The meaning of anti-Müllerian hormone levels in patients at a high risk of poor ovarian response

  • Measurements of ovarian reserve play an important role in predicting the clinical results of assisted reproductive technology (ART). The ideal markers of ovarian reserve for clinical applications should have high specificity in order to determine genuine poor responders. Basal follicle-stimulating hormone levels, antral follicle count, and serum anti-$M{\ddot{u}}llerian$ hormone (AMH) levels have been suggested as ovarian reserve tests that may fulfill this requirement, with serum AMH levels being the most promising parameter. Serum AMH levels have been suggested to be a predictor of clinical pregnancy in ART for older women, who are at a high risk for decreased ovarian response. We reviewed the prognostic significance of ovarian reserve tests for patients undergoing ART treatment, with a particular focus on the significance of serum AMH levels in patients at a high risk of poor ovarian response.

Associations Between Thyroid Hormone Levels and Urinary Concentrations of Bisphenol A, F, and S in 6-Year-old Children in Korea

  • Objectives: Bisphenol A (BPA) is used in the electrical, mechanical, medical, and food industries. Previous studies have suggested that BPA is an endocrine disruptor. Regulation of BPA has led to increased use of bisphenol F (BPF) and bisphenol S (BPS). However, few studies have investigated the associations of BPF and BPS with thyroid dysfunction in children. Our study investigated the associations of prenatal BPA and early childhood BPA, BPF, and BPS exposure with thyroid function in 6-year-old children. Methods: Prenatal BPA concentrations were measured during the second trimester of pregnancy in an established prospective birth cohort. We measured urinary BPA, BPF, and BPS concentrations and thyroid hormone levels (thyroid-stimulating hormone, total T3, and free T4) in 6-year-old children (n=574). We examined the associations between urinary bisphenol concentrations and percentage change of thyroid hormone concentrations using multivariate linear regression. We also compared thyroid hormone levels by dividing the cohort according to BPA, BPF, and BPS concentrations. Results: The associations between prenatal BPA and total T3 levels were statistically significant in all models, except for girls when using a crude model. The associations between urinary BPA and BPS concentrations and levels of all thyroid hormones were not statistically significant. However, we observed that lower free T4 levels (-1.94%; 95% confidence interval, -3.82 to -0.03) were associated with higher urinary BPF concentrations in girls only. Conclusions: Our findings identified significant associations between prenatal BPA exposure and total T3 levels in all children and between BPF exposure and free T4 levels in girls only.

Clinical and Laboratory Features to Consider Genetic Evaluation among Children and Adolescents with Short Stature

  • Conventional evaluation method for identifying the organic cause of short stature has a low detection rate. If an infant who is small for gestational age manifests postnatal growth deterioration, triangular face, relative macrocephaly, and protruding forehead, a genetic testing of IGF2, H19, GRB10, MEST, CDKN1, CUL7, OBSL1, and CCDC9 should be considered to determine the presence of Silver-Russell syndrome and 3-M syndrome. If a short patient with prenatal growth failure also exhibits postnatal growth failure, microcephaly, low IGF-1 levels, sensorineural deafness, or impaired intellectual development, genetic testing of IGF1 and IGFALS should be conducted. Furthermore, genetic testing of GH1, GHRHR, HESX1, SOX3, PROP1, POU1F1, and LHX3 should be considered if patients with isolated growth hormone deficiency have short stature below -3 standard deviation score, barely detectable serum growth hormone concentration, and other deficiencies of anterior pituitary hormone. In short patients with height SDS <-3 and high growth hormone levels, genetic testing should be considered to identify GHR mutations. Lastly, when severe short patients (height z score <-3) exhibit high levels of prolactin and recurrent pulmonary infection, genetic testing should be conducted to identify STAT5B mutations.

