The effect of occupational exposure on quality of life in dental hygienists (치과위생사의 방사선 피폭정도가 삶의 질에 미치는 영향)

  • Objectives : This study was to determine the level of occupational exposure and quality of life and to investigate the effect of variables related occupational exposure on quality of life in dental hygienists. Methods : A survey questionnaire was mailed to dental hygienists from April 4 to May 15, 2010. The subjects were 256(56.9%), who were worked in dental hospital and clinic with mean age of $29.59{\pm}7.30$. Quality of life was measured using the WHOQOL-BREF. It consists of 26 items, each with 5-point likert scale. Self control of occupational exposure was measured using the 3-items. The data were analyzed with chi-square, t-test, one-way ANOVA, pearson correlation coefficients and multiple regression using the SPSS WIN 17.0 program. Results : Regarding quality of life, the subjects was a mean of 90.73 out of a maximum 130 points. The level of self control occupational exposure a mean of 3.20 out of a maximum 5 points. Self control of occupational exposure, frequency of panorama radiography, work experience of radiography had a significant impact on quality of life in dental hygienists. Conclusions : Based on the findings, occupational exposure is negatively associated with quality of life in dental hygienists. These results suggest that health promotion program should be considered various factors related occupational exposure in dental hygienists.

  • We have been used super sensitive screen/film system which is thought to be the limit of sensitivity, however, we think this method is not satisfactory. As a general radiographic rule, reduction of exposure is inconsistent with improvement of image quality, but it is necessary to challenge this rule to get better method. We tried air gap method for minimal diffuse lung shadow which is difficult to find out by our routine method. As a result, air gap method with super sensitive screen/film system(SRO 1000/TMH) showed lower exposure dose, 50% of our routine system(SRO 750/SRH with grid), and better image quality for pulmonary pattern in the lower lung field.

  • Asbestos is widely used in the textile, asbestos cement, construction products, friction material, paper products, insulation products, chemical and plastic products because of its heat resistance, flexibility, tensile strength, and texturability. It is now generally recognized that longterm and excessive inhalation of asbestos dust causes asbestosis, lung cancer, malignant mesothelioma and malignancies in other organs such as cancer of gastrointestinal tract, leukemia, lymphoma. Although eighty thousand tons of asbestos has been annually consumed since 1979 in korea, it has not been reported asbestos and lung cancer by asbestos dust so far, while a case of mesothelioma was officially diagnosis as a occupational disease at 1993. We experienced firstly a case of asbestosis and lung cancer caused simultanously by occupational asbestos exposure 11 years, which was confirmed by chest x-ray, pulmonary function test, chest CT and HRCT, bronchoalveolar lavage, and gallium scan. And so We present a case of asbestosis, pleural effusion and lung cancer with a review literature.

Risk Factors of Blood and Body Fluid Exposure in Clinical Nurses (임상간호사의 혈액과 체액 노출 위험요인)

  • Purpose: The purpose of this study was to investigate the risk factors of exposure to blood and body fluid by clinical nurses. Method: The participants in this descriptive study were 276 clinical nurses involved in nursing care in a general hospital located in Seoul. The collected data were analyzed using a logistic regression model. Results: The annual exposure rate to blood and body fluid by clinical nurses was 66.3%, and the ratio was higher in the exposed group than in the non-exposed group for nurses under the age of 25, nurses who were unmarried, nurses with low clinical experience, and nurses who work night shifts more than six days a week. Clinical nurses who were unmarried were 1.9 times more likely to have been exposed compared to married nurses. Nurses whose work experience was less than 18 months were 2.7 times more at risk than nurses with more than 18 months of experience. Also, nurses whose fatigue scores were high had an increased chance of exposure (1.9 times). Conclusion: It is necessary to provide intensive training programs for clinical nurses who are in the early months of their career and are likely to be young and unmarried, in order to prevent exposure to blood and body fluid. Administrative supports to self-control and hospital ward operation measures to relieve nurses' fatigue should be provided.

