Spontaneous Closure of Delayed Esophageal Perforation: By temporary cervical fistulation with dual drainages (식도천공의 자연폐쇄치료[경부식도루 조성술과 이중배액법에 의한]:1예 보고)

  • The esophageal perforation is the most rapidly fatal and most serious perforation of the gastrointestinal tract. The 53 year old male patient was admitted because of substernal and epigastric pain altar esophageal bougienage for the indigestion and the difficult swallowing before about 18 hours. On esophagogram, there was the extravasation of contrast media at the right side of the lower esophagus [retrocardiac segment]. The emergency thoractotomy, debridement and suture closure with drainage were performed. But after 7 days the esophageal leakage was complicated again with pus discharge, although primary repair was done. On the 13th hospital day, the temporary cervical esophageal fistulation with dual drainages was made under general anesthesia. On the 38th day after this procedure, the esophageal leakage was closed spontaneously. On the 63rd hospital day the cervical fistulation was repaired and ever since the esophageal passage was good without leakage or swallowing difficulty.

  • Foreign body indigestion during dental procedures is uncommon but sometimes associated with severe and life threatening morbidity. The dentist should decide whether multidisciplinary intervention is necessary and how urgently it has to be done. The active removal of foreign bodies depends on the size, shape, type and duration of impaction, along with the location in the gastrointestinal tract. In this report, we presented how to manage gastric foreign body related swallowing during dental procedure and investigated the predictive risk factors, precaution, and prompt management in foreign body ingestion.

  • 'Chung Kwei Won(淸快園)' is a Chinese patent medicine, which is used for stomachic disorder, acute and chronic indigestion, dispepsia, vomiting and diarrhea in Korea. This preparation consists of 16 kinds of powdered crude drugs. For the identification of individual crude drugs in such powdery mixtures, microscopic method may be used conviniently as it requires only a small amount of specimens. In this paper, the effectiveness of this method is exemplified by the identification of the ingredients in 'Chung Kwei Won' which contains 16 powder crude drug ingredients.

  • Han Jung Pyeong Yi San is a Chinese Patent medicine, which is used for stomachic disorder, acute and chronic indigestion, dyspepsia and diarrhea in Korea. Saenggangeonbi-tang's Effect on Alcoholic Liver Cirrhosis - 1 Case (생간건비탕가감을 투여한 알콜성 간경변증 환자 치험 1례)

  • We treated a 53 year-old male patient. He had alcoholic liver cirrhosis and diabetes. The main symptoms were fatigue, ascites, edema, anorexia, indigestion, constipation, and abdominal pain. We diagnosed him as 'the retention of pathogenic damp-heat in the liver and gall bladder’ and adminstered Saenggangeonbi-tang three times a day. We observed clinical symptoms, liver function test(AST, ALT, ${\gamma}-GT$, ALP, total bilirubin), nutritional state(total protein, albumin) and capillary blood glucose. After treatment, all the main symptoms disappeared and the patient's nutritional state has improved. Child's grade using the improvement index in liver cirrhosis has changed from B to A. Although the very mechanism of Saenggangeonbi-tang was not proven, we could suggest that Saenggangeonbi-tang is not toxic to the liver and has a good effect on alcoholic liver cirrhosis with diabetes mellitus.

Microscopic Identification of the Chinese Patent Medicine 'Cheong Wi Dan' (청위단의 현미감정 연구)

  • 'Cheong Wi Dan (淸胃丹)' is a Chinese patent preparation which has been used for various diseases in Korea. This preparation consists of 19 powder crude drugs. Cheong Wi Dan is used for catarrh of the gastrointestines, indigestion, a pain in the chest and nausea. The microscopic method is ve교 useful to identify individual gradients in powdery mixture because it requires only a small amount of specimens. In this study, it is demonstrated that the microscopic method is very useful for the identification of 19 crude drug ingredients included in Cheong Wi Dan.

  • Recurrent Pyogenic Cholangitis(RPC) is a distinct clinical entity characterized by recurrent attacks of fever, chills, abdominal pain, and jaundice caused by bacterial infection. In 27, July, 2000, We treated a 40-year-old male patient, complained of these symptoms : severe abdominal pain, fatigue, indigestion, interval chilling. We diagnosed as stagnation of the liver-qi and Qi(肝鬱氣滯), gave herbal medicine composited with Herba Lysimachiae(金錢草), Spora Lygodii(海金沙), Herba Artemisiae Scopariae(茵蔯).etc. After treatment, main symptoms were disappeared and stone of common bile duct was removed. As a consequence, herbal medicine treatment was not toxic to liver function and good effect on RPC induced symptoms.

  • To investigate the documental records of the medi-alcohol theraphy on the Gastrointestinal disease, the 33 kinds of medical books editioned from the old to the present were used. the results were followed Many herbs used on the Gastrointestinal disease were sequently Atractylodis Macrocephalae Rhizoma, Zingiberis Rhizoma, Ginseng Radix. The herbs of invigorating and warming body, regulating the flow of qi etc. were many occupied(used) in treating the Gastrointestinal disease. In analyzing treatment symptom as medi-alcohol theraphy were sequently many used abdominal pain, inappetence, gastrointestinal cancer, indigestion, fatigue, vomiting, diarrhea, abdominal dropsy. The kinds of using medi-alcohol on the Gastrointestinal disease were totally 120 ones.

Clinical Case Studies on Hepatoma Patients with Dietary Therapy (식이요법에 의한 간암환자의 일부증상의 호전)

  • This study was performed to evaluate the effects of high supplementary diet of vitamins, minerals, and polysaccharides on enhancing immunity and preventing from cancer in patients with hepatocellular carcinoma(hepatoma). Dietary therapy for patients was performed with dried yeast, wheat germ. Gonoderma extract, and vegetable juice. The vegetable juice was made of fresh green edible vegetables. Ten patients of total 30 patients were considered as improved state in tumor sizes, AFP, or symptoms. Most of patients complained of weight loss, fatigue, indigestion, anorexia before dietary therapy, but most of symptoms disappeared after dietary therapy. There was no significant side effects because of dietary therapy except for occasional slight diarrhea.

A Study of Nutritional Assessment and Dietary intake after Gastrectomy of Gastric Cancer patients (위암환자의 위절제술 후 영양상태 평가 및 식사섭취도에 관한 연구)

  • The purpose of this study is to investigate the nutritional status and dietary intake of gastrectomized cancer patients. For this study, from 1993. 1 to 1993. 8, 50 postoperative gastric cancer patients were selected to examine anthropometric and laboratory data(Body Weight, Body Fat, serum Albumin, Total Lympocyte count), and dietary intake related symptoms. The results were 1) All anthropometric and laboratory data were significantly deteriorated by gastrectomy(s-Albumin, TLC. Body Fat : p<0.001). Weight loss of gastrectomized patients was 8.23$\pm$3.72% from admission to discharge. 2) In many gastrectomized cancer patients, preoperative dietary intake was decreased by abdominal discomfort, indigestion, early satiety, and anorexia. 3) Postoperative energy intake was 602$\pm$158㎉, and it is correspond to 31.18$\pm$.90% of daily energy requirement(1918$\pm$236㎉). The cause of poor oral intake is mostly fear, abdominal distension and fullness, and early satiety. In consideration of the fact that an inadequate energy intake was the main cause of the decreasing nutritional status, a careful nutritional care and dietary education is necessry after gastrectomy.
