A 72-year-old woman with metastatic lung cancer to bone and brain and with left external iliac vein thrombosis was under the care of a community palliative care provider. She experienced an acute pain crisis due to acute limb ischemia of the left lower limb. Goals-of-care discussions were held with the patient and her family; she prioritized symptom control and end-of-life care at home. The family and patient were aware of her short prognosis. Her complex pain was managed by the community palliative team, and her family was empowered to give subcutaneous injections. We illustrate a case showing the importance of community health services with palliative care support in providing symptom management and support to patient and family caregivers throughout the course of a life-limiting illness. It also highlights family caregivers' potential psychological distress in delivering subcutaneous injections in terminal care for a patient at home.

The energy intakes were significantly increased 56.29 Kcal after program, and protein 3.61g, lipid 3.12 g, calcium 49.71mg, iron 0.39 mg, vitamin B$_1$ 0.084 mg, vitamin B$_2$ 0.116 mg, and vitamin C 10.10 mg, respectively, except vitamin A and niacin. the significant changes of nutrient intakes were more at higher age group compared to at the lower age group, and more at girls compared to boys, Although the significant increase fo nutrient intakes at child care center, most nutrient intakes at home were not significantly changed, except energy, lipid, iron, vitamin A. The changes at changes of fat and calcium intakes for 3 months were positively correlated to the change of Z-score for weight, and the change of A-score for weight was positively correlated to changes of Z-score for height. The results showed that the nutritional management program for 3 month at child care center can increase nutrient intakes significantly and the incresed nutrient intakes can enhance children\`s growth. Korean society is experiencing rapid changes which are certain to shape the lives of the older people and their families. So, the purpose of this study was to improve the quality of the elderly welfare service in Korea. Even though the interest about the elderly welfare in Korea is rapidly on the increase, the reality of Korean Society still leaves much to be desired, quantitatively as well as qualitatively. Pay attent to this point, now going to grasp the situation of not the handicapped elder people but the general elder people actual life condition and demand for welfare. So this study focused on the demand of not out-of home service but in-home care service. It was based on the data from by Korea National Statistical Office, having observed 6,139 elderly people. The results were as follows. They hope to get the elderly welfare services about health examination service, nursing survice, supporting service for household. That was, the most of them wanted in-home care service than out-of home care service. To ensure effective care to the elderly, it needed development and settlement of welfare service in face of their daily living. And, even though they recognized that they should cover the elderly life expenses themselves, the rate was hit that indicated their adult children as a parents supporter. This means that we should consider not only to the elder people but also to the family which contained the old people as the elderly welfare service. As a remedy, we can find the cooperation between elder welfare service at the social welfare organization and counsel, education at the family strengthen center. To improve the quality of the elderly welfare service in Korea, the elderly welfare service should focus not only on the physical and spiritual health maintenance but also active understanding living environment and growing ability to arbitrate between individual and living.
