과 분석에 유용할 것이다.

복식에 나타난 해체주의 양식연구 -건축과 복식의 비교- (A Study of Deconstruction in Clothing -Comparison of Clothing with Architecture-)

  • Deconstructionism is a philosophical stream in the latter half of 20'th century which goes against western metaphysics and tries to deconstruct the dualism e.g. eastern/west-ern man/woman. Its main concepts are 'indi-vidual' 'other' 'difference' 'restoration of the repressed' 'decentralization' 'Today it shows strong influences in literature art, and other social fields. This study investigated inner meanings and exterior forms represented in clothing on the base of Jacques Derrida's theory in order to re-search modes of deconstruction in is. And it compared clothing with architecture among other genres of art in order to discover similarities between through and modes of art in a certain period. For illuminating concept of deconstruction I referred to the literatures of preceding studies and for deconstruction's characteristics in both clothing and architec-ture I referred work's collection book $\ulcorner$collec-tions$\lrcorner$ and other related books dealing from 1960's to this point. As a rsult there are four modes of decons-truction in both clothing and architecture as follows; 1) Differenance which is a concept of the dif-ference in time and space as being trace. 2) In termeaning of meanings which is not present in reality but re-interpreted a new in the future. 3) Interxtuality in which a texte is not alone but with others 4) Dis.De phenomenon in which distortion and fragmentation forms occur as the result of the denial of perfection and reson. There are characteristics of modes of de-construction in clothing as follows; 1) Differance; R.Gigli, P. Rabanne. G.Bersace, V.Westwood are representatives of the cloth-ing which is combined with the past the pres-ent and the future or is re-viewed as the clothing of the past in the present point of view. J. Watanabe R. Dawakubo I miyake are representatives of the clothing which is out-date but re-viewed in the sense of recollec-tion and re-usage. 2) Intermeaning of meanings: R. kawakubo I, Miyake Y,Yamamoto are representatives of the clothing which is incomplete but re-interpreted in the future and appears to be new-fashioned according to ways of bnding wearing throwing on and tying J.C. Castel-bajac K.Hamnett S.Sprouse are representa-tives of the clothing which is made up of ab -stract forms scribbling or symbolic letters which appears to be different according to view-point of observers. 3) Intertextuality ;J.P.Gaultier K.Hamnett, Comme des Gar ons are representatives of intertextuality of gender which avoids the 여-ality of man/woman J.P Gaultier G,Versace J. galliano are representatives of the intertex-tuality of time place and occasion which mixes temporality extensity and purposiveness. J.P Gaultier G,Versace are representatives of the intertextuality of coordination which combines items different in image purpose and use. P.Rabanne A,Courr ges R,Gernreic I,Miyake are representatives of the intertextuality of mat-ter which uses heterogeneous matter different from cloth. 4) Dis.De-phenomenon: R, Kawakubo I.Mi-yake J.P.Gaultier are representatives of the de-composition which discloses distortion and exag-geration of form through destructing the estab-lished way of construction J.Galliano R.Kawa-kubo Devota Y Lomba arte representatives of the decentring which restores the repressed and the alienated on the one hane and shows front-centrality on the other. Comme des Gar ones is representative of the discontinuity because of which right and left up and down are not in har-mony with one another. J,Galliano J.P,Gaultier T,Mugler are representatives of the disruption by way of which one makes one's body exposed through intentional slashes or holes. As a consequence deconstructionism enabled us to investigate similarities between through of deconstruction and modes of art interms of diffrance Intermeaning of meanings intertextuality and Dis De-phenomenon. And we found that deconstruction was a phase of development in that it as a all-comprising and multiple concept tries to pursue the new through deconstruction.

