인공 관절 설계를 위한 바이오미메틱 복합재료에 관한 연구 (A Study on Biomimetic Composite for Design of Artificial Hip Joint)

  • This study suggests the design of the functionally gradient composite, [0/90/0/core]$_s$ cross-ply laminate, to prevent stress concentration induced from the difference of rigidity between the bone and the artificial hip joint and to reinforce the wear property of the surface and the expectation of their mechanical properties. First, the four-point bending test is done about wet bones and dry bones to know the mechanical properties of the cortical bones. In result, the wet bone shows the viscoelastic behavior and the dry bone shows the elastic behavior. Moreover, we expect the properties of the proposed gradient composites as a function of carbon fiber volume fraction in each layer to apply Halpin-Tsai equation, CLPT(classical laminate plate theory), and Bernoulli beam theory etc. and decide the thickness ratio of each lamina in order to match Young's modulus of the anisotropic cortical bone with the proposed gradient composites.

연령에 따른 고관절주위 대퇴골 및 절구뼈 골수전환 양상 : MR 평가 (Marrow Conversion Pattern of the Femora and Acetabulum Around Hip Joints According to Age Group : MR Evaluation)

  • 목적 : 고관절주위 대퇴골 및 절구뼈에 질병이 이환되었을 때는 골수 변성이나 부종이 발생하지만 연령에 따른 정상 골수전환이나 재전환을 감별하지 못하면 진단이 어려워진다. 이에 정상 골수 전환 양상을 연령군으로 분류하여 분석해 보았다. 대상 및 방법 : 자기공명영상에서 고관절 및 고관절주위 대퇴골와 절구뼈에 이상소견이 없고 혈청검사에서 골수이상을 초래할 만한 자가면역질환이나 혈액질환이 없는 144명 288개 고관절을 대상으로 하였다. T1강조영상으로 고관절주위 대퇴골 및 절구뼈의 골수를 관찰하였으며 골수 신호강도가 근육 신호강도에 가까우면 적색골수, 지방 신호강도에 가까우면 황색골수로 간주하였다. 대퇴골 골수는 머리, 목, 그리고 큰돌기, 작은돌기 그리고 돌기사이를 포함한 돌기 부위로 나누었고 절구뼈 골수는 황색골수 전환 절반이상과 이하로 나누었다. 연령은 10년 단위로 나누었고 또한 태생부터 10살까지는 1년 단위로 나누었다. 결과 : 절구뼈 절반이하 골수전환 되고, 대퇴골 머리 목 그리고 돌기가 골수전환 된 경우가 226례 78.5%, 대퇴골 머리와 돌기만 골수전환 된 경우는 44예 15.3% 였다. 연령별 골수전환 범위는 만2세전후부터 커지기 시작하여 만20세전후에서 끝나는 양상을 보였다. 결론 : 고관절주위 대퇴골 및 절구뼈의 연령별 정상 골수전환 양상을 이해함으로써 질병으로 인한 골수 변성이나 부종을 감별하고 드물지 않게 고관절 통증을 호소하는 전이암이나 백혈병 같은 골수대체성 질환을 진단 할 수 있을 것으로 사료된다.

  • The pelvic girdle function as an integrated unit with all three bones moving at all three joints, are influenced by the lower extremities below and vertebral column and trunk above sacroiliac movements are caused by spinal motion, whereas iliosacral movements are caused by movements of the lower limbs. Concept of normal functional integration among the lumbar spine, pelvic and hip joint is basic to the understanding of dysfunction in this region and also functional movement of the lumbo-pelvic-hip region are part of the clinical examination, consequently the integrated biomechanics of these region need to be understood. The purpose of this review is to ascertain the integrated biomechanics among the lumbo-pelvic-hip complex by consideration of literature and to give sufficient information to be able to render accurate assessment and treatment for the syndromes described.

고관절을 이루는 골격의 골절 환자의 통증 감소에 대한 한의학적 치료 효과 : 후향적 관찰연구 (Effect of Korean Medicine Treatments for Pain Reduction in Patients with Hip Fracture : A Retrospective Observational Study)

  • The aim of this study was to observe the effectiveness of Korean Medicine treatments on pain reduction of patients who were diagnosed hip fracture - Acetabulum and Proximal Femur. We reviewed the 17 patients' records who had admitted to Daejeon Korean Medical hospital and diagnosed fracture of acetabulum or proximal femur bones from January 1, 2011 to April 30, 2020 during the admission retrospectively. Statistical analysis was done using the IBM SPSS statistics 25 program. We measured Numeric Rating Scale to evaluate the treatment's efficacy. Korean Medicine treatments reduced median of Numeric Rating Scale reduced statistically significantly from 6.00 to 3.00(p<0.05). During the admission, patients didn't experience the side effects of Korean Medicine treatments. In conclusion, these results implied that Korean medicine treatments showed a positive effect on pain reduction of hip fracture patients. Further research is required to confirm the effectiveness of Korean Medicine treatments.

