Objectives : The purpose of this in vitro study was to evaluate the effect of firing cycles on the marginal and internal fit of metal ceramic crown. Methods : Ten same cases of stone models (abutment teeth 11) were manufactured. Ten cobalt-chrome cores were made per each models and the marginal and internal fit was evaluated through a silicone replica technique. The marginal and internal fit of specimens was measured twice. The first measurement was done after manufacturing cobalt-chrome alloy core and the second measurement was done after porcelain firing. T-test of paired sample for statistical analysis was executed with SPSS 12.0K for Windows (${\alpha}$=0.05). Results : Mean(SD) marginal and internal fit were 77.1(23.3) ${\mu}m$ for the cobalt-chrome alloy core group and 84.4(21.9) ${\mu}m$ for the metal-ceramic crown group. They were statistically significant differences between groups for marginal and internal fit (p<.05). Conclusions : All metal ceramic crowns showed marginal and internal fit ranged within the current clinical recommendations.
PURPOSE. To evaluate the impact of five different tooth preparation designs on the marginal and internal fit discrepancies of cobalt-chromium (CoCr) crowns produced by computer-aided designing (CAD) and selective laser melting (SLM) processes. MATERIALS AND METHODS. Five preparation data were constructed, after which design crowns were obtained. Actual crowns were fabricated using an SLM process. After the data of actual crowns were obtained with structural light scanning, intaglio surfaces of the design crown and actual crown were virtually superimposed on the preparation. The fit-discrepancies were displayed with colors, while the root means square was calculated and analyzed with one-way analysis of variance (ANOVA), Tukey's test or Kruskal-Wallis test (α = .05). RESULTS. The marginal or internal color-coded images in the five design groups were not identical. The shoulder-lip and sharp line angle groups in the CAD or SLM process had larger marginal or internal fit discrepancies compared to other groups (P < .05). In the CAD process, the mean marginal and internal fit discrepancies were 10.0 to 24.2 ㎛ and 29.6 to 31.4 ㎛, respectively. After the CAD and SLM processes, the mean marginal and internal fit discrepancies were 18.4 to 40.9 ㎛ and 39.1 to 47.1 ㎛, respectively. The SLM process itself resulted in a positive increase of the marginal (6.0 - 16.7 ㎛) and internal (9.0 - 15.7 ㎛) fit discrepancies. CONCLUSION. The CAD and SLM processes affected the fit of CoCr crowns and varied based on the preparation designs. Typically, the shoulder-lip and sharp line angle designs had a more significant effect on crown fit. However, the differences between the design groups were relatively small, especially when compared to fit discrepancies observed clinically.
Purpose: This in vitro study was conducted to evaluate the marginal and internal fit of three-unit Co-Cr frameworks fabricated by computer-aided metal milling and direct metal laser sintering(DMLS) systems in comparison to conventional casting method. Methods: Three-unit Co-Cr frameworks were fabricated by conventional wax up with casting(CWC), computer-aided metal milling(MM) and direct metal laser sintering(DMLS)(n=10 each). The marginal and internal fit of specimens were examined using a light-body silicone impression material. The thickness of light-body silicone was measured at eight reference points each, divided in the mesio distal and bucco lingual directions. All measurements were conducted by a stereomicroscope. Digital photos were taken at 150× magnification and then analyzed using a measurement software. The Kruskal-Wallis test and Bonferroni correction were used for analyzing the results. Results: The mean(SD) is ㎛ for fabrication methods, the mean marginal fit were recorded respectively, DMLS 39(27), followed by CWC 63(38), MM 220(128). and the mean internal fit CWC 95(47), DMLS 116(49), MM 210(152). In addition, the largest gap was found in the occlusal surface area among the internal measurement areas of all groups. Conclusion: As a result, the direct metal laser sintering method showed better marginal and internal fit than the metal milling method. The marginal and internal fit were statistically different according to the three fabrication methods(p<0.001). Except the MM group, the marginal fit of the CWC and DMLS groups was below the clinical standard of 120 ㎛. Based on the results of this study, it can be applied to clinical use in the future.
