A Review of Clinical Studies for Tuina Treatment on Attention Deficit Hyperactivity Disorder (ADHD) - Focused on Chinese Randomized Controlled Trials - (소아 주의력결핍 과잉행동장애 (ADHD)의 추나요법 효과에 대한 임상연구 고찰 - 중의학 무작위 대조군 임상연구를 중심으로 -)

  • Objective This study aimed to analyze the efficacy and safety of Tuina therapy for attention-deficit hyperactivity disorder (ADHD) through a randomized controlled clinical study in China. Methods We used the China National Knowledge Infrastructure (CNKI) as a resource and conducted a search using key words: 'ADHD', '推拿'. Six randomized control studies were identified and analyzed in this study. Results 1. The Tuina treatment group showed significant changes compared to the control group. 2. The most commonly used meridians were Spleen meridian (脾經穴), Liver meridian (肝經穴), Errenshangma (二人上馬穴), Xiaotianxin (小天心), Qinghetianshu (清河天水), Kidney meridian (腎經穴). Conclusions The results of this study showed that Tuina therapy is effective for ADHD patients with ADHD. It can be used for future research on Tuina therapy for ADHD.

Systematic Review of Suicidal Behaviors Related to Methylphenidate and Atomoxetine in Patients With Attention Deficit Hyperactivity Disorder

  • Objectives: This study investigated the relationship between suicidal behavior and the use of methylphenidate (MPH) or atomoxetine (ATX) in patients with attention deficit hyperactivity disorder (ADHD). Methods: The Preferred Reporting Items for Systematic Review and Meta-Analysis guidelines were used to conduct a meta-analysis. The Physiotherapy Evidence Database scale was used to score the quality of the studies. Results: Nine studies were included in this quantitative analysis. The analysis included 602864 patients with ADHD (521125 and 81739 patients were taking methylphenidate [MPH group] and atomoxetine [ATX group], respectively) and 19230 healthy controls. The overall estimates were in the order of the control, MPH, and ATX groups; however, no statistically significant between-group difference was observed in the incidence of events (p=0.553 for control vs. MPH; p=1.000 for control vs. ATX; p=1.000 for MPH vs. ATX). Conclusion: The rate of suicidal behavior was higher in the ADHD groups treated with MPH and ATX than in the control group. However, no statistically significant difference was observed between the ADHD groups treated with MPH and ATX, and the control group. Therefore, MPH and ATX did not increase suicidal behavior.

  • Objectives: This study aimed to investigate the correlation between the Comprehensive Attention Test, Korean-Wechsler Intelligence Scale for Children-Fourth Edition, and Attention-Deficit/Hyperactivity Disorder (ADHD) Rating Scale-IV scores in children and adolescents with ADHD. Methods: Fifty-five children and adolescents diagnosed with ADHD and not taking psychiatric medications were included in this retrospective study. A correlation analysis was performed. Results: Although simple visual and auditory selective attention have diagnostic value in traditional continuous performance tests, this study revealed that inhibition-sustained attention and interference-selective attention are also effective in evaluating ADHD. Furthermore, the correlation between the attention and intelligence test scores varied depending on the use of visual or auditory stimuli. Conclusion: The findings of this study contribute to clarifying our understanding of the cognitive characteristics of children and adolescents with ADHD and can be used in future research.

Association between Atopic Dermatitis and Attention Deficit Hyperactivity Disorder Symptoms in Korean Children (소아.청소년에서 아토피 피부염과 주의력결핍과잉행동장애와의 관계)

  • Objective : The aim of this study was to examine the relationship between IgE-mediated atopic dermatitis and attention deficit hyperactivity disorder(ADHD) in Korean children. Method : This case-control study included 69 patients with atopic dermatitis and 42 normal students. Diagnoses of atopic dermatitis and assessments of its severity were made by the dermatologist. The MAST tests were administered to atopic dermatitis patients. The parents of all of the subjects completed the parent versions of the ADHD Rating Scale-IV(ARS). Results: Compared to the normal control group, the atopic dermatitis group scored significantly higher in the total, inattention, and hyperactivity/impulsivity profiles of the ARS(t=3.35, p<0.01 : t=6.41, p<0.01 ; t=3.93, p<0.01, respectively), The atopic dermatitis patients with ADHD hyperactivity/impulsivity subtype showed more severe symptoms of atopic dermatitis than those with ADHD inattentive subtype. The MAST-positive atopic dermatitis patients showed significantly increased incidence of ADHD symptoms than the MAST-negative counterparts. Conclusion : The results of this study suggest the possibility of an association between IgE-mediated atopic responsiveness and ADHD symptomatology.

