Inter-rater Reliability of Korean Triage and Acuity Scale (KTAS) among the Research Nurses and the Triage Nurses (한국형 응급환자 분류도구의 간호사 간 신뢰도 평가)

  • Purpose: The study aims to assess the inter-rater reliability of the Korean Triage and Acuity Scale between the research nurses and the triage nurses. Methods: Interrater reliability was measured on 400 adult (≧15) and 400 pediatric (<15) patients who visited the emergency medical center from January 4 to June 30, 2018. Results: The study result showed that the inter-rater reliability of the Korean Triage and Acuity Scale was substantial, with κ=.73 (95% Confidence interval= .68-.78) and 77.0 percent agreement. The inter-rater of Pediatric Korean Triage and Acuity Scale was also substantial, with κ=.76 (95% Confidence interval= .71-.82) and 83.8 percent agreement. Conclusion: Although the inter-rater reliability of the Korean Triage and Acuity Scale was acceptable, the percent agreement was lower than the desirable level (<80.0%). It was confirmed that Pediatric Korean Triage and Acuity Scale had an acceptable level of inter-rater reliability and percent agreement for clinical use. Efforts should be made to improve the reliability in the future.

Inter-rater·absolute Reliability and Concurrent Validity of Tinetti-gait Scale (Korean version) in Stroke Patients (뇌졸중 환자들의 한국판 Tinetti-보행 척도의 측정자간·절대적 신뢰도와 동시 타당도)

  • PURPOSE: The purpose of this study was to investigate the inter-rater absolute reliability and the concurrent validity of the Tinetti-gait scale that was translated into Korean for chronic stroke patients. METHODS: Fifty-two patients with post-stroke participated in this study. Inter-rater reliability was analyzed by Intraclass Correlation Coefficient ($ICC_{3,1}$) and Kappa coefficient, and absolute reliability was analyzed by the Standard Error of Measurement (SEM), and the Minimal Detectable Change (MDC). Concurrent validity was analyzed by correlating between the Tinetti-gait scale and physical functions. The physical functions were measured by using the Dynamic Gait Index(DGI), 10m walking test(10WT), One Leg Standing Test of affected/non-affected side(OLST), Sit to Stand test(STS), (Fugl Meyer assessment of Lower Extremity(FM-LE). RESULTS: The inter-rater reliability of the Tinetti-gait scale was high; $ICC_{(3,1)}=.91$ (95% CI=.85~.95) (very reliable), the range of Kappa coefficient were .73~.92 (substantial~good). The inter-rater agreement of the each item in Tinetti-gait scale ranged from .74 to .92 (95 % CI=.59~.95) (reliable~very reliable). The SEM and MDC were .56 and 1.55, respectively. In the results of concurrent validity, there were moderate positive correlation between Tinetti-gait scale and DGI (r=.78), 10WT (r=.74), OLST (r=.65~.73), FM-LE (r=67). And there was moderate negative correlation between Tinetti-gait scale and STS (r=-79) (p<.01). CONCLUSION: The Tinetti-gait scale(Korean version) was a reliable and valid tool to measure gait ability in patients with chronic stroke. Thus, it could be a useful tool for examining a gait ability of post-stroke patients. Further study should be conducted to investigate the usability to predict fall risk of post-stroke patients of the Tinetti-gait scale.

The Effect of an Education Program on Inter-rater Reliability of Neonatal/Infant Braden Q Scale for Clinical Nurses (신생아중환자실 간호사를 대상으로 한 Neonatal/Infant Braden Q Scale 사용교육이 측정자 간 일치도에 미치는 효과)

  • Purpose: This study was aimed to investigate the effect of an education program on inter-rater agreement of Neonatal/Infant Braden Q Scale for clinical nurses working at a neonatal intensive care unit (NICU). Methods: This was single-arm pre and post experimental study. The participants were 12 nurses and 128 hospitalized neonates at a NICU from December, 2012 to March, 2013. Twelve nurses were divided into four different groups; for two groups were assigned nurses with 3 to 5 years of clinical experiences, and for the others with less than 1 year of clinical experience. The interventions were given by one wound ostomy specialist and two NICU nurses with over 5 years of clinical experiences for 1 hour twice. The inter-rater agreement was measured by intraclass-correlation coefficient. Results: Overall inter-rater agreement was improved from .87(95% CI: .80~.92) at the pre-test to .94(.91~.96) at post-test. Each inter-rater agreement except moisture and nutrition was also improved. Conclusion: The developed education program on scoring for Neonatal/Infant Braden Q scale was effective to improve the inter-rater agreement among clinical nurses. We suggest to privide an education for NICU nurse before using the Neonatal/Infant Braden Q scale in clinical settings.

