암환자의 불면에 대한 발반사요법의 효과 : 예비임상연구 (A Pilot study to estimate the efficacy of foot reflexology on insomnia in cancer patient)

  • Objective: Insomnia is a very common symptom of cancer and has a substantial effect on the patients' quality of life. This study aimed to identify the effects of foot reflexology on insomnia in cancer patients. Methods: The study employed a before-after test design on a single group of 13 cancer patients with insomnia, selected by convenience sampling in Daegu. The foot reflexology treatment comprised 30-min sessions twice a week over four weeks. Pittsburgh Sleep Quality Index (PSQI) and Insomnia Severity Index (ISI) were measured at the baseline, at 2 and 4 weeks, and after 4 weeks post treatment completion. Brief Fatigue Inventory-Korean (BFI-K), Functional Assessment of Cancer Therapy-General (FACT-G), and Heart Rate Variability (HRV) were measured at baseline, post-treatment, and 4 weeks after completing treatments. Results: Thirteen patients (mean age 44.77 ± 6.70 years), with thyroid, breast or cervical cancer, were included in the study. Post-treatment, PSQI (from 12.33 ± 3.17 to 8.54 ± 3.43, p<0.001), ISI (from 20.33 ± 3.88 to 13.38 ± 4.23, p<0.001), FACT-G (from 49.6 ± 13.27 to 61.35 ± 14.45, p<0.001), and BFI (from 6.46 ± 0.91 to 5.00 ± 2.02, p<0.001) scores improved significantly over those at the baseline. Conclusion: Foot reflexology can be a good alternative treatment to improve insomnia, fatigue, and quality of life in cancer patients

향기요법의 암 환자 불면에 대한 효과에 관한 예비임상연구 (A Pilot study to estimate the efficacy of aroma therapy on insomnia in cancer patient)

  • Objective: Insomnia is a very common symptom and has a great effect on the quality of life in cancer patients. This study was conducted to identify the effects of the aroma therapy on insomnia in cancer patients. Methods: The study was performed with one group in before-after test design. The subjects were 15 cancer patients with insomnia selected by convenience sampling in Daegu. The aroma therapy consists of 10-minute sessions twice a week over 6 weeks. The PSQI (Pittsburgh Sleep Quality Index), ISI (Insomnia Severity Index) were measured at baseline and at 3 and 6 weeks, after 6 weeks after finishing treatment. STAI (State-Trait Anxiety Inventory), FACT-G (Functional Assessment Cancer Therapy-General), HRV (Heart Rate Variability) were measured at baseline, post-treatment and after 6 weeks after finishing treatments. Results: Fifteen patients (mean age 50.93 ± 7.29 years), with breast, ovarian or gastric cancer, participated this study. At post-treatment, PSQI (from 14.18 ± 2.64 to 6.18 ± 3.03), ISI (from 19.00 ± 3.19 to 8.18 ± 5.14), FACT-G (from 55.65 ± 11.20 to 66.91 ± 12.33), and STAI (from 40.36 ± 8.10 to 34.73 ± 9.81, from 45.64 ± 6.07 to 39.73 ± 9.37) scores improved significantly over baseline(p<0.001). Conclusion: Aroma therapy improved insomnia, anxiety and quality of life on cancer patients.

대사증후군이 동반된 간화상염 변증 환자에 대한 한의정신요법 병행 한의치료 증례보고 (A Patient with Liver Fire Flaming Upward and Metabolic Syndrome Improved by Korean Medicine: A Case Report)

  • The purpose of this study is to report a case of a patient with pattern identification of liver fire flaming upward and metabolic syndrome improved by Korean medicine. The patient felt completely lost. She was treated for headache, insomnia, and metabolic syndrome with Korean medicine and psychotherapy. Pittsburgh Sleep Quality Index (PSQI), Insomnia Severity Index (ISI), Pattern Identification Tool for Insomnia (PIT-Insomnia), Minnesota multiphasic personality inventory-2 (MMPI-2), and bioelectrical impedance analysis (BIA) were used for assessment. During treatment for six months, the patient stopped taking headache pills. Her blood pressure was stabilized. Her body weight, body fat, body mass index (BMI), and visceral fat decreased significantly. The sum of ISI, PSQI, PIT-insomnia also decreased significantly. Her Ego strength increased in the MMPI-2 supplementary scale. We assume that Korean medicine treatment through acupuncture, herbal medicine, and psychotherapy is effective in treating pattern identification of liver fire flaming upward and metabolic syndrome.