AMH 수치가 상승된 희발월경 환자 증례보고 (A Case Study on Korean Medicine Treatment for the Oligomenorrhea with Increased Anti-mullerian Hormone Level)

  • Objectives: In recent studies, increased AMH level has been suggested as objective surrogate marker for diagnosis PCOS, one of the major causes of oligomenorrhea. The purpose of this study is to report the clinical effect of Korean medicine treatment on oligomenorrhea patient with increased Anti-mullerian hormone level, who can be diagnosed who can be ruled out PCOS. Methods: A 27 year old woman with oligomenorrhea was enrolled in this study. We measured serum hormone levels and ruled out PCOS. The patient received Korean medicine treatment for 3 months, we assessed the result of treatment through observation of the menstrual cycle and follow-up measurements of serum hormone levels. Results: 1. The patient had menstrual cycle regularly. 2. Increased serum AMH level of the patient decreased from 12.16 ng/ml to 8.51 ng/ml. 3. The other serum hormone levels such as testosterone, LH/FSH ratio decreased Conclusion: This case shows that Korean medicine treatment could have a beneficial effect on menstrual cycle and decrease the increased serum hormone levels of ruled out PCOS patient.

염분농도에 따른 넙치 (Paralichthys olivaceus)의 성장호르몬, 프로락틴 및 수용체 유전자의 발현변화 (Gene Expression Levels of Growth Hormone, Prolactin and Their Receptors of Olive Flounder Paralichthys olivaceus by Salinity Changes)

  • To investigate the effects of environmental salinity on the expression of the genes for growth hormone (GH) and prolactin (PRL) in the pituitary, and their receptors (GHR, PRLR) In the kidney, intestine, and gills in teleosts, we acclimated juvenile olive flounders (Paralichthys olivaceus) to different salinities (5, 15, 25, or 32 psu) for 3 days and examined their mRNA levels using the reverse transcription-polymerase chain reaction (RT-PCR). In the fish adapted to low salinity, the PRL mRNA levels in the pituitary were elevated dramatically, whereas the GH mRNA levels did not differ significantly. PRLR mRNA increased significantly in fish exposed to low salinity, whereas GHR mRNA levels did not differ. These results suggest that PRL is an important hormone for flounders that are acclimated to brackish water and it may control ion homeostasis with PRLR in the osmoregulatory organs.

THE EFFECTS OF SOMATOSTATIN INFUSION ON THE PLASMA PROFILE OF GROWTH HORMONE, INSULIN AND CORTISOL IN SHEEP

  • Four castrated Corriedale sheep were used in an experiment to observe the changes in insulin, growth hormone and cortisol in blood plasma following a prolonged infusion of a high rate of somatostatin (SRIF). The animals wee infused with either saline, 25 or $50{\mu}g/kg/h$ of SRIF for 3 hours. Blood samples wee taken every 20 minutes until 1 hour following the end of the SRIF infusion. Both SRIF infusion levels suppressed the release of insulin into plasma to approximately 3.5 mU/l. The SRIF infusions reduced the concentration of growth hormone to barely detectable levels. Following the withdrawal of SRIF there was a massive release of growth hormone. The plasma concentration of growth hormone reached 60 ng/ml within 20 minutes, the length of the growth hormone discharge was in excess of 1 hour. The extent of the discharge of growth hormone following the SRIF infusions was greater than that suppressed by the infusion. The SRIF apparently caused an increase in the plasma concentration of cortisol at the end of the infusion and following is withdrawal. This is possibly associated with some change in the metabolic rate associated with the suppression of insulin or glucagons release. The present experiment demonstrates that a high rate of SRIF infusion can not completely inhibit the release of insulin into the plasma.

Effects of aerobic exercise, fat oxidation, and diet limitation on target fat mass reduction and appetite-regulating hormone levels

  • This study aims to investigate changes in plasma lipid concentrations and appetite-regulating hormone levels after a 4% body fat reduction using a 9-week intervention involving aerobic exercise, a fat-oxidizing agent, and diet limitation. After the 9-week intervention, the aerobic exercise plus hydroxycitric acid (EX+HCA), exercise (EX), and diet limitation (DIET) groups achieved the target 4% body fat reduction from the baseline value. None of the plasma lipid indicators showed significant intergroup differences, indicating that plasma lipid levels are not influenced by body weight regulation. With regard to appetite-regulating hormones, no significant intergroup differences were observed in glucose, insulin, or glucagon-like peptide-1 levels, unlike ghrelin and leptin. Ghrelin levels in particular tended to decrease in the DIET group and increase in the HCA+EX and EX groups. Leptin levels significantly decreased in the HCA+EX and EX groups, whereas no differences were observed in the DIET group. Such results indicate that exercise alone without the administration of obesity diet supplements induces elevation in ghrelin levels and reduction in leptin levels, but that diet restriction alone does not influence changes in leptin levels. Taken together, we could not confirm any synergic effects arising from the use of a fat-oxidizing agent during an exercise program to control body weight. Furthermore, diet limitation unsupported by exercise had no effect on muscle mass reduction or appetite-regulating hormone levels; thus, it is not recommended as an effective body weight control method.

