Screening for depression and anxiety disorder in children with headache

  • Purpose: The purpose of this study was to investigate the importance of initial screening tests for depression and anxiety disorders in children with headache. In addition, this study evaluated whether the Children's Depression Inventory (CDI) and Revised Children's Manifest Anxiety Scale (RCMAS) are suitable for screening symptoms of depression and anxiety. Methods: A retrospective chart review was conducted of 720 children aged 7-17 years who had visited a pediatric neurology clinic for headaches and were referred to a pediatric psychiatric clinic for psychiatric symptoms from January 2010 to December 2011. All patients completed the CDI and RCMAS. Among them, charts of patients with clinically significant total scores (cutoff>15) for psychiatric symptoms, as defined by the CDI and RCMAS scoring scales, were reviewed. Results: Nineteen patients had headaches and clinically significant total scores for psychiatric symptoms. The mean age at headache diagnosis was 11.7 years, and 57% were male. Mean duration of headache was 11.5 months. Two point eight percent of the patients were diagnosed with psychiatric disorders including major depression (1.7%), generalized anxiety disorder (1.1%), and bipolar disorder (0.1%). Four patients (0.6%) were diagnosed with attention deficit/hyperactivity disorder (ADHD). Total mean CDI and RCMAS scores of patients referred to the psychiatric clinic were 18.8 and 22.2, respectively. There was no correlation between CDI or RCMAS total scores and headache frequency, duration, or severity. Conclusion: We recommend that all patients with headache should be screened for depression and anxiety by CDI and RCMAS scores.

Depression, Anxiety, ADHD, and Impulsivity with Fire Officers (소방관의 우울, 불안, ADHD, 충동성)

  • Objective : This study aimed to evaluate the association of exhaustion, ADHD, self-esteem, impulsivity, suicidal tendencies and quality of life as well as depression and anxiety in fire officers in Korea. Methods : This study was undertaken for the first time in Korea, from September 2016 to December 2016. A set of questionnaires was provided to a total of 113 fire officers and 284 other participants as a comparison group. All subjects were evaluated using the Beck's Depression Inventory, Beck's Anxiety Inventory, Malach Burnout Inventory, Korean Adult Attention Deficit Hyperactivity Disorder Scales, Rosenberg's Self-esteem Inventory, Baratt's Impulsivity Scale, Beck's Scale of Suicide Ideation, and Quality of Life Scale. Furthermore, the odds ratio of depression, anxiety, exhaustion, ADHD, self-esteem, impulsivity, suicidal tendency and quality of life were evaluated the fire officer group and control group were compared through logistic regression analysis. Result : The results of this study indicated that the fire officers were more likely to show depression, anxiety, exhaustion, ADHD, low self-esteem and higher impulsivity. Also, the association between the fire officer group and depression, ADHD and impulsivity was confirmed through logistic regression analysis. Conclusion : Timely and effective evaluation and treatment of impulsivity, as well as depression is required for fire officers.

  • Method : Chronic motor tic or vocal tic disorders are characterized which have one of the two kinds of tics for more than a year. It is characterized chronic motor tic or vocal tic disorders that have one of the two kinds of tics for more than a year. But Tourette's disorder is a disease which has one or more motor tics and vocal tics for more than a year. Contemporary empirical studies have suggested the rate of comorbid attention-deficit hyperactivity disorder(ADHD) or obsessive compulsive disorder(OCD) in children with tic disorders be high. Objects : As society is confusing, tic disorders are increasing. Therefore, determining the treatment strategy in children with tic disorders is very important. So we studied the literatures of traditional chinese medicine about tic disorders and reported the results. Results : The principals for medical treatment were So-gan(疏肝), I-gi(理氣), Sik-pung(熄風), Gin-kyung(鎭痙), Chung-oel(淸熱) etc. Medicines treatment is used more than acupuncture treatment. The herbs are used Uncariae ramulus et Uncus(釣鉤藤), Paeoniae Radix Alba(白芍藥), Poria(白茯岺), Glycyrrhizae Radix(甘草), Bupleuri Radi.x(柴胡), Fossilia ossis Mastodi(龍骨) etc. in the order named. Acupuncture points were used Naegwan(PC6), Shinmuin(HT7), P'ungnyur4(ST40), Paek'oe. (GV20), Chung-wan(CV12), Samumgye(SP6), Kongson(SP4), T'aech'ung(LR3) and so on.

