  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made. Comparisons between the valves were made on the transvalvular pressure drop, regurgitation volume and maximum valve opening area. The polymer valve showed smaller regurgitation volume and transvalvular pressure drop compared to the mechanical valve at higher heart rate. Because the point of maximun temperature of laser-irradiated gelatin exists below the surface due to surface cooling, evaporation at the boiling temperature is made explosively from below the surface. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. We obtained a two-dimensional current density distribution within a phantom by processing the real and imaginary MR images from a 4.77 NMR machine. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. We use combination of MLP and fuzzy filter for pattern recognition to estimate the direction of a muscle and Hogan's method for the force estimation. $[^{11}C]Raclopride$ PET을 이용한 흡연에 의한 도파민 유리 영상 연구 (Smoking-Induced Dopamine Release Studied with $[^{11}C]Raclopride$ PET)

  • 목적: 흡연에 보상 강화 효과는 흡연 시에 담배 내에 포함되어 있는 니코틴 성분에 의한 선조체에서의 도파민 유리가 중심 역할을 할 것으로 생각된다. 지금까지의 여러 동물실험 에서 니코틴과 도파민의 상호 관계와 니코틴에 의한 도파민 유리가 연구되었다. 이 연구에서는 $[^{11}C]raclopride$ PET을 이용하여 생체에서 흡연에 의한 도파민 유리를 영상화 하고자 하였다. 대상 및 방법: 비흡연자이거나 과거 흡연력이 있으나 1년 이상의 금연을 시행한 5 명의 정상인 남자를 대상으로 하였고, 이들의 평균 연령은 24세 이었다. 도파민 D2 수용체 영상을 위한 방사성 리간드인 $[^{11}C]raclopride$를 볼루스+연속 주입법에 의하여 주사하면서 120 분간 30개의 동적 영상($3{\times}20$초, $2{\times}60$초, $2{\times}120$초, $1{\times}180$초, $22{\times}300$초)을 얻었다. 영상 촬영 시작 50분에 니코틴 함량 1mg의 담배를 피도록 하였으며, 담배를 피우기 직전과 흡연시작 5분 후부터 흡연에 의하여 흡수된 혈중 니코틴 농도를 측정하기 위하여 일정 간격으로 정맥혈 샘플을 획득하였다. 30 개의 프레임은 인접 프레임과의 정합에 의하여 움직임을 보정하였고, 움직임이 보정된 120분 간의 동적 영상을 합하여 평균 영상을 만든 다음, 뇌 MRI와 공간 정합을 하였다. 평균 영상과 MRI 의 공간 정합 정보를 이용하여, 각 프레임을 MRI 에 공간 정합시켜, 공간 정합된 동적 영상에서 선조체에 MRI 정보를 이용하여 좌우 각각 3 개의 관심 영역(ventral striatum : VST, precomissural dorsal caudate; caudate nucleus; precommissural putamen; anterior putamen)과 소뇌에 관심영역을 설정하였고, 동적 영상으로부터 각 관심 영역 별로 시간-농도 곡선을 구하였다. $[^{11}C]raclopride$주사 후 선조체에서의 리간드 특이적 결합에 의한 항정상태(statedy state) 에 도달한 후 흡연전 평형 상태(equilibrium state)인 30-50 분과 흡연 후 재평형에 도달한 70-90분 영상에서 각 관심 영역에서의 방사성 농도를 구하였고, 조직비 방법에 근거하여 기저상태 및 흡연 상태의 방사성 리간드의 수용체 결합능 (binding potential;BP)을 구하였다($BP=C_{ROI}/C_{cerebellum}-1$). 흡연에 의한 도파민의 유리는 흡연 전후의 $[^{11}C]raclopride$의 수용체 결합능의 변화율로 계산하였다. 흡연에 의한 혈중 니코틴의 상승은 흡연후 90 분간의 혈중 니코틴의 축적 농도로 계산되었으며, 흡연에 의한 $[^{11}C]raclopride$의 수용체 결합능의 감소율과 혈중 니코틴의 축적 농도와의 상관관계를 스피어만 상관분석법(Spearman's correlation)에 의하여 알아보았다. 결과: 흡연에 의한 선조체에서의 평균 $[^{11}C]raclopride$의 수용체 결합능의 변화는 미상핵에서 4.7%, 전피각에서 4.0%, 복측 선조체에서 7.8% 의 감소를 보여 흡연에 의한 선조체내 도파민 유리를 정량화 하였다. 특히 선조체에서의 도파민 유리에 의한 수용체 결합능의 감소는 흡연에 의한 혈중 니코틴의 축적 농도와 양의 상관관계를 보였다(rho=0.9, p=0.04). 결론: $[^{11}C]raclopride$ PET을 이용하여 비흡연 정상인에서 흡연에 의한 도파민 유리를 영상화 및 정량화 하였고, 흡연에 의한 선조체내 도파민 유리는 흡연시 흡수된 니코틴의 축적 농도와 상관관계를 가짐을 보였다. 이 연구에서의 확립된 방법과 결과는 앞으로 흡연자에서 니코틴에 의한 도파민 신경계의 활성화 연구에 기여할 것이다.

