The Effect of the Lower Trapezius Strengthening Exercise using PNF on Pain, Range of Motion, and Disability in Patients with Frozen Shoulder (PNF를 이용한 아래등세모근 강화 운동이 동결견 환자의 통증, 관절가동범위 및 장애에 미치는 영향)

  • Purpose: The study aimed to determine the effect of the proprioceptive neuromuscular facilitation (PNF) lower trapezius muscle strengthening exercise on pain, shoulder range of motion, and shoulder pain and disability index (SPADI) in patients with frozen shoulder. Methods: Following baseline measurements, 30 subjects (n=30) with frozen shoulder were randomized into two groups: the PNF group (n=15), which received PNF strength training of the lower trapezius muscles, and the control group (n=15), which received gentle palpation of the skin. Each group participated in the intervention for 30 minutes, three times per week, for six weeks. The visual analogue scales for pain, range of motion, and SPADI of both groups were recorded at both pre- and post-intervention. Paired t-tests were used to determine significant changes in the post-intervention period compared with pre-intervention, and independent t-tests were used to analyze differences in the dependent variables between the two groups. Results: After the six-week intervention, both groups experienced significantly decreased pain and SPADI (p < 0.05) and significantly increased shoulder flexion, abduction, internal rotation, and external rotation range of motion (p < 0.05). The PNF group that received the PNF strength exercise of the lower trapezius muscles showed greater improvements in pain and range of motion than those of the control group (p < 0.05). Conclusion: These results suggest that the PNF lower trapezius strengthening exercise reduces shoulder pain and disability levels and enhances shoulder range of motion in patients with frozen shoulder.

Microstructure and Strength Characteristic of Hydropobic Cement Mortar with Silan Admixture (실란계 혼화제가 혼입된 소수성 시멘트 모르타르의 미세구조 및 강도특성)

  • A hydrophobic emulsion consisting of PMHS and PVA was mixed into a cement mortar to observe changes in cement hydrate and microstructure, and to experimentally evaluate compressive strength and flexural strength. The hydrophobic emulsion was added with metakaolin and PVA fibers, and the stirring speed and sequence were adjusted to prepare a shell-concept hydrophobic emulsion. It was then mixed when mixing mortar to enhance filling of the internal pores and change of the hydrates. It was observed that the mortar mixed with a hydrophobic emulsion was filled with micropores and a coating film was formed on the surface of the hydrates by the emulsion. It was analyzed that the total pore area and porosity of the mortar mixed with the emulsion decreased from 30% to 60% compared to OPC, excluding the 50MK variable, which was extremely reduced and the median pore diameter decreased in some variables. It was also found that the compressive strength of the mortar mixed with emulsion 1% was increased up to 20%, but the strength of the mortar specimen mixed with 2% decreased to 50%.

The Changing Epidemiology of Gastroesophageal Reflux Disease: Are Patients Getting Younger?

  • Background/Aims Gastroesophageal reflux disease (GERD) is a common disease globally with increasing prevalence and consequently greater burden on the Healthcare system. Traditionally, GERD has been considered a disease of middle-aged and older people. Since risk factors for GERD affect a growing number of the adult population, concerns have been raised that increasingly younger people may develop GERD. We aim to determine if the proportion of younger patients has increased among the GERD population. Methods The incidence of GERD as well as several variables were evaluated during an 11-year period. Explorys was used to evaluate datasets at a "Universal" and Healthcare system in northern Ohio to determine if trends at a local level reflected those at a universal level. GERD patients were classified into 7 age groups (15-19, 20-29, 30-39, 40-49, 50-59, 60-69, and ${\geq}70$ years). Results The proportion of patients with GERD increased in all age groups, except for those who were ${\geq}70$ years in the universal dataset (P < 0.001) and those who were ${\geq}60$ years in the Healthcare system (P < 0.001). The greatest rise was seen in 30-39 years in both datasets (P < 0.001). Similarly, the proportion of GERD patients who were using proton pump inhibitors increased in all age groups except for those who were ${\geq}70$ years in both datasets (P < 0.001), with the greatest increase being the group 30-39 years (P < 0.001). Conclusion Over the last decade, there has been a significant increase in the proportion of younger patients with GERD, especially those within the age range of 30-39 years.

