가와사끼병 환자에서 분리한 CD14양성 세포에서 Toll-like Receptor-2의 발현 (Expression of Toll-like Receptor-2 on the Peripheral Blood Monocytes in Kawasaki Disease Patients)

  • 목 적 : 본 연구에서는 가와사끼병에서 toll-like receptor(TLR)의 발현정도를 살펴 염증반응이 유발되기 시작하는 기전에 대해 접근하고자 하였다. 방 법 : 2003년 3월부터 8월까지 연세의료원에서 가와사끼병으로 진단 받은 환아 10명과 발열대조군 10명 및 정상대조군 10명의 말초혈액을 얻은 후 유세포분석기(flow cytometry)를 시행하여 CD14 양성인 단핵구에서의 TLR-2 발현정도를 측정하였다. 또한 말초 혈액 단핵구의 total RNA를 분리한 후 역전사중합효소 연쇄반응(RT-PCR)을 시행하여 TLR-2의 mRNA 발현을 살펴보았다. 결 과 : 환자군에서의 TLR-2 발현은 정상대조군보다 통계적으로 유의하게 증가되어 있었으나 임상경과에 따른 양상을 보면 급성기보다 아급성기에서 감소하였지만 통계적으로 유의한 차이는 보이지 않았고 환자군과 발열대조군의 TLR-2 발현도 의미있는 차이를 보이지 않았다. 또한 급성기 환자군의 말초혈액 단 핵구에서 TLR-2의 mRNA 발현이 증가되어 있었다. 결 론 : TLR-2의 발현은 가와사끼병 환자에서 정상대조군과 비교하여 증가되어 있었으며 이는 TLR 및 이를 통한 선천성 면역계(innate immunity)가 가와사끼병의 병인과 연관될 수 있음을 시사한다. 앞으로 TLR의 발현이 가와사끼병에서의 염증유발에 있어 구체적으로 어떤 역할을 하는지에 대한 연구가 더 필요할 것으로 사료된다.

미역포자엽추출 푸코이단의 X-선 조사 흰쥐 폐장상해에 대한 방오기능 평가 (Protection Effect of Undariia pinnatifida sporophylls-derived Fucoidan in Rat Lung Injury from X-ray Irradiation)

  • The radioprotect effects of fractinated fucoidan, derived from Undaria pinnatifida sporophylls, were examined in lung injured rats treated with partial body irradiation. The right lung of Sprague-Dawley rats, eight separately grouped by including radiation only control group (ROG) and fucoidan+radiation treated group (FRG), were treated with various fraction of Undaria-derived fucoidan every other day by intraperitoneal injection 6 days prior to irradiation and during monitoring at 24 hours, 48 hours, 2 weeks, 4 weeks and 12-weeks post-irradiation of 6 Mev/2 Gy. The rats were euthanized at completion of the treatment. Tissue samples from the injured lung, fixed in formaldehyde using lung perfusion prior to extraction, were H/E stained for histological analysis using optical microscopy. Interstitial congestion (IC), hemorrhage in alveolar sac (HM), number of macrophage (MP) and alveolar wall thickness (AWT) as a measure of early indication of fibrosis were measured. AWTs in 24 hrs post-irradiation rats increased in comparison with $2.49{\pm}0.06{\mu}m$ of control group by $3.67{\pm}0.09{\mu}m$, $3.21{\pm}0.08{\mu}m$ (p=0.013), $2.98{\pm}0.08{\mu}m$ (p=0.00) in ROG, F1-fucoidan+radiation, and F3-fucoidan+radiation group, respectively. AWT of the ROG was further increased by $4.30{\pm}0.13{\mu}m$ in the 12 weeks post-irradiation group, but AWT on average was $2.56{\pm}0.05{\mu}m$ in the FRG. MP in the 24 hrs post-irradiation group markedly increased in comparison with $2.6{\pm}0.34/0.14\;mm^2$ of the control group by $8.0{\pm}1.48/0.14\;mm^2$ in the ROG, but it was only $3.6{\pm}0.48/0.14\;mm^2$ in F3-FRG. MP in the 12 weeks post-irradiation group was $7.2{\pm}1.28/0.14\;mm^2$ in ROG, but it was $2.8{\pm}0.37/0.14\;mm^2$ or $2.4{\pm}0.4/0.14\;mm^2$ (p<0.05) in the FRG. In addition, increased IC and HM in ROG were relatively smaller in FRG of the 24 hr and 12 weeks post-irradiation rats. In conclusion, Undaria pinnatifida sporophylls-derived fucoidan fractions exhibited radioprotectant activity on injured rat lung subjected to X-ray irradiation. However, success was variable according to the fractions and also time of injection post injury.

