The nursing-care insurance system started in April, 2000 in Japan. It was a kind of business opportunities for a lot of private entrepreneurs. They came into this care service business from another type of business rapidly. They opened the pay nursing homes with 24 hour nursing. However, the expense load of the nursing-care insurance system was large, local governments started controlling on total numbers of facilities with 24 hours nursing in 2006. So the group houses besides the pay nursing home without nursing care are paid to attention. The pay nursing home has been often managed by the nursing business. Most of the group houses are managed by community based NPO. This study is a case study by the visit and the interview form for one of the group houses. The group house "Shalom Tsukimino" in Kanagawa started operations comparatively at early time among them. Through the study we recognized that a lot of people were helping the NPO as a community service. So they can manage the group house and people who live there get the services with low cost. They show that they separate residence and nursing and use the community service efficiently. We can find a new direction in this case for the life in elderly.
Recently, the long-term care insurance for the elderly was carried out according to the elderly is increased rapidly and the formation of sympathy that a nation and society try commonly to share health and welfare promotion of the elderly. The purpose of this study is to analyze the present status of nursing home after that the long-term care insurance is enforced in chungcheongbuk-do and to utilize as basic data. The study limited its survey to those facilities that refer to the Ministry of Health and Welfare data, that had the capacity of more than 50 people. The result are as followings. Firstly, most of the nursing home were located on the outskirts of the city. But it must be constructed in the city center if the recent deinstitutionalization trend is reflected. Secondly, notwithstanding the provisions of the Elderly Welfare Law, if the Livability and amenity are considered, the plan of a single or a twin room is needed. The ondol(溫突) system bedroom for the safety of the elderly had to be planned and for the color planning of a bedroom, a heating, the furniture, the form of a door, corridors, etc. should be partly improved. The fastener in which it is appropriate for the main exit, a stair, an elevator, the lighting device, and etc. is needed and the installation of a wandering path for the dementia patient and etc. is required. Thirdly, most of the dining room arranged on the first floor but it is not nearly used and it used for employee or the other use. Therefore, we have to consider the system in which it can deliver the meal to a bedroom. If the smell of the elderly and etc. is considered, the sufficient height of the floor should be reflected for the ventilation equipment. Lastly, The improvement of the existing law are required.
The purpose of this study was to investigate the effects of home care hair treatment characteristics and frequency on hair during permanent wave treatment. The hair treatments distributed on the market were classified into cationic surfactants and silicone oil. Hair treatment was performed 5 times, 10 times, 20 times differently according to the frequency of use, and hair permanent wave was performed. The hair thickness, tensile strength, epidermal observation using SEM, and hair component analysis using EDS were performed as experimental methods. The results of the experiment showed that the natural-derived ingredient was the main ingredient, and the cationic surface activity and silicon oil-free product group had high hair protection effect. Therefore, the product group using natural ingredients should be further researched and developed, and the study on the method and frequency of use of products with irritant chemical ingredients should be conducted.

Background: To implement medication management service in Korean nursing home (NH), medication review tool for residents in Korean long-term care facilities was developed. This prospective pilot study aimed to verify the applicability of this newly developed tool and to evaluate the drug related problems (DRPs) identified by pharmacists' medication review in NH setting. Methods: This study was carried out in two NHs in Korea. The elderly residents (65 or older) using 5 or more medications were eligible. Pharmacists conducted medication review and identified the DRPs and potentially inappropriate medications (PIMs) based on the newly developed tool. Results: Among 43 NH residents, 27 residents agreed to participate. The median age was 87 and about 55.6% of them were taking 10 or more medications. Pharmacists identified a total of 37 PIMs in 18 NH residents (66.7%) and 54 DRPs in 22 residents (81.5%). The most frequent PIM was general-PIM (26 cases) followed by PIM under specific diseases or conditions (7 cases). Out of 77 items contained in the tool, 15 items were detected in study participants. 'Continued use or regular daily use of hypnotics' was the most highly detected item (9 cases) followed by 'multiple prescriptions within each class of hypnotics/sedatives' (5 cases). Among 54 DRPs identified, pharmacist intervened 39 cases (72.2%) and interventions were accepted in 18 cases (46.1%). Conclusion: This pilot study demonstrated that newly developed tool is feasible for the nursing home residents. However, further studies with larger population are warranted.
The purpose of this study was to develop the classification of home-based long-term care activities and to test its validity. In this study, the taxonomy of long-term care activities was structured according to the service domain and process. Two expert groups participated in making a draft of the taxonomy that was composed of 7 service domains, 22 care needs, 22 service objectives, and 114 activities. Reliability and validity of the taxonomy was tested in a sample of 152 elderly subjects who used the home-based long-term care services. Based on the factor analysis of 114 activities, 21 factors were extracted from 114 activities. Internal consistency of the factors was high. Content validity was confirmed by the CVI. Long-term care insurance grade was used to assess the criterion validity. Among 21 care needs, 12 cares needs were significantly different from their grade. The classification of home-based long-term care activities demonstrated reliability and validity. In conclusion, the use of this classification is recommended while communicating with the elderly subjects, service providers, and the 3rd party payers.
