Prevalence of Antibody for Hepatitis A Virus among Hospital Workers

  • Hepatitis A virus (HAV) is an epidemiologically important virus with a worldwide distribution. It causes acute hepatitis in humans. HAV infection is often subclinical or asymptomatic in children; however, symptomatic acute infections become more common with age. In this study, we investigated the prevalence of anti-HAV among hospital workers in Jeonbuk province. 447 (127 males, 320 females) persons were included in the study from January to June, 2011. Anti-HAV (total and IgM) in the sample serum was measured by VIDAS Hepatitis A (bioMerieux, France), and IgM positive samples were tested in terms of their serum alanine aminotransferase (ALT) levels by a HITACHI 7600-010 automatic analyzer (HITACHI, Japan). The overall prevalence of total anti-HAV was 45.9% (205/447), and rate for males (60.6%) was higher than that for females (40%). According to the age group, the prevalence rates of total anti-HAV were 20% (1/5) in those under 20 years old, 11.3% (18/160) in those 20-29 years, 49.7% (71/143) in those 30-39 years, 86.5% (83/96) in those 40-49 years, 92.9% (26/28) in those 50-59 years, and 40% (6/15) in those over 60 years old. The total prevalence of anti-HAV IgM was 0.8% (4/447), and rate for males (1.6%) was higher than that for females (0.6%). Through this study, we determined the prevalence of anti-HAV among hospital workers in Jeonbuk province. The prevalence of the HAV antibody overall was low in hospital workers under 40 years old and in those over 60 years old. Therefore, an improvement in sanitary conditions and hygiene and vaccinations against HAV in this population are recommended.

Study of Work-Related Musculoskeletal Pain and Health-related Quality of Life among Hospital Workers

  • Purpose: This study examined the work-related musculoskeletal pain and quality of life of hospital workers. Methods: Self-reported questionnaires were sent to 350 hospital workers at Seoul and Gyeonggi-do, of which 341 were returned. The questionnaire had four items that covered the demographic information, areas of musculoskeletal problems, pain rating scale, and quality of life. The analysis was completed using descriptive statistic, and the differences between pain and demographic variables were identified using a chi-square test. The differences between the type of occupation and quality of life were analyzed by one-way analysis of variance and a Kruskal-Wallis test. Results: The 12-month prevalence of work-related musculoskeletal pain was 86.1% of physical therapists, 86.5% of occupational therapists, 77.1% of dental hygienists, and 75.8% of nurses. A significant difference in the general and work related variables was observed between the subject with a physical burden and type of occupation. The most affected pain sites of the physical therapist included low back, hand and wrist, shoulder, and neck. The occupational therapists included the hand and wrist, shoulder, neck, but the nurses and dental hygienists reported the shoulder, back, hand and wrist. A significant difference in the quality of life was noted between the subjects in physical therapists and dental hygienists and the subjects in the nurses and occupational therapists (p=0.00). Conclusion: These findings show that hospital workers appear to be high risk for work-related musculoskeletal pain, and the quality of life of physical therapists and nurses was higher than that of dental hygienists and occupational therapists.

비의료직 병원 종사자들의 감정노동과 신체증상 및 불면증 간의 관계에서 우울의 매개효과 (The Mediating Effect of Depression on the relationship between Emotional Labor and Somatic Symptoms and Insomnia in Non-Medical Hospital Workers)

  • 본 연구의 목적은 비의료직 병원 종사자의 감정노동과 신체증상 및 불면증 간의 관계에서 우울의 매개효과를 검증하여, 이들의 감정노동과 건강문제에서 위험을 예방하는 것이다. 이를 위하여 상급종합병원 비의료직 종사자 533명에게 한국형 감정노동 평가척도, 신체증상 질문지, 불면증 심각도 질문지, 우울 질문지를 실시하였다. 수집된 자료는 SPSS 25.0 프로그램으로 분석하였다. 주요한 결과는 다음과 같다. 1. 감정노동 하위요인 중에서 감정 부조화가 우울, 신체증상, 불면증 모두에 유의한 영향을 미쳤다. 다른 하위요인들은 영향이 없거나 그 영향력이 상대적으로 작았다. 2. 감정부조화와 신체증상 간의 관계에서 우울의 부분매개효과가 나타났다. 3. 감정부조화와 불면증 간의 관계에서 우울의 완전매개효과가 나타났다. 이러한 결과를 기초로, 비의료직 병원 종사자의 감정노동 및 건강문제에 감정부조화와 우울을 다률 필요성을 제언한다.

