Changes in the Prosthesis Types Used for Aortic Valve Replacement after the Introduction of Sutureless and Rapid Deployment Valves in Korea: A Nationwide Population-Based Cohort Study

  • Background: Sutureless and rapid deployment valves for aortic valve replacement (AVR) were introduced in Korea in December 2016. This study evaluated changing trends in the prosthetic valves used for AVR in Korea after the introduction of sutureless and rapid deployment valves. Methods: From December 2016 to December 2018, 4,899 patients underwent AVR in Korea. After applying the exclusion criteria, 4,872 patients were analyzed to determine changes in the type of prosthetic valve used for AVR. The study period was divided into 5 groups corresponding to 5-month intervals. Results: The total number of AVR cases was 194.88±28.78 per month during the study period. Mechanical valves were used in approximately 27% to 33% of cases, and the proportion of mechanical valve use showed a tendency to decrease, with marginal significance overall (p=0.078) and significant decreases in patients less than 60 years of age and in men (p=0.013 and p=0.023, respectively). The use of sutureless valves increased from 13.4% to 25.8% of cases (p<0.001), especially in elderly patients (>70 years) and those requiring concomitant surgery. In a comparison between sutureless and rapid deployment valves, the use of Perceval S valves (a type of sutureless valve), gradually increased (p<0.001). Conclusion: After the introduction of sutureless and rapid deployment valves in Korea, the rate of use of these new valves remarkably increased, especially in elderly patients and those requiring concomitant surgery. Further studies should investigate the clinical outcomes of these new prostheses.

The Pattern of Clothing Export in Leading Countries (세계 주요 의류수출국의 의류수출 패턴)

  • The objective of the study is to review trends of clothing export in important countries of the world and thus to know the present stage of clothing export in Korea. For the purpose, changes of export amount, competitiveness and product life cycle were analysed in the countries from the 1960's to the 1990's. The results are as following: First, on the aspect of export amount, industrialized countries lost the lead to newly industrializing countries from the 1960's. Developing countries, as China, win the lead. Second, on the competitiveness aspect, industrialized countries lost competitiveness to newly industrializing countries before the 1960's. And newly industrializing countries were outpaced by China in the export competitiveness from the early 1990's. Third, on the aspect of product life cycle, industrialized countries go first through each period of product life cycle, introduction, growth, maturity and decline, and they all, excluding Japan, sustained period of Maturity for a long time. newly industrializing countries and developing countries go orderly through each period of product life cycle after industrialized countries. As to Korea, duration of each period was short. Specially characteristics of decline period appeared partly in the early 1990's. From the results, the theory of product life cycle, helping to explain changes in production and trade in new product lines, estimated to applicable to clothing export. The phenomena, Italy, Hong Kong and America sustain high competitiveness in the world clothing market for a long time, is needed to be studied carefully. The study about strong points in their clothing industries and competitiveness drive program be to influence Korean export policy in the future.

Design Development of Fashion Cultural Products using the Sari Container of Baekje’s Sabi Period Temples (백제 사비시대 사찰의 사리장엄구를 활용한 패션문화상품 디자인 개발)

  • Buddhist culture had a significant impact on the entire mode of Korean living after the introduction of Buddhism to Korea in the Three Kingdom Period. Baekje embraced Buddhism in 384 A.D.; subsequently, diverse artifacts have now been excavated from the temples. Various research on Korean temples are now in progress; however there is inadequate research on the relics and patterns excavated from the temples due to the focus on the temples’ architectural form. There is limited research on the development of fashion cultural products that use relics excavated from the temples. This study develops designs for fashion cultural products using Baekje Sabi Period relics; specifically, the sari container excavated from Buyeo’s Wangheungsaji, Neungsanrisaji, and Iksan’s Mireuksaji. The sari container’s original form, patterns, and writing were developed into patterns and applied to fashion products such as t-shirts, bags and scarves. Traditional multicolored paintwork exhibited on the temples, ‘dancheong’, was selected as the color for products that can symbolically express the nature of their origin. Adobe Illustrator CC and Adobe Photoshop CC were used to extract the motifs and develop the designs. Six patterns and nine fashion products were designed, accounting for a total of fifteen developed items. We hope that the fashion cultural product design expresses the distinct characteristics of Baekje’s Sabi Period and can be applied to various products and related fields.

