Unilateral cleft lip repair: a comparison of treatment outcome with two surgical techniques using quantitative (anthropometry) assessment

  • Objectives: The unilateral cleft lip (UCL) repair technique has evolved extensively over the past century into its modern form and has been identified as an important determinant of treatment outcome. The aim of this study was to evaluate and compare treatment outcomes following repair of UCL using either the Tennison-Randall (triangular) technique or the Millard rotation-advancement technique. Materials and Methods: This was a prospective randomized controlled study conducted at the Lagos University Teaching Hospital between January 2013 and July 2014. A total of 48 subjects with UCL presenting for primary surgery and who satisfied the inclusion criteria were recruited for the study. The subjects were randomly allocated into two surgical groups through balloting. Group A underwent cleft repair with the Tennison-Randall technique, while group B underwent cleft repair with the Millard rotation-advancement technique. Surgical outcome was assessed quantitatively according to anthropometric measurements, using a method described by Cutting and Dayan (2003). Results: Our 48 enrolled subjects were evenly divided into the two surgery groups (n=24 for both group A and group B). Twenty-seven subjects were male (56.3%) and 21 were female (43.8%), making a sex ratio of 1.3:1. The Millard group showed a greater increase in postoperative horizontal length and vertical lip height and a greater reduction in nasal width and total nasal width. Meanwhile, the Tennison-Randall group showed better reduction of Cupid's-bow width and better philtral height. Conclusion: We did not find any significant differences in the surgical outcomes from the two techniques. The expertise of the surgeon and individual patient preferences are the main factors to consider when selecting the technique for unilateral cleft repair.

An Algorithm for Labia Minora Reduction Based on a Review of Anatomical, Configurational, and Individual Considerations

  • A variety of reduction labiaplasty techniques have been introduced to date, but no single technique will offer the optimal solution for every patient. Rather, the technique should be chosen based on anatomical, configurational, and technical considerations, as well as on patients' personal preferences regarded maintenance of the labial rim, maintenance of labial sensitivity, and prevention of iatrogenic thickening of the labium. We reviewed, defined, and assessed labial configurational variety, neurovascular supply, reduction techniques, and patient's preferences as the considerations relevant to the choice of labiaplasty technique. Based on this review, an algorithm was constructed that leads to a choice of reduction technique through five decisions to be made regarding (1) resection or (partial) retention of the labial free rim, (2) the measure of required labial width reduction, (3) labial vascular status, (4) prevention of iatrogenic labial thickening, and (5) preservation of labial sensibility. The choice of techniques includes edge trimming, central spindle form de-epithelialization or full-thickness resection, and three modifications of the wedge resection or de-epithelialization technique. These three modifications comprised a modified anterior resection or de-epithelialization combined with posterior flap transposition, a custom flask resection or de-epithelialization, and a modified posterior wedge resection or de-epithelialization combined with anterior flap transposition. Use of the five decisional steps and the inclusion of modifications of all three conventional reduction techniques offer an improved algorithm for the choice of labioplasty technique.

Effect of Fodder Tree Species with Condensed Tannin Contents on In vitro Methane Production

  • The objective was to evaluate the effect of fodder tree species (FTS) with condensed tannin contents: Cordia elaeagnoides, Platymiscium lasiocarpum, Vitex mollis, and Haematoxylon brasiletto, on in vitro methane ($CH_4$) production at 24 h post incubation. The analysis was performed using the in vitro gas production technique, with three levels of inclusion/species: 600, 800, and 1,000 mg and with 4 replicates/species/level of inclusion. The substrate was incubated at $39^{\circ}C$, and the gas and $CH_4$ production were recorded at 4, 8, 12, and 24 h post incubation. The data collected was analyzed through Pearson correlation, polinomial regression and fixed effects models. There were negative correlations between FTS-total gas volume (r = -0.40; p<0.001); FTS-volume of $CH_4$ produced (r = -0.40; p<0.001) and between the inclusion level-volume of $CH_4$ produced (r = -0.20; p<0.001). As well as a positive correlation between hours post incubation-total gas volume (r = 0.42; p<0.001) and between hours post incubation-volume of $CH_4$ produced (r = 0.48; p<0.001). The FTS: C. elaeagnoides, V. mollis, and H. brasiletto have potential, in the three inclusion levels analyzed, to reduce $CH_4$ emission on in vitro trials (>32.7%), taking into account the total $CH_4$ production at 24 h of the forage used as reference (Avena sativa). It's suggested that C. elaeagnoides-according to its crude protein, neutral detergent fiber, and condensed tannins content- is the best alternative within the FTS analyzed, for feeding ruminants and for the control of $CH_4$ emissions during the dry season.

