Effects of Iterative Reconstruction Algorithm, Automatic Exposure Control on Image Quality, and Radiation Dose: Phantom Experiments with Coronary CT Angiography Protocols (반복적 재구성 알고리즘과 관전류 자동 노출 조정 기법의 CT 영상 화질과 선량에 미치는 영향: 관상동맥 CT 조영 영상 프로토콜 기반의 팬텀 실험)

  • In this study, we investigated the effects of an iterative reconstruction algorithm and an automatic exposure control (AEC) technique on image quality and radiation dose through phantom experiments with coronary computed tomography (CT) angiography protocols. We scanned the AAPM CT performance phantom using 320 multi-detector-row CT. At the tube voltages of 80, 100, and 120 kVp, the scanning was repeated with two settings of the AEC technique, i.e., with the target standard deviations (SD) values of 33 (the higher tube current) and 44 (the lower tube current). The scanned projection data were reconstructed also in two ways, with the filtered back projection (FBP) and with the iterative reconstruction technique (AIDR-3D). The image quality was evaluated quantitatively with the noise standard deviation, modulation transfer function, and the contrast to noise ratio (CNR). More specifically, we analyzed the influences of selection of a tube voltage and a reconstruction algorithm on tube current modulation and consequently on radiation dose. Reduction of image noise by the iterative reconstruction algorithm compared with the FBP was revealed eminently, especially with the lower tube current protocols, i.e., it was decreased by 46% and 38%, when the AEC was established with the lower dose (the target SD=44) and the higher dose (the target SD=33), respectively. As a side effect of iterative reconstruction, the spatial resolution was decreased by a degree that could not mar the remarkable gains in terms of noise reduction. Consequently, if coronary CT angiogprahy is scanned and reconstructed using both the automatic exposure control and iterative reconstruction techniques, it is anticipated that, in comparison with a conventional acquisition method, image noise can be reduced significantly with slight decrease in spatial resolution, implying clinical advantages of radiation dose reduction, still being faithful to the ALARA principle.

Artifactual Perfusion Defects due to the Parameters of Reconstruction Filter in Tc-99m-MIBI Myocardial SPECT Images (Tc-99m-MIBI 심근 SPECT 영상에서 재구성 필터에 의한 인위적 관류결손에 관한 연구)

  • Tc-99m-MIBI(Sestamibi) myocardial SPECT along with T1-201 tomographic imaging has demonstrated wide application and high image qualify sufficient for the diagnosis of myocardial perfusion defect, which consequently reflects regional myocardial blood flow, The qualitative values of myocardial SPECT with Tc-99m-MIBI as well as the quantitative cases depend in some degree on the reconstruction techniques of multiple projections. Filtered backprojection(FBP) is the common standard for reconstruction rather than the complicated and time-consuming arithmetic methods. In FBP it Is known that the distribution of radioactivity in reconstructed transverse slices varies with the selected filter parameters such as cutoff frequencies and order(Butterworth case). The cutoff frequencies basically remove and decrease the true radioactive distribution and alter the pixel counts, which lead to underestimation of true counts in specific myocardial regions. In this study, we have investigated the effect of cutoff frequencies of reconstruction filter on the artifactually induced perfusion defects, which are often demonstrated near inferior and/or inferoseptal cardiac walls due to the intense hepatic uptake of Tc-99m-MIBI. A computerized method for identifying the relative degree of artifactual perfusion defect and for comparing those degrees along with the relative amount of hepatic uptake to myocardium was developed and patient images were studied to observe the quantitative degree of underestimation of myocardial perfusion, and to propose some reasonable thresh-old of cutoff frequency in the diagnosis of perfusion defect quantitatively. We concluded that from the quantitative viewpoint cutoff frequencies may be used as high as possible with the sacrifice of homogeneity of image quality, and those frequencies lower than the common 0.3 Nyquist frequency would reveal severe degradation of radio-active distribution near inferior and/or Inferoseptal myocardium when applying Butterworth or low pass filter.

