  • A novel approach for modeling the thermic endfire interstitial applicator is presented. A hypothetical semi-infinite circular cylinder is added in the endfire direction in order to facilitate the theoretical modeling approach. The Fourier transform and mode-matching technique is utilized to obtain a solution in fast-convergent series Numerical computations for the input impedance are performed to check the Validity of the theoretical model.

  • Studies on drug delivery using nano-size particles of magnetic fluid and hyperthermia have been performed by some researchers [1] because interests in human health increased according to industry development. However, there are few studies on systems which can accurately control delivery of the magnetic fluids to a diseased part of body [2]. In this study, Cu-added magnetic ferrofluid was prepared and the external magnetic field system was designed for drug localization.

The Effectiveness of Infrared Thermography in Patients with Whiplash Injury

  • Objective : This study aims to visualize the subjective symptoms before and after the treatment of whiplash injury using infrared (IR) thermography. Methods : IR thermography was performed for 42 patients who were diagnosed with whiplash injury. There were 19 male and 23 female patients. The mean age was 43.12 years. Thermal differences (${\Delta}T$) in the neck and shoulder and changes in the thermal differences (${\Delta}dT$) before and after treatment were analyzed. Pain after injury was evaluated using visual analogue scale (VAS) before and after treatment (${\Delta}VAS$). The correlations between ${\Delta}dT$ and ${\Delta}VAS$ results before and after the treatment were examined. We used Digital Infrared Thermal Imaging equipment of Dorex company for IR thermography. Results : The skin temperature of the neck and shoulder immediately after injury showed $1-2^{\circ}C$ hyperthermia than normal. After two weeks, the skin temperature was normal range. ${\Delta}T$ after immediately injuy was higher than normal value, but it was gradually near the normal value after two weeks. ${\Delta}dT$ before and after treatment were statistically significant (p<0.05). VAS of the neck and shoulder significantly reduced after 2 week (p=0.001). Also, there was significant correlation between ${\Delta}dT$ and reduced ${\Delta}VAS$ (the neck; r=0.412, p<0.007) (the shoulder; r=0.648, p<0.000). Conclusion : The skin temperature of sites with whiplash injury is immediately hyperthermia and gradually decreased after two weeks, finally it got close to normal temperature. These were highly correlated with reduced VAS. IR thermography can be a reliable tool to visualize the symptoms of whiplash injury and the effectiveness of treatment in clinical settings.

Systematic Review and Meta-Analyses of Diagnostic Accuracy of Infrared Thermometer when Identifying Fever in Children (아동에서의 적외선 체온 측정의 진단정확도 연구 : 체계적 문헌 고찰과 메타분석)

  • Purpose: Infrared thermometers are increasingly used as a convenient, non-invasive assessment method for febrile children. However, the diagnostic accuracy of the infrared thermometer for children has been questioned, particularly in relation to sensitivity and specificity. The aim of this study was to evaluate diagnostic accuracy of infrared thermometers in febrile children. Methods: Articles published between 1966 and 2012 from periodicals indexed in the Ovid Medline, Embase, CINAHL, Cochrane, KoreaMed, NDSL, KERIS and other databases were selected, using the following keywords: 'infrared thermometer'. The QUADAS-II was applied to assess the internal validity of the diagnostic studies. Selected studies were analyzed using meta-analysis with MetaDisc 1.4. Results: Nineteen diagnostic studies with high methodological quality, involving 4,304 children, were included. The results of meta-analysis showed that the pooled sensitivity, specificity and AUC (Area Under the Curve) of infrared tympanic thermometers in children over 1 year were 0.80 (95% CI 0.78, 0.81), 0.94 (95% CI 0.93, 0.95) and 0.95 respectively. However the diagnostic accuracy of infrared tympanic thermometers in children with hyperthermia was low. Conclusion: The diagnostic accuracy of infrared tympanic thermometer was similar to axillary and rectal thermometers indicating a need for further research to substantiate these findings in children with hyperthermia.