The Relationship of Stress and Infant Perception of Mothers in Postpartum Period (산욕기 어머니의 스트레스와 영아지각과의 관계)

  • Purpose: This study was done to investigate into the relationship between mother's stress in postpartum period and perception of infant in order to provide basic data for developing nursing intervention program. Methods: The data were collected from September 1, 2004 to October 15, from one women's hospital and three facilities for taking care of women's health after delivery located in B metropolitan city, and analyzed by SPSS/12.0 program frequency, percentage rate, mean, standard deviation, average, the maximum value, the minimum value and Person's correlation coefficient. Results: 1. The degree of stress of subjects was $75.6{\pm}38.3$(0-376) in average, and $.8{\pm}\;.4$(0-4) of average point, and the degree of perception of infant was $-5.8{\pm}5.6$(15-75) in average and $-\;.4{\pm}.4$(1-5) of average point. The relationship of degrees between the stress of subject and conception of infants that showed weak reverse relation(r= .210, p= .030). That is, the more the stress of subject was high, the more the perception of infant is relatively indefinite. Conclusion: As a result, The stress of mother in postpartum period was a level to be in need of management and the perception of infant was slightly negative. thus, the introduction of nursing assistance program is necessary to relive the stress of mother in postpartum period to promote the affirmative perception of infant.

  • The marketization reform from the open-door policy in 1978 was not only booming export-oriented industries with foreign investment but also expanding the role of private actors in the Chinese water sector. Private Sector Participation (PSP) has become an important element in developing urban infrastructure by providing better services with advanced facilities. The rapid development of PSP-driven urban water infrastructure in China has a positive impacted on Chinese economic development, particularly in coastal areas. PPPs in some coastal areas have successfully spread out over China since China applied the first Build-Operate-Transfer (BOT) mode in the water sector in the early 1990s. The market-oriented water and wastewater, Public-Private Partnership (PPP) mechanism in the initial period of China has been transformed into a state-dominated PPP mechanism. The development pattern of the water and wastewater PPPs in China has been divided in four stages: the first period from 1984 to 2002, the second period from 2003 to 2008, the third period from 2009 to 2014, and the last period after 2015. The study aims to investigate the successful process of water and wastewater PPPs in local areas through five socioeconomic elements: export-oriented economic strategy, urbanization, cheap land policy, infrastructure investment, and water issues and climate change. In addition, the study focuses on analyzing the extent to which the Chinese government re-asserted its control over the PPP mechanism by classifying five elements in three different development Phases from early 2000 to 2020. The Jiangsu Province in the estern coastal area has actively invited PPP projects in the water and wastewater sectors. The successful introduction and rapid growth of PPPs in the urban water infrastructure has made the province an attractive area for a foreign investor.

Gender Inequalities in Mental Health During the COVID-19 Pandemic: A Population-based Study in Korea

  • Objectives: This study explored the effect of the coronavirus disease 2019 (COVID-19) pandemic on psychosocial stress in prime working-age individuals in Korea, focusing on gender inequalities. We hypothesized that the impact of COVID-19 on mental health would differ by age and gender, with younger women potentially demonstrating heightened vulnerability relative to men. Methods: The study involved data from the Korea Community Health Survey and included 319 592 adults aged 30 years to 49 years. We employed log-binomial regression analysis, controlling for variables including age, education, employment status, marital status, and the presence of children. The study period included 3 phases: the period prior to the COVID-19 outbreak (pre-COVID-19), the early pandemic, and the period following the introduction of vaccinations (post-vaccination). Results: The findings indicated that women were at a heightened risk of psychosocial stress during the early pandemic (relative risk [RR], 1.01; 95% confidence interval [CI], 0.98 to 1.05) and post-vaccination period (RR, 1.07; 95% CI, 1.04 to 1.10) compared to men. This pattern was prominent in urban women aged 30-34 years (pre-COVID-19: RR, 1.06; 95% CI, 1.02 to 1.10; early pandemic: RR, 1.16; 95% CI, 1.08 to 1.25; post-vaccination period, RR, 1.22; 95% CI, 1.14 to 1.31). Conclusions: The COVID-19 pandemic has exerted unequal impacts on psychosocial stress among prime working-age individuals in Korea, with women, particularly those in urban areas, experiencing a heightened risk. The findings highlight the importance of addressing gender-specific needs and implementing appropriate interventions to mitigate the psychosocial consequences of the pandemic.

The Epidemiological Trend of Rotavirus Gastroenteritis in Children in a Single Center from 2004 to 2012: A Retrospective Study (2004-2012년 소아 로타바이러스 위장관염의 역학적 추세에 관한 단일 기관 연구)