  • Objectives : To prove the effectiveness of acupuncture treatment targeting the insomnia-complaining patients from the group of hospital inpatients. Methods : Among Wonkwang University Iksan Oriental Medical Hospital impatients, needling Sinmaek($BL_{62}$) and Johae($KI_6$) and Venesection on Eunbaek(SP1) on the 15 insomnia-complaining patients. Then, confirmed the degree of insomnia symptom improvements through Saint's Mary Hospital(SMH) questionnaire, insomnia severity index(ISI), and Korean sleep scale(KSS). Results : 1. Result indicated after the acupuncture treatment, the total hours of sleeping increased in average per hour. 2. When evaluating the degree of improvement using Insomnia Severity Index(ISI) after the treatment, average score was lowered and the statistical data showed the result was significant. 3. When evaluating the degree of improvement using Korean Sleep Scale(KSS), the average score was increased and the statistical data showed the result was significant. Conclusions : Among the insomnia patients from the group of Oriental Medical Hospital inpatients, the total hours of sleeping increased, showed the score improvement by using the tool for evaluating sleeping, and indicated statistically significant treatment effectiveness.

The Effect of Hypnotics on Sleep Quality and Cognitive Function in Patients with Brain Tumors

  • Objective : We investigated the effect of hypnotics on sleep quality, cognitive function, and depressive mood in patients with insomnia following brain tumor resection. Methods : From patients who underwent brain tumor resection, we recruited 10 patients with insomnia who received hypnotics for more than 1 week during a 3-week follow-up period (insomnia group). We also recruited 12 control patients with brain tumors but without insomnia (control group). We evaluated sleep quality at baseline and 3 weeks later using the Insomnia Severity Index (ISI), the Pittsburgh Sleep Quality Index (PSQI), the Stanford Sleepiness Scale (SSS), and the Epworth Sleepiness Scale (ESS) and investigated cognitive function and depression using the Computerized Neuropsychological Test and the Beck Depression Inventory (BDI). Results : At baseline, SSS, ISI, PSQI, and BDI scores were significantly higher and visual continuous performance test (VCPT) and auditory continuous performance test (ACPT) scores were significantly lower in the insomnia than in the control group. Three weeks later, the patients who had received hypnotics had significantly higher ISI, PSQI, ESS, VCPT, ACPT, visual span forward and backward, and visual recognition test scores, and significantly lower BDI scores. Conclusion : Quality of sleep in patients with insomnia following brain tumor resection was initially poor but improved significantly after taking hypnotic medication. Further, the hypnotic medications appeared to contribute to the amelioration of cognitive impairments and depressive moods in patients who previously underwent brain tumor resection. We thus recommend the use of hypnotics for patients with brain tumors with insomnia.

  • Objectives: The purpose of this study is to report the case of a patient with Parkinson's disease experiencing anxiety and insomnia treated with Korean medicine therapies. Methods: A patient diagnosed with Parkinson's disease with anxiety and insomnia was treated with Korean medicine therapies, including herbal medication (Gyejigayonggolmolye-tang), acupuncture, electro-acupuncture, and moxibustion, for 18 days. To evaluate the therapeutic effect, we checked the Beck Anxiety Inventory (BAI) and Insomnia Severity Index (ISI) scores to monitor anxiety and insomnia. Results: After treatment, the patient's BAI and ISI scores for anxiety and insomnia decreased. Further, the patient and her protector mentioned that their quality of life had improved. Conclusion: These results suggest that Korean medicine therapies with Gyejigayonggolmolye-tang have a beneficial effect on anxiety and insomnia in patients with Parkinson's disease.