NEUROPSYCHOLOGICAL ASSESSMENT OF CHILDREN WITH ATTENTION DEFICIT/HYPERACTIVITY DISORDER (주의력결핍/과잉운동장애 아동의 신경심리학적 평가)

  • This paper first reviewed the current neurological theories concerning the etiology of ADHD and secondly, examined results of studies that applied neuropsychological assessment methods in the examination of ADHD children both here in Korea and abroad. ADHD children were found to exhibit characteristic responses indicating deficits in vigilance, sustained attention, distractibility, allocation and regulation of attention in many assessments of attention, in addition to deficits in executive functioning, working and associative memory. Such neuropsychological assessment results suggest that in addition to dysfunction in the frontal lobe and the reticular activation system, dysfunction may exist in other neural pathways involving many areas of the brain. However, because a substantial number of neuropsychological assessment tools being employed in Korea for ADHD children had been developed abroad, a Korean standardization project involving ADHD and normal control children, in addition to other child psychiatric population pools must be conducted in order to obtain appropriate age norms and test validity, and in order to make possible a more accurate and precise comparison and interpretation in the assessment of ADHD children.

COMORBIDITY OF CHILD AND ADOLESCENT INPATIENTS (소아정신과 입원환자의 공존질병(Comorbidity))

  • The objective of this paper was to determine the degree of diagnostic overlap. In a pilot study of 56 inpatients(mean age 12) with DSM-III-R axis I and/or II disorders, the degree of psychiatric comorbidity was examined. 64.3% had two or more diagnoses. The samples were divided into the following 9 groups 1) attention deficit hyperactivity disorder 2) conduct disorder 3) oppositional defiant disorder 4) schizophrenia 5) mood disorders 6) tie disorders 7) elimination disorders 8) mental retardation 9) personality disorders Substantial overlap(especially tic disorders, elimination disorders, disruptive behavior disorders) occured among inpatients Patients had about 2 DSM-III-R axis I & II diagnoses. Additional research with increased sample size is necessary to clarify its relationship with other psychiatric diagnoses.

  • A 8 year old boy with problems of hyperactivity, impulsivity received playpsychotherapy. Through the detailed examination of their contents and group supervision of play therapy session, common mistakes novice therapist make frequently and tasks in the initial phase of play psychotherapy are illustrated. Therapist's personality and anxiety frequently interferes therapeutic processes. Common mistakes therapist makes during initial phase of play therapy are ; therapist's over-enthusiasm, overinvolvement or non-responsiveness, premature or inappropriate interpretation and direct connection of play theme to reality. It is utmost important for the therapist to make positive and trusting relationship with the patient in the initial phase of the therapy. In order to do so therapist must accept the patient as he is, permit the patient takes his leads. And as a result, the Patient can go through corrected developmental experience. In conclusion, it is a therapist's main task to establish relationship.

Discriminant Validity of the CBCL 1.5-5 in Diagnosis of Developmental Delayed Infants (발달지체 진단에서 CBCL 1.5-5 유아행동평가척도-부모용의 변별력)

  • Objectives:The purpose of this study was to verify discriminant validity and the clinical cutoff score of Child Behavior Checklist 1.5-5 in the diagnosis of developmental delayed infants. Methods:The participants were screened by Denver II which includes 156 developmental delayed infants and 288 normal infants. Chi-squared test, t-test, ROC curve analysis, odds ratio analysis were performed on the data. Results:Only 47 items out of 99 items among the CBCL 1.5-5 of total groups, 36 items of boys and 48 items of girls, discriminated developmental delayed infants well. Discriminant validity was confirmed by mean differences on the subscales of Withdrawn, Sleep Problems, Attention Problems, Internalizing Problems, Externalizing Problems, Total Problems, DSM Pervasive Developmental Problems and DSM Attention Deficit/Hyperactivity Problems between the two groups. Additionally, ROC analyses demonstrated that Withdrawn, Attention Problems, Internalizing Problems, Total Behavior Problems and DSM Pervasive Developmental Problems significantly predicted developmental delayed infants compared to normal infants. Also, the clinical cutoff score criteria adopted in the Korean CBCL 1.5-5 for subscales of Withdrawn, Attention Problems, Internalizing Problems, Total Behavior Problems and DSM Pervasive Developmental Problems were shown to be valid. Conclusion:The subscales of Withdrawn, Attention Problems, Internalising Problems, Total Behavior Problems and DSM Pervasive Developmental Problems significantly discriminated in the diagnosis of developmental delayed infants well.

