요양병원 근무 간호사의 직무배태성에 미치는 영향: 근무환경과 역할갈등 중심으로 (The Effects of Nursing Work Environment and Role Conflict on Job Embeddedness among Nurses of Long-term Care Hospital)

  • 본 연구는 요양병원 근무 간호사의 근무환경, 역할갈등 및 직무배태성을 확인하고 직무배태성에 미치는 영향요인을 파악하기 위하여 수행되었다. 본 연구를 위하여 일개 도에서 자료 수집을 허락한 10개 기관에서 총 200명의 간호사를 대상으로 2018년 7월과 8월에 걸쳐 자료를 수집하였다. 본 연구에서는 구조화된 설문지를 통해 일반적 특성과 간호사의 근무환경, 역할갈등 및 직무배태성을 평가하였으며 배포한 200부 중 분석에 적합한 190부를 최종 분석하였다. 수집된 자료는 SPSS 버전 25.0를 이용하여 서술적 통계, t-test, ANOVA, Pearson's correlation coefficients, 다중회귀분석을 실시하였다. 분석 결과 직무배태성은 5점 만점에 2.98±0.46점이었고, 하위영역별로는 적합성, 연계, 희생 순으로 나타났다. 간호사의 근무환경은 평균 3.14±0.42점이었고 하위영역별로는 수간호사의 리더십, 실무체계, 동료와의 관계, 기관의 지원 순이었다. 역할갈등은 평균 3.43±0.51점이었고 하위영역 중 환경장애가 가장 높았다. 직무배태성은 간호사들이 인식하는 근무환경과 통계적으로 유의한 정적 상관관계를 보였고, 역할갈등과는 통계적으로 유의한 부적 상관관계를 보였다. 최종적으로 본 연구의 종속변수인 요양병원 근무 간호사들의 직무배태성에 영향을 미치는 요인은 근무환경, 연령, 역할갈등이었으며 설명력은 50.4% 이었다. 본 연구결과를 통해 요양병원에 근무하는 간호사들의 근무환경, 역할갈등과 직무배태성을 확인하였으며 현재 문제가 되고 있는 간호사들의 높은 이직률과 관련하여 직무배태성 제고를 위한 방향성을 점검하였다. 즉 요양병원 조직의 차원에서 간호근무환경 개선과 역할갈등 완화 노력이 요구되는데 이를 위한 조직과 정책적 뒷받침이 필요해 보인다. 또한 근무 중인 간호사 연령층의 특성을 고려할 필요성을 확인한 것 역시 의미있는 발견이라 하겠으며 앞으로 더욱 가속화될 고령인구 증가와 더불어 요양병원 근무 간호사들의 직무배태성 제고에 더욱 관심을 가질 필요성이 제기된다.

수도권 200병상 이상 노인요양병원 간호사의 이직의도에 영향을 미치는 요인 (Factors Influencing Nurse Turnover Intention of Senior Convalescence Hospitals in the Metropolitan Area)

  • Purpose: This study was a descriptive research to investigate the factors influencing nurses' turnover intention of Senior Convalescence hospitals in the metropolitan area. Methods: A cross-sectional survey design was used. A questionnaire was distributed to the nurses in Senior Convalescence hospitals. The data of 210 nurses were analyzed using the descriptive statistics, t-test, ANOVA, Pearson correlation coefficient and multiple regression. Results: Turnover intention was significantly correlated with professionalism, job involvement, organizational commitment, job stress, practice environment. Organizational commitment, job stress, age, and practice environment were identified as factors influencing turnover intention. These factors explained 53.3% of variance of turnover intention. Conclusion: The results suggest that strategies to decrease turnover intention should be discussed and continued to develop ways to establish organizational commitment, to lower job stress levels and to improve practice environment of nursing work. And further study is needed to identify the key mechanism in nurses' turnover intention of Senior Convalescence hospitals.

요양병원 간호사의 임상의사결정능력에 미치는 영향요인 (Factors Influencing on Clinical Decision Making Ability of Nurses in Long-term Care Hospitals)

  • 본 연구의 목적은 요양병원 간호사의 임상의사결정능력에 미치는 영향요인을 규명하기 위함이다. 2021년 10월 11일부터 12월 10일까지 G도 소재 요양병원 간호사 174명을 대상으로 조사하였다. 수집된 자료의 분석은 SPSS 23.0을 이용하여 서술통계, t-test, ANOVA, Pearson's correlation, stepwise multiple regression으로 분석하였다. 연구결과 연구대상자의 임상의사결정능력은 비판적 사고성향(r=.494, p<.001), 간호근무환경(r=.451, p<.001)과 양의 상관관계를 나타내었으며, 비판적 사고성향과 간호근무환경이 임상의사결정능력을 33.1% 설명하였다. 간호사의 핵심능력인 임상의사결정능력의 향상을 위해서는 비판적 사고성향을 높이는 교육과 바람직한 간호근무환경 조성이 필요하다. 추후연구에서는 요양병원 간호사의 임상의사결정능력에 영향을 미치는 다양한 변수를 확인하는 연구와 임상의사결정능력 향상을 위한 프로그램 개발 및 효과를 위한 연구를 제언한다.