Re-188과 Tc-99m 표지 단일클론항체 CEA79.4의 생체외 특성과 생체내 분포 (In Vitro Properties and Biodistribution of Tc-99m and Re-188 Labeled Monoclonal Antibody CEA79.4)

  • 목적: 현재 Re-188 표지 단일클론항체는 종양의 방사면역치료제로서 주목받고 있으며 많은 연구가 진행되고 있다. 이 연구에서는 골수성 백혈구 세포에 존재하는 NCA-95에 대한 단일클론항체 CEA79.4를 사용하여 Re-188의 항체 표지법을 개발하고 그 특성을 분석하여 Tc-99m 표지항체와 비교하고자하였다. 대상 및 방법: 표지는 항체에 방사성 핵종을 직접 붙이는 직접법을 사용하였다. 환원제인 ${\beta}$-mercaptoethanol로 단일클론항체 CEA79.4를 환원하여 SH-기를 생성시키고 여기에 발생기로부터 얻은 Tc-99m과 Re-188을 표지 하였다. Tc-99m의 표지를 위해서는 pH 6.5인 PBS에서 반응시켰으며 착화제와 환원제로 MDP kit를 사용하였다. Re-188은 50 mM acetate buffered saline (ABS, pH 5.3)에서 stannous tartrate를 환원제로 사용하여 표지 하였다. 표지효율은 ITLC로 확인하였으며 표지항체는 PD-10 칼럼을 이용하여 분리하였다. 분리한 표지항체를 1% 사람혈청 알부민과 비타민 C를 넣어 산화를 방지하고 실온과 사람혈청(5% $CO_2$, $37^{\circ}C$)에서 안정성 검사를 실시하였다. 대장암세포주인 SNU-C4와 세포결합검사를 실시하여 면역반응성과 항원에 대한 친화상수를 측정하였다. 또한 정상 ICR 마우스에 꼬리정맥으로 주사하여 1, 14, 24시간에 체내분포를 보았다. 결과: Tc-99m의 항체 표지효율은 $92.4{\pm}5.9%$ (n= 15)이고 Re-188은 $84.7{\pm}4.6%$ (n=4)로 우수한 표지효율을 보였다. 표지항체의 체외안정성검사를 실시한 결과 실온에서는 Tc-99m 표지항체의 경우 24시간까지 90% 정도로 안정하였으나 Re-188 표지항체는 70% 정도를 유지하였다. 사람혈청에서는 Tc-99m 표지항체는 24시간에 60% 이상이었으나 Re-188은 50% 이하로 낮았다. Tc-99m과 Re-188의 표지항체의 면역반응성은 각각 59.2%, 41.6%, 친화상수는 $6.59{\times}10^9\;M^{-1}$, $4.2{\times}10^9\;M^{-1}$ 이었다. 또한 정상 마우스에서 체내 분포는 비 특이적으로 집적되지 않고 배설되어 유사한 경향을 보였다. 결론: 기존의 Tc-99m 표지법과 같은 원리를 이용하여 단일클론항체 CEA79.4를 Re-188과 표지하였다. Re-188-CEA79.4는 면역반응성과 친화력등 체외 면역학적 특성과 체내분포에서 Tc-99m 표지항체와 큰 차이를 보이지 않았다. 따라서 Re-188-CEA79.4는 골수성 백혈병, 림프종 등 질병의 방사면성역치료에 적용 할 수 있을 것이다.