Validity and Reliability of the Korean Version of the Partners In Health Scale (PIH-K) (한국어판 자기관리 측정도구(Partners In Health scale)의 타당도 및 신뢰도 분석)

  • Purpose : The purpose of this study was to validate the Korean version of Partners In Health scale (PIH-K) which is used to measure the self-management of patients with chronic illnesses in Korea. Methods : Translation of the 12-item PIH-K was conducted according to the World Health Organization guidelines. Data from 306 participants who took medicines over 3 months by doctor's prescription were collected from October to November 2017. Validity such as content validity, construct validity, and concurrent validity were conducted using content validity index (CVI), exploratory and confirmatory factor analyses (CFA). To evaluate concurrent validity, the correlation coefficients between the PIH-K and concurrent scales (Self-As-Carer Inventory) were calculated. The reliability of the PIH-K was examined using the internal consistency and test-retest reliability tests. Results : The CVI of the PIH-K was 0.91. According to the CFA, factor loadings for four factors ranged from .64 to .97, which explained 67.5% of the total variance. The PIH-K was significantly correlated with concurrent variables such as those on the Self-As-Carer Inventory. The Cronbach's ${\alpha}$ was .86 and the intraclass correlation coefficient for the two-week test-retest reliability was .88. Conclusion : Findings show that the PIH-K is reliable and valid in measuring self-management of patients with chronic illnesses.

A Dualistic Development in Korean Industrialization (한국 산업화의 이중구조)

  • Among the hypotheses regarding the internal process of industrialization, the debates about 'labor-surplus' model have been intensive. The basic idea of this neoclassical theory is that industrial development is brought about by the transfer of the unlimited cheap labor to the modern sector, and thus, under the labor-surplus situation labor-using technologies should be used for industrialization. Fei and Rannis attempted to confirm this theory by applying it to the Japanese economy. The purpose of this paper is to study whether the theory can be applied to Korean economic development. The neoclassical dualistic model which was designed by Kelly and Williamson was utilized. Simulating Korea's major economic variables for the period of 1965-1992 by using computable general equilibrium (CGE) model, we found that there are significant differentials between the simulation and the actual data. It suggests that Korea's economic development has not followed the neoclassical path -- creation of comparative advantage on the basis of market force.

Scale Development of Wisdom among Korean Elderly ('한국노인의 지혜척도' 개발에 관한 연구)

  • Purpose: The purpose of this study was to develop a wisdom scale among Korean elderly with high validity and reliability. Method: The process of the development of this scale follows: A conceptual framework was identified based on literature a review of the elderly and discussion with experts in physiology and sociology. Total 128 items were developed. Through reliability testing, factor analysis, 60 preliminary items, and 4-points scale were selected. By a means of the internal consistency for 60 items, 33 items were deleted. Finally, 27 items were remained. To verity 27 items, factor analysis, reliability testing, and correlation were done. Data were collected from 480 elderly subjects in Daegu, Kyungpook Province, and Busan, Kyungnam Province from August, 2008 to February, 2009. Results: From the results of the factor analysis for 27 items, 3 factors were extracted. These factors were labeled as 'empathic emotion', 'self-reflection', and 'bitter experience of life'. Cronbach's Alpha for the 27 items was .921 and correlation coefficient of ego-integrity was .69. Conclusion: An exploratory study of the variables related to wisdom among the elderly is needed for criterion validity of this scale. A study on wisdom for different age groups is needed for re-verification.

Risk Factors of the Masticatory Function in Patients with Temporomandibular Disorders: A Cross-Sectional Cohort Study

  • Purpose: To investigate the masticatory function of patients with different temporomandibular disorders (TMD) phenotypes, and to explore the risk factors for the masticatory function of TMD patients among multiple biopsychosocial variables using patient-reported outcomes (PROs). Methods: Clinical features and TMD diagnoses of 250 cases were investigated by reviewing medical records. Psychosocial factors were evaluated using four questionnaires representing pain severity and pain interference (Brief Pain Inventory), pain catastrophizing (Pain Catastrophizing Scale, PCS), psychological distress (Symptom Check List-90-Revised, SCL-90R) and kinesiophobia (Tampa Scale for Kinesiophobia for Temporomandibular Disorders, TSK-TMD). Masticatory function, as a dependent variable, was determined using the Jaw Functional Limitation Scale (JFLS). Kruskal-Wallis test and Spearman's rank correlation were used for analyses. Results: A total of 145 cases were included and classified into four subgroups including group 1: TMD with internal derangement without pain (n=14), group 2: TMD with muscle pain (n=32), group 3: TMD with joint pain (n=60) and group 4: TMD with muscle-joint combined pain (n=39). Pain severity (p=0.001) and interference (p=0.022) were the highest in group 2, but the mean global score of JFLS was the highest in group 3, followed by group 4, group 2, and group 1 (p=0.013). Pain severity, pain interference, the mean global score of PCS and the mean global score of TSK-TMD showed significant and moderate correlation with the mean global score of JFLS. All subdimensions and the global severity index of SCL-90R had significant, but weak correlations with all scores of JFLS. Conclusions: The results suggest that masticatory functional limitation depends on the TMD phenotypes. Among the various PROs, pain perception, pain catastrophizing and kinesiophobia seem to be more influential risk factors on jaw function than psychological distress, such as depression and anxiety.