해외파견 병원근로자의 문화적응 스트레스와 직무만족이 조직몰입에 미치는 영향 -아랍에미리트 중심으로- (The Effects of the Acculturation Stress and Job Satisfaction on Organizational Commitment among dispatched hospital-workers in United Arab Emirates)

  • 본 연구는 해외파견 병원근로자를 대상으로 문화적응 스트레스와 직무만족이 조직몰입에 미치는 영향을 파악하기 위해 시행되었다. 이를 위해 아랍에미리트에 파견된 병원근로자 107명을 설문조사하여 SPSS 25.0을 이용하여 분석하였다. 연구결과는 첫째, 문화적응 스트레스는 평균 83.01점, 직무만족은 평균 26.61점, 조직몰입은 평균 20.85점으로 나타났다. 둘째, 문화적응 스트레스가 낮을수록 직무만족도와 조직몰입은 높게 나타났으며, 직무만족도가 높을수록 조직몰입은 높게 나타났다. 셋째, 조직몰입에 영향을 미치는 요인으로는 직무만족도가 가장 높게 예측되었으며 72%의 설명력을 나타내었다. 그 밖의 요인으로는 가족동반, 해외체류경험, 사전교육, 문화적응 스트레스가 조직몰입에 영향을 미치는 것으로 나타났다. 그러므로, 해외파견 병원근로자의 조직몰입향상을 위해서 문화적응 스트레스를 줄이고 직무만족도를 높이는 다양한 프로그램 개발 및 방안이 마련되어야 하겠다.

일개 병원의 환자안전문화에 대한 인식 (A Study on Worker's Perception of Patient Safety Culture in a hospital)

  • Background : The purpose of study in to grasp the level of perception of hospital workers on the patient safety culture, consider the difference in perception of patients safety culture according to medical service and finally find out a way to establish patient safety culture in hospital. Methods : As for the data, the analysis on frequency, t-test, ANOVA and tukey test were carried out by using SPSS 12.0. Result : The results of comparison among the positive response ratios on the patients culture of hospital workers showed that the subjects had perceived the teamwork within units most positively(74.1%), and perceived most negatively on the non-punitive response to error(16.2%)and the staffing(26.2%). 68.6% of subjects answered that the medical error were mostly of always reported. when daytime working hours are longer, perception of patient safety culture ranked low. In general, departments for direct medical service than departments for indirect medical service assessed patient safety culture high. Conclusion : Organizational learning and teamwork within units, communication openness, active support of hospital management for patient safety, and cooperation across the units would be crucial to promote the overall perceptions of patients safety of hospital workers and the level of patients safety in the units and to improve the quality of the event reporting system.

건강보험 빅데이터를 통한 전체 근로자 및 공무원 근로자의 암 발생률 분석 (Analyzing Cancer Incidence among Korean Workers and Public Officials Using Big Data from National Health Insurance Service)

  • Objectives: This study aimed to establish a control group based on the big data from National Health Insurance Service. We also presented presented the number of incidences for each cancer, and analyzed the cancer incidence rate among Korean workers. Methods: The cohort definition was separated by 'baseline cohort', 'dynamic cohort', and 'fixed- industry cohort' according to the definition. Cancer incidence was calculated based on the Korean Standard Classification of Disease code. Incidence rate was calculated among the group of all workers and public officials. Based on the study subjects and each cohort definition, the number of observations, incidences, and the incidence rate according to sex and age groups was calculated. The incidence rate was estimated based on the incidence per 100,000 person-year, and 95% confidence intervals calculated according to the Poisson distribution. Results: The result shows that the number of cancer cases in the all-worker group decreases after the age of 55, but the incidence rate tends to increase, which is attributed to the retirement of workers over 55 years old. Despite the specific characteristics of the workers, the trend and figures of cancer incidence revealed in this study are similar to those reported in previous studies of the overall South Korean population. When comparing the incidence rates of all workers and the control group of public officials, the incidence rate of public officials is generally observed to be higher in the age group under the age of 55. On the other hand, for workers aged 60 or older, the incidence rates were 1,065.4 per 100,000 person-year for all workers and 1,023.7 per 100,000 person-year for civil servants. Conclusions: This study analyzed through health insurance data including all workers in Korea, and analyzed the incidence of cancer of workers by sex and age. In addition, further in-depth researches are needed to determine the incidence of cancer by industry.

