하악 구치부에 식립한 sintered porous surfaced implants의 후향적 다기관 연구 (A Retrospective Study of Sintered Porous-surfaced Dental Implants in Restoring the Edentulous Posterior Mandible: Up to Eight Years of Functioning)

  • Purpose : The aim of this study was to evaluate the survival rate of sintered porous-surfaced implants placed in the edentulous posterior mandibles, in relation to implant length and diameter, crown-to-implant ratio, and types of prostheses, for a maximum of eight years of functioning. Material and Methods : The study group consisted of 43 partially edentulous patients who visited Catholic University Hospital of Daegu and one private dental clinic. A total of 122 sintered porous-surfaced implants n $Endopore^{(R)}$ (Inn ova Life Sciences, Toronto, Ontario, Canada) -- were placed in the edentulous posterior mandibles, Two diameter sizes (4.1 mm and 5.0 mm) and four lengths (5.0 mm, 7.0 mm, 9.0 mm, and 12.0 mm) were used. One hundred and three implants were splinted and 21 implants were nonsplinted. The survival rates of the implants in relation to length, diameter, crown-to-implant ratio, and types of prostheses were investigated. Statistical data were analyzed using SPSS Win.Ver 14.0 software with the Chi-square test. Results : The survival rate of the 4.1mm diameter implants was 100% and 91.2% for the 5.0mm diameter implants. The survival rates of the implants of differing diameters were found to be statistically different (p=0.005). The survival rates of both the 5.0mm and 7.0 mm length implants were 100%. The survival rate of the 9.0mm length implants was 97.9% and for the 12.0mm length implants was 95.1%. There was no statistical difference in survival rates for the differing lengths of implants. Of the 103 prostheses that were splinted, the survival rate was 98.0%. The survival rate of splinted prostheses was higher than that of the non-splinted prostheses, but was found to be not statistically different. There were no failed cases when the crown-to-implant ratio was under 1.0. When the crown-to-implant ratio was between 1.0 and 1.5, the failure rate of the implants was 6.7%. No failure was recorded with the ratio range of 1.5 to 2.0. Relative to the crown-to-implant ratio of 1.0, the failure rates were statistically different (p=0.048). Discussion and Conclusion : The cumulative survival rate of the porous-surfaced implants placed in the edentulous posterior mandibles was 97.5%. Short porous-surfaced implants showed satisfactory results after a maximum of nine years of functioning in the edentulous posterior mandibles.

Implant Therapy and Temporomandibular Disorder

  • Though implant treatment is considered as a common treatment option for edentulous patients, there have been few studies on the temporomandibular disorder (TMD) related with implant treatment. The purposes of this study were to evaluate the relevance of TMD to the implant patients and to evaluate the risk factors of TMD in relation with implant treatment. For the evaluation of various risk factors of TMD in relation with implant therapy, clinical evaluation focused on patient factors and implant factors. From a group of 694 patients, 25 patients (3.6 %) were included in this study. The majority of the patients were included in the asymptomatic 'adaptive' group. Parafunction was detected in 11 patients, 8 patients were male. Four patients having parafunction showed complications such as implant failure or fracture of the suprastructure. From the results, it is possible that TMJ related symptoms are developed or aggravated after implant therapy, which requires relatively more chair time; so TMJ examination should be included in the pre-operative evaluation for dental implant patients. Also, it is important to treat patients after they sign an informed consent that includes a detailed explanation on the possibility of TMD during treatment.

  • The proper occlusal design is one of the most important factors for the long-term success. The purpose of this research is to investigate and define occlusal considerations to reduce failure of implant-supported fixed prostheses. The physiological movement of implants is markedly lower than that of natural teeth and they also lack in occlusal sensitivity. Proper occulsal pattern may be assigned to compensate for the biological disadvantages and occlusal contacts must be formed where the cantilever effect is minimized. Moreover, the long-term success can be assured by reducing early occulsal loading to avoid implant overloading and selecting appropriate occlusion material.