ATTENTION-DEFICIT HYPERACTIVITY SYMPTOMS IN CHILDHOOD OF ADULT BIPOLAR DISORDER/SCHIZOPHRENIA PATIENTS (성인 양극성장애와 정신분열병 환자의 아동기 주의력결핍-과다활동 증상)

  • Object:The authors compared the attention-deficit hyperactivity(ADH) symptoms in childhood of adult male patients with bipolar disorder, schizophrenia and normal control subjects and attempted to find out whether attention-deficit hyperactivity disorder and bipolar disorder are related each other. Method:The authors compared ADH symptoms in childhood assessed with Wender Utah Rating Scale(WURS), selected 25 items of WURS(WURS-25), and Parent Rating Scale(PRS), and compared them between 26 bipolar, 29 schizophrenic, and 27 normal control subjects. Result:The subjects with bipolar disorder had significantly higher mean score of WURS compared with normal control group(One-way ANOVA, duncan test, WURS:DF=2, F=3.77 p=0.027), and the differences between the mean scores of WURS-25 and PRS of bipolar subjects and the other two groups were also highly significant(One-way ANOVA, Duncan test, WURS-25:DF=2, F=4.24 p=0.0178, PRS:DF=2, F=13.97 p<0.001). The mean scores of WURS, WURS-25, and PRS of schizophrenic subjects were higher than those of normal control group, though not significant. WURS and PRS were correlated for subjects with bipolar disorder(r=0.7495) and the normal control(r=0.5305), and there was no correlation for schizophrenic subjects. Conclusion:The ADH symptoms in childhood were much more evident for adult bipolar subjects than schizophrenic and normal control subjects. And these results are very suggestive that there might be some relationship between bipolar disorder and attention-deficit hyperactivity disorder and these two disorders might have a shared common pathophysiology which needs further study.

Change of Psychological Characteristics after Joining the Army According to Internet Addiction Tendency in Young Male Army Recruits (군 병사들에서 인터넷 중독 경향에 따른 입대 후 심리특성 변화)

  • Objectives The aim of this study was to find out the psychological changes associated with internet addiction between before and after the military drill among the young male army recruits. Methods All 1091 participants were army recruits aged from 19 to 22 years who participated in the 5-week military drill. They were assessed with some self-reported questionnaires [Sociodemographic questionnaires, Young's Internet Addiction Scale (IAS), the State-Trait Anxiety Inventory (STAI), the Beck Depression Inventory (BDI), the Barratt Impulsiveness Scale (BIS), the Attention-Deficit/Hyperactivity Disorder Self-Rated Scale (ASRS)]. They were divided into two groups, One is the internet addiction tendency group and the other is non-addiction group according to the IAS score. And the severity of the internet addiction tendency, depression, anxiety and impulsiveness were evaluated by the IAS, the STAI, the BDI, the BIS, the ASRS before and after military drill. Results The result of the paired t-test shows that the IAS, the STAI, the BDI, the BIS, the ASRS scores decreased after military drill in both non-addiction group and internet addiction tendency group. The result of the repeated measures ANOVA shows that there is an interaction effect between the changes of ASRS-hyperactivity (F = 23.437, p < 0.001), ASRS-impulsiveness (F = 4.896, p = 0.027), BIS-total (F = 4.057, p = 0.044), BIS-motor impulsiveness (F = 13.609, p < 0.001) scores and groups. The result of the generalized estimating equation shows that internet addiction tendency is associated with ASRS-inattention (${\beta}=0.075$, p < 0.001), ASRS-hyperactivity (${\beta}=0.092$, p = 0.002), STAI-trait anxiety (${\beta}=0.046$, p < 0.001), BIS-motor impulsiveness (${\beta}=0.119$, p = 0.028). Conclusions The results show that psychopathologies such as internet addiction tendency, depression, anxiety, impulsiveness may improve after military drill regardless of the groups. Internet addiction tendency may be related to the ASRS-inattention, ASRS-hyperactivity, STAI-trait anxiety, motor-impulsiveness. So these findings may be considered in the treatment of internet addiction in terms of the impulsiveness control.

