Reliability and Validity of Gait Assessment Tools for Elderly Person (노인의 보행에 대한 평가 도구의 신뢰도와 타당도 조사 연구)

  • Purpose: The examine the Reliability and Validity of the modified Emory Functional Ambulation Profile (mEFAP), Tinetti. Gait (TG), Timed Up & Go Tes t (TUG), Comfortable Gait Speed (CGS), Berg Balance Scale (BBS) in assessing gait function and balance in elderly person. Methods: The 45 community-dwelling subjects were participated in this study. Reliability was determined by intra-class correlation coefficient (ICC$_{3,1}$), Bland and Altman method (Standard Error of Measurement (SEM), Smallest Real Difference (SRD)). Results: Validity was examined by correlation the mEFAP, TG, TUG, CGS, BBS. The intra-rater reliability were High (ICC$_{3,1}$ : mEFAP=0.95, TG=0.96, TUG=0.94, CGS=0.96, BBS=0.92) and Absolute reliability were excellent (SEM: mEFAP=1.90, TG=0.21, TUG=0.28 CGS=0.25, BBS=0.52), (SRD: mEFAP=5.26, TG=0.58, TUG=0.77, CGS=0.69, BBS=1.44). There were significant correlations between assessment tool (r=.0.58$\sim$0.78, p<0.01) indicating good validity. Our results provide strong evidence that the assessment tool has good reliability, validity for assessing elderly person undergoing rehabilitation. Conclusion: The gait assessment tool is a useful scale for measuring walking function and recovery in elderly person.

  • Carbon Fiber Reinforced Plastic (CFRP) and Titanium Alloy (Ti6Al4V) stack, extensively used in aerospace structural components are assembled by fasteners and the holes are made using drilling process. Drilling of stack in one shot is a complicated process due to dissimilarity in the material properties. It is vital to have optimal machining condition and tool geometry for better hole quality and tool life. In this study the tool wear and hole quality were analysed by experimental analysis using three modified twist drills and online tool condition monitoring using Acoustics Emission (AE) sensor. Helix angle and point angle influence tool performance and cutting force. It was found that a tool geometry (TG1) with high helix angle of $35^{\circ}$ with low point angle $130^{\circ}$ results in reduction in thrust force of 150-500 N range but the TG2 also perform almost similar to TG1, but when compared with the AErms voltage generated during drilling it was found that progressive rise in voltage in TG1 is less with respect to TG2 which can be attributed to tool life. In process wear monitoring was done using crest factor as monitoring index. AErms voltage were measured and correlated with the performance of the drills.

Effects of Cholesterol Feeding on HDL-Cholesterol, Total Cholesterol and Triglyceride of Plasma and Tissues of Rats Fed the Different Dietary Fat Level and P/S Ratio (총지방량과 P/S Ratio가 다른 식이에 첨가된 Cholesterol이 Plasma HDL-Cholesterol 과 조직의 Cholesterol과 Triglyceride에 미치는 영향)

  • The present studies were designed to observe the effects of both dietary fat levels and P/S ratio on lipid components in plasma and tissues of young rats when cholesterol was supplemented at 1%(w/w) to four dietary groups providing total fat as 10%( LF ) or 45% ( HF ) of calories and P/S ratio as 0.2 or 4.0, Low Fat-0.2 : Low Fat-4.0 : High Fat-0.2 : High Fat-4.0. Plasma total cholesterol was increased almost to the same level in all dietary groups after the cholesterol supplement but the value of HF -0.2 was slightly higher than that of LF -0.2. Plasma TG was also increased in all dietary groups when cholesterol was supplemented but was more significantly increased in LF group than in HF group. HDL-cholesterol was slightly increased by cholesterol supplement but there was no effect by the total dietary fat level of previous diet. However, plasma HDL-cholesterol, total cholesterol and TG levels were slightly reduced in high fat diet of P/S 4.0. Total cholesterol per g -liver was higher in HF group than in LF group before and after cholesterol supplement even though it was increased more in LF group with cholesterol addition. Liver cholesterol was also higher in rats fed diets of P/S 4.0 at high fat level before and after cholesterol supplement. In contrast, total cholesterol per g-muscle was reduced by cholesterol supplement in all groups and it was significantly higher in LF than in HF. There was no significant effect in liver TG by total fat level and P/S ratio when cholesterol was supplemented. Muscle TG was lowered in all groups by cholesterol supplement but no effect by total fat level and P/S ratio.