  • Sinus Schneiderian membrane elevation surgery is widely performed for dental implant placement in the maxillary posterior region. With regard to sinus elevation surgery, various complications can occur and lead to implant failure. For successful implants in the maxillary posterior region, the clinician must be well acquainted with sinus anatomy and pathology, a variety of bone graft materials, the principles of sinus elevation surgery, and prevention and management of complications.

최후방 단일치 임플란트의 생존율에 대한 후향적 연구 (A retrospective study on survival rate of the most posterior single tooth implant)

  • Purpose: The purpose of this study was to assess the long term survival rates of the most posterior single tooth implant and to evaluate the influence of implant characteristics on implant survival. Material and Methods: This retrospective report presents findings on 37 patients with 43 implants replacing single molars. The inclusion criteria were having implants replacing a molar of the most posterior region and follow-up data over at least 6 months. Data were recorded regarding the incidence of complications and survival rates of these implants. Results: The range of follow-up was from 9 to 66 months(mean: 40.2 months). The cumulative survival rate of total implants was 93.0% which reflects the loss of three implants: one had broken neck, one implant failed because of infection, one implant showed failed osseointegration. Abutment- screws loosening occurred in five implants(11.6%). Conclusion: Within the limits of this study, a single tooth-implant can serve as a good long-term and predictable treatment modality to replace the most posterior teeth with low complication and failure rates.

임플란트 보철 교합의 임상적 고려 사항 (Clinical considerations for appropriate occlusion of implant restorations)

  • The appropriate occlusion is one of the most important factors for the long-term success of implant and its restorations. The purpose of this review is to investigate and define occlusal considerations to reduce failure of implant prostheses. Proper occlusal pattern may be assigned to compensate for the biological disadvantages and occlusal contacts must be formed where the cantilever effect is minimized. Moreover, the long-term success of implants after osseointegration can be assured by reducing early occlusal loading to avoid implant overloading and selecting appropriate occlusion material. Occlusal overload was brought by the number and location of occlusal contacts, which are under the clinician's control. The concept of implant occlusion is based on the concepts derived from traditional prosthetics. Moreover, there are few evidence on the concept or design of implant occlusion. Several occlusal design was recommended for implant prosthesis. Mutually protected occlusion, group function occlusion and bilateral balance occlusion was recommended for the specific types of implant restorations. This article reviews proper design of occlusion for implant restoration and offers occlusion strategy clinically.

장기간의 유지관리 실패로 인해 파절된 임플란트 피개의치 재수복 증례 (Retreatment of fractured implant overdenture due to long-term maintenance failure)

  • 임플란트 피개의치에서 치주적, 보철적 유지관리는 보철물과 임플란트의 장기적 성공에 매우 중요한 요인이다. 유지관리의 실패는 임플란트 주위 골소실 및 보철물이나 구성품의 파절 및 마모로 인해 보철적 실패를 유발할 수 있다. 이 증례에서는 유지관리 실패로 파절된 임플란트 피개의치의 재치료를 위해 기존의 골드 milled bar를 코발트 크롬 milled bar로 외형에 방해되지 않는 형태로 재제작하였고, 하악의 기존 고정성보철은 임플란트 주변 골흡수와 환자의 전신적 문제로 인해 전략적으로 임플란트를 선택하여 두 개의 임플란트에 CM LOC을 연결하여 임플란트 피개의치를 제작하여 재수복하였다. 결과적으로 환자의 위생관리를 수월하게 하는 디자인의, 기능적 심미적으로 장기간 유지관리에 유리한 보철물로 수복하였고 정기적인 유지관리를 시행하였다.