  • 최근 전통적인 액체상 공정을 대체하는 기술로서 고체 담체와 단백질 사이의 '생물인식' 기능을 이용하는 새로운 생물공정기술이 개발되고 있다. 통상 고체 담체로는 표면에 특정한 기능기가 노출되어 있는 크로마토그래피용 담체를 사용한다. 단백질의 반응이나 상호작용이 단백질이 담체 표면에 부착되어 있는 상태에서 일어나기 때문에 이 '고체상 기술'은 액체상 기술에 비해 뚜렷한 장점을 갖고 있다. 고체상 재접힘은 변성제에 의해 용해된 내포체 형태의 재조합 단백질을 이온교환수지 표면에 흡착시켜 시작한다. 변성제를 단백질 주위로부터 서서히 제거시키면서 고유의 3차 구조로 재접힘시킨다. 재접힘이 완료되면 염 구배와 같은 전통적인 방법에 의해 재접힘된 단백질을 정제된 상태로 용출시킨다. 이 개념은 '확장층 흡착 재접힘'에도 연장 적용된다. 세포파쇄액에 변성제를 첨가하여 용해한 내포체 단백질은 확장층 흡착 크로마토그래피용 Streamline 담체에 흡착되고 세포찌꺼기와 불순 단백질들은 확장층 사이로 빠져 칼럼 밖으로 제거된다. 흡착된 목적 단백질은 고체상 재접힘 방법에 의해 재접힘 된 후 용출된다. 수년간 연구 발전되어 온이 새로운 재접힘 기술은 정제수율 향상, 공정 단계 감축, 공정 시간 및 부피 감소에 따라 생물의약공정의 경제성을 크게 향상시킬 수 있는 것으로 증명되고 있다. 본 논문에서는 실험실에서 수행한 여러 생물의약용 단백질들을 대상으로 한 연구 실험 자료를 바탕으로 고체상 재접힘 기술의 적용 사례를 서술하였다.

기하정보 기반 이상탐지분석을 이용한 BIM 개별 부재 IFC 분류 무결성 검토에 관한 연구 (Using Geometry based Anomaly Detection to check the Integrity of IFC classifications in BIM Models)

  • Although Industry Foundation Classes (IFC) provide standards for exchanging Building Information Modeling (BIM) data, authoring tools still require manual mapping between BIM entities and IFC classes. This leads to errors and omissions, which results in corrupted data exchanges that are unreliable and thus compromise the validity of IFC. This research explored precedent work by Krijnen and Tamke, who suggested ways to automate the mapping of IFC classes using a machine learning technique, namely anomaly detection. The technique incorporates geometric features of individual components to find outliers among entities in identical IFC classes. This research primarily focused on applying this approach on two architectural BIM models and determining its feasibility as well as limitations. Results indicated that the approach, while effective, misclassified outliers when an IFC class had several dissimilar entities. Another issue was the lack of entities for some specific IFC classes that prohibited the anomaly detection from comparing differences. Future research to improve these issues include the addition of geometric features, using novelty detection and the inclusion of a probabilistic graph model, to improve classification accuracy.

Probabilistic bearing capacity of strip footing on reinforced anisotropic soil slope

  • The probabilistic bearing capacity of a strip footing placed on the edge of a purely cohesive reinforced soil slope is computed by combining lower bound finite element limit analysis technique with random field method and Monte Carlo simulation technique. To simulate actual field condition, anisotropic random field model of undrained soil shear strength is generated by using the Cholesky-Decomposition method. With the inclusion of a single layer of reinforcement, dimensionless bearing capacity factor, N always increases in both deterministic and probabilistic analysis. As the coefficient of variation of the undrained soil shear strength increases, the mean N value in both unreinforced and reinforced slopes reduces for particular values of correlation length in horizontal and vertical directions. For smaller correlation lengths, the mean N value of unreinforced and reinforced slopes is always lower than the deterministic solutions. However, with the increment in the correlation lengths, this difference reduces and at a higher correlation length, both the deterministic and probabilistic mean values become almost equal. Providing reinforcement under footing subjected to eccentric load is found to be an efficient solution. However, both the deterministic and probabilistic bearing capacity for unreinforced and reinforced slopes reduces with the consideration of loading eccentricity.

관절가동범위 증진을 위한 이완 기법의 적절한 수축강도와 수축시간은? (What is the Optimal Contraction Intensity and Duration in the Performance of Relaxation Techniques for Maximal Increase of Range of Motion?)

