혈부축어탕 스트레스에 미치는 영향 (Effects of Hyeolbuchukyeo-tang on Stress)

  • Backgrounds : Two stress pathways, the hypothalamic-pituitary-adrenal (HPA) axis and the sympathetic nervous system, regulate immune system responses through release of corticosteroids, norepinephrine and epinephrine. respectively. These neuromediators act on immune cells via specific receptors on their surface to modulate the production of key regulatory cytokines. Objectives : To evaluate the preventive effects of oriental medicine Hyeolbuchukyeo-tang (HC) on stress. Methods : Mice were divided into three groups: nounal group, control group under immobilization stress and HC group which received Hyeolbuchukyeo-tang (HC) under immobilization stress. following sacrifice, their splenocytes were isolated and splenocyte surface markers were determined. The brains were removed and mRNA determined. In vivo, we separated RNA Iron cultured macrophages (RAW264.7). Results : In our study, immune functions were decreased in stress due mainly to changes of various neuromediators, cytokines and macrophage activities, and the treatment of HC increased those expressions. Conclusions : In summary, the present study documents the anti-stress effects of HC through stress-immune regulation.

만성(慢性) 간(肝) 질환(疾患)에서 경구적(經口的) 당부하(糖負荷) 시험시(試驗時) 혈당(血糖) 및 혈장(血漿) 인슐린의 변동(變動)에 관(關)한 연구(硏究) (A Study on the Glucose and Immunoreactive Insulin Response during Oral Glucose Tolerance Test in Patients with Chronic Liver Diseases)

  • The blood glucose and plasma immunoreactive insulin(IRI) levels were measured glucose tolerance test in 7 healthy subjects and 6 patients with chronic liver diseases. The glucose tolerance was impaired in 5 of the 6 patients and normal in 1. Plasma IRI responses were markedly increased and delayed in all patients, suggesting insulin resistance. Patients with more glucose intolerance showed less increase in plasma IRI than the group with less intolerance. It is suggested that some insulin antagonists may decrease the peripheral insulin sensitivity and stimulate compensatory hyperactivity of pancreatic islets. If the compensatory hyperactivity is inadequate due to genetic predisposition to diabetes mellitus or exhaustion of ${\beta}$-cells of pancreatic islsts, the glucose intolerance and overt diabetes mellitus may ensue.

Study on rock fracture behavior under hydromechanical loading by 3-D digital reconstruction

  • The coupled hydro-mechanical loading conditions commonly occur in the geothermal and petroleum engineering projects, which is significantly important influence on the stability of rock masses. In this article, the influence of flaw inclination angle of fracture behaviors in rock-like materials subjected to both mechanical loads and internal hydraulic pressures is experimentally studied using the 3-D X-ray computed tomography combined with 3-D reconstruction techniques. Triaxial compression experiments under confining pressure of 8.0 MPa are first conducted for intact rock-like specimens using a rock mechanics testing system. Four pre-flawed rock-like specimens containing a single open flaw with different inclination angle under the coupled hydro-mechanical loading conditions are carried out. Then, the broken pre-flawed rock-like specimens are analyzed using a 3-D X-ray computed tomography (CT) scanning system. Subsequently, the internal damage behaviors of failed pre-flawed rock-like specimens are evaluated by the 3-D reconstruction techniques, according to the horizontal and vertical cross-sectional CT images. The present experimental does not only focus on the mechanical responses, but also pays attentions to the internal fracture characteristics of rock-like materials under the coupled hydro-mechanical loading conditions. The conclusion remarks are significant for predicting the rock instability in geothermal and unconventional petroleum engineering.