Application of Borehole Radar to Tunnel Detection (시추공 레이다 탐사에 의한 지하 터널 탐지 적용성 연구)

  • The borehole radar methods used to tunnel detection are mainly classified into borehole radar reflection, directional antenna, crosshole scanning, and radar tomography methods. In this study, we have investigated the feasibility and limitation of each method to tunnel detection through case studies. In the borehole radar reflection data, there were much more clear diffraction signals of the upper wings than lower wings of the hyperbolas reflected from the tunnel, and their upper and lower wings were spreaded out to more than 10m higher and lower traces from the peaks of the hyperbolas. As the ratio of borehole diameter to antenna length increases, the ringing gets stronger on the data due to the increase in the impedance mismatching between antennas and water in the boreholes. It is also found that the reflection signals from the tunnel could be enhanced using the optimal offset distance between transmitter and receiver antennas. Nevertheless, the borehole radar reflection data could not provide directional information of the reflectors in the subsurface. Direction finding antenna system had a advantage to take a three dimensional location of a tunnel with only one borehole survey even though the cost is still very high and it required very high expertise. The data from crosshole scanning could be a good indicator for tunnel detection and it could give more reliable result when the borehole radar reflection survey is carried out together. The images of the subsurface also can be reconstructed using travel time tomography which could provide the physical property of the medium and would be effective for imaging the underground structure such as tunnels. Based on the results described above, we suggest a cost-effective field procedure for detection of a tunnel using borehole radar techniques; borehole radar reflection survey using dipole antenna can firstly be applied to pick up anomalous regions within the borehole, and crosshole scanning or reflection survey using directional antenna can then be applied only to the anomalous regions to detect the tunnel.

  • An anterior cruciate ligament (ACL) reconstruction is one of the most frequent surgical procedures in the knee joint, but despite the better understanding of anatomy and biomechanics, surgical reconstruction procedures still fail to restore rotational stability in 7%-16% of patients. Hence, many studies have attempted to identify the factors for rotational laxity, including the anterolateral ligament (ALL), but still showed controversies. Descriptions of the ALL anatomy are also confused by overlapping nomenclature, but it is usually known as a distinctive fiber running in an anteroinferior and oblique direction from the lateral epicondyle of the femur to the proximal anterolateral tibia, between the fibular head and Gerdy's tubercle. The importance of the ALL as a secondary restraint in the knee has been emphasized for successful ACL reconstructions that can restore rotational stability, but there is still some controversy. Some studies reported that the ALL could be a restraint to the tibial rotation, but not to anterior tibial translation. On the other hand, some studies reported that the role of ALL in rotational stability would be limited as a secondary structure because it bears loads only beyond normal biomechanical motion. The diagnosis of an ALL injury can be performed by a physical examination, radiology examination, and magnetic resonance imaging, but it should be assessed using a multimodal approach. Recently, ALL was considered one of the anterolateral complex structures, as well as the Kaplan fiber in the iliotibial band. Many studies have introduced many indications and treatment options, but there is still some debate. The treatment methods are introduced mainly as ALL reconstructions or lateral extra-articular tenodesis, which can achieve additional benefit to the knee stability. Further studies will be needed on the indications and proper surgical methods of ALL treatment.

Modeling of Vegetation Phenology Using MODIS and ASOS Data (MODIS와 ASOS 자료를 이용한 식물계절 모델링)

  • Recently, the seriousness of climate change-related problems caused by global warming is growing, and the average temperature is also rising. As a result, it is affecting the environment in which various temperature-sensitive creatures and creatures live, and changes in the ecosystem are also being detected. Seasons are one of the important factors influencing the types, distribution, and growth characteristics of creatures living in the area. Among the most popular and easily recognized plant seasonal phenomena among the indicators of the climate change impact evaluation, the blooming day of flower and the peak day of autumn leaves were modeled. The types of plants used in the modeling were forsythia and cherry trees, which can be seen as representative plants of spring, and maple and ginkgo, which can be seen as representative plants of autumn. Weather data used to perform modeling were temperature, precipitation, and solar radiation observed through the ASOS Observatory of the Korea Meteorological Administration. As satellite data, MODIS NDVI was used for modeling, and it has a correlation coefficient of about -0.2 for the flowering date and 0.3 for the autumn leaves peak date. As the model used, the model was established using multiple regression models, which are linear models, and Random Forest, which are nonlinear models. In addition, the predicted values estimated by each model were expressed as isopleth maps using spatial interpolation techniques to express the trend of plant seasonal changes from 2003 to 2020. It is believed that using NDVI with high spatio-temporal resolution in the future will increase the accuracy of plant phenology modeling.

