고혈압 의료비 지역 간 변이 및 변이 요인 분석 (Analysis on geographic variations and variational factors in expenditures for hypertension)

  • 본 연구는 우리나라의 고혈압 의료비의 지역별 변이와 지역의 사회학적, 의료공급, 건강행태 등의 변이 요인을 규명함으로써 이를 기반으로 지역별 고혈압 관리 사업 방안 마련에 기초자료를 제시하는 것이다. 이를 위해 전국 시 군 구 247개 지역을 대상으로, 2012년 지역별 의료이용 통계자료(국민건강보험공단), 지역사회건강조사(질병관리본부) 및 국토해양부 자료를 이용 분석하였다. 연구결과 전국 247개 시 군 구의 고혈압 건강보험진료비는 평균 6만2천원이었고 변동계수는 30.0으로 지역 간 높은 고혈압 의료비 변이를 보였다. 주요 변이 요인으로는 인구밀도, 유배우자율, 평균가구원수입, 인구십만명당병원수, 관외의료비비율, 월간음주율, 중증도이상신체활동실천율, 평생의사진단율 등으로 나타났다. 의사결정나무를 이용한 분석 결과 평생의사진단율, 평균가구원수입, 유배우자율, 인구십만명당병원수, 비만율, 월간음주율에서 유의한 차이가 있었다. 본 연구의 결과 고혈압 의료비의 지역 간 변이요인으로는 의료공급이나 인구사회학적 특성뿐만 아니라 건강행태에 있음을 확인하였고 이는 고혈압의료비 절감을 위한 지역보건사업 정책 결정에 참고가 될 수 있을 것이다.

신규 고혈압 환자의 치료지속성과 고혈압 조절 여부가 심뇌혈관질환 합병증에 미치는 영향 (The Effects of Adherence and Hypertension Control on Complication among Newly Diagnosed Hypertension Patients)

  • Background: This study aimed to research on how adherence and blood control could make a difference when it comes to develop complications. Methods: The study's subjects were 255,916 patients who were newly diagnosed with hypertension in 2009 using data collected by National Health Insurance Cooperation. Patients are considered as a group under adherence if visit days and prescription days are more than 300 days. Patients are considered to have successfully controled their hypertension based on actual value measured by National Health Insurance Cooperation and the study takes a look at whether they were diagnosed with complications of cerebrocardiovascular disease in 2012. Chi-square test and logistic regression was used to analyze. Results: Patients who were able to control their hypertension show 0.80 times chance of developing cerebrovascular disease, and 0.89 times chance of developing cardiocerebrovascular disease. The group of adherence shows lower chance of developing complication in general than the group of non-adherence. Conclusion: The study revealed that hypertension's constant treatment could control the blood pressure and prevent complications. It is important that encourages patients to effort for persistent treatment for reducing complication.

고혈압 관리에 영향을 미치는 요인 분석: 제4기 국민건강영양조사자료를 중심으로 (Related Factors of Awareness, Treatment, and Control of Hypertension in Korea : Using the Fourth Korea National Health & Nutrition Examination Survey)

  • 본 연구는 고혈압 환자를 대상으로 효과적인 관리대책을 개발하는데 유용하게 활용할 수 있는 기초자료를 얻기 위한 목적으로 수행되었다. 제4기 국민건강영양조사자료(2007~2009)를 이용하였으며, 분석방법은 다변량 로지스틱 회귀분석을 적용하였다. 본 연구의 주요 결과로는 첫째, 고혈압 인지와 통계적으로 유의한 관련성이 있는 변수는 남녀 모두에서 연령, 직업, 주관적 건강상태, 체질량지수 및 건강검진 여부 변수였다. 둘째, 고혈압의 치료는 남녀 공히 연령, 주관적 건강상태, 체질량지수 및 건강검진 여부 변수가 유의했다. 셋째, 고혈압의 조절에는 남녀 모두 연령, 주관적 건강상태, 체질량지수 및 건강검진 여부 변수가 유의한 변수였다. 정책적 시사점으로는 고혈압 관리수준을 전반적으로 향상시키려면 선택과 집중의 원칙하에 취약 집단에 초점을 맞추어 이들에게 적합한 대책을 집중적으로 시행해야 할 것이다.

