Case Series of Headache Patients Diagnosed with Subarachnoid Hemorrhage by Computed Tomography in a Korean Medicine Hospital (두통으로 한방병원에 내원하여 컴퓨터단층촬영후 지주막하출혈로 진단된 증례 보고)

  • The purpose of this study was to investigate the cases diagnosed as subarachnoid hemorrhage by noncontrast computed tomography (NCCT) after the appearance of symptoms related to headache and paralysis in patients who visited a Korean medicine hospital. A search of electronic medical records of patients diagnosed with subarachnoid hemorrhage on NCCT in W Korean medicine hospital from January 2010 to December 2017 identified five patients. The patients (four women and one man) ranged in age from 64 to 79 years old. Three were inpatients and two were outpatients. Two patients had hypertension, two patients had diabetes, two patients had heart disease, and two patients had a history of cerebral infarction. Development of a cooperative system is needed for accurate diagnosis and appropriate treatment of this disease in the Korean medicine clinic and for establishment of an NCCT scan application method.

  • Purpose: To establish a database for breast cancer patients to save and manage clinical data and to preliminarily investigate its clinical application. Materials and Methods: Information on breast cancer patients hospitalized in our department from 2008.01 to 2013.01 were input into our breast cancer management system. SPSS 16.0 software was used as a convenient reference to evaluate the accuracy of the newly built database. Results: A database of 2403 breast cancer patients was successfully established. Information in the database clearly displayed capabilities of storage, addition, retrieval, statistical analysis and other functions. As the continuously updated database showed, the distribution of age, sex, nationality, allergy history, pausimenia and marriage of patients was identical to that achieved by SPSS analysis, indicating reliable and accurate data analysis. Conclusions: The described database is easy and convenient to operate and manage, and should prove suitable for application in clinical research and treatment.

  • Mucopolysaccharidosis type II (MPS II) is a rare X-linked recessive lysosomal storage disease caused by mutation of the iduronate-2-sulfatase gene. The mutation results in iduronate-2-sulfatase deficiency, which causes the progressive accumulation of heparan sulfate and dermatan sulfate in cellular lysosomes. The phenotype, age of onset, and symptoms of MPS II vary; accordingly, the disease can be classified into either the early-onset type or the late-onset type, depending on the age of onset and the severity of the symptoms. In patients with severe MPS II, symptoms typically first appear between 2 and 5 years of age. Patients with severe MPS II usually die in the second decade of life although some patients with less severe disease have survived into their fifth or sixth decade. Here, we report the establishment of a preimplantation genetic diagnosis (PGD) strategy using multiplex nested polymerase chain reaction, direct sequencing, and linkage analysis. Unaffected embryos were selected via the diagnosis of a single blastomere, and a healthy boy was delivered by a female carrier of MPS II. This is the first successful application of PGD in a patient with MPS II in Korea.

  • The purpose of this study was to review national mental health policy goal in Korea and America, and to suggest strategies for advancing the national mental health policy in Korea. Objectives: First, the review of nation mental health policy in Korea. Second, the review of nation mental health policy in America. Third, the development of nation mental health plan in Korea. Methods: To achieve this objectives, review the books, journals, and national published papers and so on. Results: First, 15 goals are planned for mental health promotion in Korea Health Plan 2010. Second, 14 goals and multiple strategies are planned for mental health promotion in America Healthy People 2010. Third, 3 plans are suggested for mental health promotion in Korea. Conclusion: Mental Health is essential dimension in holistic health. And mental health promotion has been increasingly emphasized in national health plan. Therefore, effective nation mental health policy establishment would be needed continuously.

Characteristics about Patients using the Emergency Room of a General Hospital of a partial area Residents (일부 지역 주민의 종합병원 응급실 이용 환자에 관한 특성)

  • Objectives: The purpose of this study was to verify about characteristics about patients using the emergency room of a general hospital of a partial area residents. Methods: The analysis of the data was used by SAS program(ver. 9.2). Chi-square test was used in analyzing degree of emergency of patients visiting the emergency room of a general hospital. Using cluster analysis, subjects were classified into 3groups. Results: Urgent and emergent patients at the age of 40 or greater were significantly higher than in under 10 years. the rates of urgent and emergent patients in male(3.2% and 3.3%) were significantly higher than in female(2.5% and 2.2%)(p<0.001). Urgent patients were the highest at 11 p.m.-3 a.m. and on Thursday and in winter. Emergent patients were the highest at 4 a.m.-8 a.m. and on wednesday and in winter. Patients as cause of disease was the highest in illness. Conclusion: Policy establishment and measures of government and public health authority were needed for emergent diseases at the age of 40 or greater of male and at midnight time and on thursday and wednesday and in winter.