길경(桔梗)발효 추출물의 알레르기성 소양행동 억제효과연구 (Anti-pruritic Effect of Ethanol Extracts from Platycodon grandiflorum and its fermented production in Scratching Behavior Mouse Models)

  • Objectives : Platycodon Root is frequently used in traditional medicine to treat inflammatory diseases of the throat. The purpose of this study was to characterize the effect of the EtOH extract of fermented Platycodon grandiflorum on the ameliorative effects on the Antipruritic Effect of atopic dermatitis mouse model induced by compound 48/80 and ovalbumin (OVA)-induced allergic responses in mice. Methods : In the present study, we examined the anti allergic effect of Platycodon grandiflorum (PR) and its fermented production (FPR) in several mouse model. We measured acute ear edema in a mouse model caused by TPA and consecutively histological change of Ear tissue was observed by hematoxylin and eosin (H&E) staining. and also Scratching behaviors by compound 48/80 was investigated. The levels of allergic mediators such as immunoglobulin (Ig) E, and anti-oxidant markers such as SOD and MDA in the sera of OVA induced allergic mice were measured by enzyme-linked immunosorbent assay. Results : FPR inhibited compoud 48/80-induced scratching behavior in mice, as well as acetic acid-induced writhing in mice. The anti-scratching behavioral effect of FPR was more potent than PR. FPR extract significantly decreased the serum levels of IgE and MDA compared with those of OVA control group. Conclusions : These results indicate that Anti allergic effect of Platycodon grandiflorum is enhanced by fermentation with Saccharomyces cerevisae and FPR may be useful for protection from the itching reactions, which are IgE-mediated representative skin allergic diseases.

The safety, immunological benefits, and efficacy of ginseng in organ transplantation

  • Korean ginseng (Panax ginseng) is associated with a variety of therapeutic effects, including antioxidative, anti-inflammatory, vasorelaxative, antiallergic, antidiabetic, and anticancer effects. Accordingly, the use of ginseng has reached an all-time high among members of the general public. However, the safety and efficacy of ginseng in transplant recipients receiving immunosuppressant drugs have still not been elucidated. Transplantation is the most challenging and complex of surgical procedures and may require causation for the use of ginseng. In this regard, we have previously examined the safety, immunological benefits, and protective mechanisms of ginseng with respect to calcineurin inhibitor-based immunosuppression, which is the most widely used regimen in organ transplantation. Using an experimental model of calcineurin inhibitor-induced organ injury, we found that ginseng does not affect drug levels in the peripheral blood and tissue, favorably regulates immune response, and protects against calcineurin inhibitor-induced nephrotoxicity and pancreatic islet injury. On the basis of our experimental studies and a review of the related literature, we propose that ginseng may provide benefits in organ transplant recipients administered calcineurin inhibitors. Through the present review, we aimed to briefly discuss our current understanding of the therapeutic benefits of ginseng related to transplant patient survival.

복분자 자죽염의 마우스 대식세포주 및 복강 대식세포에 대한 면역증진 효과 (Immunostimulatory Effects of Purple Bamboo Salts Composed with Rubus coreanus in Raw264.7 Cells and Mouse Peritoneal Macrophages)

  • 본 연구에서는 전통적인 기능성 식품이자 의약품으로 사용되었던 자죽염(PubS)과 복분자를 조합한 복분자 자죽염(PuBS-R)을 사용해 비특이적, 선천성 면역에 중요한 역할을 하는 대식세포에서의 영향을 확인하였다. PuBS-R은 마우스 대식세포인 Raw264.7 세포에서 $1,000{\mu}g/mL$ 농도처리 시 세포의 증식을 유발하였으며, 그에 반해 PuBS를 $1,000{\mu}g/mL$ 처리 시 유의하게 세포 증식률이 억제되었다. 이어서 면역 관련 사이토카인 $TNF-{\alpha}$, $IFN-{\gamma}$, IL-12, IL-10 등을 유전자 단계와 단백질 단계에서 평가하였다. 그 결과 PuBS-R은 50, 100, $500{\mu}g/mL$에서 유의한 증가율을 보였고, 무엇보다 이것은 PuBS 군에 비해서도 월등한 증가율을 확인할 수 있었다. 마지막으로 마우스의 복강에서 분리한 peritoneal macrophage에 PuBS-R을 처리 후 $TNF-{\alpha}$, $IFN-{\gamma}$, IL-12, IL-10, iNOs를 단백질 단계에서 평가한 결과, Raw264.7 세포에서 얻은 결과와 동일한 결과를 확인할 수 있었다. 이러한 결과로 미루어 보아 PuBS-R은 인체의 대식세포 활성의 증가를 통해 비특이적, 선천성 면역력을 증가시킬 것으로 판단되며, 이러한 결과를 바탕으로 추후 실험동물을 이용한 in vivo 후속 연구를 통해 PuBS-R의 면역증강 기능성 식품 개발이 가능할 것으로 생각된다.