Induction of Apoptosis by a Combination of Paclitaxel and Carboplatin in the Presence of Hyperthermia

  • Purpose: To study enhancing effects of paclitaxel in the thermochemotherapy of osteosarcoma cell lines and related mechanisms. Materials and Methods: Paclitaxel and carboplatin were used alone or jointly on OS732 cell lines in the presence of hyperthermia. Inhibition of proliferation was measured by MTT assay and cellular changes were assessed with inverted phase contrast and fluorescence microscopy. Apoptosis was analyzed with flow cytometry (FCM) and Fas expression by immunocytochemistry. Results: At $43^{\circ}C$, one hour after the application of 10ug/ml paclitaxel and $5{\mu}g/ml$ carboplatin on OS732 cells jointly, the survival rate was 15.8% which was significantly lower than with $10{\mu}g/ml$ paclitaxel (45.8%) and $5{\mu}g/ml$ carboplatin (47.7%) respectively (P<0.01). Moreover, changes of morphology and apoptotic rates indicated that the apoptosis-inducing effect of combined application was also much enhanced, as evident also regarding Fas expression. Conclusion: Paclitaxel is conducive to thermochemotherapy of osteosarcoma cell lines, possibly accomplished by up-regulation of Fas expression with induction of apoptosis.

  • Magnetic hyperthermia mediated by superparamagnetic particles is mainly based in sinusoidal waveforms as excitation signals. Temperature changes are conventionally explained by rotation of the particles in the surrounding medium. This is a hypothesis quite questionable since habitual experimental setups only produce changes in the magnetic module, not in the field lines trajectories. Theoretical results were tested by changing the waveform of the exciting signal in order to compare non-sinusoidal signals against sinusoidal signals. Experiments were done at different frequencies: 200 KHz, 400 KHz, 600 KHz, 800 KHz and 1 MHz. Superparamagnetic Iron Oxide samples (SPION), made of magnetite ($Fe_3O_4$) and suspended in water (100 mg/ml), were used. Magnetic field strength varies from $0.1{\pm}0.015KA/m$ to $0.6{\pm}0.015KA/m$. In this study was observed that the power loss depends on the applied frequency: for 1 to 2.5 RMS current the responses for each signal are part of the higher section of the exponential function, and for 3.5 to 8 RMS current the response is clearly the decrement exponential function's tale (under $1{\times}10^3LER/gr$).

Biologic rationale of cancer treatment with hyperthermia (고온온열치료장치를 사용한 종양치료의 생물학적 원리)

  • 고온온열치료는 radiofrequency, ultrasound , microwave, 원적외선 등을 이용하여 신체의 부분 혹은 전신을 4$0^{\circ}C$ 이상으로 가열하여 암을 치료하는 방법이다. 우리나라에 도 입된 15기의 기계중 현재까지 사용되고 있는 것은 대부분이 radiofrequency를 사용하는 기 계이며 현재 고신의대, 동아의대, 부산메리놀병원, 여의도 성모병원, 영남의대, 전주예수병원 등에서 환자치료에 사용하고 있다. 고온 온열치료제(hyperthermia)는 직접 암세포를 죽이는 작용, 방사선치료나 항암제치료와 병행하여 그 효과를 증강시키는 작용으로 크게 나눌수 있 다. 직접 암세포를 죽이기 위하여는 43$^{\circ}C$이상의 고온을 사용하여야 하나 인체에서는 42.5$^{\circ}C$ 이상으로 가온하기가 쉽지않아 4$0^{\circ}C$~42$^{\circ}C$ 정도의 온도에서 방사선 치료나 항암제 치료효과 를 증진시키는 작용을 임상에서 주로 사용하고 있다. 특히 방사선 치료와 병합 사용시 그 효과가 뛰어나 간암, 난소암, 대장 직장암, 식도암, 위암, 자궁암, 전립선안, 췌장암, 폐암등, 거의 모든 암에서 부작용을 증가시키지 않으면서 그 효율을 1.1-6.14배나 증가시킨다고 보 고되고 있어 지난 10여 년간 제자리걸음을 하고 있는 암의 치료에 희망을 주고 있다. 방사 선 치료와 병합시 효과를 증대시키는 기전은 1)세포의 핵 합성기 (S-phase)는 방사선 치료 에는 매우 저항력이 강하여 잘 죽지 않으나 고온온열치료에는 예민함으로 암세포는 정상조 직에 비해 산소가 부족하여 염기성대사(anaerobic metabolism)를 많이 함으로 그 부산물인 유산 (lactic acid)이 많이 생성됨으로 정상조직보다 pH가 낮아 암 조직이 정상조직에 비해 고온온열치료에 더 잘 듣는 원인이 된다. 3) 영양이 부족한 상태의 세포는 고온온열치료에 훨씬 예민하다. 4) 암조직은 혈관상태가 정상조직에 비해 좋지 않음으로 정상조직보다 쉽게 가온이 되며, 일단 가온된 온도는 잘 식지 않음으로 정상조직에 비해 훨씬 효율적이다. 5)고 온온열치료는 4$0^{\circ}C$~43.5 $^{\circ}C$정도에서만 이 작용이 일어남으로 정상인체에서 43$^{\circ}C$이상의 가온 은 쉽지 않음으로 이 효과는 암조직에서 주고 일어나게 된다. 6)고온온열치료는 방사선치료 후에 생기는 손상의 재생을 억제함으로 방사선의 치료효과를 높인다. 7)38.5$^{\circ}C$~41.5$^{\circ}C$의 낮 은 온도에서도 암조직의 산소 상태를 호전시켜 방사선 치료효과를 증대시키는 역할을 한다.