Bilateral Congenital Deafness in a White Bull Terrier; Brainstem Auditory Evoked Response Findings

  • A 2-month-old, intact female white Bull Terrier presented because of suspected deafness. The coat color was predominantly white and the iris color, of both eyes, was brown. The dog did not respond to the owner's voice when the sound stimuli were presented outside of the visual field; however, the dog responded to visual gestures. The other physical, neurological, otoscopic, radiographic, and blood examinations were unremarkable. To assess the apparent deafness, brainstem auditory evoked responses (BAER) were recorded and analyzed in the dog with suspected deafness as well as a normal littermate. The response in the normal littermate consisted of a series of five wave peaks (I-V) with decreased amplitude and prolonged latency as the stimulus intensity decreased. The BAER from the dog suspected of deafness appeared as a flat line and did not reveal identifiable peaks that corresponded to those found in the normal littermate. Thus, congenital, sensorineural and bilateral deafness was confirmed by the BAER.

Analytic responses of slender beams supported by rotationally restrained hinges during support motions

  • This paper presents an analytic solution procedure of the rotationally restrained hinged-hinged beam subjected to transverse motions at supports based on EBT (Euler-Bernoulli beam theory). The EBT solutions are compared with the solutions based on TBT (Timoshenko beam theory) for a wide range of the rotational restraint parameter (kL/EI) of slender beams whose slenderness ratio is greater than 100. The comparison shows the followings. The internal loads such as bending moment and shearing force of an extremely thin beam obtained by EBT show a good agreement with those obtained by TBT. But the discrepancy between two solutions of internal loads tends to increase as the slenderness ratio decreases. A careful examination shows that the discrepancy of the internal loads originates from their dynamic components whereas their static components show a little difference between EBT and TBT. This result suggests that TBT should be employed even for slender beams to consider the rotational effect and the shear deformation effect on dynamic components of the internal loads. The influence of the parameter on boundary conditions is examined by manipulating the spring stiffness from zero to a sufficiently large value.

Measured structural response of a long irregular pit constructed using a top-down method

  • A 1257-m-long irregular deep foundation pit located in the central of Nanjing, China was constructed using the combined full-width and half-width top-down method. Based on the long-term field monitoring data, this study analyzed the evolution characteristics of the vertical movement of the columns, internal force of the struts, and axial force of the structural beam and slab. The relevance of the three mentioned above and their relationship with the excavation process, structural system, and geological conditions were also investigated. The results showed that the column uplift was within the range of 0.08% to 0.22% of the excavation depth, and the embedded depth ratio of the diaphragm wall and the bottom heave affected significantly on the column uplift. The differential settlement between the column and diaphragm wall remained unchanged after the base slab was cast. The final settlement of the diaphragm wall was twice the column uplift. The internal force of the struts did not varied monotonically but was related to numerous factors such as the excavation depth, number of struts, and environmental conditions. Additionally, the dynamic force and deformation of the columns, beams, and slabs were analyzed to investigate the inherent relationship and variation patterns of the responses of different parts of the structure.

초음파 영상에서 급성 위점막 병변으로 진단된 환자에 대한 한의치료 치험 1례 (A Case Report on Korean Medicine Treatment for a Patient Diagnosed with Acute Gastric Mucosal Lesion on Ultrasound Imaging)

  • Objectives: This study reports a clinical case that showed improvement in the symptoms of a patient with acute gastric mucosal lesion (AGML) through ultrasound. Methods: A 90-year-old woman with AGML was treated with Korean medicine treatments, including Hyangsapyeongwisan Soft Ext, acupuncture, and moxibustion. The responses before and after treatment were evaluated using ultrasound imaging and the numerical rating scale (NRS). Results: After treatment, ultrasound imaging showed improvement in gastric mucosa thickness, from 1.45 cm to 0.53 cm at the anterior wall and from 1.25 cm to 0.36 cm at the posterior wall. The NRS scores for epigastric pain, nausea, and dizziness all decreased, and the symptoms of diarrhea disappeared. Conclusion: This study suggests that Korean medicine treatment can be a therapeutic option in treating the symptoms of patients with AGML.