Purpose: To evaluate the marginal and internal fit of metal coping fabricated by a metal three-dimensional (3D) printer that uses selective laser melting (SLM). Methods: An extraoral scanner was used to scan a die of the prepared maxillary right first molar, and the coping was designed using computer-aided design software and saved as an stereo lithography (STL) file. Ten specimens were printed with an SLM-type metal 3D printer (SLM group), and 10 more specimens were fabricated by casting the castable patterns output generated by a digital light processing-type resin 3D printer (casting the 3D printed resin patterns [CRP] group). The fit was measured using the silicon replica technique, and 8 points (A to H) were set per specimen to measure the marginal (points A, H) and internal (points B~G) gaps. The differences among the groups were compared using the Mann-Whitney U-test (α=0.05). Results: The mean of marginal fit in the SLM group was 69.67±18.04 ㎛, while in the CRP group was 117.10±41.95 ㎛. The internal fit of the SLM group was 95.18±41.20 ㎛, and that of the CRP group was 86.35±32 ㎛. As a result of statistical analysis, there was a significant difference in marginal fit between the SLM and CRP groups (p<0.05); however, there was no significant difference in internal fit between the SLM group and the CRP group (p>0.05). Conclusion: The marginal and internal fit of SLM is within the clinically acceptable range, and it seems to be applicable in terms of fit.
Purpose: The goal of this study was to determine the clinical acceptability of various cement space settings for the marginal and internal fit of a zirconia core manufactured using additive manufacturing. Methods: The maxillary right incisor served as the master model. After scanning the maxillary right incisor with a dental 3D (three-dimensional) scanner, the stereo lithography file was created using different cement space settings of 40, 120, and 200 ㎛ using computer-aided design software (Dental System 2018; 3Shape). The marginal and internal fit of the 3 groups were determined using the silicon replica technique. Measurement points were divided into the following three categories: margin, axial wall, and incisal. To ensure more accurate measurements, these three measurement points were divided into 8 points. The Shapiro-Wilk, one-way ANOVA, and Tukey's honestly significant difference test (for all tests α=0.05) were the statistical analyses that were included in the study. Results: The CS (cement space)-200 group had better marginal and internal fit than the CS-40 and CS-120 groups, and there were statistically significant differences at the marginal and incisal points, except for the axial wall points. CS-200 group, both marginal and internal fit were within 120 ㎛, which is the clinically acceptable value. Conclusion: This study suggests that a 200 ㎛ cement space setting is ideal for optimal marginal and internal fit of 3D-printed ceramic crowns.
Purpose : The purpose of this study was to analyze the change in thickness of transvers abdominis, internal oblique, and external oblique when virtual reality based ring fit adventure is applied to young adults in order to investigate the effect of ring fit adventure on core stabilization. Methods : 30 subjects participated in the experiment. Subjects were randomly assigned to two groups. 15 subjects performed ring fit adventure core exercise (experimental group) and 15 subjects bridge and dead bug exercise (control group). The ring fit adventure core exercise program consists of 6 types, 1) bow pull, 2) overhead lunge twist, 3) pendulum bend, 4) seated ring raise, 5) plank, 6) warrior III pose. Each exercise was performed for 5 minutes, for a total of 30 minutes. The bridege and dead bug exercise were performed for 15 minutes each for a total of 30 minutes. All interventions were performed 3 times a week for 4 weeks. Thickness of the abdominal muscles was measured with a ultrasound. The paired t-test was used to compare the thickness of the transverse abdominis, internal oblique, and external oblique before and after intervention, and the comparison between groups was analyzed using the independent t-test. Results : As a result, in the experimental group, thickness of transverse abdominis and internal oblique increased significantly (p<.05), but external oblique decreased significantly (p<.05), and in the control group, thickness of transverse abdominis, internal oblique, and external oblique increased significantly (p<.05). There was a significant difference in external oblique in the difference between groups (p<.05). Conclusion : These study results showed that core exercise using ring fit adventure can reduce external oblique and increased selective muscle activity of transverse abdominis and internal oblique of the deep abdominal muscles, so it is meaningful as an effective intervention for core stabilization.