  • 우리나라 성인에서 아침결식이 증가하고 있으며, 아침결식은 대사적 위험을 증가시킨다고 한다. 본 연구는 한국성인을 위한 대사적 위험성이 낮은 아침식사 에너지 수준을 제시하고자 제4기 (2007~2009) 국민건강영양조사 자료를 이용하여 19~64세 성인 12,238명을 대상으로 1일 에너지섭취량이 500~5,000 kcal에 속하며, 임신 또는 수유중인 자, 대사 이상 질환이나 암으로 치료받는 자를 제외한 7,769명 (남자 38.4%, 여자 61.6%)의 자료를 분석하였다. 아침식사 에너지비율은 개인별 아침식사의 에너지섭취량이 에너지 필요추정량의 10% 미만 (극저에너지군), 10~20% 미만 (저에너지군), 20~30% 미만 (중간에너지군), 30% 이상 (충분에너지군)인 네 군으로 분류하여 대사질환의 위험요인별 교차비와 영양상태를 비교하였다. 모든 자료는 해당 년도의 가중치를 적용한 후 일반선형모델을 사용하여 성, 연령, 거주지역, 소득수준, 교육수준, 직업 유무 또는 에너지 섭취량을 추가 보정하여 분석하였다. 유의성은 ${\alpha}=0.05$ 수준에서 검토하였다. 1) 전체 대상자 중 아침식사 에너지가 1일 에너지필요추정량의 30% 이상에 속하는 대상자는 16.9%, 20~30% 미만군에 39.2%, 10~20% 미만군에 37.6%, 10% 미만군에 6.3% 순으로 나타났다. 극저에너지군과 저에너지군에는 여자보다 남자, 19~39세의 젊은 성인, 도시 거주자가 많았고 소득수준은 중상이상, 교육수준은 고졸이상이 많았고 직업을 가진 사람, 가족과 아침식사를 하지 않는 사람이 많았으나 중간에너지군과 충분에너지군에는 그와 대조적으로 여자, 40세 이상 성인, 읍면거주자, 소득과 교육수준이 낮고, 무직, 가족과 식사하는 사람이 많았다. 2) 충분에너지군에서 곡류 (주식류, 스낵류), 당류와 함께 감자류, 두류, 육류 및 난류, 어패류, 채소류, 과일류, 유지류, 조미료류의 섭취량이 네 군보다 높았다. 극저에너지군은 채소류의 섭취량이 네 군 중 가장 낮았고, 스낵류, 당류와 함께 육류 및 난류, 유지류, 조미료류의 섭취량이 저에너지 군과 중간에너지군보다 높았다. 3) 아침식사 에너지가 낮은 군일수록 지방에너지비와 단백질에너지비는 높고 탄수화물에너지비는 낮았으며, 에너지와 모든 영양소 섭취량 및 영양소적정섭취비 (NAR, MAR)가 낮아 영양소별 평균필요량 (EAR)에 미달되게 섭취한 대상자의 비율이 높았다. 충분에너지군은 에너지와 모든 영양소의 섭취실태가 가장 양호하였다. 4) 체중, 신장, 체질량지수와 허리둘레가 극저에너지군에서는 높은 반면, 충분에너지군에서는 낮아서 복부 비만위험은 충분에너지군을 기준으로 아침에너지섭취가 낮을수록 증가하여 극저에너지군에서 1.9배로, 저에너지군 1.44배, 중간에너지군 1.32배에 비해 높았으며, 체질량지수에 의한 비만 위험도 유사한 결과를 보였다. 또한 충분에너지군에서 혈청총콜레스테롤이 가장 낮아서 이상지질혈증 발생 위험은 충분에너지군에 비해 극저에너지군에서 1.84배 높았고 공복혈당에 의한 당뇨 발생 위험도 극저에너지군에서 1.57배 높았다. 이상의 연구결과에서 19~64세 성인에서 아침식사 에너지가 개인별 에너지 필요추정량의 10% 미만인 군은 비만, 이상지질혈증, 당뇨의 위험과 동시에 영양부족의 우려가 있는 반면에, 에너지 필요추정량의 30% 이상군은 대사위험성이 가장 낮고 영양상태가 가장 양호하였기 때문에 우리나라 19~65세 미만 성인을 위한 아침식사의 에너지 가이드라인으로 에너지 필요추정량의 30% 이상을 제시할 수 있을 것으로 보인다.

