The development and evaluation of a pediatric nurse education program to improve nursing competency for newly graduated nurses in a children's hospital (어린이병원 신규간호사를 위한 간호역량 향상 교육프로그램 개발 및 효과검증)

  • Purpose: The purpose of this study is to identify the effects of a standardized educational program to improve nursing competency on newly graduated nurses in a children's hospital after developing and applying a pediatric nurse education program. The effectiveness of the program was confirmed by evaluating the clinical competency and field adaptation. Methods: In the first step, an education program was developed using the analysis, design, development, implementation and evaluation (ADDIE) model. As a second step, a similar experimental study of a single group repeat measures design was conducted to evaluate the clinical competency and field adaptation over time after application of the program. Additionally, a focus group interviews were conducted to collect subjective data on the effects and improvement points of the program. Results: As a result of applying the program, there was a significant change in the clinical competence and the field adaptation of newly graduated nurses in a children's hospital. The categories derived from the focus group interviews were "getting special guidance," "better care," "becoming a nurse at a children's hospital" and "winning together." Conclusion: It was confirmed that the education program enhances the clinical competency of new nurses in children's hospitals. In addition, it provided the necessary data to understand the experiences of new nurses, help them adapt effectively, and establish appropriate interventions.

  • In this paper, We focused the issue of creating a socially problematic nurse schedule. The nurse schedule should be prepared in consideration of three shifts, appropriate placement of experienced workers, the fairness of work assignment, and legal work standards. Because of the complex structure of the nurse schedule, which must reflect various requirements, in most hospitals, the nurse in charge writes it by hand with a lot of time and effort. This study attempted to automatically create an optimized nurse schedule based on legal labor standards and fairness. We developed an I/O Q-Learning algorithm-based model based on Python and Web Application for automatic nurse schedule. The model was trained to converge to 100 by creating an Fairness Indicator Score(FIS) that considers Labor Standards Act, Work equity, Work preference. Manual nurse schedules and this model are compared with FIS. This model showed a higher work equity index of 13.31 points, work preference index of 1.52 points, and FIS of 16.38 points. This study was able to automatically generate nurse schedule based on reinforcement Learning. In addition, as a result of creating the nurse schedule of E hospital using this model, it was possible to reduce the time required from 88 hours to 3 hours. If additional supplementation of FIS and reinforcement Learning techniques such as DQN, CNN, Monte Carlo Simulation and AlphaZero additionally utilize a more an optimized model can be developed.

Relationship between Nurse Managers' Facilitative Communication and Nurses' Self-esteem (간호관리자의 촉진적 의사소통과 간호사의 자아존중감과의 관계)

  • Purpose: The purpose of this study was to investigate the relationship between nurse managers' facilitative communication as perceived by nurses and nurses' self-esteem. Methods: The subjects were 256 staff nurses at B hospital of C University in Kyeonggi-do. The data were collected between December 1, 2009 and January 12, 2010. The collected data were analyzed using the SAS program through t-test, ANOVA, Scheff$\acute{e}$ test, Pearson's correlation coefficients. Results: Nurses' self-esteem was higher when nurse managers perceived them as facilitative communicators in empathic understanding and genuineness subgroup of facilitative communication (p<.05). There was a positive correlation between nurse managers' facilitative communication and nurses' self-esteem(r=.15, p=.015). Also there was a positive correlation among the nurse managers' facilitative communication subgroups, empathic understanding; genuineness; concreteness(r=0.18, p=.004; r=0.18, p=.003; r=0.19, p=.002) and nurses' self-esteem. Conclusion: Nurse managers are in need of facilitative communication. Therefore, job education and training for improving nurse managers' communication skills particularly related to empathic understanding, genuineness and concreteness are required.