Effects of Straight Leg Lifts and Double Leg Lowering Exercise on Abdominal Muscle Activity, Back Pain, and Flexibility in Patients with Chronic Low Back Pain in their 50s (50대 만성허리통증 환자들을 대상으로 다리들기와 다리내리기 운동이 배 근육의 활성도, 허리통증, 그리고 유연성에 미치는 영향)

  • Purpose : The purpose of this study was to investigate the effects of Straight leg lifts (SLL) and double leg lowering (DLL) exercise on abdominal muscle activity, visual analog scale (VAS), and flexibility in patients with chronic low back pain (LBP). Methods : A total of 30 LBP patients were divided into two groups: those with SLL exercise group 15 (male=8, female=7) and those with DLL exercise group 15 (male=7, female=8). Before the intervention, the abdominal muscle activity, VAS, and flexibility were measured. After 4 weeks of intervention, the above variables were measured in the same way. The SLL exercise bends the leg $45^{\circ}$ in the supine position, and the DLL exercise was performed as opposed to SLL. At this time, the pressure biofeedback unit (PBU) was placed behind the lumbar to reduce the instability of the pelvis and muscles. The subjects were instructed to use the PBU to maintain the target pressure determined (40 mmHg) during the exercise. Results : The external oblique (EO), internal oblique (IO), and transverse abdominis (TrA) were significantly different in the SLL and DLL group, and EO, IO, and TrA activity improved more significantly increased in the DLL than SLL group (p<.05). The results on the VAS and flexibility were significantly different both group (p<.05). However, there was no significant difference between the groups (p>.05). Conclusion : SLL and DLL exercises in patients with LBP were able to confirm the increased activity of the abdominal muscles, decreased pain, and increased flexibility of the waist. In addition, DLL exercise is more effective in patients with LBP in terms of muscle activity.

Analysis of Relation between Safety Needs and Life Satisfaction of Security Safety System Users (경비안전시스템 이용자들의 안전욕구와 생활만족에 대한 관계 분석)

  • The purpose of this study is to analyze the safety needs and life satisfaction of users of security system. The contents of the questionnaire consisted of "safety need, use satisfaction, life satisfaction". As a result of the analysis, Chronbach's alpha coefficient was calculated as 0.897 for safety needs, 0.888 for use satisfaction, and 0.887 for life satisfaction. The items were found to have internal consistency. In the validation of discriminant validity, both AVE and CR values were found to be 0.6 or more than the reference value of 0.8. The intuitiveness of the variables used in this study was found to be largely secured. As the hypothesis test result, the standardization factor was calculated as 0.993 for 'safety needs ⇨ use satisfaction' and 0.453 for 'use satisfaction ⇨ life satisfaction', and both hypotheses were positive. As a result of the verification, it was analyzed that the higher the user's safety needs, the higher the use satisfaction, and the higher the use satisfaction, the higher the life satisfaction.

A Multilayer Perceptron-Based Electric Load Forecasting Scheme via Effective Recovering Missing Data (효과적인 결측치 보완을 통한 다층 퍼셉트론 기반의 전력수요 예측 기법)

  • Accurate electric load forecasting is very important in the efficient operation of the smart grid. Recently, due to the development of IT technology, many works for constructing accurate forecasting models have been developed based on big data processing using artificial intelligence techniques. These forecasting models usually utilize external factors such as temperature, humidity and historical electric load as independent variables. However, due to diverse internal and external factors, historical electrical load contains many missing data, which makes it very difficult to construct an accurate forecasting model. To solve this problem, in this paper, we propose a random forest-based missing data recovery scheme and construct an electric load forecasting model based on multilayer perceptron using the estimated values of missing data and external factors. We demonstrate the performance of our proposed scheme via various experiments.