  • Purpose: This study was performed to investigate the epidemiological trend of rotavirus acute gastroenteritis (RV-AGE) in children. Methods: A retrospective review was performed in patients (1 month to 18 years of age) with acute gastroenteritis at KEPCO Medical Center from September 2004 to August 2013. Comparative analyses were performed based on periods: pre-vaccine (2004-2006) and post-vaccine (2008-2012) in all patients; 2004-2006 (period A), 2007-2009 (period B) and 2010-2012 (period C) in patients under 5 years of age. Results: Proportion of RV-AGE decreased from 25.0% (337/1,346) in pre-vaccine period to 20.8% (459/2,210) in post-vaccine period (rate ratio (RR), 0.83 [95% CI, 0.73-0.93]; P=0.0029). The median age of patients with RV-AGE in post-vaccine period (2.6 years) was significantly (P<0.0001) higher than that in pre-vaccine period (1.6 years). In patients hospitalized with AGE, proportion of RV-AGE was significantly reduced in patients 6 to 23 months old (RR, 0.62 [95% CI, 0.51-0.75]; P<0.0001). Significant decline in proportion of RV-AGE was observed in patients under 5 years of age: period A, 26.9% (308/1,144); period B, 22.7% (295/1,299); period C, 20.6% (186/902) (P =0.0007). After the introduction of rotavirus vaccine, a significant decreasing trend of RV-AGE proportion was observed in patients 6 to 11 months old (P =0.0018) and 12 to 23 months old (P =0.0152). Conclusion: Decrease in RV-AGE proportion and increase in age of patients with RV-AGE were observed after the introduction of rotavirus vaccine in this single center study. Continued and systematic surveillance is needed to assess the impact of rotavirus vaccine.

The Effect of Different Periods of Seed Storage on Germination of Genus Lespedeza (Lespedeza속(屬)의 종자저장기간별(種子貯藏期間別) 발아시험(發芽試驗))

  • In order to guide the project for tree seedling production and direct seeding afforestation by introduction of genus Lespedeza, studies were conducted on the effect of seed storage period and on percentage and rate of germination through various kinds of treatments. L. bicolor, L. cyrtobotrya, and L. japonica. intermedia seed were storeds at different period of time. The results obtained are summarized as follows: 1. Germination capacity was not reduced until 28 months after harvest. However, germination was very poor 40 months after harvest, with less than 30% germination obtained by germination treatments. 2. Removal of seed coat and cone, sulphuric acid treatments were more effective in increasing germination than other treatments. Especially, most seeds germinated one week earlier than other treatments by removal of the seed coat. 3. The seeds stored for a short period of time showed more rapid germination compared to seeds which were stored for long period.

The clinical study of oral care for early radiation therapy in the head and neck cancer patients (두경부 악성 종양환자에서 조기 방사선치료를 위한 구강관리법에 대한 임상적 연구)

  • Introduction: Tooth requiring extraction before radiotherapy in head and neck cancer patients should be performed as long as possible before the initiation of radiation therapy. Conventionally, a minimum 2-week waiting primary healing period is recommended. Although the above 2-week period is ideal, it is not uncommon for the radiotherapist and cancer patient to feel an urgent need to proceed with radiotherapy despite the need for dental care. Therefore, alternative approaches for early radiotherapy, including conservative endodontic treatment and a 1-week waiting primary healing period after dental extraction at the time of radiotherapy were considered and applied based on a literature review Materials and Methods: The clinical study involved 120 head and neck cancer patients who were treated at Wonju Christian Hospital, Wonju College of Medicine, Yonsei University, from January 1995 to December 2004. Results: In the clinical study, there were no specific complications, such as, post-extraction wound infections, radiation osteitis and osteoradionecrosis over the recent 10 years despite the early radiotherapy. Conclusion: Based on the clinical study, a minimum 1-week waiting primary healing period for oral care before radiotherapy is suitable for early radiotherapy in head and neck cancer patients.

The Influences of Health Insurance on the Contents of Medical Services for Selected Hospitalized Patients (의료보험 실시가 입원환자의 진료내용에 미치는 영향 -한 병원의 정상분만산모와 충수절제술환자를 통한 사례연구-)

  • This study was written to discover the changes that may exist in the contents of medical services after introduction of health insurance system, and to identify the net-effect of health insurance system on medical services. Uncomplicated nornmal delivery and appendectomy patients were divided into 4 groups, the non-insured in pre-NHI periods(group A), the insured of health insurance for employees in pre-NHI periods(group B), the insured of regional health insurance for city residents in post-NHI periods(group C) and the insured of health insurance for employees in post-NHI periods(group D). The mehtod of matching was applied to control for major demographic differences among these 4 groups of each disease. In pre-NHI period, the medical services and the variation of medical services of the non-insured were compared with those of the insured. The difference between the change of medical services from group A to those of group C, and the change of medical services from group B to those group D is defined as the net-effect of health insurance. The results are as follows. First, in length of stay after delivery or operation, total length of stay, some laboratory examination, amount of several drugs used in appendectomy patients, frequency of sitz bath in delivery patients, there was net-effect of health insurance in increasing direction. Second, length of stay after delivery or operation, total length of stay, some laboratory examination, amount of several drugs used in appendectomy patients and frequency of sitz bath in delivery patients were significantly more in the insured than in the non-insured group in pre-NHI period. Third, the variation of medical services of post-NHI period was not less then those of pre-NHI period. Fourth, antenatal care on which the third party does not pay and the patient pays for all, was diffrerent by socioeconomic and educational level of patients.
