Long-tenn Patients' Reasons for Stay in Some General Hospitals (종합병원 장기입원환자들의 재원사유)

  • There has been pointed out that a great portion of hospitalized patients stay in hospitals longer than necessary, often even after the completion of necessary care. This causes that hospital resources are not used efficiently. In order to identify underlying forces in postponing inpatients' discharge, this study aimed to investigate reasons for long-term stay of patients admitted in general hospitals. A total of 135 patients, who were staying at 7 general hospitals in Inchon and Kyonggi-Do for more than 60 days, were surveyed with a self-administered questionnaire between April 3 and April 10, 2000. Medical reasons including incompleteness of necessary care, difficulty in receiving outpatient-based care, and being under physical therapy were the most significant factors associated with long-term stay, followed by the lack of familial resources to take care of patients after discharge. Financial problems such as inability to pay for hospital bills were not significant factors influencing long-term stay. Regression analyses were conducted for medical reasons, familial resources, and financial problems, respectively. It was shown that receiving physical therapy and the number of admission in the past were significant predictors for medical reasons. The lack of familial resources as a reason for long-term stay had a positive relationship with the degree of need for aid in daily living. It may be recommended for the hospitals to cope with administrative problems due to the patients' long-term stay, considering the reasons of it, and their characteristics. And also, institutional efforts like vitalizing the home care service systems by hospitals as the continuing care after discharge should be needed.

Analysis of Factors Affecting Length Of Stay for A Serious Patients Using Medical Records (의무기록자료를 이용한 중증질환자의 재원일수에 미치는 요인 분석)

  • Objectives: In this study, we tried to analyze the factors affecting Length Of Stay for serious patients in Republic of Korea. Methods: The study included 139,172 serious patients in the 2012-2016 discharge details. Using the SPSS 23.0 program, we conducted a rank regression analysis with social and social demographic characteristics as control variables, medical institution characteristics and medical use characteristics as independent variables, and Average Length Of Stay as a dependent variable. Results: Average Length Of Stay for participants was found to be 9.92days. And the location and bed size of medical institutions were not statistically significant, the hospitalization path was more urgent(B=0.43) than the outpatient (p<0.001), and there was no secondary diagnosis(B=0.35). However, Average Length Of Stay was higher (p<0.001) than there was no main surgery(B=0.80). After discharge, Average Length Of Stay for funding(B=0.43) and death(B=0.72) was long (p<0.001). Average Length Of Stay for participants was found to be 9.92days. And the location and the bed size of the medical institution were not statistically significant, and the hospitalization pass had longer Length Of Stay for emergency patients(B=0.43) than for outpatients(p<0.001). There was a longer Length Of Stay(B=0.35) than none was diagnosed. There were longer Length Of Stay(p<0.001) than there was no major surgery(B=0.80). After discharge, the outpatients had longer Average Length Of Stay(B=0.43) and deaths(B=0.72) than those who returned home(p<0.001). Conclusion: As a result of analyzing the factors affecting Average Length Of Stay of the participants, it was confirmed that regardless of the location and bed size of medical institutions, hospitalization route, department diagnosis, main surgery, and whereabouts after discharge. Therefore, appropriate interventions and necessary support must be provided so that efficient Length Of Stay can be managed according to the medical use characteristics of serious patient.

Socio-cultural Readjustment of Korean Students Returning from Overseas

  • This study examines the socio-cultural readjustment of the Korean students returning from overseas study at an early age. For this study 259 returnee students from elementary through high school completed a questionnaire which covered aspects such as school adjustment and re-acculturative stress in relationship to gender, grade, number of parents accompanying the students overseas, length of overseas stay, age of return, length of stay in Korea, and the acculturative stress experienced in a host culture. The findings indicate that re-acculturative stress level of returnee students is higher than the level of the acculturative stress, and that the two are positively related. Hierarchical multiple regression analyses reveal the grade and acculturative stress of returnee students as a significant predictor of school adjustment. The length of overseas stay and acculturative stress significantly predicted the re-acculturative stress level. The acculturative stress significantly affected both school adjustment and re-acculturative stress of returnee students back in Korea.

