Effects of Essential Oil Gargling and Purified Water Gargling on Thirst, Oral Condition and Halitosis of Postoperative Patients (에센셜오일 가글링과 정수 가글링이 수술 후 환자의 갈증, 구강상태 및 구취에 미치는 효과)

  • Purpose: The main purpose of this study was to effect oral care methods between essential oil gargling and purified water gargling for postoperative patients who had general surgery or orthopedic surgery. Methods: The postoperative patients were assigned to one of two groups. One group gargled with essential oil and the other with purified water. All group gargled three times interval 2 hours. Each patients thirst, oral condition and halitosis were assessed four times. Results: After oral care was provided once, there were significant differences in thirst level between two groups. when oral care was provided once and three times, there were significant differences in oral condition between two groups. but there were no significant differences in halitosis between two groups. Conclusion: The results show that essential oil gargling is a more effective intervention than purified water gargling for post operative patients oral care in reducing the thirst level and improving the condition of the oral cavity.

Effect of Auricular Acupuncture for Mental Stress on Heart Rate Variability(HRV) (이침 요법이 정신적 스트레스를 가한 성인의 심박변이도에 미치는 영향)

  • Objectives : The purpose of this study was to assess the effect auricular acupuncture for acute mental stress using power spectrum analysis of the heart rate variability(HRV). Methods : 10 healthy volunteers participated in this study. After 5 minute rest, the first mental stress was provided for 5 minute. And then subjects rested for 15 minute. The second mental stress was provided for 5 minute. The acupoint, Shin-Mun point of the ear was stimulated for 15 minute. HRV was recorded before and after the first and second mental stress, and after auricular acupuncture stimulation. Results : After mental stress, normalized LF and LF/HF ratio is significantly increased. Before and after simple rest, normalized LF and normalized HF is significantly changed, but LF and LF/HF ratio is not significantly changed. On the other hand, before and after auricular acupuncture treatment, normalized LF, normalized HF is significantly changed, and also LF and LF/HF ratio is significantly decreased. Conclusion : The result suggest that auricular acupuncture can decrease more significantly in cardiac sympathetic activity due to mental stress than simple rest.

  • The purpose of this study was to analyze the differences in the outcome for CABG according to whether hospitals provided heart related surgeries. The 2011 National Inpatient Sample (NIS) and inpatient quality indicator principles from the Healthcare Research and Quality (AHRQ) were used for analysis. Hospitals were divided into three groups according to the surgeries they provided. The length of stay and in-hospital deaths were adjusted for the differences in risks. ANOVA was performed to examine the differences for the risk-adjusted in-hospital mortality rate and risk-adjusted length of stay among the three groups. The analysis results showed that hospitals providing CABG, PTCA, and PHS had lower risk-adjusted in-hospital mortality rates or similar risk-adjusted lengths of stay compared to those of hospitals providing only CABG. However, the three groups did not have statistically significant differences in outcome indicators. Another study will be needed with a larger sample.

  • In the recent years, with a purpose to provide visual and psychological stability to patients and hospital users, Affordance design theory based on perceptual psychological thought has been applied as hospital design. In the past, hospital service design was focused on but Affordance design theory focuses on the patients' experience and methodological research on . The characteristics of the application of Affordance design in the case of patients' experience and service point is as follows:- Firstly, Affordance design is applied when the objective of the patient's experience and the functionality of the object are combined. In other words, affordance design is a condition that arises when encounter between service contact point and patients' experience takes place. Secondly, In the field of Hospital service design, Affordance concepts in and its application can be divided on the functions of experience, cognition and sense following the application of Affordance concepts in Hospital service design. Thirdly, Affordance design method sets meaning of the action received through information provided by morphological features. The condition and surrounding has to be designed in a way that it can induce active behavior of the patient. This study has significance as it has been studied from the perspectives of Affordance theory of design psychology in addition to the physical environment of the hospital, the point of interaction between the hospital and patient, along with the elements of experience design. This study proposes the necessity of analytical study in the field of service design implementation considering various stakeholders in the service contact points of small and medium hospitals in Korea. Furthermore, study of the application of Affordance design can be complemented by reflecting the size of hospitals, the characteristics of specialties, and the specific service environment of each hospital can also be conducted.

