Er:YAG 레이저 조사가 산화 티타늄 블라스팅 임플란트 표면 미세 구조 및 거칠기에 미치는 영향 (The effect of Er:YAG laser irradiation on the surface microstructure and roughness of $TiO_2$ implant)

  • Purpose: The aim of this study was to evaluate the effect of Er:YAG laser on microstructure and roughness of $TiO_2$ blasting implant surface. Materials and Methods: Ten $TiO_2$ blasting implant were used in this experiment. One implant was control group, and nine $TiO_2$ blasting implant surfaces were irradiated with Er:YAG laser under 100 mJ/pulse, 140 mJ/pulse, and 180 mJ/pulse condition for 1 min, 1.5 min, and 2 min respectively. Optical interferometer and scanning electron microscopy was utilized to measure roughness and microstructure of specimens. Results: The surface roughness was decreased after Er:YAG laser irradiation in all groups, but there was no significant difference. 100 mJ/pulse and 140 mJ/pulse group did not alter the $TiO_2$ blasting implant surface in SEM study while 180 mJ/pulse group altered the $TiO_2$ blasting implant surface. Implant surfaces showed melting, microfracture and smooth surface in 180 mJ/pulse group. Conclusion: Detoxification of implant surface using Er:YAG laser must be irradiated with proper energy output and irradiation time to prevent implant surface alteration.

Number of implants for mandibular implant overdentures: a systematic review

  • PURPOSE. The aim of this systematic review is to address treatment outcomes of Mandibular implant overdentures relative to implant survival rate, maintenance and complications, and patient satisfaction. MATERIALS AND METHODS. A systematic literature search was conducted by a PubMed search strategy and hand-searching of relevant journals from included studies. Randomized Clinical Trials (RCT) and comparative clinical trial studies on mandibular implant overdentures until August, 2010 were selected. Eleven studies from 1098 studies were finally selected and data were analyzed relative to number of implants. RESULTS. Six studies presented the data of the implant survival rate which ranged from 95% to 100% for 2 and 4 implant group and from 81.8% to 96.1% for 1 and 2 implant group. One study, which statistically compared implant survival rate showed no significant differences relative to the number of implants. The most common type of prosthetic maintenance and complications were replacement or reattaching of loose clips for 2 and 4 implant group, and denture repair due to the fracture around an implant for 1 and 2 implant groups. Most studies showed no significant differences in the rate of prosthetic maintenance and complication, and patient satisfaction regardless the number of implants. CONCLUSION. The implant survival rate of mandibular overdentures is high regardless of the number of implants. Denture maintenance is likely not inflenced substantially by the number of implants and patient satisfaction is typically high again regardless os the number of implants.

Influence of implant-abutment connection structure on peri-implant bone level in a second molar: A 1-year randomized controlled trial

  • PURPOSE. This study aimed to evaluate the effect of two different implant-abutment connection structures with identical implant design on peri-implant bone level. MATERIALS AND METHODS. This clinical study was a patient-blind randomized controlled trial following the CONSORT 2010 checklists. This trial was conducted in 24 patients recruited between March 2013 and July 2015. Implants with internal friction connection were compared to those with external hex connection. One implant for each patient was installed, replacing the second molar. Implant-supported crowns were delivered at four months after implant insertion. Standardized periapical radiographs were taken at prosthesis delivery (baseline), and one year after delivery. On the radiographs, distance from implant shoulder to first bone-to-implant contact (DIB) and peri-implant area were measured, which were the primary and secondary outcome, respectively. RESULTS. Eleven external and eleven internal implants were analyzed. Mean changes of DIB from baseline to 1-year postloading were 0.59 (0.95) mm for the external and 0.01 (0.68) mm for the internal connection. Although no significant differences were found between the two groups, medium effect size was found in DIB between the connections (Cohen's d = 0.67). CONCLUSION. Considering the effect size in DIB, this study suggested the possibility of the internal friction connection structure for more effective preservation of marginal bone.

프랙탈 분석을 통한 임플란트 주변골 평가 (Evaluation of peri-implant bone using fractal analysis)

  • Purpose : The purpose of this study was to investigate whether the fractal dimension of successive panoramic radiographs of bone after implant placement is useful in the characterization of structural change in alveolar bone. Materials and Methods. Twelve subjects with thirty-five implants were retrospectively followed-up from one week to six months after implantation. Thirty-six panoramic radiographs from twelve patients were classified into 1 week, 1-2 months and 3-6 months after implantation and digitized. The windows of bone apical and mesial or distal to the implant were defined as periapical region of interest (ROI) and interdental ROI; the fractal dimension of the image was calculated. Results There was not a statistically significant difference in fractal dimensions during the period up to 6 months after implantation. The fractal dimensions were higher in 13 and 15mm than 10 and 11.5mm implant length at interdental ROIs in 3-6 months after implantation (P<0.01). Conclusion : Longer fixtures showed the higher fractal dimension of bone around implant. This investigation needs further exploration with large numbers of implants for longer follow-up periods.