Semantic Network Analysis about Comments on Internet Articles about Nurse Workplace Bullying (간호사 괴롭힘 관련 인터넷 포털 기사에 대한 댓글의 의미연결망 분석)

  • Purpose: A significant amount of public opinion about nurse bullying is expressed on the internet. The purpose of this study was to analyze the linkage structures among words extracted from comments on internet articles related to nurse workplace bullying using semantic network analysis. Methods: From February 2018 to April 2019, comments made on news articles posted to the Daum and Naver web portal containing keywords such as "nurse", "Taeum", and "bullying" were collected using a web crawler written in Python. A morphological analysis performed with Open Korean Text in KoNLPy generated 54 major nodes. The frequencies, eigenvector centralities, and betweenness centralities of the 54 nodes were calculated and semantic networks were visualized using the UCINET and NetDraw programs. Convergence of iterated correlations (CONCOR) analysis was performed to identify structural equivalence. Results: This paper presents results about March 2018 and January 2019 because these months had highest number of articles. Of the 54 major nodes, "nurse", "hospital", "patient", and "physician" were the most frequent and had the highest eigenvector and betweenness centralities. The CONCOR analysis identified work environment, nurse, gender, and military clusters. Conclusion: This study structurally explored public opinion about nurse bullying through semantic network analysis. It is suggested that various studies on nursing phenomena will be conducted using social network analysis.

Analysis of the Adequacy of Nurse Staffing Level through the Estimation of Nursing Activity Hours and Implementation of Focus Group Interviews in a Tertiary Hospital: Using a Mixed-Method Design (일 상급종합병원 병동간호사의 업무량 측정 및 간호사 배치수준의 적절성 연구: 혼합연구 설계 적용)

  • Purpose: This study aimed to examine the adequacy of current nurse staffing levels by identifying nursing activities and workload. Methods: The study used a mixed-method design. A nursing activity survey was conducted using the work sampling method over 2 working days with 119 general ward nurses. A focus group interview was conducted with 12 nurses. Quantitative and qualitative data were analyzed using SPSS 20.0 and content analysis, respectively. Results: The most amount of time was spent on medication (in direct nursing) and electronic medical record documentation (in indirect nursing). The appropriate nurse-to-patient ratio is 1:7.7 for the day shift, 1:9.0 for the evening shift, and 1:11.9 for the night shift. However, the current nurse-to-patient ratio is 1:9.4, 1:11.0, and 1:13.8 for the day, evening, and night shifts, respectively. Therefore, the current nurse staffing level is insufficient for the workload. In the focus group interview, the main reasons cited for being unable to complete tasks within working hours were communication and coordination, and the nursing electronic medical record. The essential nursing activities of basic nursing and emotional support were overlooked owing to a heavy workload. Therefore, an adequate nurse staffing level should be higher than the measured quantitative workload. Conclusion: These results suggest the general wards of tertiary hospitals should evaluate the adequacy of their current nurse staffing and allocate sufficient nurses to improve patient safety and nursing care quality.

Effectiveness of the Nurse Hand Cream on Hand Skin Moisturization (간호사 맞춤 핸드크림 사용이 손 피부 보습에 미치는 효과)

  • Purpose: This study sought to verify the effectiveness of the Nurse Hand Cream developed to protect the skin of the hands of nurses damaged by frequent hand hygiene. Methods: A randomized controlled trial was conducted that compared the hand skin moisture content of subjects using the Nurse Hand Cream that had been developed for nurses with that of the other hand cream. Results: After applying the hand cream, the subjects' skin moisture content were measured using a skin moisture measuring device; the moisture content of the experimental group significantly increased compared with the control group (p<.001). In particular, as a result of the second experiment that involved the four-time performance of hand hygiene every 30 minutes for two hours, the skin moisture content of the experimental group significantly increased compared with the control group, and 84.4% of the experimental group maintained a normal or higher skin moisture content. Conclusion: The hand cream for nurses was statistically confirmed to strengthen the moisturizing power and prevention of skin moisture loss compared with the other hand cream.

Influence of the Nursing Practice Environment on Job Satisfaction and Turnover Intention

  • Objectives: To examine whether the nursing practice environment at the hospital-level affects the job satisfaction and turnover intention of hospital nurses. Methods: Among the 11 731 nurses who participated in the Korea Health and Medical Workers' Union's educational program, 5654 responded to our survey. Data from 3096 nurses working in 185 general inpatient wards at 60 hospitals were analyzed using multilevel logistic regression modeling. Results: Having a standardized nursing process (odds ratio [OR], 4.21; p<0.001), adequate nurse staffing (OR, 4.21; p<0.01), and good doctor-nurse relationship (OR, 4.15; p<0.01), which are hospital-level variables based on the Korean General Inpatients Unit Nursing Work Index (KGU-NWI), were significantly related to nurses' job satisfaction. However, no hospital-level variable from the KGU-NWI was significantly related to nurses' turnover intention. Conclusions: Favorable nursing practice environments are associated with job satisfaction among nurses. In particular, having a standardized nursing process, adequate nurse staffing, and good doctor-nurse relationship were found to positively influence nurses' job satisfaction. However, the nursing practice environment was not related to nurses' turnover intention.