'아유르베다'($\bar{A}yurveda$)의 의경(醫經)에 관한 연구 (A Study of The Medical Classics in the '$\bar{A}yurveda$')

  • Through a simple study of the medical classics in the '$\bar{A}yurveda$', we have summarized them as follows. 1) Traditional Indian medicine started in the Ganges river area at about 1500 B. C. E. and traces of medical science can be found in the "Rigveda" and "Atharvaveda". 2) The "Charaka" and "$Su\acute{s}hruta$(妙聞集)", ancient texts from India, are not the work of one person, but the result of the work and errors of different doctors and philosophers. Due to the lack of historical records, the time of Charaka or $Su\acute{s}hruta$(妙聞)s' lives are not exactly known. So the completion of the "Charaka" is estimated at 1st${\sim}$2nd century C. E. in northwestern India, and the "$Su\acute{s}hruta$" is estimated to have been completed in 3rd${\sim}$4th century C. E. in central India. Also, the "Charaka" contains details on internal medicine, while the "$Su\acute{s}hruta$" contains more details on surgery by comparison. 3) '$V\bar{a}gbhata$', one of the revered Vriddha Trayi(triad of the ancients, 三醫聖) of the '$\bar{A}yurveda$', lived and worked in about the 7th century and wrote the "$A\d{s}\d{t}\bar{a}nga$ $A\d{s}\d{t}\bar{a}nga$ $h\d{r}daya$ $sa\d{m}hit\bar{a}$ $samhit\bar{a}$(八支集)" and "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$(八心集)", where he tried to compromise and unify the "Charaka" and "$Su\acute{s}hruta$". The "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$" was translated into Tibetan and Arabic at about the 8th${\sim}$9th century, and if we generalize the medicinal plants recorded in each the "Charaka", "$Su\acute{s}hruta$" and the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$", there are 240, 370, 240 types each. 4) The 'Madhava' focused on one of the subjects of Indian medicine, '$Nid\bar{a}na$' ie meaning "the cause of diseases(病因論)", and in one of the copies found by Bower in 4th century C. E. we can see that it uses prescriptions from the "BuHaLaJi(布哈拉集)", "Charaka", "$Su\acute{s}hruta$". 5) According to the "Charaka", there were 8 branches of ancient medicine in India : treatment of the body(kayacikitsa), special surgery(salakya), removal of alien substances(salyapahartka), treatment of poison or mis-combined medicines(visagaravairodhikaprasamana), the study of ghosts(bhutavidya), pediatrics(kaumarabhrtya), perennial youth and long life(rasayana), and the strengthening of the essence of the body(vajikarana). 6) The '$\bar{A}yurveda$', which originated from ancient experience, was recorded in Sanskrit, which was a theorization of knowledge, and also was written in verses to make memorizing easy, and made medicine the exclusive possession of the Brahmin. The first annotations were 1060 for the "Charaka", 1200 for the "$Su\acute{s}hruta$", 1150 for the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$", and 1100 for the "$Nid\bar{a}na$", The use of various mineral medicines in the "Charaka" or the use of mercury as internal medicine in the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$", and the palpation of the pulse for diagnosing in the '$\bar{A}yurveda$' and 'XiZhang(西藏)' medicine are similar to TCM's pulse diagnostics. The coexistence with Arabian 'Unani' medicine, compromise with western medicine and the reactionism trend restored the '$\bar{A}yurveda$' today. 