Anticancer Efficacies of Doxorubicin, Verapamil and Quercetin on FM3A Cells under Hyperthermic Temperature

  • Hyperthermia (HT) in combination with anticancer drugs (ACDs) had proven to more efficacious in various cancers, although efficacies vary according to chemotherapeutic compounds and cancer types. Presently there are few data that compares anticancer efficacies among ACDs under hyperthermic conditions. Therefore, we selected three commonly used ACDs (quercetin, verapamil and doxorubicin) and compared their antitumor effects when each was treated with 43$^{\circ}C$ HT exposure. Firstly, FM3A, a murine breast cancer cell line, was treated with each ACD for 1 h followed by 43$^{\circ}C$ exposure for additional 1 h, and examined the effects of: 1) each drug, 2) 43$^{\circ}C$ HT exposure, and 3) the combination of each drug and 43$^{\circ}C$ HT exposure for 1, 6 and 24 h. The determined overall effects on FM3A cells were arrested cell proliferation, clonogenic efficiency and apoptosis. Pre-treatment of FM3A cells to each ACD followed by 43$^{\circ}C$ HT exposure produced greater antitumor effects including suppressed cell proliferation, reduced clonogenic efficiency and increased apoptotic cell death, compared to ACD treatment or HT exposure alone. Apoptotic cell death occurred in a time-dependent manner. Among the ACDs, antitumor efficacies varied in the order of doxorubicin > verapamil > quercetin. It was concluded that heat exposure during ACD treatment of caner cells may be an important factor to get a better antitumor benefit, even though this benefit may differ from one drug to another.

Temperature distribution in VX-2 hepatoma heated with thermoseed hyperthermia (열소자 온열요법시 VX-2 hepatoma내의 온도 변화에 대한 연구)

  • It was the purpose of present study to develop a new thermoseed for heating deep-seated tumors and assessment of the effect of magnetic control on thermoseeds. Aqueous suspension of iron micro spheres (Ferropolysaccharide) was injected directly into the VX-2 hepatoma and heated with 1.2 MHz inductive radiofrequency unit. Aqueous thermoseed suspension was delivered to the tumor by simple percutaneous injection. The limitation of the thermoseed heating method is the positional change of thermoseed particles in the tumor after implantation. The thermoseed particles could enter the systemic blood circulation and cause a severe embolization of a critical organ. To minimize this limitation, we have used the magnetic control after loading the thermoseed in the tumor, W hen ferropolysaccharides were exposed to a strong magnetic field, they magnetized and subsequently exerted a magnetic force on each other, forming larger aggregates of particles. The size of aggregated Particles were too big to enter the systemic blood circulation. Thus, unlike other thermoseed method, we hold the thermoseed particles stationary in the tumor. The temperature of the injected site and immediate vicinity elevated by $4-5^{\circ}C$. The temperature of the surrounding normal hepatic tissue elevated by $1-2^{circ}C$ only. The heating effect within the tumor was variable depending on the density of ferromagnetic aqueous suspension. Our results suggest that inductive heating of tumor injected with ferropolysaccharide solution offers the possibility of effective heat delivery to the defined tumor volume, which is difficult to heat with other heating devices.