고혈압 치료를 위한 상용하는 한약 처방 10종의 혈관이완 평가 (Evaluation of Vasodilative Effects on 10 Traditional Herbal Formulas for Treatment of Hypertension)

  • Objectives : Hypertension (high blood pressure), one of the world's major chronic diseases, has a high mortality rate due to its high prevalence and complications, but its control rate is low. The proper management and control through appropriate exercise, diet management, and optimal drug choice can reduce the risk of death from hypertension. Although various antihypertensive drugs are used to treat hypertension, they also have numerous adverse effects. Alongside increased interest in the use of Traditional Herbal Formulas (THF) for hypertension treatment, the purpose of this study was to examine the vasodilative effects of 10 THF in the rat thoracic artery pre-contracted by potassiumchloride (KCl). Methods : THF were extracted with distilled water for 2 hours. The rat thoracic artery was suspended and contracted by KCl in the organ bath which contained 10 ml Krebs Henseleit (KH) buffer. THF extracts were added in a dose-dependent increase (10-1,000 ㎍/mL) to examine vasodilative effects. The vasodilative effects produced by THF were expressed as the percentage in response to KCl-induced contraction. Results : Among the 10 THF, Banhasasim-tang, Buhnsimgieum, Sagunja-tang, and Samul-tang showed vasodilative effects. And, Sipjeondaebo-tang, Ssanghwa-tang, Ojeok-san, Onkyung-tang, Yongdamsagan-tang, and Hyangsayukgunja-tang showed no significant vasodilative effects. Also, in co-administration with amlodipine, Banhasasim-tang showed higher vasodilative effects than amlodipine alone, and Buhnsimgieum showed greater vasodilative effects at low concentrations, but inhibited amlodipine's vasodilative effects at high concentrations. Conclusion : As a result of these studies, they will be expected to provide useful data to establish guidelines of combined administration of THF and western antihypertensive drugs for the treatment of hypertension.

머신러닝 기법을 활용한 고혈압 환자의 건강 관련 삶의 질 요인 예측 (Using Machine Learning Techniques to Predict Health-Related Quality of Life Factors in Patients with Hypertension)

  • Purpose : This study aims to identify the factors influencing health-related quality of life through machine learning of the general characteristics of patients with hypertension and to provide a basis for related research on patients, such as intervention strategies and management guidelines in the field of physical therapy for health promotion. Methods : Annual data from the second Korean Health Panel (Version 2.0) from 2019 to 2020, conducted jointly by the Korea Health and Social Research Institute and the National Health Insurance Service, were analyzed (Korea Health Panel, 2024). The data used in this study was collected from January to July 2020, and the data was collected using computer-assisted face-to-face interviews. Of the 13,530 household members surveyed, 1,368 were selected as the final study participants after removing missing values from 3,448 individuals diagnosed with hypertension by a doctor. Results : The results showed that walking (P2) was the most significant factor affecting health-related quality of life in random forest, followed by perceived stress (HS1), body mass index (BMIc), total household income (TOTc), subjective health status (SRHc), marital status (Marr), and education level (Edu). Conclusion :To prevent and manage chronic diseases such as hypertension, as well as to provide customized interventions for patients in advanced stages of the disease, research should be conducted in the field of physical therapy to identify influencing factors using machine learning. Based on the findings of this study, we believe that there is a need for additional content that can be utilized in the field of physical therapy to improve the health-related quality of life of patients with hypertension, such as diagnostic assessment and intervention management guidelines for hypertension, and education on perceived stress and subjective health status.

심뇌혈관질환 고위험군의 교육정보센터 영양실습 교육프로그램 효과 (Effects of Nutritional Education Practice Program for Cardiocerebrovascular High-risk Group at the Education Information Center)

  • This research was performed to investigate the effects of NEP (Nutritional Education Practice) program developed by KHyDDI (Korea Hypertension Diabetes Daegu Initiative) for hypertension and diabetes patients. The subjects were 116 patients (hypertension 70, diabetes 46) who had completed basic education program at the education information center and four-session program was implemented for them. Nutrient intake was analyzed and compared before and after the program by 24-hr recall method and evaluate weight, waist circumference, body fat, blood pressure and eating habits in terms of nutrition knowledge, eating behavior, salty taste assessment. The improved results after the program were observed in weight, waist circumference, body fat ratio, blood pressure, slightly salty taste in salty taste assessment, nutrition knowledge, eating behavior, sodium, energy, carbohydrate and protein intake ratio to total energy (p < 0.001). Therefore, this program is effective in the improvement of weight, waist circumference and eating behavior, and the continued management would lead to the prevention of cardio-cerebrovascular diseases in the community.