상악골 골수염을 수반한 골화석증 (OSTEOPETROSIS WITH MAXILLARY OSTEOMYELITIS)

  • 상악골 골수염을 수반한 골화석증에 이환된 33세의 남자환자에서 다음과 같은 사항이 관찰되었다. 1. 간비대, 비장비대, 궁다리소견 등을 볼 수 있었다. 2. X선사진상에서 전반적인 골밀도의 증가와 부비동의 폐색등을 볼 수 있었다. 3. 대퇴골과 좌골의 여러 부위에서 오래된 골절이 관찰되었으며 고관절의 아탈구도 관찰되었다. 4. 상ㆍ하악골 모두 골밀도가 증가되어 있었으며 다수의 기형치와 매복치가 관찰되었다. 5. 골주사 소견에서 큰 관절과 장골에서 다발성의 불규칙한 섭취증가를 볼 수 있었다.

유한요소해석을 이용한 하중조건에 따른 대퇴골의 거동예측 (Predicted the behavior of the femur according to the loading condition using FEM)

  • Falling related injuries are categorized as the most serious and common medical problems experienced by the elderly. Hip joint fracture, one of the most serious consequences of falling in the elderly, occurs in only about 1% of falling. In this study, according to the loading conditions, the analysis is the behavior of the femur. The CT images using the commercial program "Mimics" the bones of three-dimensional CAD data generated, and we will analyze the results of finite element analysis. The boundary conditions on the basis of existing research has been simplified. In this paper, the whole femur was assumed to be isotropic linear elastic material. Predicted the behavior of the femur according to the loading condition, it can be help the development of high-precision artificial bones and joints can be treated with surgery and will be able to perform efficiently.

발생 부위에 따른 단순성 골낭종의 치료방법에 대한 연구 (The Treatment's Modality of Simple Bone cyst According to the Location)

  • Twenty-nine cases of unicameral bone cyst developed in long bone of children have been treated and followed up for 4.5 years in average form Department of Orthopaedic Surgery, Guro Hospital, Korea University, College of Medicine since September, 1983, Treatment for those lesions differed to form largely two groups, one of which consised of insillation of Methyl-prednisolone for non-weight bearing bones(12 humeri) and the other of curettage and autogenous bone graft for weight-bearing bones(7 femur). Methl-prednisolone group required repetition of instillation for 3.5 time in average spanning over 4 years until cloudy obliteration occurs. Curettage and bone graft had healed in 3 year 6 months' time in average. There were neither recurrence nor pathologic fractures of the lesions with the latter group. Immobilization period was virtually non with Methyl-prednisolone group and 4-6 weeks by hip spica with curettage and bone graft group. As conclusions, It seems confirmed that treatment strategy of unicameral bone cyst consisted of Methyl-prednisolone instillation for humerus lesions and early curettage and bone graft for femur lesions is applicable as guideline having solid ground in clinical experiences.

A Rare Case of Solitary Osteochondroma at the Temporomandibular Joint: A Case Report

  • Osteochondroma is a bone tumor with cartilaginous growth potential that generally appears near the growth plate of long bones in areas such as hip, knee, and shoulder joints, related to the nature of endochondral ossification and it is known a common benign bone tumor. However, it has been very rare in craniofacial region possibly because craniofacial bone is largely formed by intramembranous ossification. Moreover, reports on the solitary type of osteochondroma in mandibular condyle has been extremely rare. Osteochondroma in mandibular condylar may show various symptoms similar to general temporomandibular joint disorders (TMDs), such as pain in the condylar area during mouth opening, internal derangement, facial asymmetry or posterior open bite. Therefore, it can be disregarded for a long time period without any adequate treatment. Surgical excision has been the treatment option for the solitary osteochondroma with very low recurrence rate reportedly. In this case report, a rare case of solitary osteochondroma developed in unilateral mandibular condyle is presented with emphasis on differential diagnosis with general TMDs.

  • Pigmented villonodular synovitis(PVNS) usually presents as a benign proliferative monoarticular arthritis that affects the knee in 80% of cases, followed in frequency by the hip, ankle, and calcaneocuboid joint. PVNS rarely affects temporomandibular joint area. Patients typically complain of pain, locking, and recurrent swelling. Tumor progression limits the range of movement of the joint and causes it to become stiff and firm. Sometimes a palpable mass can be appreciated. 고관절용 인공관절 시스템의 안정성 평가 (Stability Test of Artificial Joint for Hip Joint)

  • 인공관절은 관절부의 뼈가 파괴되거나 관절 손상을 입어 관절로서의 기능을 할 수 없게 된 경우에 사용된다. 고관절용 인공관절 시스템의 일차안정성을 평가하기 위해서는 인공관절이 삽입된 직후 인공관절에 반복하중을 가하여 대퇴골과 인공관절 사이의 상대변위를 측정하게 된다. 이때, 정확한 안정성을 평가하기 위해서는 인공관절이 대퇴골을 파고드는 변위와 대퇴골에서 인공관절이 회전한 각의 변화량을 정확히 측정하여야 한다. 본 연구에서는 고관절용 인공관절 시스템의 일차안정성을 평가하기 위해 새로운 상대 변위측정 방식을 제안하고 이를 이용하여 수술 방법의 차이 및 대퇴골 자체 강성의 차이에 따른 일차 안정성을 사체에서 얻은 대퇴골 실험을 통해 비교 평가하였다.