Nuclear factor erythroid 2-related factor 2 활성화를 통한 해간전(解肝煎)의 간세포 보호 효능 및 분자기전을 활용한 해간전(解肝煎) 구성 약물의 최적화 연구 (Study of hepatoprotective effect of Haegan-jeon through activation of nuclear factor erythroid 2-related factor 2 and optimization of herbal composition based on molecular mechanism)

  • Objectives : Present study investigated hepatoprotective effect of Haegan-jeon extract (HE) and tried to elucidate molecular mechanism involved. According to molecular mechanism, present study optimized herbal composition of HE (op-HE) and compared in vitro and in vivo hepatoprotective effects of op-HE to HE. Methods : For in vitro experiments, HepG2 cells were exposed to arachidonic acid (AA, $10{\mu}M$) and iron ($5{\mu}M$) for inducing oxidative stress. Cell viability, GSH contents, $H_2O_2$ production, mitochondrial membrane potential, immunoblot and reporter gene assay were performed to investigate cytoprotective effects and responsible molecular mechanisms. For in vivo experiments, hepatoprotective effect of HE and op-HE were assessed on $CCl_4-induced$ liver injury mice model. Results : HE pretreatment prevented AA+iron-mediated hepatocytes apoptosis. In addition, AA+iron-induced mitochondrial dysfunction, $H_2O_2$ production, glutathione depletion were reduced by HE pretreatment. In addition, nuclear factor erythroid 2-related factor 2 (Nrf2) phosphorylation, antioxidant response element (ARE)-driven reporter gene activity, and antioxidant genes expression were increased by HE. Based on reporter gene and MTT assays, we found that op-HE consisting three medicinal herbs also significantly increased transactivation of Nrf2 and reduced the AA+iron-mediated cytotoxicity. Moreover, in $CCl_4-induced$ liver injury mice model, HE-op had an ability to ameliorate $CCl_4-mediated$ increases in serum alanine transferase and aspartate aminotransferase activity, hepatic degeneration, inflammatory cell infiltration, and collagen deposition. Hepatoprotective effects of op-HE were comparable to those of HE. Conclusions : Present study suggests that op-HE as well as HE exhibit hepatoprotective effect against oxidative stress-mediated liver injury via Nrf2 activation.

암미세환경에서 종양관련대식세포의 역할 (Role of Tumor-associated Macrophage in Tumor Microenvironment)

  • 암세포는 종양의 성장을 지지하는 다양한 성분으로 구성되어 있는 환경에서 자란다. 암미세환경에 존재하는 주요 세포등은 섬유아세포, 내피세포, 면역세포들이며 이들세포들은 암세포들과 서로 소통을 하고 있다 종양조직에 유입된 면역세포중에서 대식세포가 종양미세환경의 주요성분으로서 다양한 면역현상들을 조절한다. 면역세포유입에 의한 암촉진과 항암효과 간의 복잡한 균형은 종양의 성장과 진행에 필요한 만성염증 환경을 생성시킬 수 있다. 대식세포는 M1과 M2 극성화로 규정된 미세환경 신호에 반응하여 기능적으로 다른 프로그램을 작동시킬 수 있다. 종양관련대식세포는 다양한 사이토카인, 케모카인, 단백질분해효소들을 분비함으로써 암 신생혈관형성, 증식, 전이 및 면역억제를 촉진시킨다. 최근에, 종양관련대식세포는 암줄기세포와 상호작용하여 종양의 진행, 전이 및 항암제 내성을 유도하는 것으로 알려져 있다. 종양관련대식세포는 암미세환경을 유지하기위해 면역억제 기능을 획득하며, 종양의 이질성과 가소성의 특성을 갖고 있어 암관련인자 및 감염등의 노출에 의해 서로 다른 극성형질로 리프로그래밍된다. 종양관련대식세포는 기질인자의 자극에 의해 암특이적인 케모카인들을 생성하기 때문에 케모카인은 질병의 활성을 반영하는 바이오마커로 작용할 수 있다. 종양조직에 종양관련대식세포가 많이 유입될수록 환자의 예후가 좋지 않으며 항암치료에 대한 저항성이 생긴다. 따라서 종양에서 대식세포를 표적화하는 항암치료는 유망한 치료전략이 될 수 있다.