  • Now, Korean health technology and health service is proud to be world class one at a level of not falling behind anywhere in the world. However, this health service has been provided, centering on hospital or medical staff, not patient so far. And there is a tendency to gradually change into patient-centered health service. According to the results of survey performed by Korea Institute for Health and Social Affairs, it was reported that, as of 2016, 30% or more of inpatients were not sufficiently given information relating to hospital rounds, and in particular, 50% or more of inpatients in their twenties to thirties expressed discontent, and approximately 17% of inpatients in their seventies and beyond expressed discontent. In the light of these data, especially inpatients in their twenties to thirties want information relating to hospital rounds and the like. And taking into consideration that they are characterized by being accustomed to using a smartphone, it is intended to design and implement a hospital rounds information smartphone application. Now, in some of medium and large hospitals, information about hospital is provided through smartphone application. However, the main purpose is hospital information or outpatient's appointment to see a doctor and so on. Even in case of providing information about hospital rounds, only information about approximate time to make rounds is provided. This paper is intended to design and implement hospital rounds information system making it possible to check their turn when rounds are made by utilizing NFC function in smart phone.

Analysis of Parental Decisions Based on Sex Chromosome Abnormalities Detected Prenatally: A Ten-year update (2001-2010)

  • Purpose: The aim of this study was to analyze parental decisions regarding pregnancies in which the fetus had sex chromosome abnormalities (SCA) over a ten-year period. Materials and Methods: We collected and reviewed records from our hospital for 2001-2010 and a genetic specialist provided-genetic counseling. Results: We diagnosed 130 cases (0.71%) with SCA out of 18,376 prenatal cases from 2001 to 2010. We reviewed the records and the results of all pregnancies. We also included cases (n=84) of apparently normal anatomic fetuses to analyze the factors influencing parental decisions. We excluded 34 cases with an obvious anomaly or a presumably bad outcome and 12 cases that were not followed up. Forty-three couples (51.2%) continued their pregnancies while forty-one (48.8%) terminated them. Of 38 mosaicism cases, 21 (55.3%) were continued. Among the 20 pregnancies assisted by reproductive techniques, 15 (75%) were continued (P=0.02). More pregnancies were continued when genetic counseling was provided (61.9%) compared to cases in which it was not provided (19%) (P=0.01). Conclusion: Genetic counseling is important in providing appropriate information to parents. Establishing guidelines and protocols will help both obstetricians and parents to make informed decisions.

The Influence of Inpatient's Experience on Hospital Recommendation Intention - Focusing on the Moderating Effects of Health Condition - (입원 환자경험이 병원 추천의도에 미치는 영향 - 건강상태의 조절 효과를 중심으로 -)

  • Purpose : This study is to analyze the inpatients's experience of medical services provided by hospital including medications, treatments, and environment. Based on the results of surveys conducted as part of the inpatient experience evaluation in A hospital in Goyang, Gyeonggi province. Methodology : A sample of 300 adults aged 19 years or older who had more than one day of hospitalization was selected. The questionnaire was conducted from April 3rd to June 21st, 2017 by telephone. Findings : It is found that recommendation intention influenced by medical services, hospital environment, medication treatment process. but it turns out that there is no moderate effects of health condition between patient's experience and recommendation. Practical Implication : In order to improve the inpatient experience, there should be a way to improve experience in providing patient-centered services in the hospital s environment, medication and treatment.