Effects of Atomoxetine on Hyper-Locomotive Activity of the Prenatally Valproate-Exposed Rat Offspring

  • to valproic acid (VPA) during pregnancy produces ASD-like core behavioral phenotypes as well as hyperactivity in offspring both in human and experimental animals, which makes it a plausible model to study ASD-related neurobiological processes. In this study, we examined the effects of two of currently available attention defecit hyperactivity disorder (ADHD) medications, methylphenidate (MPH) and atomoxetine (ATX) targeting dopamine and norepinephrine transporters (DAT and NET), respectively, on hyperactive behavior of prenatally VPA-exposed rat offspring. In the prefrontal cortex of VPA exposed rat offspring, both mRNA and protein expression of DAT was increased as compared with control. VPA function as a histone deacetylase inhibitor (HDACi) and chromatin immunoprecipitation experiments demonstrated that the acetylation of histone bound to DAT gene promoter was increased in VPA-exposed rat offspring suggesting epigenetic mechanism of DAT regulation. Similarly, the expression of NET was increased, possibly via increased histone acetylation in prefrontal cortex of VPA-exposed rat offspring. When we treated the VPA-exposed rat offspring with ATX, a NET selective inhibitor, hyperactivity was reversed to control level. In contrast, MPH that inhibits both DAT and NET, did not produce inhibitory effects against hyperactivity. The results suggest that NET abnormalities may underlie the hyperactive phenotype in VPA animal model of ASD. Profiling the pharmacological responsiveness as well as investigating underlying mechanism in multiple models of ASD and ADHD may provide more insights into the neurobiological correlates regulating the behavioral abnormalities.

A STUDY OF THE CONTENT VALIDITY IN HYPERKINETIC CONDUCT DISORDER (과잉운동 품행장애의 내용 타당도에 관한 연구)

  • Conduct disorder is a heterogenous disorder of various etiology affecting 4-10% of school-age children. There is suggestive evidence that the comorbid group of conduct disorder and attention deficit hyperactivity disorder is different from these two separated diseases groups. There findings suggest that this comorbid group may present a meaningful subgroup. This study is conducted to examine the content validity of hyperkinetic conduct disorder that accepted first in International Classification of Disease 10th edition. The results are summarized as follows : 1) Using Conners Parenting Rating Scale, Parent Rating Scale Form for DSM-III-R, the mean scores of attention deficit hyperactivity disorder in conduct disorder were significantly higher compared with those of normal controls. 2) 72.4% of conduct disorder and 41.5% of normal control groups showed simultaneous attention deficit hyperactivity disorder. The diffenence between conduct disorder and normal control groups was significant. There were many previous informations that children with attention deficit hyperactivity disorder had conduct disorder as comorbid disorder and these result show that children with conduct disorder also significantly had attention attention deficit hyperactivity disorder reversibly. Then these show that hyperkinetic conduct disorder-diagnosis used when both the overall criteria for hyperkinetic disorders and the overall criteria for conduct disorders are met-have satisfactory content validity. Biological, familial or long term studies are needed to further validate this diagnostic category.

  • Attention Deficif and Hyperactivity Disorder(ADHD) is a behavioral disoerder characterized by inattentiveness, impulsivity and hyperactivity, This is the case of a ADHD child treated with Neurofeedback therapy. ADHD Diagnotic System(ADS), Conner;s parent rating scale, ADHD-Rating Scale (ARS), Korean Child Behavior Checklist(K-CBCL) were compared between before and after Neurofeedback theraphy. And the EEG(Theta wave, SMR wave and Beta wave) data of each ession was analysed. As a result, Neurofeedback therapy is efficient in the treatment of ADHD.

Back and Chest Pain Related to Scalenus Medius Muscle (중사각근과 관련된 배부통과 흉통에 관한 연구)

  • One hundred and four patients who complained of chest pain or back pain in the region between scapular were studied. In most cases, anatomical location of pain was in the rhomboid or serratus anterior muscle. Hyperactivity of dorsal scapular nerve or long thoracic nerve which innervate those muscles was thought to be responsible for the pain. The hyperactivity of the nerves may be due to the spasm of the scalenus medius muscle which the nerves meet during their course to the rhomboid or serratus anterior muscles. Therefore, spasmolytic treatment including trigger point injection, physical therapy, laser therapy, or NSAIDs may be effective for the treatment of chest pain or back pain.