Reliability of the Modified Modified Ashworth Scale for the Muscle Tone of Poststroke Patients (뇌졸중 환자의 근긴장도 평가를 위한 개정된 개정된 Ashworth 척도의 신뢰도)

  • Purpose : The clinical scale to assess spasticity of muscle was wildly used the modified Ashworth scale (MAS). But reliability of the MAS has been controverted for ambiguity among the grades. The purpose of this study was to establish the inter-rater reliability of the modified MAS (MMAS) translated into Korean in stroke patients. Methods : Twenty-five patients (sixteen men and nine women) with hemiplegia (ten right and fifteen left) were measured by two raters who were physical therapist in the rehabilitation hospital. The raters assessed spasticity of shoulder adductor, elbow flexor, wrist flexor, hip adductor, knee extensor, and ankle plantar flexor in the same patients according to ratings criteria of the MAS and the MMAS. Results : For the inter-rater reliability of the MAS, two raters agreed on 57.3% and the Kappa values were moderate ($\kappa$=0.41) between two rater. The inter-rater reliability of the MAS was fair for the wrist flexor and the hip adductor and moderate for the other muscles. The intra-rater reliability was good for the shoulder adductor and the knee extensor and moderate for the other muscles. For the inter-rater reliability of the MMAS, two raters agreed on 84.7% and the Kappa values were good ($\kappa$=0.78) between two rater. The inter-rater reliability of the MMAS was moderate for the hip adductor, and good for the shoulder adductor and the wrist flexor, and very good for the other muscles. The intra-rater reliability was good for the wrist flexor and the hip adductor and very good for the other muscles. Conclusion : This study suggests that the MMAS translated into Korean is reliable test scale for the spasticity with stroke patients in the clinical field.

Inter-Rater and Intra-Rater Reliability of the Modified Ashworth Scale and the Modified Tardieu Scale: A Comparison Study (수정된 Ashworth 척도와 수정된 Tardieu 척도의 검사자간, 검사자내 신뢰도 비교 연구)

  • Purpose: The purpose of this study was to assess and compare the reliability of the Modified Tardieu Scale (MTS) with the Modified Ashworth Scale (MAS) in patients with hemiplegia. Methods: Two experienced physical therapists examined twenty six patients (17 male and 9 female) with an age range of 19-83 years (mean=51.9 SD=15.2). They assessed the elbow flexor/extensor muscle spasticity in the affected side. Interand intra-rater reliability of the MAS and the MTS were calculated using kappa statistics. Intraclass correlation coefficient (ICC) was calculated to determine the inter- and intra-rater reliability of the angle of muscle reactions (R2-R1). Results: The intra-rater reliability of the MAS (K=0.39-0.55) and MTS (K=0.33-0.55) was fair to moderate. The inter-rater reliability was significantly higheras measured with MTS (K=0.54-0.66) in comparison with MAS (K=0.52). Intra-rater reliability of R2-R1 was moderate to almost perfect (ICC=0.52-0.86), and inter-rater reliability was substantial (ICC=0.74-0.76). Conclusion: The MTS provides higher inter-rater reliability compared with the MAS in hemiplegia patient analysis, but intra-rater reliability of both scales was not significantly different. Thus further research is needed to examine not only reliability, but also validity of these measurement systems.

Development and Validation of an Observation Scale for Day Care Programs (어린이집 프로그램 관찰척도의 개발과 타당화)

  • This study aimed to develop an observation scale for assessing the quality of day care programs and to examine reliability and validity of the scale. Instruments included 3 self-assessment scales, the Evaluation Scale for Early Childhood Programs and an observation scale completed by 8 trained observers in 89 day care sites. Item analysis indicated that most items were acceptable in terms of item discrimination and item response distribution. Cronbach ${\alpha}^{\prime}s$ and inter-observer reliability were relatively high. Validity of the scale was evidenced by correlations between the observation scale and the criterion measure and inter-correlations among sub-areas of the observation scale. Scores on the observation scale were validated by quality ratings from self-assessment scores.