Evaluation of waterlogging tolerance using chlorophyll fluorescence reaction in the seedlings of Korean ginseng (Panax ginseng C. A. Meyer) accessions (엽록소 형광반응을 이용한 인삼 유전자원의 습해 스트레스 평가)

  • Measuring chlorophyll fluorescence (CF) is a useful tool for assessing a plant's ability to tolerate abiotic stresses such as drought, waterlogging and high temperature. Korean ginseng is highly sensitive to water stress in paddy fields. To evaluate the possibility of non-destructively diagnosing waterlogging stress using chlorophyll fluorescence (CF) imaging techniques, we screened 57 ginseng accessions for waterlogging tolerance. To evaluate waterlogging tolerance among the 2-year-old Korean ginseng accessions, we treated ginseng plants with water stress for 25 days. The physiological disorder rate was characterized through visual assessment (an assigned score of 0-5). The physiological disorder rates of Geumjin, Geumsun and GS00-58 were lower than that of other accessions. In contrast, lines GS97-62, GS97-69 and GS98-1-5 were deemed susceptible. Root traits, chlorophyll content and the reduction rates decreased in most ginseng accessions. Further, these metrics were significantly lower in susceptible genotypes compared to resistant ones. All CF parameters showed a positive or negative response to waterlogging stress, and this response continuously increased over the treatment time among the genotypes. The CF parameter Fv/Fm was used to screen the 57 accessions, and the results showed clear differences in Fv/Fm between the susceptible and resistant genotypes. Susceptible genotypes had an especially low Fv/Fm value of less than 0.8, reflecting damage to the reaction center of photosystem II. It is concluded that Fv/Fm can be used as a CF parameter index for screening waterlogging stress tolerance in ginseng genotypes.

Arthroscopic Full-Thickness Rotator Cuff Repair in Elderly Patients (고령 환자의 관절경적 회전근 개 봉합술의 결과)

  • Purpose: To examine the clinical and structural outcomes of an at least two-year follow-up of arthroscopic full-thickness rotator cuff repairs with a single-row or suture-bridge technique in patients more than 65 years of age. Materials and Methods: Patients diagnosed with a full-thickness rotator cuff tear who were more than 65 years of age, underwent arthroscopic rotator cuff repair after at least six months of conservative treatment, agreed to take a follow-up magnetic resonance imaging (MRI) six months postoperatively, and visited outpatient for at least two years were enrolled in this study. Clinical evaluations were done using The University of California Los Angeles score, Constant Shoulder Score, and visual analogue scale evaluated two years after the surgery. The structural integrity was analyzed using follow-up MRI. During surgery, a suture-bridge technique was used if the rotator cuff tendon could cover half of the footprint under constant tension. Otherwise, single-row repair was performed. Results: The samples were 158 cases, consisting of 93 single-repairs and 65 suture-bridge repairs. A preoperative comparison of the age distribution, fatty degeneration of supraspinatus and infraspinatus muscle, medial retraction of torn cuff tendon, and tear size between the two groups were not significant. The clinical scores were improved significantly in all cases. The distribution of the structural integrity by Sugaya classification were 49 cases in type 1 (31.0%), 62 cases in type 2 (39.2%), 30 cases in type 3 (19.0%), 11 cases in type 4 (7.0%), and six cases in type 5 (3.8%). The re-tear rate of the single-row group was 9.7% (nine out of 93 cases) and 12.3% (eight out of 65 cases) for the suture-bridge group. Conclusion: Satisfactory clinical and radiological outcomes were achieved after arthroscopic full-thickness rotator cuff repair in patients more than 65 years of age. Both single-row and suture-bridge techniques would be beneficial for the elderly.