Predictors of Facility Adaptation in Nursing Home Residents (노인요양시설 입소노인의 시설적응에 영향을 미치는 요인)

  • Purpose: The purposes of this study were to examine the relationships among activities of daily living, self-efficiency, nursing home care quality and nursing home adaptation, and to identify the influencing factors of nursing home adaptation in nursing home residents. Methods: The study employed a descriptive correlational design. The data were collected from 148 older adults without dementia by interview from six nursing homes in three cities from February 1, 2008 to February 28, 2008. Results: Levels of nursing home adaptation in older adult residents were different by type of decision maker of nursing home admission, reason of nursing home admission, type of payment and length of nursing home stay. The nursing home adaptation was significantly correlated with self-efficiency and nursing home care quality. The strongest predictor of nursing home adaptation was type of decision maker of nursing home admission followed by the self-efficiency. Conclusion: The study suggested that during the decision making period of nursing home admission, older adults should have enough time and careful considerations of their families to decide by themselves in positive ways. Nursing home staff should be able to identify reasons of nursing home admission and demands or expectations of older adults and their families.

Difference of Caring Needs According to Caring Experience of the Elderly with Dementia and Caring Situation (치매노인 부양경험 유무에 따른 부양요구의 차이와 치매노인 부앵실태)

  • The purpose of this study were to find out the difference of needs of caring between care-givers and non care-givers, and to suggest the way of lessening vigorous task of care-givers for the elderly with dementia. Data were collected from 130 nationwide respondents intentionally divided into two groups; care-givers and non care-givers in the middle aged with middle and upper income. Collected data were analyzed by frequency, percentage, t-test using SPSS package. Since the result of survey, unexpectedly, showed no difference between two groups, it could be explained as that these two groups commonly had same needs of caring for the elderly with dementia. Major findings were as follow; 1) Most Koreans stiff thought family should be the main care-giver for the elderly with dementia prior to nation or society. 2) Responsibility of caring for the elderly with dementia would be better to be shared with children instead of focusing to a child. 3) They thought ideal residential facilities for the elderly with dementia were small-scale professional dementia facility(group home) rather than home or general elderly housing. 4) Professional dementia care hospital was one of the most needed facilities for the elderly with dementia, followed by short-stay and dar-care center. 5) It was revealed care-giving task was vigorous showing that most care-givers spent 1-5 hours a day for caring, while 13% of respondents spent 11-24 hours a duty. 6) 90% of care-givers took the responsibility of main care-giver because of duty of offsprings or spouses, and wanted to be free from their current circumstances. From the result of this survey researchers would like to suggest the establishment of diverse facilities for professional dementia care to lessen the caring burden for the elderly with dementia: group home, chronic hospital, short-stay, day-care center. Financial support from the government for the housing renovation of the caring families should be considered seriously afterward. It is needed to give the opportunity to select proper paid dementia care facilities according to their income and situation of household.

The Study on the Consumption and the Preference of Chungkukjang among Elementary School Children in Seongnam Area (성남 지역 일부 초등학생의 청국장 섭취 실태 및 기호도 조사)

  • This study examined the preference and consumption of chungkukjang among elementary school children in Seongnam, Korea. The subjects included 583 students(male 300, female 283) who were asked to fill out a questionnaire. The collected data were analyzed using the SAS package. The results showed that 91.9% of the female students, 91.6% of children from nuclear families, and 90.7% of children with stay-at-home mothers, consumed chungkukjang. Among the children who had eaten chungkukjang previously, most had consumed it at home, and 65.5% of the students responded that chungkukjang was tasty. The data also revealed that female students, children from nuclear families, and those with stay-at-home mothers had greater preference for chungkukjang than male students, children from extended families, and those with working mothers. When asked why they dislike chungkukjang, both male and female students who had eaten it responded because 'it smells bad' and that 'it is not tasty'. However, among the students who had never eaten it, the male students replied because 'it is not tasty'(33.3%) and that 'it is not eaten at home'(27.8%), and the primary responses of the female students were 'it is not eaten at home'(65.2%) and 'it is not tasty'(21.7%). However, the subjects who had tried chungkukjang previously answered that in the future they would like to have it more frequently, as compared to the subjects who had never eaten chungkukjang. Therefore, to increase knowledge and improve perceptions of chungkukjang foods, families should give children many opportunities to try them, and in return, children will consume chungkukjang foods with pride. Furthermore, it is important that society as well as mass media, which have great influence on children, show concern and offer support in promoting chungkukjang.