  • In an attempt to evaluate the effect of high and low concentration of Injinsaryungsan and high and low concentration of Sosihotang on cholestatic liver injery induced by $ANIT({\alpha}-naphthylisothiocyanate)$, biochemical changes in serum transaminase(GOT, GPT), alkaline phosphate, lactate dehydrogenase, total cholesterol, triglyceride, total-bilirubine were studied and the following results were obtained. 1. High concentration of Injinsaryungsan(2.2g/Kg) inhibited significantly the activity increases of GOT, GPT, ALP, LDH, TC, TG, T-Bilirubine induced by $ANIT({\alpha}-naphthylisothiocyanate)$. 2. Low concentration of Injinsaryungsan(1.1g/Kg) inhibited the activity increases of ALP, LDH, TC, TG with statistical significance, while inhibited the activity increase of GOT ,but with no statistical significance. 3. High concentration of Sosihotang(2.4g/Kg) inhibited the activity increases of LDH, TG, TC with statistical significance while inhibited the activity increases of GOT, GPT, ALP, T-bilirubine with no significance. 4. Low concentration of Sosihotang(1.2g/Kg) inhibited the activity increase of TG, while inhibited the activity increase of ALP, TC with no statistical sig-nificance, but didn't inhibite the activity increases of GOT, GPT, LDH, T-Bil. These results suggest that Injinsaryungsan has more significant effect on the liver injury induced by $ANIT({\alpha}-naphthylisothiocyanate)$ compared with Sosihotang and so can be applicable clinically to virus hepatitis and cholestatic liver injury. Further study will be required to evaluate the effect of Sosibotang on cholangitis and cholecystitis.

The antioxidant activity of steamed ginger and its protective effects on obesity induced by high-fat diet in C57BL/6J mice

  • BACKGROUND/OBJECTIVES: Ginger, a root vegetable, is known to have antioxidant and antiobesity effects. Preparation, such as by steaming, can affect the chemical composition of prepared root vegetables or herbs and can change their functional activities. In the present study, we investigated the protective effects of steamed ginger against oxidative stress and steatosis in C57BL/6J mice fed a high-fat diet. MATERIAL/METHODS: The levels of polyphenols and flavonoids in two different extracts of steamed ginger, i.e., water extract (SGW) and ethanolic extract (SGE); as well, their antioxidant activities were examined. Forty male C57BL/6J mice were fed a normal diet (ND, n = 10), high-fat diet (HFD, 60% fat, w/w, n = 10), HFD supplemented with 200 mg/kg of SGE or garcinia (GAR) by weight (SGED or GARD, respectively, n = 10) for 12 weeks. Serum chemistry was examined, and the expressions of genes involved in lipid metabolism were determined in the liver. Histological analysis was performed to identify lipid accumulations in epididymal fat pads and liver. RESULTS: The SGE had higher contents of polyphenols and flavonoids and higher DPPH and $ABTS^+$ free radical scavenging activities compared to those of SGW. Treatment with SGE or GAR significantly decreased the HFD-induced weight gain. Both SGE and GAR significantly reduced the high serum total cholesterol (TC), triglyceride (TG) and low-density lipoprotein levels induced by HFD. Compared to ND, HFD significantly increased hepatic TC and TG levels. SGE or GAR supplementation significantly decreased the increase of hepatic lipids by HFD. Interestingly, SGE had a more significant effect in reducing hepatic TC and TG levels than GAR. Furthermore, hepatic genes involved in lipogenesis and lipolysis were altered in both the SGED and GARD groups. CONCLUSIONS: The present study indicates that steamed ginger supplementation can decrease plasma TC and TG and can inhibit liver steatosis by regulating the expressions of hepatic genes.