  • Titanium and titanium alloys are the most common materials used for dental and biomedical implants, owing to their biocompatibility and favourable mechanical properties. However infection of the region surrounding a dental implant by pathogenic microorganisms is a significant factor in implant failure. Prevention and control of microbial colonization of implant surfaces is considerable interest to the biomedical community. One important strategy is to render the implant surface antibacterial by impeding the formation of biofilm. A number of approaches have been proposed for this purpose. Therefore, we reviewed the researches of antibacterial coatings on titanium implants in this articles.

  • Currently increasing use of implants, especially in anterior implant esthetics has become a major concern for both the patient and dentist. In the case of thin biotype if the thickness of the gingival soft tissue is less than 2mm, human eye can detect differences of colors depends on underlying materials. The zirconia abutment can be use not only for better esthetics but also for the hygienic because it is less attractive for the plaque deposition when it compare to the metals. Zirconia itself has many advantages as a biomaterial but also has frequent mechanical problems when it use for abutment of internal connection implant. For prevention or reduction of mechanical failures, use of titanium-link with zirconia super-structure which part that connects directly into the implant can be a good alternative. In this literature, I would like to review the clinical considerations of use of titanium link - CAD/CAM zirconia abutment for dental implant in esthetically important areas.

Fiber Reinforced Framework를 이용한 Implant 수복증례 (Implant Restorations Using Fiber Reinforced Framework)

  • Fiber reinforced materials have favorable mechanical properties. Moreover, the strength to weight ratios of this material is superior to those of most alloys. Comparing to the metals, it showed many other advantages as well, including non-corrosiveness, translucency and easy repair characteristic. Since, it has the potential for the chair-side and laboratory fabrication, it is not surprising that fiber reinforced composites offer the potential for use in various applications in dentistry. To make the well-fitted restorations, Fiber reinforced composite (FRC) has been suggested as an alternative framework material for the implant supported fixed prosthesis. Two fixed partial denture fabrication procedures were tried. Vectris fiber was pressed to the EsthetiCone gold cylinder on the implant positioned cast. And then, Targis were added on it. In the other method, we used the customized component using UCLA abutment. The beads for retaining the Vectris fiber were added on the abutment. If careful laboratory and clinical techniques were done, these two techniques would fulfill the demands of the esthetics and strength.

임프란트 지지 국소의치의 임상적 연구 (Clinical study of implant supported removable partial dentures)

  • OBJECTIVE The purpose of clinical study was to evaluate the survival of dental implants used in restoring patients with implant supported removable partial dentures (ISRPDs) of different configurations. MATERIALS AND METHODS The sample consisted of 20 consecutively treated partially edentulous patients, who, between 2003 and 2008, had a total of 84implants placed in different arch sites and who were treated with ISRPDs. The mean age was 57 years. Mean follow up time from delivery of ISRPDs was 3 years 2 months (range, 1 to 6 years). Osseointegration failure, postoperative complication of dental prosthesis, and the success rate of ISRPDs were retrospectively evaluated using clinical and radiographic examination. RESULT The overall implant survival rate was 100%. During follow up, the one clasp of removable partial denture was broken in 1 year11 months after prosthesis delivery. All patients were satisfied with their prosthesis. CONCLUSIONS Implant supported removable partial dentures could serve as favorable prognosis. Careful patient selection, with an appropriate maintenance and recall system, is recommended to obtain satisfactory results.

Correlation between microleakage and screw loosening at implant-abutment connection

  • PURPOSE. This study aimed to evaluate the correlation between microleakage and screw loosening at different types of implant-abutment connections and/or geometries measuring the torque values before and after the leakage tests. MATERIALS AND METHODS. Three different abutment types (Intenal hex titanium, internal hex zirconium, morse tapered titanium) with different geometries were connected to its own implant fixture. All the abutments were tightened with a standard torque value then the composition was connected to the modified fluid filtration system. After the measurements of leakage removal torque values were re-measured. Kruskal-wallis test was performed for non-parametric and one-way ANOVA was performed for parametric data. The correlation was evaluated using Spearman Correlation Test (${\alpha}=0.05$). RESULTS. Significantly higher microleakage was found at the connection of implant-internal hex zirconium abutment. Observed mean torque value loss was also significantly higher than other connection geometries. Spearman tests revealed a significant correlation between microleakage and screw loosening. CONCLUSION. Microleakage may provoke screw loosening. Removing torque values rationally decrease with the increase of microleakage.

A study on the mechanical properties of TiN/DLC based functionally graded coatings

  • In recent, various functional coatings on artificial tooth implants have been conducted to enhance the bonding strength between implants and bones. Despite of these efforts, some previous reports argued that an adhesion strength between titanium implant and the final coatings like hydroxyapatite(HA) is weaker than the strength between coating and bone. In order to increase the adhesion force between the final coating and implant surface, TiN/DLC based functionally graded coating, which has higher mechanical strength than the titanium implant, was applied as a middle layer between titanium implant and final coating. Particularly we finally coated a biocompatible hydroxyapatite film on the DLC layer and examined the mechanical properties. As a result, TiN/DLC based functionally graded coating showed the higher adhesion strength compared with hydroxyapatite single layer coating on the titanium implant.