Background/Aims: Optimal risk stratification based on simplified geriatric assessment to predict treatment-related toxicity and survival needs to be clarified in older patients with diffuse large B-cell lymphoma (DLBCL). Methods: This multicenter prospective cohort study enrolled newly diagnosed patients with DLBCL (≥ 65 yr) between September 2015 and April 2018. A simplified geriatric assessment was performed at baseline using Activities of Daily Living (ADL), Instrumental ADL (IADL), and Charlson's Comorbidity Index (CCI). The primary endpoint was event-free survival (EFS). Results: The study included 249 patients, the median age was 74 years (range, 65-88), and 125 (50.2%) were female. In multivariable Cox analysis, ADL, IADL, CCI, and age were independent factors for EFS; an integrated geriatric score was derived and the patients stratified into three geriatric categories: fit (n = 162, 65.1%), intermediate-fit (n = 25, 10.0%), and frail (n = 62, 24.9%). The established geriatric model was significantly associated with EFS (fit vs. intermediate-fit, HR 2.61, p < 0.001; fit vs. frail, HR 4.61, p < 0.001) and outperformed each covariate alone or in combination. In 87 intermediate-fit or frail patients, the relative doxorubicin dose intensity (RDDI) ≥ 62.4% was significantly associated with worse EFS (HR, 2.15, 95% CI 1.30-3.53, p = 0.002). It was related with a higher incidence of grade ≥ 3 symptomatic non-hematologic toxicities (63.2% vs. 27.8%, p < 0.001) and earlier treatment discontinuation (34.5% vs. 8.0%, p < 0.001) in patients with RDDI ≥ 62.4% than in those with RDDI < 62.4%. Conclusions: This model integrating simplified geriatric assessment can risk-stratify older patients with DLBCL and identify those who are highly vulnerable to standard dose-intensity chemoimmunotherapy.
Purpose: This study was conducted to comparative evaluate the marginal and internal gap of three-unit metal frameworks(Co-Cr) fabricated by wax milling method and digital light projection method of CAD/CAM systems. Methods: All the specimens were fabricated by three different fabrication methods: conventional wax up with casting(CWC), milled wax block with casting(MWC), digital light projection with casting(DLPC) (n=10 each). The marginal and internal fits of specimens were examined using a replica technique. The light-body silicone thickness was measured at 8 reference points(each abutment: 16 measurements). All measurements were conducted by a stereomicroscope. Digital photo were taken at $150{\times}$ magnification and then analyzed using a measurement software. The Mann-Whitney test was used for analyzing the results. Results: Statistically significant differences were found between the fabrication methods(p<0.001). The mean(SD) is ${\mu}m$ for fabrication methods, the mean marginal fit were recorded respectively, CWC 63(38), MWC group 50(33), DLPC 103(54) and the mean internal fit CWC 96(47), MWC group 116(41), DLPC 138(66). Conclusion : The marginal and internal fit were statistically different according to the fabrication methods(p<0.001). In all fabrication methods, the greatest misfit was found the occlusal area of all specimens.
The purpose of this study was to assess the stress-induced pattern at the supporting bone, the implant fixture, the abutment and the abutment screw according to a friction-fit joint (Astra; Model 1) or slip- fit joint (Frialit-2; Model 2) in the internal connection system under vertical and inclined loading using finite element analysis. In conclusion, in the internal connection system of the implant and the abutment connection methods, the stress-induced pattern at the supporting bone, the implant fixture, the abutment and the abutment screw according to the abutment connection form had difference among them, and the stress distribution pattern usually had a widely distributed tendency along the inner surface of the implant fixture contacting the abutment post. The magnitude of the stress distributed in the supporting bone, the implant fixture, the abutment and the abutment screw was higher in the friction-fit joint than in the slip-fit joint. But it is considered that the further study is necessary about how this difference in the magnitude of the stress have an effect on the practical clinic.
Purpose: The aim of this study measured and compared the marginal and internal fit of metal cores with two base metal alloy (Ni-Cr alloy(non-beryllium), Co-Cr alloy). Methods: Maxillary right first molar abutment fabricated by titanium was prepared for this study. Impressions(10ea) were made from titanium model, and study models were poured with improved dental stone. Wax cores of twenty were prepared for burn-out and casting. Ten wax cores cast Ni-Cr alloy(non-Be), and finally ten cast Co-Cr alloy. Marginal and internal fit of cores was evaluated using silicone replica technique and digital microscope(x160). The data were statistically analyzed with the independent samples t-test (${\alpha}$ <.05). Results: Mean(standard deviation, SD) marginal and internal fit total size of Ni-Cr alloy(non-Be) group was $73.3(14.4){\mu}m$ and of Co-Cr alloy group $65.6(17.4){\mu}m$. The marginal and internal fit total size of Ni-Cr alloy group(non-Be) was statistically significantly greater than that of Co-Cr alloy group (P=.004). Conclusion: Co-Cr alloy cores in this study had a better marginal fit than Ni-Cr alloy(non-Be) cores.