  • This paper draws the home into the stream of information and communications technology (ICT) development. Considering that homes and our home life are constantly affected by persistent change influenced by ICT, it is remarkable that ICT has been relatively neglected as an object of research in the field of housing studies. This study provides an overview of conceptual movements in ICT/home relationships and their design and social impact through a critical discourse analysis. The conceptual movements must be considered by professionals in the field of housing and built environment to stay attuned to changes in the 21st Century. Diverse debates took place in the discourse about ICT in relation to home. The stream of thought was started by futurists who saw the electronic cottage as a solution to the social problems caused by the separation of work and home life in industrial cities. ICT has now entered a new phase, with WiFi P2P networks and location-based social media that encourage sharable homes. Based on the analysis of the discourse on ICT and home, we drew from three issues related to the paradigm shifts in homes in the context of ICT development to provide future directions for housing studies: from space programming to time scheduling, from individual ownership to shared access, and from live+work dwelling to live+work+community.

Development of an IoT-based Unmanned Home-Delivery Box System (사물인터넷(IoT) 기반의 무인 택배함 시스템 개발)

  • This paper is concerned with an Internet of Things (IoT)-based unmanned home-delivery box system, in which the system is developed with a prototype using a weight sensor, shock sensor, the Arduino and Raspberry-Pi of open-source hardware platforms. The developed system provides several functions such as a safe storage means, prevention of delivery-theft, and remote control of the home-delivery box, etc. Specifically, the system recognizes the arrival of goods by detecting the weight of deliveries and sends the arrived message of deliveries to the recipient' smartphone, and also controls (i.e., open and close) the locker of the unmanned home-delivery box system remotely with the smartphone. Furthermore, the developed system provides automatically a warning alarm around the unmanned home-delivery box when an external shock is applied to the home-delivery box, and sends a message on the shock to the recipient' smartphone. The major functions of the developed home-delivery box system have been verified in a realistic environment, and confirmed to work well. With the application of the developed home-delivery box system to the homes and offices where recipients are always not to stay, it is expected to significantly improve the safety of delivered goods as well as to effectively prevent the delivery operatives posing as a crime.

Effects of an Empowerment Program on the Adjustment of Older Adults to Nursing Home Life (요양시설 노인의 시설적응을 위한 임파워먼트 프로그램의 효과)

  • Purpose: This study aimed to evaluate the effectiveness of the empowerment intervention on the levels of self-esteem, interpersonal relationships and adjustment to nursing home life of the Korean nursing home residents. Methods: Participants (n=47) whose period of stay was less than one year were purposefully selected from 3 nursing homes in Korea. The experimental group (n=21) attended 10 weekly hour-long empowerment sessions. The empowerment program comprised two components; group education and group discussion. The control group (n=26) received a two-hour education about health management. Results: Compared with the control group, the experimental group showed significantly higher mean scores of selfesteem (t=5.51, p<.001), interpersonal relationships (t=2.73, p=.009) and adjustment to nursing home life (t=2.10, p=.041). Conclusion: Findings of this study suggest that empowerment interventions may be effective in enhancing nursing home residents' self-esteem, interpersonal relationships and adjustment to nursing home life. Future research is needed to determine if such an empowerment program could be used with newly institutionalized elders to speed as well as enhance their adjustment to nursing home life.