불안을 동반한 수면 장애 환자 치험 1례 (A Case Report of Sleep Disorder with Anxiety)

  • Objectives: This case report presents a 29-years-old male patient. His chief complaint was insomnia, anxiety and chest discomfort. We diagnosed him with sleep disorder. During the therapeutic period, he experienced temporary improvement. Methods: We treated him with emotional freedom techniques(EFT), giungoren-therapy. Also, acupuncture and chuna therapy were accessorily practiced. The effects of treatment were measured by Beck depression inventory(BDI), state-trait anxiety inventory(STAI), insomnia severity index(ISI). Results: After treatment, the chief complaint have been subsided and improved. But, BDI, STAI, ISI score were not remarkably decreased. Conclusions: This result suggests that EFT and giungoren-therapy might be effective for sleep disorder patient with anxiety.

분심기음을 포함한 한방치료로 호전된 섬유근육통 환자의 불면 치험 1례 (A Case Report of a Patient with Insomnia Related To Fibromyalgia Treated with Korean Medicine, including Bunshimgi-eum)

  • In this case report, we describe the effects of Korean Medicine therapies, including Bunshimgi-eum, on a patient hospitalized with insomnia due to fibromyalgia in a Korean Medicine Hospital. We treated the patient with Bunshimgi-eum for 21 of 27 hospital days. Post-treatment, we used the Insomnia Severity Index (ISI) to measure the severity of insomnia. To determine the severity of multiple pain, including neck, low back, Lt. shoulder, Lt. elbow, Rt. knee, and hand, we used the Numerical Rating Scale (NRS). For evaluation of generic health status, we used the European Quality of Life-5 Dimensions (EQ-5D) scale. After the treatment, the patient's clinical symptoms improved, according to the ISI, NRS, and EQ-5D. The results of this case study suggest that Korean Medicine therapies, including Bunshimgi-eum, may have positive effects as a treatment for insomnia related to fibromyalgia.

Risk Factors Associated with Clinical Insomnia in Chronic Low Back Pain: A Retrospective Analysis in a University Hospital in Korea

  • Background: Insomnia is becoming increasingly recognized as a clinically important symptom in patients with chronic low back pain (CLBP). In this retrospective study, we have determined risk factors associated with clinical insomnia in CLBP patients in a university hospital in Korea. Methods: Data from four-hundred and eighty one CLBP patients was analyzed in this study. The Insomnia Severity Index (ISI) was used to determine the presence of clinical insomnia (ISI score ${\geq}15$). Patients' demographics and pain-related factors were evaluated by logistic regression analysis to identify risk factors of clinical insomnia in CLBP. Results: It was found that 43% of patients reported mild to severe insomnia after the development of back pain. In addition, 20% of patients met the criteria for clinically significant insomnia (ISI score ${\geq}15$). In a stepwise multivariate analysis, high pain intensity, the presence of comorbid musculoskeletal pain and neuropathic pain components, and high level of depression were strongly associated with clinical insomnia in CLBP. Among these factors, the presence of comorbid musculoskeletal pain other than back pain was the strongest determinant, with the highest odds ratio of 8.074 (95% CI 4.250 to 15.339) for predicting clinical insomnia. Conclusions: Insomnia should be addressed as an integral part of pain management in CLBP patients with these risk factors, especially in patients suffering from CLBP with comorbid musculoskeletal pain.

  • ■ Objectives The purpose of this case report is to show the effect of Traditional Korean Medicine(TKM) on a patient with insomnia occurred after onset of thalamic hemorrhage. ■ Methods A patient with insomnia diagnosed as thalamic hemorrhage was treated with herbal medication, acupuncture, electro-acupuncture, cupping and moxibustion. Then we evaluated the improvement with the number of times the patient woke up, PSQI-K(Pittsburgh Sleep Quality Index-Korean), ISI-K(Insomnia Severity Index-Korean) and KESS(Korean version of Epworth sleepiness scale). ■ Results Decrease in scores of PSQI-K, ISI-K, KESS and the number of times the patients woke up were observed after the TKM treatment. ■ Conclusion This case showed the effect of TKM treatment on poststroke insomnia.