Agreement between Parents and Teachers on School Children's Emotional/Behavioral Problems (학령기 아동 정서.행동문제에 대한 부모-교사 평가 일치도)

  • Objectives: In assessing behavioral/emotional problems in school-aged children, the importance of multi-informant reporting has been well documented. However, in clinical settings obtaining multiple informants' opinions has proven difficult. For that reason, we researched the agreement and predictive validity of the Child Problem-Behavior Screening Questionnaire (CPSQ) in order to reveal how accurate parents' assessments reflected teachers'opinions. Methods: We conducted the first screening for second- and third-grade children from 3 elementary schools in Seoul from 2003 to 2007 using the CPSQ. There were 1178 children included in the analysis. We then administered the Korean version of the Child Behavior Checklist (K-CBCL) as a second screening tool and subsequently, the ADHD Diagnostic System (ADS) and the Korean Educational Development Institute version of the Wechsler Intelligence Scale for Children (KEDI-WISC) was administered by a psychiatrist. We examined each item on the CPSQ and the subscale's agreement between parent and teacher as well as the predictive validity of the CPSQ in children diagnosed with emotional/behavioral problems. Results: The agreement rates between parents and teachers appeared high for questions 18 (0.433), 1 (0.385), and 2 (0.325). Among the subscales, a relatively high correlation was found for externalizing problems, attention deficit hyperactivity disorder, and cognitive problems. For all diagnosed children, their parents revealed a higher sensitivity and lower specificity than teachers. Conclusion: From these results, we confirmed that the CPSQ can be useful for sorting out externalizing and cognitive problems. There is a need for further study, however, with a larger sample size.

The Effects of Social Skills Training as a Preventive Approach for Children in the Community Setting (지역사회에서 예방적 접근으로 실시한 사회기술훈련의 효과)

  • Objectives : The purpose of this study was to evaluate the effect of social skills training (SST) for children in two different community settings, an elementary school and a child community center. Methods : Two separate groups of 15 children (10 boys and 5 girls) and 13 children (4 boys and 9 girls) participated in a SST program twice per week at an elementary school and a child community center, respectively. Age range of participants was from 8 to 10 years. Sixty-minute-long sessions of SST were continued twice per week for 4 weeks. Korean-Child Behavior Checklist (K-CBCL), Conner's Rating Scales, Korean-Attention-Deficit Hyperactivity Disorder Rating Scale (K-ARS), Social Skills Rating System (SSRS), and Matson's Evaluation of Social Skills with Youngsters (MESSY) were evaluated by their parents before and after the SST program. Results : Pre-and-post analysis of SST indicated no significant statistical differences in the elementary school group. However, the child community center group showed significantly increased scores for total social ability (p<.05) and decreased scores for attention problem (p<.05) on the K-CBCL. In addition, scores of MESSY were significantly increased (p<.05). These differences between the two groups were still significant after controlling pre-SST scores ; and difference of SST scores between the two groups disappeared after SST. Conclusion : Results suggest that SST in the community setting is an effective tool for prevention of social deficits and problems in attention by promoting social function and attention.

A Study on Perception and Needs of Urban Park Users on Off-Leash Recreation Area (도시 공원이용자들의 애견전용공간 계획에 대한 인식 및 요구도 연구)

  • The recent increase in the number of dogs in high-density urban environments causes conflicts between dog owners and non-dog owners and the access of dogs to urban parks has been controversial among park users. A lack of exercise and socialization, however, can cause canine behavior problems such as aggression and hyperactivity which are potential dangers to people and there has increased demand for a place where people and their dogs can interact and exercise together. In many developed countries, off-leash recreation areas (OLRA) are considered as safe and effective places that dogs can exercise, play, and socialize with other dogs while reducing conflicts between dog owners and other park users. This study aimed to review the issue of dogs' access to public parks and to examine the perception of park users on OLRA to provide insights and guidelines in establishing OLRA in Korea. The survey respondents, even non-dog owners, perceived that it is necessary to provide a separate space for dogs from other park users by providing OLRAs in urban parks and showed the willingness to use. Most of dog owners consider their dogs as companions or family members and feel the lack of places to exercise their dogs. As local government plays a major role in urban animal management which emerge as a social issue at present, more active interventions of municipalities are necessary to support various needs and activities of park users. An experimental OLRAs based on community participations would be a meaningful starting point to effective urban animal management.