간호사가 인식한 조직의 특성과 투약오류보고장애요인간의 정준상관관계 (Canonical correlation between organizational characteristics and barrier to medication error reporting of nurses)

  • 본 연구는 간호사들이 인식한 안전 분위기와 업무환경을 포함한 조직의 특성과 투약오류보고장애요인간의 관련성을 검증하기 위한 연구이다. 수정된 안전 분위기, 업무환경, 투약오류보고장애 문항을 활용하여 7개 병원의 334명의 간호사들을 대상으로 하여 조사하였다. 자료수집기간은 2012년 9월 한달 간이었고, 서술적 통계, Pearson correlation coefficient, canonical correlation을 적용하였다. 조직의 특성은 투약오류보고장애와 세 가지 유의한 상관이 있었다. 첫 번째 정준상관계수는 .50(Wilks' ${\lambda}$=0.61, df=32, p<.001)이었고, 두 번째는 .35(Wilks' ${\lambda}$=0.81, df=21, p<.001), 세 번째는 .22(Wilks' ${\lambda}$=0.93, df=12, p=.018)이었다. 첫 번째 정준상관은 높은 안전 분위기와 업무환경이 오류보고에의 두려움을 제외한 투약오류보고와 관련이 있었다. 두 번째 식은 '의료인간의 안전 분위기'의 높은 지각과 높은 '병원에의 간호사 참여'와 '인적 자원의 적절성'은 낮은 '불안감'과 '행정자들의 반응'과 관련이 있었다. 투약오류보고장애를 위한 전략과 안전 분위기와 업무환경과 같은 조직의 특성의 진보가 적용되어야 한다.

일 대학병원 간호사의 잔류의도 영향요인 (Affecting Factors on Intention to Stay of Nurses Working at a University Hospital)

  • Purpose: The purpose of this study was to explore professional self-concept, work environment, resilience and intention to stay for nurses in university hospitals, and to identify the relationship between these variables and factors that influence intention to stay. We aimed to provide basic data to develop measures for increasing intention to stay. Methods: This study used a descriptive design, the subjects were nurses who worked for more than 1 year at a university hospital with 744 beds, located in city C. Data were collected using a structured questionnaire from 120 nurses. The data collection period were from September 15 to 30, 2020. The data were analysed using the SPSS/WIN 25.0 program. Results: There was a positive correlation between the subject's intention to stay and the professional self-concept (r=.57, p<.001), the nursing working environment (r=.29, p=.002), and resilience (r=.50, p<.001). Significant factors influencing the subject's intention to stay was the sense of satisfaction (β=.40, p<.001), a sub-factor of professional self-concept, indicating the higher the satisfaction level, the higher the intention to stay. The regression model explained 45.0% of the variance of the outcome variable (F=9.64, p<.001). Conclusion: Sense of satisfaction was identified as a factor impacting the intention to stay. Therefore, it is necessary to develop and to apply intervention programs to improve the satisfaction of the professional self-concept in order to increase the nurses' intention to stay.

병원 영양부서의 급식 및 임상영양 서비스의 중점관리 요인 (Task Analysis on Foodservice , Clinical Nutrition Service in Hospital Dietetic Departments)

  • The purpose of this research was to analyze the tasks on foodservice ․ clinical nutrition service in hospital dietetic departments. A survey of 30 hospital food and nutrition service departments was undertaken and detailed informations were collected from each, including surveys of 176 dietitians and 30 foodservice managers. Statistical data analysis was completed using the SAS/win 6.11 package for descriptive analysis and t-test. The results of this study can be summarized as follows : Regarding dietitian's training period, 38.6% dietitians answered that they spent 1~2 years in mastering food service management. 28.4% dietitians replied that they needed 2~3 years in learning clinical nutrition management. It was shown that 48.9% dietitians were engaged in food service management, while 26.7% were engaged in clinical nutrition and 26.7% were engaged in both. The 13 elements of food service management showed low performance level. These elements were food temperature, food intake, menu selection, contaminated substances and serving mistake and reliance on hospital food. These should be controlled with caution. The average score of dietitian's life satisfaction within the work place was 3.42 out of 5. The most dissatisfactory element was physical environment of the workplace. 5 elements for improving nutrition service activities showed low performance level with high importance score. Problem analysis showed low difference score between importance and performance level. Hospitals under contract foodservice management received higher points on clinical nutrition performance(P<.05) than hospitals under self-operated foodservice management.