An event-related potential study of global-local visual perception in female college students with binge drinking (폭음 여자대학생의 전체-세부 시지각 처리에 관한 사건관련전위 연구)

  • It is reported that binge drinkers show cognitive impairment similar to alcohol use disorder patients. A previous studies using neuropsychological tests and brain imaging techniques to investigate the visual perception of alcohol use disorder patients reported that they had global-local visual perception defects. Although the neurological basis for the global-local visual perception deficit in the heavy drinking group has been presented, there are no studies to date that have investigated the global-local visual perception in the heavy drinking group. This study investigated local-biased visual perception in female college students with binge drinking (BD) using event-related potentials (ERPs). Based on the scores of the Korean version of Alcohol Use Disorder Identification Test and the Alcohol Use Questionnaire, participants were assigned into BD (n=25) and non-BD (n=25) groups. Local-global visual processing was assessed using a local-global paradigm, in which large stimuli (global level) composed of small stimuli (local level) were presented. The stimuli presented at global and local levels were either congruent or incongruent. The behavioral results exhibited that the BD and non-BD groups did not differ in terms of accuracy and response time. In terms of ERPs, the BD and non-BD groups did not show difference in N100, P150 and N200 amplitude. However, the BD group showed significantly smaller P300 amplitude than non-BD group especially in the local condition. In addition, a negative correlation between P300 amplitude and binge drinking score was observed, i.e., severer binge drinking smaller P300 amplitude. The P300 is known to reflect cognitive inhibition and attentional allocation. In the global-local paradigm, the local condition required to attend to local target while ignoring global non-target. Therefore, the present results indicate that female college students with BD do not have local-biased visual processing, instead they seem to have difficulties in inhibition of irrelevant stimuli.

Radiation Dose Reduction in Digital Mammography by Deep-Learning Algorithm Image Reconstruction: A Preliminary Study (딥러닝 알고리즘을 이용한 저선량 디지털 유방 촬영 영상의 복원: 예비 연구)

  • Purpose To develop a denoising convolutional neural network-based image processing technique and investigate its efficacy in diagnosing breast cancer using low-dose mammography imaging. Materials and Methods A total of 6 breast radiologists were included in this prospective study. All radiologists independently evaluated low-dose images for lesion detection and rated them for diagnostic quality using a qualitative scale. After application of the denoising network, the same radiologists evaluated lesion detectability and image quality. For clinical application, a consensus on lesion type and localization on preoperative mammographic examinations of breast cancer patients was reached after discussion. Thereafter, coded low-dose, reconstructed full-dose, and full-dose images were presented and assessed in a random order. Results Lesions on 40% reconstructed full-dose images were better perceived when compared with low-dose images of mastectomy specimens as a reference. In clinical application, as compared to 40% reconstructed images, higher values were given on full-dose images for resolution (p < 0.001); diagnostic quality for calcifications (p < 0.001); and for masses, asymmetry, or architectural distortion (p = 0.037). The 40% reconstructed images showed comparable values to 100% full-dose images for overall quality (p = 0.547), lesion visibility (p = 0.120), and contrast (p = 0.083), without significant differences. Conclusion Effective denoising and image reconstruction processing techniques can enable breast cancer diagnosis with substantial radiation dose reduction.

Patterns in the Use and Perception of Digital Breast Tomosynthesis: A Survey of Korean Breast Radiologists (디지털 유방 토모신테시스에 대한 국내 사용 현황과 인식에 관한 설문조사 연구)

  • Purpose To evaluate the pattern of use and the perception of digital breast tomosynthesis (DBT) among Korean breast radiologists. Materials and Methods From March 22 to 29, 2021, an online survey comprising 27 questions was sent to members of the Korean Society of Breast Imaging. Questions related to practice characteristics, utilization and perception of DBT, and research interests. Results were analyzed based on factors using logistic regression. Results Overall, 120 of 257 members responded to the survey (response rate, 46.7%), 67 (55.8%) of whom reported using DBT. The overall satisfaction with DBT was 3.31 (1-5 scale). The most-cited DBT advantages were decreased recall rate (55.8%), increased lesion conspicuity (48.3%), and increased cancer detection (45.8%). The most-cited DBT disadvantages were extra cost for patients (46.7%), insufficient calcification characterization (43.3%), insufficient improvement in diagnostic performance (39.2%), and radiation dose (35.8%). Radiologists reported increased storage requirements and interpretation time for barriers to implementing DBT. Conclusion Further improvement of DBT techniques reflecting feedback from the user's perspective will help increase the acceptance of DBT in Korea.