  • The extensor digitorum brevis (EDB) muscle island flap is a reliable, safe method for coverage of foot and ankle. There are many variation in approach such as curvilinear, zigzag, L-shaped or vertical longitudinal incision for exposure of the EDB muscle. These approaches use only single incision excluding the distal incision for exposure of the distal tendon. Since dorsalis pedis artery vascular bundle and sinus tarsi branch of the lateral tarsal artery both requires careful dissection, single incision alone may cause not only difficulty in exposure but also skin sloughing at donor site. So we tried to modify the approach into two parallel longitudinal incision, one for dorsalis pedis vascular bundle and the other for sinus tarsi branch exposure. The author treated 9 patient with EDB muscle flap. We used single incision in six patients, and two parallel incision in three patients. All the flap survived. In two parallel incision group, dissection was more easy and rapid. So we would like to suggest that two parallel longitudinal incision approach is better method than the single incision technique for exposure of the EDB muscle flap.

  • Increasing prevalence of childhood obesity poses threats to the global health burden. Because this rising prevalence cannot be fully explained by traditional risk factors such as unhealthy diet and physical inactivity, early-life exposure to endocrine disrupting chemicals (EDCs) is recognized as emerging novel risk factors for childhood obesity. EDCs can disrupt the hormone-mediated metabolic pathways, affect children's growth and mediate the development of childhood obesity. Many organic pollutants are recently classified to be EDCs. In this review, we summarized the epidemiological and laboratory evidence related to EDCs and childhood obesity, and discussed the possible mechanisms underpinning childhood obesity and early-life exposure to non-persistent organic pollutants (phthalates, bisphenol A, triclosan) and persistent organic pollutants (dichlorodip henyltrichloroethane, polychlorinated biphenyls, polybrominated diphenyl ethers, per- and polyfluoroalkyl substances). Understanding the relationship between EDCs and childhood obesity helps to raise public awareness and formulate public health policy to protect the youth from exposure to the harmful effects of EDCs.

Therapeutic Effects and Adverse Drug Reactions are Affected by Icotinib Exposure and CYP2C19 and EGFR Genotypes in Chinese Non-Small Cell Lung Cancer Patients

  • Background: The aim of this study was to evaluate how CYP2C19 affects icotinib and metabolite' exposure, and to determine whether the exposure and EGFR genotype influences survival time, tumor metastasis and adverse drug reactions. Materials and Methods: 274 NSCLC patients who accepted 125mg icotinib/t.i.d. were chosen from a phase III study. Blood samples were obtained in $672^{nd}$ ($4^{th}$ week) and $1,680^{th}$ hours ($10^{th}$ week), and plasma was used to quantify the concentration of icotinib and blood cells were sampled to check the genotypes. Clinical data were also collected at the same time, including EGFR genotypes. Plasma concentrations were assessed by HPLC-MS/MS and genotype by sequencing. All data were analyzed through SPSS 17.0 and SAS 9.2. Results: CYP 2C19 genotypes affected bio-transformation from icotinib to M24 and M26, especially in poor-metabolisers. Higher icotinib concentrations (>1000 ng/mL) not only increased patient PFS and OS but also reduced tumor metastasis. Patients with mutant EGFR experienced a higher median PFS and OS (234 and 627 days), especially those with the 19del genotype demonstrating higher PR ratio. Patients who suffered grade II skin toxicity had a higher icotinib exposure than those with grade I skin toxicity or no adverse effects. Liver toxic reactions might occur in patients with greater M20 and M23 plasma concentrations. Conclusions: CYP2C19 polymorphisms significantly affect icotinib, M24 and M26 exposure. Patients with mutant EGFR genotype and higher icotinib concentration might have increased PFS and OS and lower tumor metastasis. Liver ADR events and serious skin effects might be respectively induced by greater M20, M23 and icotinib concentrations.

Use of autonomous maximal smile to evaluate dental and gingival exposure

  • Objective: This study was performed to validate the autonomous maximal smile (AMS) as a new reference for evaluating dental and gingival exposure. Methods: Digital video clips of 100 volunteers showing posed smiles and AMS at different verbal directives were recorded for evaluation a total of three times at 1-week intervals. Lip-teeth relationship width (LTRW) and buccal corridor width (BCW) were measured. LTRW represented the vertical distance between the inferior border of the upper vermilion and the edge of the maxillary central incisors. Intraclass correlation coefficients (ICCs) for reproducibility, and the m-value (minimum number of repeated measurements required for an ICC level over 0.75), were calculated. Results: LTRW and BCW of the AMS were 1.41 and 2.04 mm, respectively, greater than those of the posed smile (p < 0.05), indicating significantly larger dental and gingival exposure in the AMS. The reproducibility of the AMS (0.74 to 0.77) was excellent, and higher than that of the posed smile (0.62 to 0.65), which had fair-to-good reproducibility. Moreover, the m-value of the AMS (0.88 to 1.05) was lower than that of the posed smile (1.59 to 1.85). Conclusions: Compared to the posed smile, the AMS shows significantly larger LTRW and BCW, with significantly higher reproducibility. The AMS might serve as an adjunctive reference, in addition to the posed smile, in orthodontic and other dentomaxillofacial treatments.