7) The "Charaka" is a book inclined to internal medicine that investigates the origin of human disease which used the dualism of the 'Samkhya', the natural philosophy of the 'Vaisesika' and the logic of the 'Nyaya' in medical theories, and its structure has 16 syllables per line, 2 lines per poem and is recorded in poetry and prose. Also, the "Charaka" can be summarized into the introduction, cause, judgement, body, sensory organs, treatment, pharmaceuticals, and end, and can be seen as a work that strongly reflects the moral code of Brahmin and Aryans. 8) In extracting bloody pus, the "Charaka" introduces a 'sharp tool' bloodletting treatment, while the "$Su\scute{s}hruta$" introduces many surgical methods such as the use of gourd dippers, horns, sucking the blood with leeches. Also the "$Su\acute{s}hruta$" has 19 chapters specializing in ophthalmology, and shows 76 types of eye diseases and their treatments. 9) Since anatomy did not develop in Indian medicine, the inner structure of the human body was not well known. The only exception is 'GuXiangXue(骨相學)' which developed from 'Atharvaveda' times and the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$". In the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$"'s 'ShenTiLun(身體論)' there is a thorough listing of the development of a child from pregnancy to birth. The '$\bar{A}yurveda$' is not just an ancient traditional medical system but is being called alternative medicine in the west because of its ability to supplement western medicine and, as its effects are being proved scientifically it is gaining attention worldwide. We would like to say that what we have researched is just a small fragment and a limited view, and would like to correct and supplement any insufficient parts through more research of new records.

피부병의 형상의학적 치료 (Treatment of Skin Diseases in Hyungsang Medicine)

  • By treating skin diseases with Hyungsang medicine and reviewing the clinical cases of them through classifying skin diseases into general care and into Hyungsang medical care, the writer came to the conclusion as followings. The skin is a house of Kyungnak, and it' s a place which Ki comes in and out, which human body controls conditions of his physiological function and which doctors can make a diagnosis from seeing it. The skin is subject to lung and large intestine in the point of organ, but in the point of movement, to spleen and stomach. Skin diseases come from unbalance of Ki and Hyul, mostly from fever in blood and also from Dam, Wind, Dampness and Dryness because of including fire. Generally skin diseases are treated according to classifying into distinctive symptoms, region, cause or time being occurred, and concomitant symptoms. After being divided into man and woman, age and youth, fatness and leanness, Jung Ki Shin Hyul type, visceral figures, Yukkyung shapes on Hyungsang medicine method, the remedy of skin diseases which is combined common meditation with Hyungsang medical method is efficacious. In distinguishing into man and woman, age and youth, man gets sick mainly by drinking liquor and exhausting Jung, woman is taken ill generally by indigestion and Damwha, especially by drinking liquor. Young child gets ill mostly from inherent