The Effect of a Computerized Pharmacist Communication Application-based SBAR Tool

  • Background: Pharmacists communicate with a variety of healthcare experts to prevent medication errors. Situation-Background-Assessment-Recommendation (SBAR) is a tool used for concise and accurate communication. In 2018, we developed the pharmacy-SBAR (P-SBAR) to deliver pharmacists intervention more quickly and effectively through quality improvement activities. Objectives: This study evaluates the efficacy of P-SBAR on pharmacists' intervention activities before and after the implementation of P-SBAR applications. We assessed the impact of P-SBAR on reducing the burden of intervention work, promoting pharmacists' participation, and enhancing the acceptance rate. Methods: This is a retrospective study of the two groups before and after P-SBAR implementation. All pharmacists' intervention records during two periods (2016-2017 and 2019-2020) were extracted from the data warehouse system at Kyunghee University Hospital at Gangdong, Seoul. The outcome was the number of inpatients and pharmacists who participated in the prescription monitoring activity, the number of interventions, and the physicians' acceptance rate. Results: Although the total number of inpatients decreased (364,753 vs. 348,229), the number of pharmacists who participated in intervention activity increased (monthly mean: 15.8 vs. 18.0, p=0.001). The total number of interventions (2,767 vs. 4,389), the frequency of full acceptance (2,018 vs. 3,710), and the monthly acceptance rate increased significantly (73.8% vs. 83.8%, p<0.001). Conclusion: P-SBAR improved accessibility and convenience by digitalizing the intervention activities performed in an offline environment. Improvement in work burden and acceptance rate using P-SBAR is expected to contribute toward reducing medication errors.

중소병원 간호사의 직무·조직·경력 특성과 이직의도와의 관계에서 일-가정 갈등의 조절효과 (Moderating Effects of Work-family Conflict between Job·Organizational·Career Characteristics and Turnover Intention among Nurses in Small and Medium-sized Hospitals)

  • Purpose: The purpose of this study was to identify moderating effects of work-family conflict in the relationship between job, organizational, career characteristics and turnover intention among nurses working in small and medium-sized hospitals. Methods: A self report questionnaire survey was completed by 286 nurses working in five small or medium-sized hospitals in P city. Data were gathered during October, 2014 and analyzed using the SPSS 21.0 program. Results: Work-family conflict had significant moderating effects between job, organizational, career characteristics and turnover intention of nurses working in small or medium-sized hospitals. Career commitment was the biggest factor in reducing turnover intention. Conclusion: The results of the study indicate that work-family conflict and career commitment are important factors in turnover intention among nurses working in small and medium-sized hospitals. Therefore, to understand job and organizational career characteristics of nurses in small and medium hospitals, consideration must be given to the role of conflict in the nurses' workplace and homes. Support from the organization focusing on career development, and provision of an innovative system for the environment of small hospitals are needed.

신규간호사의 재직의도 영향요인 (Affecting Factors of New Nurse's Intention to Retention in Hospitals)

  • Purpose: The purpose of this study is to investigate relationship among intention to retention, role conflicts, psychosocial job conditions and job satisfaction of new nurses, and identify affecting factors for intention to retention. Methods: Data were collected from 136 new nurses who were graduated at 2016 and working now in general hospitals by self-reported questionnaire. The relationship among variables were analyzed with Pearson's coefficient correlation and affecting factors for intention to retention were identified by using multiple linear regression analysis. Results: The mean score of intention to retention was $2.63{\pm}0.28$. Intention to retention was significantly different by the working ward, hope period for work, and reason for retention. Intention to retention had positive relationships with work organization and job contents, and job satisfaction, but negative relationships with frequency of role conflict, demands at work. Job satisfaction and meaning of work identified as influencing factors inn intention to retention. Conclusion: It is needed a positive hospital environment to improve adaptation in clinical nursing practice and to emphasize professionalism including value of nursing and meaning of nurses' work for new nurses.

VDT 작업 간호사들의 근골격계질환과 요통 문제예방을 위한 기초연구 (Pilot Study of Nurses with VDT Work to Prevent Musculosketal Disorders and Back-Pain Problems)

  • Back pain is painful for people at a lower lumber(L4/L5 or L5/S1). It is experienced as a most frequent disaster 70% of workers have suffered from back-pain. Especially, the occurrence rate of back pain for nurses is very high. Therefore, this study investigates female nurses who worked as a part of a medical cost management team, which mainly deal with the medical insurance in a general hospital at Seoul area. These nurses had different job tasks, which used computers for 50% of their time, so it can be treated as VDT workers. As a first step of this project, a muscle fatigue was measured for these special VDT workers. First, survey for nurses was conducted to figure out what is a real problem of them. Second, to evaluate an experimental data, a medical cost management team was chosen for subjects. Areas of measurement were 3 places that were the trapezius (TR), the medial deltoid (MD), and the erector spine muscle (ES: L4/L5). These areas are most frequently used, so they were chosen for this study. Measurements were taken before work and after work. From these measurements, it was revealed that a fatigue really comes from their main VDT task excluding natural fatigue after work, so their work environment need to be corrected.