Analysis on the Gray Scale Capability of TFT-LCD using Three-dimensional Simulation (3차원적 시뮬레이션에 의한 TFT-LCD의 Gray Scale 성능 분석)

  • We analyzed the effect of a pixel and all the inter-electrode capacitances in a unit pixel of TFT-LCDs on the gray scale capability. The pixel and all the inter-electrode parasitic capacitances were obtained from the tree dimensional profiles of potential distribution and molecular director considering lateral fields generated at the edge of the pixel. To obtain the RMS and kickback voltages of the pixel, we constructed an equivalent circuit of the panel containing all the parasitic capacitances. The calculation was performed though H-SPICE. As results, we confirmed that the pixel becomes smaller, the effect of parasitic capacitances on the gray scale capability becomes larger.

Inter-Examiner Reliability of the Korean Version of the Pediatric Balance Scale (한글판 아동 균형척도(Pediatric Balance Scale)의 측정자간 신뢰도)

  • The Pediatric Balance Scale (PBS) is a modified Berg's Balance Scale developed as a balance measure for school-aged children with mild to moderate motor impairments. The purpose of this study was to determine the inter-examiner reliability of the Korean version of PBS when applied to children with developmental delays. In this study, PBS was administrated to a total of 79 children with developmental delays (17 with global developmental delay, 31 with cerebral palsy, and 31 with mental retardation) in the Seoul Community Rehabilitation Center. Two pediatric trained physical therapists with longer than 13 year of clinical experience scored the children's performance blind, while replaying videotaped data. The inter-examiner reliability was statistically determined by intraclass correlation coefficients (ICCs). The results of this study revealed that the Korean version of PBS seems to have high inter-examiner reliability when applied to children with movement disorders such as global developmental delay (ICC=.96) and cerebral palsy (ICC=.97); however, it has relatively lower inter-examiner reliability (ICC=.78) for children with developmental delay secondary to mental retardation. therefore, the results support that the Korean version of the PBS could be a useful clinical measurement to assess the balance skills for children with developmental delay who have an adequate level of cognition to enable them to fol1ow the verbal instructions to complete the test.

Analysis on the Electric Properties of TFT-LCDs (TFT-LCD의 전기적 특성 연구)

  • In this paper, we calculated the pixel and all the inter-electrode capacitances in a unit pixel of three kinds of TFT-LCD modes in order to realize the full color image. And we analyzed the effect on applied voltage. We fixed size of sub-pixel and designed a unit pixel of three kinds of TFT-LCD modes. We calculated rigorously the pixel and all the inter-electrode capacitances by the three dimensional simulation. And we expanded one pixel into whole panel using H-SPICE. As a result, we calculated the RMS voltage and kickback voltage which most affect gray scale of the pixel and calculated the number of bit according to permissible range of minimum gray scale using V-T curve.

Inter-Rater Reliability of the Modified Ashworth Scale of Spasticity (경련성 (spasticity) 평가를 위한 Modified Ashworth Scale의 측정자간 신뢰도)

  • This study was performed to determine the inter-rater reliability of manual tests of elbow, knee flexor, and ankle dorsiflexor muscle spasticity graded on the Modified Ashworth Scale. Two raters each independently graded the spasticity of 32 patients with intracranial lesions after moving the paretic limb passively through the available range of motion. The patients were asked to simultaneously squeeze therapeutic putty with their non-paretic hand for reinforcement. The ratings were compared by the Wilcoxon matched pairs signed-rank test and by the Kendall's coefficient of rank(tau) correlation. There was singificant correlation between two raters for spasticity at the elbow, knee flexor, and ankle dorsiflexor. The correlations of the two raters ranged from .6746 to .9308. The highest correlation was for the elbow with reinforcement and the lowest was for the knee without reinforcement. Poorer correlation was evident in the knee joint. The positive results of this study encourage the continued use of manual tests of muscle spasticity, using the Modified Ashworth Scale.