  • Objectives: The aim of this study was to develop the efficient program protecting shift to cerebrovascular disease as complication for patients with diabetes and hypertension. Methods: Chronic disease self management program(CDSMP), implement manual, action plan, Q&A card and motivation methods were suggested based on Bandura's social learning theory through reviewing various literatures and cases. Results: This program can increase self-efficacy, individual health behavior change and quality of life and it makes to continuous care of chronic disease. Conclusion: In order to operate chronic disease self-management program, standardized education courses training of specialist leaders and expert patients leaders would be required. And the development enlargement of self-management program for various other chronic disease such as arthritis, back pain, atopy, asthma would be required in the future.

Comparative Analysis of American Academy of Pediatrics and European Society of Hypertension Guidelines for the Diagnosis and Treatment of Pediatric Hypertension

  • Childhood hypertension (HTN) has become a significant public health issue because of the increased risk of cardiovascular disease in adulthood. However, childhood HTN is underrecognized and underdiagnosed in clinical practice. The European Society of Hypertension in 2016 and the American Academy of Pediatrics (AAP) in 2017 published updated guidelines for the screening, prevention, and management of pediatric HTN. There were notable differences between the two guidelines as well as many similarities. The updated AAP guidelines have clarified and simplified the recommendations for screening, diagnosis, and treatment of childhood HTN based on current evidence. This review highlights the important developments in both guidelines, focusing on recent advances in the classification and treatment of childhood HTN.

  • Pulmonary hypertension is a rare and progressive illness with a devastating prognosis. Promising research efforts have advanced the understanding and recognition of the pathobiology of pulmonary hypertension. Despite remarkable achievements in terms of improving the survival rate, reducing disease progression, and enhancing quality of life, pulmonary arterial hypertension (PAH) is not completely curable. Therefore, an effective treatment strategy is still needed. Recently, many studies of the underlying molecular mechanisms and technological developments have led to new approaches and paradigms for PAH treatment. Management based on stem cells and related paracrine effects, epigenetic drugs and gene therapies has yielded prospective results for PAH treatment in preclinical research. Further trials are ongoing to optimize these important insights into clinical circumstances.

과천시민의 고혈압 인지, 치료, 조절과의 관련요인 (Awareness, Treatment, Control, and Related Factors of Hypertension in Gwacheon)

  • Objectives : To identify the factors related to awareness, treatment, and control of hypertension in a Gwacheen population. Methods : This study surveyed 1,176 Gwacheon residents older than 40 years, and measured blood pressure using a standardized guideline in 1999. The study subjects were 473 adults (175 males, 295 females) with hypertension defined as a systolic blood pressure $\geq$140 mmHg, diastolic blood pressure $\geq$90 mmHg or reported treatment with antihypertensive medication. Information on awareness, treatment, and control of hypertension, and sociodemographic and health-related factors .was collected through person-to-person interviews with a structured questionnaire. Results : Overall, 252 (53.3%) of hypertensive subjects were aware of their condition, of whom as many as 193 (76.6%) were being treated, and 81 (42.0%) had their blood pressure controlled at the recommended level (<140/90 mmHg). However, of the 473 subjects found to have hypertension, only 40.8% were being treated, and 17.1% were under control. There were no significant differences in the proportions of awareness, treatment, and control of hypertension. In multiple logistic regression models, awareness of hypertension was positively associated with age and family history of hypertension in females. Control of hypertension was also positively associated with haying a partner and marital status in females.. Conclusion : These findings demonstrate that much greater efforts on improving awareness, treatment, and control of hypertension are needed, even in urban community settings, considering related factors such as a9e, family history, and marital status. However, these factors should be further investigated for their causal relationship.