weakness, but in many cases he is occurred by disturbed digestion, so he must be taken care of ingesting food. The aged becomes ill from weakening, so he must be treated according to reasons of diseases. To fatness and leanness, skin diseases are showed symptoms by weakened Yang, insufficient Yin or excess fever on the theory of upper and lower parts, by unbalance of Ki and Hyul on the theory of left and right, by Dryness and Dampness on the theory of front and rear and by insufficiency of vitality and combined fever on the theory of inside and outside. Therefore doctors must cure the symptoms according to the theory of upper and lower, left and right, front and rear and inside and outside. In type of Jung Ki Shin Hyul, the balances between Jung-Hyul and Shin-Ki are important points. Concretely Jung type comes to easily shortage of Jung or gathering abnormally Dampness and Dam, Ki type is easy to make a mass with Ki or to be deficient of Ki, Shin type comes to press seven emotions togather or to be insufficient of Yin and fever inside, and Hyul type is inclined to be deficient of Hyul and to become feverish or to be deficient of Ki. Therefore on the ground of the above reasons skin diseases come into existence. Inclusively in Jung-Hyul type , Ki must be moved on the base of full Jung-Hyul, so Wukmijiwhangtang or Samultang is prescribed basically. In Shin-Ki type , Jung-Hyul is saved on the base of moving Ki, so Hyangsosan or Yijintang is the proper prescription. Considering the visceral figures, doctors judge for diagnosis and remedy by putting together the elements, that is, great and small sizes of a set of features (eye, ear, mouth, nose), color and temper. The yukkyung types are classified according to many or few of Ki-Hyul, and then skin diseases appear by being inclined toward one side between Ki and hyul, or among the six atmospheric elements(wind, cold, heat, dampness, dryness, and fire). Especially because Yangmyung type is full of much Ki and Hyul, and also has much fever in stomach, he gets skin symptoms frequently. So his prescription is based on taking off fever in stomach, and also he must be careful about regimen certainly.

아미노산 수송체 TAT1에 의한 방향족 아미노산의 수송특성 (Transport Properties of Aromatic Amino Acids by Amino Acid Transporter TAT1)

  • hTATl에 의해 수송되는 방향족 아미노산들의 수송특성을 밝히기 위해 hTATl의 cRNA를 미세주입한 Xenopus laevis oocyte에서 hTATl에 의 해 유도되는 방향족 아미노산의 up-take를 여러 조건 하에서 관찰하였긴. hTATl은 L-[$^{14}$ C]tryp-tophan의 uptake를 유도하였으며, 그 uptake는 $Na^{+}$-과 Cl$^{-}$-비 의존적이었다. hTATl은 L-($^{14}$ C)tryptophan의 uptake를 시간의 존적으로 유도함을 알 수 있었다. hTATl에 의한 L-($^{14}$ C) tryptophan의 uptake는 방향족 아미노산인 phenylalanine, tyrosine 및 tryptophan에 의해서 억제되었으며 , hTATl에 의한 아미노산들의 uptake 실험에서 L-($^{14}$ C)phenylalanine, L-($^{14}$ C)tyrosine 및 L-($^{14}$ C)tryptophan의 수송을 확인하였다 hTATl에 의 한 L-($^{14}$ C)tryptophan의 uptake는 포화되었으며, Km치는 452.2$\pm$27.8 UM, V$_{max}$ 값은 2.1 $\pm$0.3 pmol/oocyte/min 이었다. L-($^{14}$ C)tyrosine 및 L-[$^{14}$ C]phenylalanine의 Km치는 각각 636.3$\pm$59.4 UM과 740.5$\pm$96.7 HM이었다. 실험용액의 pH 5.5에서 8.5까지의 변화는 hTATl에 의한 L-[$^{14}$ C]tfpto- phan의 uptake에 별다른 영향을 미치지 못하였다. hTATl의 CRNA를 미 세주입한 oocyte에서 배양시간 의존적 인 L-($^{14}$ C) tryptophan의 efflux를 볼 수 있었으며, 이 efflux는 oocyte 외 용액의 tryptophan존재 유무에는 영향을 받지 않았다. 따라서 본 연구의 결과로 hTATl이 상피세포로부터 혈류로의 방향족 아미노산의 수송에 중요한 역할을 할 것으로 사료된다.