One University Staff Members' Life Styles, Body Mass Indices, Lipid Profiles and Plasma Glucose Levels (일 대학교 교직원의 생활습관과 체질량지수 및 생화학적 지수에 관한 연구)

  • Purpose: To analyze the differences of the Body Mass Index (BMI), blood biochemical indices (TC, TG, HDL, PP2) among college faculty members depending on their life styles and thereupon, provide for some basic data useful for healthcare education. Methods: 163 faculty members were sampled for a questionnaire survey and a medical checkup, both conducted from Jan. 20 to Jan. 26, 2010. Results: First, such blood biochemical indicesas BMI, TG, HDL and PP2 differed significantly depending on gender, while TC, TG and HDL differed significantly depending on age. Second, BMI differed significantly depending on drinking. Third, smokers showed significantly lower BMI and PP2 than non-smokers. Among the smokers, those smoking for 1-10 years showed a significantly lower level of TG. Fourth, those exercising as hard as sweating 3 times a week showed significantly lower BMI, TC and TG. Fifth, BMI was correlated positively with TC and TG, while being correlated negatively with HDL. On the other hand, TC was correlated positively with the TG which was correlated negatively with the HDL which was correlated positively with PP2. Conclusion: In order to prevent chronic diseases and live a healthy life, it must be necessary to control drinking, stop smoking and exercise regularly.

Effects of Dietary Calcium and Sodium Levels on Lipid Metabolism in Hyperlipidemic/Hypercholesterolemic Rats (고지혈증 모델 흰쥐에서 칼슘과 소디움 섭취수준이 체내 지질대사에 미치는 영향)

  • The effects of dietary Ca and Na levels on lipid metabolism in hyper lipidemic/hypercholesterolemic rats were examined. In Expt. 1, normal rats were divided into six groups and fed high fat(15%, w/w)/cholesterol(1%, w/w) diet containing two levels of Na, low (0.05) or high(1.5%) and three levels of Ca, low(0.1%), normal (0.5%), or high(1.5%) for 8 weeks. In Expt. 2, hyperlipidemia / hypercholesterolemia rats were induced by feeding high fat / cholesterol diet for 4 weeks. They were divided into four groups and fed the high fat / cholesterol diet, containing two levels of Na, low or high and two levels of Ca, low or high for 4 weeks. In Expt. 1, total lipid and total cholesterol contents in serum and liver were significantly lower in rats fed high Ca diet than in rats fed normal or low Ca diet regardless of dietary Na levels. Serum TG was the highest in rats fed low Ca and low Na diet. In Expt. 2, Serum total lipid, TG, and total cholesterol levels decreased by 24, 35, 26% respectively in rats fed high Ca diet regardless of dietary Na levels. Serum total lipid level tended to increased in rats fed low Na diet. The total lipid and TG contents in liver slightly decreased in rats fed high Ca diet. Another observation was that high Ca intake significantly faciliated the fecal lipid and cholesterol excretion regardless of dietary Na levels. There results suggest that the hypolipidemidc/hypocholesterolemic effects of high Ca diet could be partly due to increase in lipid and cholesterol excretion and these effects may be independent of dietary Na levels.

  • The present studies were designed to compare the effects of both dietary fat levels and P / S ratio on lipid components in plasma and tissues. Changes in plasma HDL-cholesterol, cholesterol and TG, and also in tissue cholesterol and TG were determined in young rats fed diets providing total dietary fat as 10%, 25% or 45% of calories and P / S ratio as 0.2 or 4.0. Plasma cholesterol levels were getting higher as dietary fat levels increased at P / S 0.2. Plasma cholesterol was lower in rats fed dietary fat either 25% or 45 %, each with P / S 4.0. But at 10% no change in plasma cholesterol were observed by P / S 4.0 because of a possible insufficiency of the absolute amount of PUFA. HDL-cholesterol was rather less sensitive to the modification of dietary fat level, but was reduced in rats fed diets of P / S 4.0 at either 25% or 45% fat, even though HDL-cholesterol were increased in the group of 10% with P / S 0.2. Total cholesterol per g- liver were significantly increased as dietary fat levels increased. Liver cholesterol levels were higher in rats fed diets of P / S 4.0 at higher fat levels (25% or 45%) which possibly suggested that a reduction of plasma cholesterol by high PUFA diet was not at least from a decreased synthesis of cholesterol in liver. However, in muscle no significant differences were found by feeding high P / S ratio at each levels of fat. At 10% fat level, compared to 25% or 45%, cholesterol level was lower in g-liver but higher in g- muscle. Plasma TG was decreased as more dietary fat were supplied at P / S ratio, but no consistant response obtained at low P / S ratio. TG per g-liver were reduced by feeding P / S 4.0 diet at 10% or 45% fat level but no differences were found in muscle. P / S 4.0 diet was more efficient in lowering plasma cholesterol TG and HDL-cholesterolt levels only if fat level was more than 25% of the total calories And young rats were more resistant to dietary fat modification.