체간안정화 운동이 측만증 환자의 폐기능과 호르몬에 미치는 효과 (The Effect of Hormone and Pulmonary Function in the Scoliosis Patients after Trunk Stabilization Exercise)

  • Purpose: The purpose of this study was to evaluate the effect of a trunk stabilization exercise program on the participants' scoliosis angle, pulmonary function, and growth hormones. Method: In the study, 30 participants were divided into a normal scoliosis exercise group (n=15) and an obese scoliosis exercise group (n=15). The participants performed a trunk stabilization exercise program three times a week for 12 weeks, and the exercise sessions lasted 50 minutes. The participants' pulmonary function [FVC, FEV1, FEV1/FVC, and PEF] was measured using a CardioTouch 3000S, and their scoliosis angles were measured using the Cobb's angle. The levels of growth hormone (GH) and insulin-like growth factor-1 (IGF-1) were analyzed on an immunoradiometric assay (IRMA) and radioimmunoassay (RIA), respectively. Results: After the intervention, the scoliosis angle, hormone levels, and pulmonary function increased significantly in both groups (p<.05). The result of the intergroup difference test indicated statistically significant differences in the three items (scoliosis angle, hormone levels, and pulmonary function) between the two groups (p<.05). Conclusion: Therefore, this program may be recommended as a therapeutic intervention for patients with scoliosis.

Effect of Thyroid Hormone on the Gene Expression of Myostatin in Rat Skeletal Muscle

  • Modification of thyroid hormone levels has a profound effect on skeletal muscle differentiation, predominantly through direct regulation involving thyroid hormone receptors. Nevertheless, little is known about the regulation of myostatin gene expression in skeletal muscle due to altered concentrations of thyroid hormone. Thus, the goal of our study was to find out whether altered thyroid states could change the gene expression of myostatin, the most powerful inhibitor of skeletal muscle development. A hyperthyroid state was induced in rats by daily injections of L-thyroxine 20 mg/100 g body weight for 14 days, while a hypothyroid state was induced in another group of rats by administering methimazole (0.04%) in drinking water for 14 days. After a period of 14 days of L-thyroxine treatment we observed a significant increase of myostatin expression both in mRNA and protein level. However, decreased expression of myostatin mRNA and protein were observed in hypothyroid rats. Furthermore, our studies demonstrated that the upregulation of myostatin gene expression might be responsible for the loss of body weight induced by altered thyroid hormone levels. We concluded that myostatin played a role in a metabolic process in muscle that was regulated by thyroid hormone.

갑상선호르몬 수용체 베타 유전자 돌연변이(H435Y)가 확인된 갑상선호르몬 저항성 증후군 1례 (Identification of a de novo mutation (H435Y) in the THRB gene in a Korean patient with resistance to thyroid hormone)

  • 갑상선 호르몬 저항성 증후군은 갑상선 호르몬에 대한 조직의 반응이 감소되어 나타나는 드문 유전 질환이다. 대부분은 갑상선 호르몬 수용체 (TR) 유전자의 돌연변이로 인한 갑상선 호르몬 수용체의 결함에 의한다. TR 유전자의 변이는 일반적으로 이형접합성이며 상염색체 우성 유전 양상을 보인다. 혈청 갑상선 호르몬 수치가 증가되어 있음에도 불구하고 혈청 갑상선 자극호르몬 수치가 억제되지 않으며, 임상 양상은 다양하다. 본 증례는 경미한 갑상선종, 총 및 유리 $T_4$, $T_3$의 증가, 정상 범위의 TSH 소견을 보이는 4세 여아로서 TR 유전자 분석에서 과오돌연변이(H435Y)를 확인하였다. 부모에서는 돌연변이가 관찰되지 않았으며, 갑상선 기능도 정상이었다. 특별한 투약 없이 추적 관찰 중에 갑상선종의 증가나 다른 증상의 악화는 없는 상태이다.