알레르기성 천식환자에 있어서 투베르쿨린 피부반응 검사와 Cytokine의 변화 (Tuberculin Skin Test and Change of Cytokines in Patients with Allergic Asthma)

  • 연구배경: 알레르기성 기관지 천식은 가역적인 기도폐색과 기관지의 과민성을 동반하는 기도의 만성적인 호산구성 염증성 질환으로서 기도의 염증발현에 관여하는 세포는 여러가지가 있지만 Th 림프구는 cytokine을 분비하여 염증반응을 조절하는 중요한 역할을 하고 있다. Th 림프구는 cytokine의 분비양상에 따라 Th1 림프구와 Th2 림프구로 나뉘어지며 Th1 림프구는 지연형 과민반응과 결핵균이나 바이러스 감염등에 대한 방어작용 그리고 종양에 대한 숙주반응에 관여하고 Th2 림프구는 즉시형 과민반응과 알레르기성 천식과 같은 알레르기성 질환 그리고 기생충 감염등에 대한 방어작용에 관여한다. Th1 림프구와 Th2 림프구는 서로 길항작용을 하는 것으로 알려지고 있어 대표적인 Th1 림프구 매개 질환인 결핵과 Th2 림프구 매개질환인 알레르기성 기관지 천식은 서로의 발생을 억제할 것으로 추정되며 알레르기성 기관지 천식환자에서는 Th2 림프구의 기능이 항진되어 Th1 림프구의 기능이 억제되고 반대로 Th1 림프구의 기능이 정상인 집단에서는 알레르기성 천식환자에 비해 Th2 림프구의 기능이 저하되어 있을 것으로 추정된다. 방 법: 정상 대조군과 알레르기성 천식환자군, 그리고 내인성 천식환자군에서 투베르쿨린 피부반응의 양상을 실시하여 지연형 과민반응의 양상을 관찰하고 혈청 IgE의 농도와 말초혈액 호산구 수를 조사하였다. 그리고 Th1 림프구에서 주로 생산되는 cytokine인 IFN-$\gamma$와 IL-12, Th2 림프구에서 주로 생산되는 IL-4, IL-5, IL-10의 혈청 농도를 측정하였다. 결 과: 투베르쿨린 피부반응의 양성율은 알레르기성 천식환자군이 내인성 천식환자군에 비해 투베르쿨린 피부반응에 대한 양성율이 유의하게 낮았으며(p<0.05), 투베르쿨린 피부반응의 정도는 내인성 천식환자군에 비하여 알레르기성 천식환자에서 유의하게 감소되어 있었다 (p<0.05). IL-4와 IL-10은 알레르기성 천식환자군에서 다른 두 군에 비하여 통계적으로 유의하지 않았으나 증가되어 있었고 IL-5는 알레르기성 천식환자군에서 다른 두 군에 비하여 유의하게 증가되어 있었다 (p<0.01). IL-12와 IFN-$\gamma$는 알레르기성 천식환자군과 내인성 천식환자군에서 정상 대조군에 비하여 유의하게 감소되어 있었다(p<0.05). 알레르기성 천식환자군에서 말초 혈액 호산구 수와 혈청 IgE 농도는 정상 대조군에 비하여 유의하게 증가하여 있었다(p<0.05). 알레르기성 천식환자에서 말초혈액 호산구 수는 혈청 IgE(r=0.737, p=0.003), IL-5(r=0.352, p=0.038), IL-10(r=0.827, p=0.001)과 서로 유의한 상관관계를 보이면서 증가하고 있었다. 전체적으로 Th2 cytokine인 IL-5와 IL-10의 혈청 농도는 서로 유의한 상관관계를 나타내고 있었고(r=0.340, p=0.046), IL-12와 IFN-$\gamma$ 혈청 농도도 서로 유의한 상관관계를 나타내고 있었다(r=0.593, p=0.001). 결 론: 알레르기성 기관지 천식환자는 정상 대조군에 비하여 Th1 림프구의 기능이 저하되어 있었고 Th2 림프구의 기능은 항진되어 있었으며, 이러한 Th2 림프구의 기능 항진은 말초혈액 호산구 수와 혈청 IgE와 유의한 상관관계를 보이고 있어 Th2 림프구 기능 항진이 알레르기성 기관지 천식의 병인에 중요한 역할을 할 가능성이 있음을 알 수 있다.