The Affecting for Awareness of Exposure at Hazardous Materials on the Working Satisfaction of Female Workers in the Small Scale Industry (중소규모 산업장 여성근로자들의 작업환경에 대한 인식이 직무만족에 미치는 영향)

  • 458 female workers working in the small and medium scaled industries in Inchon area were surveyed to study their general characteristics, health behavior, working and environmental conditions, awareness of exposure at hazardous materials, current health status, and degree of working satisfaction. Self-administered questionnaire was distributed to the female workers with the explanation by trained interviewer on the purpose of survey and the method how to answer. The results were as follows : (1) 75.5% of the respondents are living with their families and others are living in domitary or rent house for herself. Age distribution ranges from teenagers to warren in their forties. In marital status 69.0% of workers are single and 31.0% of them have ever married. 69.4% of workers are high school graduate. The size of factory is 48.3% with less than 300 workers, 42.1% with 300~500 workers. (2) Most of the workers(86.7%) are full time workers, 52% of the respondents have working experiences in other factories. More than 65% have fewer than 5 years working experience. (3) The awareness of exposure at noise was 33% of respondents, it was significantly different by skilled status. The awareness of exposure at dust, other hazardous material was each 13.3%, 12.4% of respondents. (4) 26% of them are unsatisfied with their working cognitions and 43.7% of them are unsatisfied with their welfare in occupational system unsatisfactory rate of promotion is higher in the unmarried workers(45.6%), skilled workers(47.2%), full time workers(47.4%), long term workers(50%), and awareness group of exposure hazardous material(51.3%), compared to married women workers, unskilled workers, part-time workers, short term workers and unawareness group of exposure at hazardous materials. (5) The index of working satisfaction is rated on a scale of 1 to 10 with the higher point indicating higher satisfaction. Among the selected variables, marital status, working status, the awareness of exposure at hazardous materials, were statistically significant with the index of working satisfaction. (6) By multiple regression analysis showed that the determinant factors of index of working condition were health index, the awareness of exposure at dust, the awareness of exposure other hazardous materials, marital status and working period. In conclusion, there is a significant relationship between the awareness of exposure at hazardous material the index of working satisfaction. Therefore if the workers are less exposed to the hazardous material and are educated properly they will be more satisfied with their work thus improving their health and productivity. Finally, with the result of this study, there is a great need for the development of a health promotion program and a welfare policy for small-scale factories.

Association of Sleep Disturbance, Fatigue, Job Stress and Exposure to Blood and Body Fluid in Shift-work Nurses (교대근무 간호사의 혈액과 체액 노출 경험과 수면장애, 피로, 직무스트레스와 상관성)

  • Purpose: The purpose of this study was to identify associations among the sleep disturbance, fatigue, job stress, and blood and body fluid (BBF) exposure of shift-work nurses. Methods: A total of 299 shift-work nurses from two tertiary hospitals were enrolled in this study. We used the Insomnia Severity Index (ISI), and the Fatigue Severity Scale (FSS), and the Korean Occupational Stress Scale-Short Form (KOSS-SF) to evaluate sleep disturbance, fatigue, and job stress, respectively. The data were analyzed using t-test or chi-squared test and Logistic regression analysis using the SPSS 23.0 program. Results: We found that 43.8% of participants reported BBF exposure over the past year. Splash or exposure to broken skin of BBF were most frequent (56.9%), and followed by needlestick injuries (30.4%) and sharp injuries (12.8%). Age, hospital, working period, level of stress, sleep disturbance ($ISI{\geq}15$), fatigue (FSS $score{\geq}4$), job demand and organizational climate subset in KOSS-SF were significantly associated with BBF exposure in shift-work nurses. In multivariate analysis after adjusting age and hospital, the risk factors of BBF exposure in shift-work nurses were the level of stress and fatigue (FSS $score{\geq}4$). Conclusion: Fatigue and job stress were related to BBF exposure in shift-work nurses. Our results suggest that management of sleep disturbance, fatigue, and high job stress in shift-work nurses is needed to reduce risk of BBF exposure.