비소세포성 폐암 환자의 방사선 치료 시 제한 호흡 주기의 유용성 평가 (Evaluation of the Usefulness of Restricted Respiratory Period at the Time of Radiotherapy for Non-Small Cell Lung Cancer Patient)

  • 목 적: 비소세포성 폐암 환자의 호흡 조절 방사선 치료 시 호흡에 의한 종양의 움직임을 최소화하는 것이 필수적이다. 이에 본 연구에서는 자유호흡 주기와 제한호흡 주기를 각각 적용한 치료 계획을 비교, 분석하여 제한 호흡 주기의 유용성을 평가하고자 한다. 대상 및 방법: 2011년 4월부터 12월까지 비소세포성 폐암 환자 9명(tumor n=10)을 대상으로 환자에게 평소의 호흡주기를 바탕으로 측정한 '자유호흡 주기'와 의도적으로 줄인 '제한호흡 주기'를 '신호모니터-호흡(guided-breathing)'법을 사용하여 각각 훈련을 실시한 후 RPM과 4차원 전산화 단층촬영 모의치료기를 이용해 치료계획용 4D CT를 실시하여 총 10개의 호흡 위상(respiration phase)별 CT 영상을 획득하였다. 각 호흡 주기의 CT 영상에 관측자 두 명이 각각 설정한 육안적 종양체적(Gross Tumor Volume, GTV)과 내부표적체적(Internal Target Volume, ITV)부피를 측정, 비교하였고 들숨(end-inspiration, EI)인 0%와 날숨(end-exhalation, EE)인 50% 위상에서의 center of mass (COM)를 측정하여 종양 움직임의 진폭을 측정했다. 또한 관측자 두명이 각각 두 호흡 주기를 적용한 치료계획을 수립하였고 정상 폐 평균선량(mean dose to normal lung, MDTNL)과 정상 폐 용적의 정상조직합병증확률(normal tissue complication probability, NTCP)을 비교, 분석하였으며 측정된 자료의 정량적 평가를 위해 통계 분석을 실시했다. 결 과: 관측자 두 명의 '제한호흡 주기'를 적용한 치료 계획을 분석한 결과 '자유호흡 주기'에 비해 종양의 3D 방향 움직임이 관측자 1의 경우 38.75%, 관측자 2의 경우 41.10%의 감소율을 보였고 GTV와 ITV의 부피를 측정, 비교한 결과 GTV의 부피는 관측자 1의 경우 $14.96{\pm}9.44%$, 관측자 2의 경우 $19.86{\pm}10.62%$, ITV의 부피는 관측자 1의 경우 $8.91{\pm}5.91%$, 관측자 2의 경우 $15.52{\pm}9.01%$의 감소율을 보였으며 MDTNL과 NTCP를 분석, 비교한 결과 MDTNL은 관측자 1의 경우 $3.98{\pm}5.62%$, 관측자 2의 경우 $7.62{\pm}10.29%$의 감소율을, NTCP의 경우 관측자 1의 결과 $21.70{\pm}28.27%$, 관측자 2의 결과 $37.83{\pm}49.93%$의 감소율을 나타냈다. 또한 두 관측자의 결과 값의 상관관계를 분석한 결과 '자유호흡 주기'에서는 관측자간의 유의한 차이를 보이는 데 비해 '제한호흡 주기'에서는 관측자간의 차이가 없는 감소율을 보였다. 결 론: '자유호흡 주기'에 비해 '제한호흡 주기'를 적용한 치료 계획에서 평가인자들의 상대적인 감소를 나타내어 비소세포성 폐암 환자의 호흡 조절 방사선 치료 시 '제한호흡 주기'의 유용성 및 타당성을 확인할 수 있었다.