발효과정이 솔잎 착즙액의 항산화, alpha-Glucosidase 및 Angiotensin Converting Enzyme 저해 활성에 미치는 영향 (Effects of Fermentation on the Metabolic Activities of Pine Needle Juice)

  • 솔잎착즙액과 솔잎발효액의 항산화활성을 분석하기 위하여 총 페놀함량, DPPH 라디칼 소거능, 총 항산화능, $ORAC_{ROO{\cdot}}$ 활성, CAC 활성을 분석하였고, 항유전독성을 분석하기 위하여 DNA 손상 억제능을 분석하였으며, 항당뇨효과와 항고혈압 효과를 분석하기 위하여 각각 ${\alpha}$-glucosidase 및 ACE 저해활성을 분석하였다. 총 페놀 함량은 솔잎착즙액(17.3 mg GAE/g)이 솔잎발효액(4.6 mg GAE/g)보다 유의적으로 3.7배 높았으며, 이는 발효가 진행됨에 따라 페놀 성분이 침전된 결과로 보인다. DPPH 라디칼 소거능, 총 항산화능 및 $ORAC_{ROO{\cdot}}$ 활성은 솔잎착즙액이 솔잎발효액 보다 유의적으로 높았다. 즉 1 mg/mL 수준에서 솔잎착즙액과 솔잎발효액의 DPPH 라디칼 소거능은 각각 33.3%, 21.4%로 나타났고, $60{\sim}530{\mu}g/mL$ 농도에서 솔잎착즙액이 솔잎발효액보다 유의적으로 높은 총 항산화능을 나타내었으며, $2{\sim}100{\mu}g/mL$ 농도에서 솔잎착즙액이 솔잎발효액보다 유의적으로 높은 $ORAC_{ROO{\cdot}}$ 활성을 나타내었다. 솔잎착즙액과 솔잎발효액의 CAC 활성은 AAPH 처리군보다 솔잎착즙액 및 솔잎발효액 처리군의 CAC 활성이 농도 의존적으로 증가하였으며, $50{\mu}g/mL$ 농도를 제외하고 두 그룹 간의 유의적인 차이는 나타나지 않았다. 솔잎착즙액과 솔잎발효액의 항산화활성의 상관관계는 총 항산화능과 DPPH 라디칼소거능(r=0.836, p=0.000) 및 ORAC assay(r=0.918, p=0.000) 간의 높은 양의 상관관계가 나타났을 뿐만 아니라 DPPH 라디칼 소거능과 ORAC assay(r=0.909, p=0.000) 간에도 높은 양의 상관관계가 나타나 솔잎착즙액과 솔잎발효액의 높은 항산화활성을 추측할 수 있었다. $H_2O_2$로 유도된 산화적 스트레스에 대한 DNA 손상 억제능은 솔잎착즙액과 솔잎발효액에서 농도의존적으로 증가하였으며, $50{\mu}g/mL$ 농도에서 솔잎발효액이 솔잎착즙액보다 더 높은 경향을 나타내었다. 솔잎착즙액과 솔잎발효액의 항당뇨 효과를 알아보기 위하여 ${\alpha}$-glucosidase 억제능을 실험한 결과 희석배율이 증가할수록 솔잎발효액의 ${\alpha}$-glucosidase 억제능은 급격히 감소하는 반면, 솔잎착즙액은 솔잎발효액보다 높은 활성을 유지하는 것으로 나타났다. 항고혈압 효과 분석을 위한 ACE 저해활성은 2배 희석 시 솔잎착즙액 87.1%, 솔잎발효액 60.0%로 솔잎착즙액의 ACE 저해활성이 높은 경향을 나타내었다. 본 연구를 통해 솔잎착즙액의 다양한 생리활성을 평가하고자 하였으며, 발효가 이러한 생리활성에 미치는 영향을 분석하고자 하였다. 본 연구에서 2년간 발효한 솔잎발효액만을 분석하였고, 발효기간에 따른 활성의 변화를 평가하지 못한 제한점이 있으나 본 연구의 결과는 지금까지 연구가 미흡했던 솔잎착즙액의 생리활성을 보고함으로써 솔잎을 활용한 기능성 소개개발에 유용한 자료로 제안될 수 있을 것이다. 또한 향후 연구에서 솔잎착즙액의 기능성을 증대시킬 수 있는 발효과정에 대한 연구가 수행되어야 할 것이다.