Comparison of Dosimetry Protocols in High Energy Electron Beams (고에너지 전자선에 대한 표준측정법간의 비교)

  • Any detector inserted into a phantom should have such a geometry that it caused as small as possible perturbation of the electron fluence. Plane parallel chambers meet this requirement better than other chambers of configurations. IAEA protocol recommends the use of plane parallel chambers for this reason. However, the cylindrical chambers are widely used for convenient. The purpose of this study is to evaluate the absorbed dose due to the differences of four different dosimetry protocols such as IAEA protocol using cylindrical chamber, TG 21 protocol using cylindrical chamber, Markus protocol using plane parallel chamber, and TG 39 report for the calibration of plane parallel chamber in electron beams. Depth-ionization measurements for the electron beams of nominal energy 6, 9, 12, 15, and 18 MeV from Siemens accelerator with a 10$\times$10 cm$^2$ field size were made using a radiation field analyser with 0.125 cc ion chamber. Dosimetric measurements by IAEA and TG 21 protocol were made with a farmer type ionization chamber in solid water for each electron energy, respectively. Dosimetric measurements by Markus protocol were made with a plane parallel ionization chamber in solid water for each electron energy, respectively. The cavity-gas calibration factor for the plane parallel chamber was obtained with the use of 18 MeV electron beam as guided by TG 39 report. Dosimetric measurements by TG 39 were performed with a plane parallel ionization chamber in solid water for each electron energy, respectively. For all the energies and protocols, measurements were made along the central axis of the distance of 100 cm (SSD = 100 cm) with 10$\times$10 cm$^2$ field size at the depth of d$_{max}$ for each electron beam, respectively. In the case of 18 MeV, the discrepancy of 0.9 % between IAEA and TG 21 was found and the two protocols were agreed within 0.7 % for other energies. In the case of 18 MeV and 6 MeV, the discrepancies of $\pm$ 0.8 % between Markus and TG 39 was found, respectively and the two protocols were agreed within 0.5 % for other energies. Since the discrepancy of 1.6 % between cylindrical and plane parallel chamber was found for 18 MeV, it is suggested to get the calibration factor using other method as guided. by TG 39.9.

A Study on the Ultrasonographic Liver Fat Density and Serum Enzymes for Testing Liver Function in Korean Adults (성인 남녀의 초음파 조영술로 측정한 간지방밀도와 혈청 간기능효소와의 상관성 연구)

  • This study was done to investigate the relationship between ultrasonograph fat density (ULFD) using objective measurement and serum enzymes for testing liver function in 101 healthy adults(43 males and 58 females). Average serum enzyme activities in males and females were GOT27.111U/L and 22.46IU/L, GPT 34.06IU/L and 18.501U/L, and ${\gamma}$-GTP 37.67IU/L and 17.201U/L, respectively. Males showed significantly higher activities of GPT and ${\gamma}$-GTP than females. ULFD of the obese group (BMI$\geq$25) was significantly higher than that of the nonobese group. GOT, GPT, and ${\gamma}$-GTP tended to be high in the obese group. GPT and ${\gamma}$-GTP of the high TG group (TG$\geq$170) tended to be markedly high for males, but not for females. GPT was positively correlated with ULFD, body weight , and weight-to-height, ratio, and ${\gamma}$-GTP was positively correlated with body weight, weight-to-height ratio. BNI, and KI. ULFD and ${\gamma}$-GTP were positively correlated with serum TG. These results suggests that , among serum enzymes for testing liver function, GPT has a close relationship with ULFD using objective measurement, while GOT does not. Also , ${\gamma}$-GTP has a close relationship with parameters for obesity.
