A Study on Medical Costs for Patients with Vertigo Based on 2014 Health Insurance Review & Assessment Service-National Patients Sample Data (2014년 건강보험심사평가원 환자표본데이터 분석을 이용한 현훈환자의 의료비용에 관한 연구)

  • Objectives: The purpose of this study was to analyze the medical cost for patients with vertigo and to examine associations between chronic vertigo and mental disorder using 2014 Health Insurance Review & Assessment Service-National Patients Sample (HIRA-NPS). Methods: We analyzed sociodemographic characteristics, medical cost and medical care use pattern for vertigo patients. We used hierarchical multiple logistic regression analysis to examine odds ratio between chronic vertigo and mental disorder. Results: A total of 46,502 people and 118,504 claims data were identified for vertigo cases. Characteristics of vertigo patients have significant differences on proportion of female patients (68.36%), patients' average age (54.98) and proportion of medical assistance (5.76%) compared with non-vertigo patients. Results revealed that Korean medicine are one of frequent methods among total treatments for vertigo patients. Total days of medical care and total costs are 2.78 days and 111,362 won, respectively, and days for outpatients in Korean medical care (mean: 2.26 days) are more than those (mean: 5.05 days) in Western medical care. There is significant difference relative to sex between acute vertigo and chronic vertigo. The odds ratio between chronic vertigo and mental disorder is estimated as 1.34, that means risk of becoming chronic is 34% higher for vertigo patients with mental disorder. Conclusions: This study assessed socio-demographic characteristics, medical care use and expenses related to vertigo, and estimated associations between chronic vertigo and mental disorder. Findings provide a basis for economic evaluation studies on vertigo patients and development of clinical practice guidelines for vertigo patients with mental disorder.

Nutritional Status and the Characteristics Related to Malnutrition in Children Under Five Years of Age in Nghean, Vietnam

  • Objectives: This study was conducted to assess the nutritional status and characteristics related to malnutrition in children less than five years of age in Nghean, Vietnam. Methods: In this study, which was conducted in November 2007, 650 child-mother pairs were selected using a two-stage cluster sampling methodology. A structured questionnaire was then administered to the mothers in their home settings. Anthropometric measurement was then used to determine if children were underweight (weight-for-age), wasting (weight-for-height) and stunting (height-for-age) based on reference data from the National Center for Health Statistics (NCHS)/World Health Organization (WHO). Logistic regression analysis was then used to describe the hierarchical relationships between potential risk factors and malnutrition. Results: The mean Z-scores for weight-for-age, height-for-age and weight-for-height were -1.46 (95% CI=-1.57, -1.35), -1.44 (95% CI=-1.56, -1.32) and -0.71 (95% CI=-0.82, -0.60), respectively. Of the children included in this study, 193 (31.8%) were underweight, 269 (44.3%) were stunting and 72 (11.9%) were wasting. Region of residence, the mother's level of education and occupation, household size, number of children in the family, weight at birth and duration of exclusive breastfeeding were found to be significantly related to malnutrition. Conclusions: The findings of this study indicate that malnutrition is still an important problem among children less than five years of age in Nghean, Vietnam. In addition, maternal, socio-economic and environment factors were found to be significant factors for malnutrition among children under five.

Risk Factors for Suicidal Ideation across the Life Cycle among Korean Adults: Korean Psycho-social Anxiety Survey (한국 성인의 생애주기별 자살생각의 위험요인: 한국 사회·심리적 불안 조사)

  • Purpose: To identify the risk factors for suicidal ideation across the life cycle among Korean adults using data from 2015 Korean Psycho-social Anxiety Survey. Methods: The data were statistically analyzed using hierarchical logistic regression analysis. Results: The factors associated with suicidal ideation among the young adult group were education level (odds ratio [OR] 1.62, 95% confidence interval [CI] 1.05~2.49), self-esteem (OR 1.19, 95% CI 1.12~1.27), stress (OR 3.26, 95% CI 2.07~5.15), anger control problems (OR 3.58, 95% CI 2.34~5.50), and depression (OR 2.59, 95% CI 1.66~4.04) whereas among the middle-aged adults the factors were education level (OR 1.58, 95% CI 1.12~2.23), existence of a spouse (OR 2.55, 95% CI 1.72~3.78), self-esteem (OR 1.13, 95% CI 1.08~1.18), stress (OR 2.46, 95% CI 1.81~3.33), anger control problems (OR 1.70, 95% CI 1.22~2.36), and depression (OR 2.64, 95% CI 1.91~3.64). Among the older adult group the findings were the existence of a spouse (OR 2.16, 95% CI 1.37~3.39), self-esteem (OR 1.22, 95% CI 1.13~1.32), stress (OR 2.21, 95% CI 1.38~3.54), anxiety (OR 2.42, 95% CI 1.46~4.02), and depression (OR 2.48, 95% CI 1.30~4.72). Conclusion: The findings suggest that there may be a need for different suicide intervention programs to decrease suicidal ideation across the life cycle.