A Study on Ward of Public Hospital for Spatial Composition of Efficient Integrated Nursing Care Service - Focused on the Regional Public Hospital - (효율적 간호·간병통합서비스 공간 조성을 위한 공공병원 병동부에 관한 연구 - 지역거점 지방의료원을 중심으로 -)

  • The purpose of this study is identify the status of the ward of public hospital for integrated nursing care service. Integrated nursing care service has been expanding on a national scale from 2016 but most of public hospitals are currently unable to operate due to nursing shortage. In this study 8 wards of public hospital have been chosen and analyzed. The measure of space program and area distribution(patient area percentage, staff area percentage, circulation area percentage), nurses's walking distance(distance from ns to patient room, distance from ns to core) were calculated by depthmapX and autocad programs. The result of this study is as follows. Nurse's walking distance is more than 24m so the efficiency of nurse's patient care is reduced. The percentage of patient area in double-corridor is higher and the patient feels more comfortable but the Nurse's walking distance is longer and the accessibility is lowered. NS should be located in the center of the ward and close to the core but some wards are not composed of proper space-separation and flow of human trrafic is overlapped. This study may serve as basic research for the architectural plan for future integrated nursing care ward.

A Survey for Developing Strategies to Improve the Fees for Nursing Care (건강보험 간호관리료 수가체계 개선의견 조사분석)

  • Purpose: To examine the current fees for nursing care and propose the strategies for improvement. Method: The number of subjects for this study was 86, including 36 chief executives of the nursing department, 14 of the health insurance department and 33 nursing managers. Data were analyzed by SPSS WIN 12.0 program. A researcher-developed questionnaire with 30 items was utilized. Results: 61% hospital had improved the nursing management fee grade after adoption of the differentiated nursing management fee schedules. After grade improvement, the time for direct nursing care increased. Also, the patient health outcome, nurse's job satisfaction were improved and more nurses were employed in general nursing units. Many subjects addressed that ICU and more nursing units were needed to adopt the differentiated nursing management fee schedules and "bed to nurse ratio" needed to be changed to "patients to nurse ratio" and specialized by the nursing units. Conclusion: The health policy in reference to fees for nursing care needs to get improved further in order to provide the quality-assured nursing care.

Impact of Increased Supply of Newly Licensed Nurses on Hospital Nurse Staffing and Policy Implications (신규면허간호사 공급량 증가가 의료기관 간호사 확보수준에 미친 영향과 정책적 함의)

  • Purpose: This study aimed to analyze the impact of increasing the supply of newly licensed nurses on improving the hospital nurse staffing grades for the period of 2009~2014. Methods: Using public administrative data, we analyzed the effect of newly licensed nurses on staffing in 1,594 hospitals using Generalized Estimating Equation (GEE) ordered logistic regression, and of supply variation on improving staffing grades in 1,042 hospitals using GEE logistic regression. Results: An increase of one newly licensed nurse per 100 beds in general units had significantly lower odds of improving staffing grades (grades 6~0 vs. 7) (odds ratio=0.95, p=.005). The supply of newly licensed nurses increased by 32% from 2009 to 2014, and proportion of hospitals whose staffing grade had improved, not changed, and worsened was 19.1%, 70.1%, and 10.8% respectively. Compared to 2009, the supply variation of newly licensed nurses in 2014 was not significantly related to the increased odds of improving staffing grades in the region (OR=1.02, p=.870). Conclusion: To achieve a balance in the regional supply and demand for hospital nurses, compliance with nurse staffing legislation and revisions in the nursing fee differentiation policy are needed. Rather than relying on increasing nurse supply, retention policies for new graduate nurses are required to build and sustain competent nurse workforce in the future.