Effect of night shift work on the reduction of glomerular filtration rate using data from Korea Medical Institute (2016-2020)

  • Background: Shift work increases the risk of chronic diseases, including metabolic diseases. However, studies on the relationship between shift work and renal function are limited. The aim of this study was to investigate the association between shift work and a decreased glomerular filtration rate (GFR). Methods: Data were evaluated for 1,324,930 workers who visited the Korean Medical Institute from January 1, 2016 to December 31, 2020 and underwent a health checkup. Daytime workers were randomly extracted at a ratio of 1:4 after matching for age and sex. In total, 18,190 workers aged over 40 years were included in the analyses; these included 3,638 shift workers and 14,552 daytime workers. Participants were categorized into the shift work group when they underwent a specific health checkup for night shift work or indicated that they were shift workers in the questionnaire. The odds ratio was calculated using a conditional logistic regression to investigate the relevance of shift work for changes in GFR. Results: 35 workers in the shift group and 54 in the daytime group exhibited an estimated GFR (eGFR) value of < 60 mL/min/1.73m2 (p < 0.01). The difference in eGFR values between two checkups differed significantly depending on the type of work (p < 0.01); the difference in the shift work group (-9.64 mL/min/1.73 m2) was larger than that in the daytime work group (-7.45 mL/min/1.73 m2). The odds ratio for eGFR reduction to < 60 mL/min/1.73 m2 in the shift group versus the daytime group was 4.07 (95% confidence interval: 2.54-6.52), which was statistically significant. Conclusions: The results of this study suggest that eGFR decreases by a significantly larger value in shift workers than in daytime workers; thus, shift work could be a contributing factor for chronic kidney disease (CKD). Further prospective studies are necessary to validate this finding and identify measures to prevent CKD in shift workers.

  • In this paper, we propose a plan for the introduction of a knowledge-based system proposed by the traditional computing sciences, in order to operate the computer system computerized hospital workers effectively. Hospital computational features of the operating system for this purpose, were made about the working conditions, computational difficulties of the hospital administrator. It is necessary for a knowledge management system for results-effective IT system operational review findings apply to the hospital, where a hospital computer system having features and hospitals computational workers must be resolved on the technical and environmental constraints with It became aware of the need. This study further shows that an effective hospital computer system to derive additional operating a variety of issues to consider for the base and the introduction of a knowledge management system can gradually solve the problem.

Association between coronavirus disease 2019-related workplace interventions and prevalence of depression and anxiety

  • Background: Although coronavirus disease 2019 is causing a variety of psychological problems for workers, there are few longitudinal studies on changes in workers' mental health by workplace intervention. This study aimed to evaluate the change in the prevalence of depression and anxiety according to the active involvement of the workplace. Methods: This study was conducted on 1,978 workers at a workplace who underwent a health screening from January 2019 to August 2020, and classified depression and anxiety disorders using a self-report questionnaire. After the first pandemic, the company stopped health screening, took paid leave and telecommuting, and conducted interventions such as operating its own screening clinic. To see if this workplace intervention affects workers' mental health, we conducted generalized estimating equations to compare odds ratio (OR). Results: In the pre-intervention group, 384 people (16.86%) had depression, and 507 people (22.26%) had anxiety disorder. Based on the OR before intervention, the OR of depression decreased to 0.76 (0.66-0.87) and the OR of anxiety disorder decreased to 0.73 (0.65-0.82). Conclusions: As a result of this study, it was confirmed that workplace intervention was related to a decrease in depression and anxiety. This study provides basic data to improve workers' mental health according to workplace intervention, and further research is needed according to workplace intervention in the future.

Association between Work-related Communication Devices Use during Work Outside of Regular Working Hours and Depressive Symptoms in Wage Workers

  • Background: This study aimed to investigate the relationship between work-related communication devices use during work outside of regular working hours and depressive symptoms in wage workers. Methods: Data from 50,538 workers aged 15 years or older who had participated in the 6th Korean Working Condition Survey (KWCS) were used. The final sample was 32,994 wage workers. The questionnaire asked the respondents how often they used communication devices for work during work outside of regular working hours. Depressive symptoms were assessed using WHO-5 Well-Being Index. Multiple logistic regression analysis was used to analyze the association between work-related communication devices use during work outside of regular working hours and depressive symptoms. Results: The rate of depressive symptoms was highest among workers who did not use work-related communication devices during work outside of regular working hours. After adjusting for socio-demographic and work-related factors, the odds ratio of depressive symptoms among workers who used communication devices when working outside of regular working hours was 1.20 (95% CI: 1.09-1.32); the odds ratio of depressive symptoms in the group not using communication devices for free-time work was 1.66 (95% CI: 1.37-2.00), which was higher than that of the reference group, that is, workers who did not work outside of regular working hours, and was statistically significant. Conclusion: Regardless of whether work-related communication devices are used, working outside of regular working hours increases depressive symptoms. The use of work-related communication devices during work outside of regular working hours can reduce the rate of depressive symptoms.