기능성 소화불량증 환자에서 헬리코박터 파일로리 제균 치료 효과 및 관련 요인: 국내 전향적, 다기관 연구 (Female Gender is a Poor Predictive Factor of Functional Dyspepsia Resolution after Helicobacter pylori Eradication: A Prospective, Multi-center Korean Trial)

  • Background/Aims: The predictive factors of functional dyspepsia (FD) remain controversial. Therefore, we sought to investigate symptom responses in FD patients after Helicobacter pylori (H. pylori) eradication and used predictive factor analysis to identify significant factors of FD resolution at one-year after commencing eradication therapy. Methods: This prospective, multi-center clinical trial was performed on 65 FD patients that met Rome III criteria and had H. pylori infection. Symptom responses and factors that predicted poor response were determined by analysis one year after commencing H. pylori eradication therapy. Results: A total of 63 patients completed the one-year follow-up. When an eradication success group (n=60) and an eradication failure group (n=3) were compared with respect to FD response rate at one year, results were as follows; complete response 73.3% and 0.0%, satisfactory response 1.7% and 0.0%, partial response 10.0% and 33.3%, and refractory response 15.0% and 66.7%, respectively (p=0.013). Univariate analysis showed persistent H. pylori infection (p=0.021), female gender (p=0.025), and medication for FD during the study period (p=0.013) were associated with poor FD response at one year. However, age, smoking, alcohol consumption, and underlying disease were not found to affect response. Finally, multivariate analysis showed that female gender (OR, 4.70; 95% CI, 1.17-18.88) was the sole independent risk factor of poor FD response at one year after commencing H. pylori eradication therapy. Conclusions: Female gender was found to predict poor response in FD patients despite H. pylori eradication. Furthermore, successful H. pylori eradication appears to be associated with FD improvement, but the number of non-eradicated patients was too small to conclude.

Influence of Bornyl Acetate on Blood Pressure and Aortic Strips Contractility of the Rat

  • The present study was conducted to investigate the effects of bornyl acetate on arterial blood pressure and vascular contractile responses in the normotensive rats and to establish the mechanism of action. Both phenylephrine (an adrenergi$\alpha$-receptor agonist) and high potassium (a membrane-depolarizing agent) caused greatly contractile responses in the isolated aortic strips. These phenylephrine (10$^{-5}$ M)-induced contractile responses were depressed in the presence of high concentrations of bornyl acetate (10∼20 $\mu\textrm{g}$/ml), but not affected in low concentrations of bornyl acetate (2.5∼5$\mu\textrm{g}$/ml). High potassium (5.6 ${\times}$ 10$^{-2}$ M)-induced contractile responses were also greatly inhibited in the presence of bornyl acetate (2.5∼20 $\mu\textrm{g}$/ml) in a dose-dependent fashion. Bornyl acetate (1∼10 mg/kg) given into a femoral vein of the normotensive rat produced a dose-dependent depressor response, which is transient (data not shown). Interestingly, the infusion of a moderate dose of bornyl acetate (3mg/kg/30 min) made a significant reduction in pressor responses induced by intravenous norepinephrine. Collectively, these results obtained from the present study demonstrate that intravenous bornyl acetate causes a dose-dependent depressor action in the anesthetized rat at least partly through the blockade of adrenergic $\alpha$$_1$-receptors. bornyl acetate also causes vascular relaxation in the isolated aortic strips of the rat via the blockade of adrenergic $\alpha$$_1$-receptors, in addition to the unknown mechanism of direct vasorelaxation.