  • The significance of occlusion has regained its popularity in dentistry with the introduction of implant therapy. Literature has reported that the clinical success and longevity of dental implants can be achieved by biomechanically controlled occlusion. Occlusal overload is known to be one of the main causes for implant failure. Evidences have suggested that occlusal overload contribute to early implant bone loss as well as deosseointegration of successfully integrated implants. Unlike natural teeth, osseointegrated implants are ankylosed to surrounding bone without the periodontal ligament (PDL) which provides mechanoreceptors as well as shock-absorbing function. Moreover, the crestal bone around dental implants may act as a fulcrum point for lever action when a force (bending moment) is applied, indicating that implants/implant prosthesis could be more susceptible to crestal bone loss by applying force. Hence, it is essential for clinicians to understand inherent differences between teeth and implants and how force, either normal or excessive force, may influence on implants under occlusal loading. The purposes of this paper are to review the importance of implant occlusion, to establish the optimum implant occlusion with biomechanical rationale, to provide clinical guidelines of implant occlusion and to discuss how to manage complications related to implant occlusion.

Long-term cumulative survival and mechanical complications of single-tooth Ankylos Implants: focus on the abutment neck fractures

  • PURPOSE. To evaluate the cumulative survival rate (CSR) and mechanical complications of single-tooth $Ankylos^{(R)}$ implants. MATERIALS AND METHODS. This was a retrospective clinical study that analyzed 450 single $Ankylos^{(R)}$ implants installed in 275 patients between December 2005 and December 2012. The main outcomes were survival results CSR and implant failure) and mechanical complications (screw loosening, fracture, and cumulative fracture rate [CFR]). The main outcomes were analyzed according to age, sex, implant length or diameter, bone graft, arch, and position. RESULTS. The 8-year CSR was 96.9%. Thirteen (2.9%) implants failed because of early osseointegration failure in 3, marginal bone loss in 6, and abutment fracture in 4. Screw loosening occurred in 10 implants (2.2%), and 10 abutment fractures occurred. All abutment fractures were located in the neck, and concurrent screw fractures were observed. The CSR and rate of screw loosening did not differ significantly according to factors. The CFR was higher in middle-aged patients (5.3% vs 0.0% in younger and older patients); for teeth in a molar position (5.8% vs 0.0% for premolar or 1.1% for anterior position); and for larger-diameter implants (4.5% for 4.5 mm and 6.7% for 5.5 mm diameter vs 0.5% for 3.5 mm diameter) (all P<.05). CONCLUSION. The $Ankylos^{(R)}$ implant is suitable for single-tooth restoration in Koreans. However, relatively frequent abutment fractures (2.2%) were observed and some fractures resulted in implant failures. Middle-aged patients, the molar position, and a large implant diameter were associated with a high incidence of abutment fracture.

임플란트 구성요소의 파절면에 관한 주사전자현미경적 연구 - Part I: 임플란트 고정체 (Fracture Analysis of Implant Components using Scanning Electron Microscope - Part I : Implant Fixture)

  • 보철물의 실패는 파절로 인해 다수 발생하게 되지만 파절 발생시 그 원인을 파악하는 것은 어렵다. 보철물의 실패를 예방하고 예후를 예측하기 위해 보철물의 원인을 분석하는 것이 중요하며, 원인을 밝히기 위해 파절면 분석을 시행하게 된다. 파절면 분석은 파절면 뿐 아니라 주위 환경(응력 상황)에 대한 분석이 동반되며, 이를 이용하여 균열 진행, 파절 양상, 파절 원인 등을 파악하게 된다. 이 연구의 목적은 임상적으로 기능 후 파절된 임플란트 고정체의 파절면 분석을 시행하여 파절 기전 및 파절 원인(하중 양상)을 밝히는 것이다. 파절된 임플란트 고정체는 3년간 강릉-원주 대학교에 임플란트 고정체의 파절을 주소로 내원한 환자를 대상으로 수집하였다. 먼저 임상 및 방사선 사진 분석을 하였으며, 시편 세척 과정을 거쳐 주사 전자 현미경을 이용한 파절면 분석을 시행하였다. 임플란트 파절면 분석 시 피로 줄무늬, 벽개 파절 등의 파절 지표를 통해 피로 파절로 인해 파절이 발생되었음을 확인할 수 있었다.