The study aims to develop a assessment tool to provide the in-home disabled in a local community with appropriate services in consideration of physical, emotional, social and environmental circumstances. After collection of assesment data of 200 in-home disabled through use of the tool, a desire-extracting algorithm was developed to provide a service to real needs through the use of decision tree analysis on data mining. The study was conducted for Five months from June 2006 through October 2006, and it is divided into development of an assessment tool and extraction of real needs through the use of the tool. The basic framework of the development of the tool was established through the examination of related literature, the subjective satisfaction of the assessment tool and items were developed through the use of a focus group and experts, and verification was implemented through the use of statistics to confirm the validity of the tool. This study looked into the current conditions in Korean and Japanese care management for the elderly, cases of consumer counseling in these countries, and the supporting institutions for Japanese users regarding the use of care management for the elderly. The number of recipients of care management for the elderly in both Korea and Japan is growing every year, and more Japanese users receive various services compared to Korean users. The results of an analysis on consumer counseling regarding the use of Korean and Japanese care management revealed, two types of complaints: counseling for the improvement of the institution and complaints related to the procurement of service. Regarding the insurance system, the complaints were mostly related to affirmation of a rating and the burden incurred by cost-sharing. Regarding the use of service, such key impairment cases were related to in-service medical accidents, illegal acts including caregiver contract violations and forced retirement, careless service by workers, and human rights violations of the elderly. Japan has developed governmental and, related-administrative and business services as well as a local governmental system to redress customer impairment issues as this pertains to service for better quality care management. In addition, they have enlarged the locally closed service, provided exact information about the services offered, and improved service appraisal techniques. The Japanese cases will serve as a good reference to improve the Korean system. In particular, the construction of a system that reduces customer losses and the availability of more counseling information are urgently required to improve the system in terms of customer experiences when they seek care management.
Objectives: The purpose of this study was to provide a basic data of nutrition services in home health care by analyzing hospital-based home-visit nutrition education needs of patients at discharge. Methods: Data was collected from September 11 to October 12, 2012 by administering questionnaires to 289 chronic disease patients to be discharged from a university hospital in Pusan. The home-visit nutrition education instruments used for collecting data were developed by the researcher. Results: Regarding the demands of home-visit nutrition education, 62.3% of subjects were willing to use home-visit nutrition education and 37.7% weren't. The main reason for using the home-visit nutrition education was "the effective nutrient management in consultation with an individual's doctor", 38.9% and 31.2% of patients who did not wish to use the service gave the reason for their decision as, "Just by managing the nutritional requirements of a family's diet and, the patient will be able to fully recover", respectively. As for the demand, classified with the areas of home-visit nutrition education, the demand for the area of basic nutrition (3.75/5.00) was the highest followed by, the area of educational nutrition (3.74/5.00), therapeutic nutrition (3.67/5.00), and dietary nutrition (3.55/5.00). The demand for the area of educational nutrition was high "Considering the state of dietary management, such as disease status and drugs", 73.7%. As for the relation between the characteristics of the study subjects and analysis of demand home-visit nutrition education, the characteristic of subjects, that is, "regular home-visit nutrition education", "practice of diet therapy after discharge" had a significant difference statistically (p < 0.01). As for the relation between the needs for fundamental home-visit nutrition education and the demand of home-visit nutrition education, basic nutrition, educational nutrition, therapeutic nutrition, and dietary nutrition had a significant difference statistically (p < 0.01).
This paper examines the value of unpaid care work using the 2004 time use survey and compared them to economic measures such as GDP, tax revenue, value of paid work, government expenditure on care-related personnel expenditure and remuneration of paid care workers. It employs the mean earnings approach and generalist approach. It finds that the value of unpaid care work (compared to GDP) is between 18% and 29% using different approaches. The value of care of household members and for others in the community (person care) is between 5% and 8%. Furthermore, the value of unpaid care work is greater than that of direct tax regardless of the approach used. In addition, the value of person care is estimated at between 22% and 35% of the total value of the paid economy. The value of unpaid care work far exceeds the value of government expenditure on care-related personnel and the remuneration of paid care workers. This research suggests that unpaid care work (mostly conducted by women) should be recognized as part of production activities. This is because caring and household maintenance activities are necessary for individual well-being but also it contributes to the national economic competitiveness particularly through human resource development.
Background: Determining patterns of habitat use is key to understanding of animal ecology. Approximately 1% of bird species use brood parasitism for their breeding strategy, in which they exploit other species' (hosts) parental care by laying eggs in their nests. Brood parasitism may complicate the habitat requirement of brood parasites because they need habitats that support both their host and their own conditions for breeding. Brood parasitism, through changes in reproductive roles of sex or individual, may further diversify habitat use patterns among individuals. However, patterns of habitat use in avian brood parasites have rarely been characterized. In this study, we categorized the habitat preference of a population of brood parasitic lesser cuckoos (Cuculus poliocephalus) breeding on Jeju Island, Korea. By using compositional analyses together with radio-tracking and land cover data, we determined patterns of habitat use and their sexual and diurnal differences. Results: We found that the lesser cuckoo had a relatively large home range and its overall habitat composition (the second-order selection) was similar to those of the study area; open areas such as the field and grassland habitats accounted for 80% of the home range. Nonetheless, their habitat, comprised of 2.54 different habitats per hectare, could be characterized as a mosaic. We also found sexual differences in habitat composition and selection in the core-use area of home ranges (third-order selection). In particular, the forest habitat was preferentially utilized by females, while underutilized by males. However, there was no diurnal change in the pattern of habitat use. Both sexes preferred field habitats at the second-order selection. At the third-order selection, males preferred field habitats followed by grasslands and females preferred grasslands followed by forest habitats. Conclusions: We suggest that the field and grassland habitats represent the two most important areas for the lesser cuckoo on Jeju Island. Nevertheless, this study shows that habitat preference may differ between sexes, likely due to differences in sex roles, sex-based energy demands, and potential sexual conflict.