자궁경부암 강내조사 3차원 치료계획 시 Packing의 유용성 분석 (Packing effects on the intracavitary radiation Therapy 3-Dimension plan of the uterine cervix cancer)

  • 목적 : CT영상을 이용한 삼차원치료계획을 수립하여 방광과 직장의 점선량(point dose)과 체적선량(volume dose)에 대해 선량-체적 히스토그램(Dose-Volume Histogram)을 이용하여 자궁경부암의 강내치료에 사용하는 packing의 효과를 분석하였다. 대상 및 방법 : 자궁경부암 환자 7명을 대상으로 하여 동일조건하에서 packing을 시행했을 때와 제거했을 때 각각 CT촬영(Ultra Z, marconi, USA)을 하여 치료계획장치(Plato Brachy the Rapy V14.2.4)에서 방광과 직장의 reference point를 각각 표시하고 ICRU38에 따라 A point에 치료계획을 시행하였다. 하지만 rectum의 경우 ICRU에서 제시한 point가 rectum 선량을 대표할 수 있는 값으로 적절하지 않기 때문에 maximum point를 찾아 비교하였다. 그리고 rectum과 bladder의 $50\%,\;80\%,\;100\%$선량의 volume에 따른 체적선량을 알아보았다. 측정한 값들을 윌콕슨 부호검정(SAS 통계분석처리 프로그램)을 통하여 packing의 효과를 분석하였다. 결과 : Packing 제거 시에서의 방광과 직장의 reference point 선량은 $116.94{\pm}35.42\%,\;117.59{\pm}21.08\%$이었고, packing 시행한 경우에는 각각 $107.08{\pm}38.12\%,\;95.19{\pm}21.32\%$이었다. Packing시행 후에 방광은 $9.86\%$, 직장은 $22.4\%$감소하였다. Packing제거시의 방광과 직장 maximum point 선량은 $164.51{\pm}50.89\%,\;128.81{\pm}33.05\%$, packing시행한 경우 각각 $142.31{\pm}44.79,\;110.08{\pm}37.03\%$이었다. packing시행 후에 방광과직장 maximum point 선량이 $22.2\%,\;18.73\%$줄어들었다. packing제거시 방광과 직장선량의 $50\%,\;80\%,\;100\%$선량의 Volume은 방광이 $48.62{\pm}18.09\%,\;16.12{\pm}11.15\%,\;7.51{\pm}6.63\%$, 직장이 $23.41{\pm}14.44\%,\;6.27{\pm}4.28\%,\;2.79{\pm}2.27\%$이었고, packing시행한 경우의 $50\%,\;80\%,\;100\%$선량의 volume은 방광이 $40.33{\pm}16.72\%,\;11.63{\pm}8.72\%,\;4.87{\pm}4.75\%$, 직장이 $18.96{\pm}8.37\%,\;4.75{\pm}2.58\%,\;1.58{\pm}1.06\%$이었다. packing시행 후에 $50\%,\;80\%,\;100\%$선량의 volume 방광은 $8.29\%,\;4.49\%,\;2.64\%$, 직장은 $4.45\%,\;1.52\%,\;1.21\%$ 감소하였다. 결론 : 자궁경부암의 강내치료 시 사용하는 packing의 효과를 CT를 이용한 삼차원치료계획을 통하여 알아본 결과 ICRU 38에서 권고하는 방광과 직장의 기준 점선량(reference point dose)의 경우 P값이 각각 0.0781, 0.0781이었고, 최대점선량(maximum point dose)은 P값이 각각 0.0156, 0.0156으로써 유의한 차이를 보이는 것으로 나타났으나 $50\%,\;80\%,\;100\%$를 초과하는 체적선량(volume dose)의 경우 p갈이 0.15이상으로써 유의하지 않은 것으로 나타났다. 다시 말해서 packing의 효과가 점선량의 경우 차이가 있는 것으로 보이지만 실제 체적선량은 별 차이가 없는 것으로 분석되었다. 그 이유를 살펴보니 방광과 직장의 용적(volume)은 넓은데 비해 packing을 하는 부분은 일부분에 지나지 않아서 큰 선량감소의 효과가 없었던 것으로 보인다. 하지만 방사선의 강도는 거리 역 제곱에 비례하므로 거리가 멀면 멀수록 방사선의 강도는 약해진다. 따라서 packing을 실시하여 방광과 직장의 장해를 최소화하는데 노력을 기울여야 할 것이다.