  • Purpose: The purpose of this study was to review articles in order to establish optimal contraction intensity and duration in the performance of relaxation techniques for maximal increase in range of motion. Methods: The Cochrane, EBSCO, Embase, Medline, ProQuest, PubMed, ScienceDirect, and Scopus databases were used to search articles from 1990 to January 2016. The search terms were "contract relax," "hold relax," "muscle energy technique (MET)," and "proprioceptive neuromuscular facilitation (PNF) stretching." Only experimental human studies (randomized controlled trials) that compared the effects of varying intensity and duration of isometric contraction were included. Non-English language and unpublished studies were excluded. Results: A total of 2,156 articles were initially identified, with only five eventually meeting the inclusion and exclusion criteria. Three studies compared the effects of varying intensity in isometric contraction and two studies compared the effects of varying duration in isometric contraction with regard to range of motion (ROM). Two articles suggested that submaximal voluntary isometric contraction was more effective than maximum voluntary isometric contraction (MVIC) in the improvement of ROM. One article showed that a longer contraction time led to greater increases in ROM. Conclusion: Submaximal voluntary isometric contraction was recommended during contract-relax exercises in healthy people. Lack of evidence makes it difficult to suggest the optimal duration of isometric contraction during relaxation techniques. For future research, high-quality evidence will be needed to establish the optimal contraction intensity for maximum improvement of ROM.

Design and Cloning of the Gene for a Novel Insulin Analogue, $(B^{30}$-Homoserine) Human Insulin

  • In order to prepare a novel human insulin analogue suhbstituted with homoserine at B$^{30}$ / position, (B$^{30}$ /-homoserine) human insulin, a synthetic gene was designed by linking directly a gene for B chain with that for A chain. This gene was constructed by enzymatic joining of 10 different synthetic oligonucleotides, and then inserted at the polylinker region of pUC19 plasmid. To achieve a high level of gene expression, the gene fusion technique region of pUC19 plasmid. To achieve a high level of gene expression, the gene fusion technique was employed using amino terminal regions of lacZ gene up to Clal or hpal, and either of them has been located under tac promoter. The chemical induction of these fused genes by isopropyl-.betha.-D-thiogalactopyranoside (IPTG) gave a satisfactory level of expression in Escherichia coli harboring the ocnstructed plasmids. It was observed that the fused gene product as a single chain insulin precusor was produced more than 30% of total cell protein of E. coli as a form of inclusion body.

무릎굽힘근의 신장성에 대한 수축-이완 기술의 효과 (The Effect of Contract-Relax with Agonist Contraction on Hamstring Extensibility in Normal Subjects)

  • Thirty healthy adults aged 20 to 29 with no history of musculoskeletal or neurogenic disorder volunteered for this study. The contract-relax with agonist contration(CRAC) was applied to the right hamstring muscles with the subject in the supine position. Each hamstring group was stretched on three sucessive days with several repetitions of the technique lasting 1min, 3min, and 5min. respectively. Hamstring extensibility at the knee(ROM) was measured before and after stretching using on electronic digital inclinometer(EDI). The results, namely the increase in ROM, were analysed using one-way repeated ANOVA at p<0.05. The differences were not significant. Possible factors influencing the results are excessive sensitibility of the measuring instrument, the psychological and physical status of the subjects, and the level of muscle fatigue. The mean increases in ROM were 3.0 at 1min., 2.6 at 3min, and 2.1 at 5min. Inclusion of a control group would have further defined the effects of the stretching technique.

Harmonic Current Compensation Using Active Power Filter Based on Model Predictive Control Technology

  • Harmonic current mitigation is vital in power distribution networks owing to the inflow of nonlinear loads, distributed generation, and renewable energy sources. The active power filter (APF) is the current electrical equipment that can dynamically compensate for harmonic distortion and eliminate asymmetrical loads. The compensation performance of an APF largely depends on the control strategy applied to the voltage source inverter (VSI). Model predictive control (MPC) has been demonstrated to be one of the effective control approaches to providing fast dynamic responses. This approach covers different types of power converters due to its several advantages, such as flexible control scheme and simple inclusion of nonlinearities and constraints within the controller design. In this study, a finite control set-MPC technique is proposed for the control of VSIs. Unlike conventional control methods, the proposed technique uses a discrete time model of the shunt APF to predict the future behavior of harmonic currents and determine the cost function so as to optimize current errors through the selection of appropriate switching states. The viability of this strategy in terms of harmonic mitigation is verified in MATLAB/Simulink. Experimental results show that MPC performs well in terms of reduced total harmonic distortion and is effective in APFs.