  • 흡연물질로서 benzopyrene, nicotine 및 cotinine등은 기관지상피세포인 Beas2B에서 CuZnSOD, thioredoxin, glutathione reductase 등의 발현량에 영향을 주며 특히 thioredoxin 및 glutathione reductase의 발현량을 노출 후 30분에서 4시간 경과 시간대에 증가시켰다가 24시간 이후에는 억제하는 특징을 나타내었다. 상기한 흡연물질에 의한 항산화효소의 조절기전에는 전사인자인 NNF-${\kappa}B$가 관여하는 것으로 추정되었다.

재조합 골형성 단백질 2형(rh-BMP-2) 함유 무세포성 진피조직(acellualr dermal matrix)의 골재생 효과 (Effects of Recombinant Human Bone Morphogenetic Protein-2 loaded Acellular Dermal Matrix on Bone Formation)

  • Introduction : The purpose of this study was to evaluate the possibility of the acellular dermal matrix (ADM) as a barrier membrane for bone regeneration, and to evaluate the osteogenic effect of ADM as a carrier system for rhBMP-2 in the rat calvarial defect model. Materials and Methods: An 8-mm, calvarial, critical-size osteotomy defect was created in each of 60 male Spraque-Dawley rats(weight $250{\sim}300g$). Three groups of 20 animals, each received either rhBMP-2(0.025mg/ml) in an ADM carrier, ADM only, or negative surgical control. And each group was divided into 2- and 8-weeks healing intervals. The groups were evaluated by histologic and histomorphometric parameters(10 animals/group/healing intervals). Data were expressed as $means{\pm}standard$ deviations($m{\pm}SD$). Comparisons between experimental and control groups were made using two-way ANOVA and post hoc t-test. Comparisons between 2 weeks and 8 weeks were made using paired t-test. The level of statistical difference was defined as P< 0.05. Results : The ADM group and rhBMP-2/ADM group results in enhanced local bone formation in the rat calvarial defect at both 2 and 8 weeks. The amount of defect closure and new bone formation were significantly greater in the rhBMP-2/ADM group relative to ADM group(P<0.05). At 8 weeks, the majority of ADM in the defect was contracted, and integrated with surrounding host tissues. In addition, host cell infiltration and neovascularization of the ADM in the absence of an inflammatory response were observed, and the newly formed bone around ADM showed a continuous remodeling and consolidation. Conclusion : The results of the present study indicated that ADM may be used as a barrier membrane for bone regeneration and that may be employed as a delivery system for BMPs.

Helicobacter pylori에 의해 호중구 및 위점막 세포로부터 유도되는 Leukotriene $B_4$의 생성에 미치는 Rebamipide의 영향 (The Effect of Rebamipide on Cellular Release of Leukotriene $B_4$ by Helicobacter Pylori)

  • Leukotrienes(LTs) are hewn to act as a mediator provoking tissue response in inflammation. This finding implicates that LTs also play important roles in the pathogenesis of H, pylori-induced gastritis and gastric ulceration. Rebamipide is being currently used as a therapeutics for gastritis and peptic ulcer, but their mechanisms of action have not been known clearly yet. One possibility is that their therapeutic effects are ascribed to interfering with the H. pylori-induced release of LTs from neutrophils and gastric mucosal cells. In the present study, this possibility was tested using $LTB_4$ as the test material in human neutrophils and Kato III cells(gastric adenoma cells as a substitute for gastric mucosal cells). The release of $LTB_4$ from both neutrophils and Kato III cells was time and H. pylori-dose dependent. The maximum release of $LTB_4$ was induced by neutrophils and Kato III cells when these cells incubated with H. pylori $(4.8{\times}10^8\;cells/ml$ for 30min. But in the presence of rebamipide the release of $LTB_4$ from these cells was suppressed in dose dependent manners. The release was completely suppressed at 1.0 mM of rebamipide in neutrophils and 2.0 mM of this drug in Kato III cells, respectively. We also obtained the results that the release of $LTB_4$ was induced by A23187$(Ca^{2+}\;ionophore)$ and the A23187-induced release was also inhibited by rebamipide. It seems that the machanism of action of rebamipide is through its interaction with the level of intracellular $Ca^{2+}$. In view of the roles of $LTB_4$ in inflammatory reaction and the roles of H. pylori in gastritis and peptic ulcer, the effects of this drug observed in this study may contribute to their therapeutic action in these gastric disorders.