Proper Time of the Sterile Tracheal Suction Catheter Change Based on Nursing Clinic (기도흡인용 카테터의 교환시기를 규명하기 위한 임상적 근거)

  • The purpose of this study was to identify the proper changing time of tracheal suction catheter by examining the microorganisms inside after used The samples were the adult and the elderly patients who were in the intensive cane unit of one university hospital in Korea from the early December, 2002 to the end of May 2003. Since the patients needed tracheal suction, sterile catheter including other equipments were used whenever tracheal suction provided, to the experimental group. The sterile catheter with others was provided when the catheter repeatedly used for four hours, to the comparative group. Research was permitted by the head of hospital administration, hospital nursing department; microbiology and intensive care unit. The main results of the study are as follows. 1) The demographic characteristics between the two groups present statistically no difference. 2) Every the experimental group showed various kinds of microorganisms, the numbers of the kinds and carries of the microorganisms were less than those of the comparative group. 3) The mean numbers of the microorganisms between the two groups were significantly different, p < 0.05. This study results strongly emphasize the needs of using sterile catheter whenever a nurse suctions to keep patient's airway open.

An Assessment of Factors Affecting Plate Waste and Its Effects in Normal & Soft Diets Provided from Hospital Foodservice (병원 환자급식 중 일반식과 연식에서 발생되는 잔반의 원인과 손실 분석)

  • The purposes of this study were to : (a) analyze the portion size & plate waste of normal & soft diets provided by dietetic departments in hospital, (b) investigate the factors affecting plate waste, and (c) determine the nutritional & monetary value of plate waste. A questionnaire for food preference and sensory evaluation was developed and a survey was carried out for the 46 normal diet and 56 soft diet patients in Sanggye Paik hospital in Seoul. Serving size and plate waste were weighed by the electric scale, and CAN-Pro program was used to evaluate the nutrition value. The data were analyzed using the SAS package program for descriptive analysis, t-test, ANOVA, and Pearson correlation. The average plate waste rate for normal diet and soft diet were 30.3% and 49.6%. More plate waste amount occurred on female patients'diet than male patients'diets regardless of the kind of diet. The plate waste percentage of male patients were higher than those of female on normal diet. On soft diet, patients who hospitalized in surgery or pediatrics department were higher than in other wards. Plate waste of normal diet and amount were correlated positively, but wastage and preference were correlated negatively Negative correlation was between taste and plate waste, and between temperature and plate waste, too. On the other hand, plate waste caused the deficiency of some nutrients such as Ca, Fe, Vit. B$_2$and a great monetary loss.

A Study on The Use of Kimchies in Dodium restricted Diet of Hospital Food Service Operation (병원합식의 염분제한식에 있어서의 김치이용에 관한 연구)

  • Although Korean people like Kimchi very much, Kimchi is generally limited in sodium restricted diet of hospital food service operation. The use of Kimchi in sodium restricted diet can improve appetite and nutritional status of patients. In this study, four kinds of Kimchi(Kwail-Nabakji, susan-Nabakji, suk-Gakduki, Oi-Gakduki) were Prepared and analyzed for their Sodium contents. the preference of Kwail-Nabakji and susan-Nabakji was compared with low sodium Juciy kimchi provided in the hospital in 25 patients who were restricted in sodium intake. The result were as follows: 1. In 2 kinds of low sodium juicy Kimchi that salt not added sodium contents of kwail-Nabakji(fruti-juicy Kimchi) and susan-nabakji(ginseng-Kimchi) were 17.8 mg/100g a 11.0 mg/100g, respectively. 2. The otehr 2 kimchies were prepared by adding dilute salted shrimp broth. suk-Gakd uki(boild radish-Kimchi) and Oi-Gakduki(cumcuber-Kimchi) had 89.8 mg/100g and 111.6 mg/100g sodium, respectively. 3. Na/k ratios of 4 kinds of low sodium kimchi were in the range of 0.34-0.62 which were lower than that of general Kimchies. 4. The patients preferred kwil-nabakji and susan-nabakji to low sodium juicy Kimchi provided in the hospital, and most liked Kwai-Nabakju. Therefore low sodium Kimchies can be recommended in sodium restricted diet because they have lower sodium contents then general Kimchies.