Strategies for Rural Hospitals in Korea (농촌지역 병원들의 경영개선을 위한 전략)

  • This thesis is a kind of case report on the managerial difficulties and strategies to overcome them for the rural hospitals in Korea. We examined environmental situations and SWOT(Strength, Weakness, Opportunity, Threat) of the hospitals, and suggested some strategies to improve the hospital business through analysing secondary data and brief survey to staffs and patients at 3 case hospitals. Among the strategies were included establishment of their identity as community hospitals, positioning towards high-touch services rather than high-tech services to improve their competence. For this positioning, patient satisfaction should be emphasized to make rapport with the residents in the community, and a few service areas should be selected and strengthened. Emergency care and geriatric health services are recommended to be covered strongly, because rural hospitals are usually the one and only hospital in the region and rural communities have aged population structure. In addition to these, networking is necessary among rural hospitals nationwide and with local clinics and tertiary hospitals near urban areas. And also, complementary role division with community health post should be developed to lessen the competitive relationship between public and private sectors. To support these strategies, fresh organizational culture should be built up to make hospital staff creative and enthusiastic on their job. Finally, inducement of governmental assistance will be necessary to make the environment advantageous to the rural hospitals.

Experiences of nurses at a general hospital in Seoul which is temporarily closed due to COVID-19 (COVID-19 감염으로 임시 폐쇄된 서울 종합병원 간호사의 경험)

  • Purpose: This study is aimed at preparing basic data to increase professionalism in nursing and effective counter-strategies against future infectious diseases. For that purpose, this study investigated the experiences of nurses in a general hospital with an 800-bed size, temporarily closed after a confirmed case of COVID-19. Methods: This study was a qualitative study using individual in-depth interviews, and data were collected from 10 nurses in a university hospital temporarily closed from May 1 to June 13, 2020. Results: As a result of this study, the subjects were classified into 'COVID-19 and nursing', 'nursing work and vocation' and 'life changed with COVID-19'. Conclusion: In conclusion, there is a need for education that nurtures the ability of nurses to screen patients with infectious diseases well and to skillfully wear and remove protective clothing. In addition, in order to cultivate the expertise of nurses and respond efficiently to infectious diseases, a separate compensation, such as a risk allowance, is required along with the establishment of an infrastructure that fully supports personnel and supplies. A mental health program to relieve stress is also required.

A Korean Case of Neonatal Nemaline Myopathy Carrying KLHL40 Mutations Diagnosed Using Next Generation Sequencing

  • Nemaline myopathy is a genetically heterogeneous neuromuscular disorder and one of the most common congenital myopathies. The clinical manifestations usually vary depending on the age of onset. Neonatal nemaline myopathy has the worst prognosis, primarily due to respiratory failure. Several genes associated with nemaline myopathy have been identified, including NEB, ACTA1, TPM3, TPM2, TNNT1, CFL2, KBTBD13, KLHL40, KLHL41, LMOD3, and KBTBD13. Here, we report a neonatal Korean female patient with nemaline myopathy carrying compound heterozygous mutations in the gene KLHL40 as revealed using next generation sequencing (NGS). The patient presented with postnatal cyanosis, respiratory failure, dysphagia, and hypotonia just after birth. To identify the genetic cause underlying the neonatal myopathy, NGS-based gene panel sequencing was performed. Compound heterozygous pathogenic variants were detected in KLHL40: c.[1405G>T];[1582G>A] (p. [Gly469cys];[Glu528Lys]). NGS allows quick and accurate diagnosis at a lower cost compared to traditional serial single gene sequencing, which is greatly advantageous in genetically heterogeneous disorders such as myopathies. Rapid diagnosis will facilitate efficient and timely genetic counseling, prediction of disease prognosis, and establishment of treatments.

Experiences of Patient Safety Accidents in General Hospital Nurses (종합병원 간호사의 환자안전사고에 대한 경험)

  • This study aims to identify and understand nurses' experience on patient safety accidents in general hospital. The qualitative research method was used to analyze the daily life of seven nurses who had experienced nursing safety accidents directly or indirectly in general hospitals. The nurses' experiences were divided into 7 categories and 24 subordinate properties and 7 categories were "unexpected experience", "missing caused by negligence", "facing the anxiety", "difficult of loneliness", "resignation from the hospital", "entering into conflict" and "being practiced a nurse". The results of this study will contribute to the establishment of policies for safety accidents in hospitals and also it will help to make a practical improvement plan to prevent the patient safety accident at the clinical site.

Bacterial Contamination of Surfaces in an Ultrasound Room (초음파실 표면의 세균 오염평가)

  • The ongoing coronavirus disease 2019 (COVID-19) pandemic has highlighted the importance of hospital hygiene and infection control in hospital settings. To raise hygiene awareness among ultrasound technicians, we evaluated the hygiene status of an ultrasound room, in comparison with that of objects used in daily life. Using the swab method, the following surfaces were examined: eight surfaces in the ultrasound room including the ultrasound probes (convex, linear, sector, 3D), ultrasound track ball, ultrasound keyboard, ultrasound gel (sealed and in use) and pillow as well as four surfaces of everyday objects including subway handles, common computer keyboards, common computer mouse, and cell phones. The streak plate technique was used for inoculation into media, which was observed for the formation of bacterial colonies following incubation for 24 h. Six bacterial strains were detected from objects used in the ultrasound room, including methicillin-resistant Staphylococcus aureus. Four strains of bacteria were detected on surfaces of everyday objects. The equipment and accessories used in an ultrasound room can act as vehicles for infecting patients. Establishment of standardized hygiene protocols and periodic training of the staff are recommended to avoid cross-infection.