West African Pregnancy Experienced Women's Self-Rated Health and Related Factors: Case of Democratic Republic of the Congo (서아프리카 임신경험 여성들의 주관적 건강상태와 관련 요인: 콩고민주공화국 사례를 중심으로)

  • Background: This study aims to analyze West African pregnancy experienced women's self-rated health and related factors in Democratic Republic (DR) of the Congo, and provide basic data which can be used in developing maternal and child heath projects and health promotion programs to enhance women's self-rated health in the DR Congo. Methods: A survey was conducted among 400 women, 300 of whom were from Kenge and 100 from Boko in Bandundu Province. Three hundred and fifty-two questionnaires were administered on factors expected to affect the self-rated health, categorized into basic information and economic status of respondents, healthcare service, and illness patterns. The hierarchical logistic regression analysis was performed taking each variable into account. Results: The results show that those who have had nutrition education, work in the office, have no health insurance, or have had no illness for the last one year perceive themselves to be in good health. Conclusion: To improve the self-rated health status and promote maternal and child health in DR Congo, the results suggest that the government or relevant agencies should develop programs to educate people regarding health and nutrition, and policies based on the right status of each country. Systems are also required for healthcare at all social levels along with aspects to avoid illness in daily life.

Prophylactic Effect of Transparent Film Dressing on Sacrum and Coccyx in SICU Patients (외과중환자실 환자의 천미골에 적용한 보호필름 드레싱의 예방적 효과)

  • Purpose: This study was done to examine the prophylactic effect of transparent film dressing on the sacrum and coccyx sites to reduce pressure ulcers. Methods: The participants were 317 patients admitted to an SICU in Seoul, Korea. Of the patients, 175 were assigned to the experimental group and 142 to the control group. For participants in the experimental group, a prophylactic transparent film dressing was applied on the sacrum and coccyx. The control group received the usual care. The nurses checked for pressure ulcers on the sacrum and coccyx at least once every duty. When pressure ulcer occurred, it reported on the record form. The results were analyzed using Poisson and Hierarchical logistic regression. Results: The prevalence and risk of pressure ulcer was lower in the experimental group compared to the control group but the difference was not significant. The ICU length of stay was significantly associated with pressure ulcer risk. Conclusion: Findings indicate that prophylactic transparent film dressing helps to reduce pressure ulcer in SICU patients.

Risk Factors for Falls in the Elderly by Life-cycle (노인생애주기에 따른 낙상요인)

  • Objectives: The objective of this study was to investigate risk factors for falls in the elderly by life-cycle. Methods: This study used the data on 435 male and female elderly generated from a survey taken at 21 Welfare Centers for the Elderly in Seoul. The incidence of demographic characteristics, physical changes brought about with ageing, health-related behavior, chronic diseases, and environmental characteristics on falls in the elderly was analyzed. Hierarchical Logistic Regression analysis was also conducted to investigate the incidence by life-cycle (early-mid-late stage) and the results were used for predicting falls. Results: The incidence of risk factors on falls varied across life-cycle. Among the elderly in the early (ages from 65 through 74), marital status, the existence of a companion, the quality of sleep, the chronic diseases, and the condition of the bathroom floor were found to be related to falls. Among those in the mid-stage (ages from 75 through 84), sex, marital status, the existence of a companion, hearing capacity, sense of balancing, chronic diseases, the exclusive use of rooms, and the side bar with bath tub were found to affect falls. Finally, for the elderly in their late stage (85 years of age and older), drinking and vision were found to be related to falls. Conclusion: The incidence of risk factors on falls was found to vary according the stage in the elderly. A key implication of this finding is that falls prevention programs and interventions must be catered to specific age sub-groups.

Effect of Sexual Attitude of Aged People on Sexual Life: Moderating Effect of General Characteristics (노인의 성태도가 성생활에 미치는 영향: 일반적 특성의 조절효과)

  • Purpose: The purpose of this study was to analyze the effect of sexual attitude among the elderly to their sexual life while exploring the moderating effect of general characteristics in the relationship between sexual attitude and sexual life. Methods: A descriptive research design was used. The subjects were 322 married elderly from four silver centers in Seoul and Gangwon-do. Aging Sexuality Attitude Scale developed by White (1982) was used to examine sexual life status and general characteristics of the subjects (gender, age, academic background, religion, economic status, and residential type). For data analysis, ${\chi}^2$-test, t-test, and ANOVA were conducted. Since the dependent variable of sexual life is a binary variable, hierarchical logistic regression was implemented. Results: There were 180 people (55.9%) who had a sexual life. Depending on gender (${\chi}^2$=35.03, p<.001), academic background (${\chi}^2$=29.45, p<.001), and economic status (${\chi}^2$=7.36, p=.025), the subjects showed significant difference in terms of sexual life. Regarding sexual attitude, the subjects showed significant difference depending on gender (t=-5.47, p<.001), age (t=-2.31, p=.021), academic background (t=5.16, p<.001), economic status (F=3.17, p=.043), and sexual life status (t=4.99, p<.001). Conclusion: It is important to devise the sex education program in consideration of gender, age, and academic background that showed moderating effects.

Effects of Job Stress and Supervisory Support on Depression of Care Givers in Elderly Care Facilities (직무 스트레스와 직장 상사의 지지가 요양보호사의 우울감 경험에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effects of job stress and support from supervisors on depression of care givers in elderly care facilities. Methods: Subjects were 191 care givers working in elderly care facilities in K-Province in Korea. Data were collected from self-administered questionnaires. Descriptive statistics, ${\chi}^2$ test, t-test, F-test, Pearson's and Spearman's correlation coefficients, and hierarchical multiple logistic regression with dummy variables were used for statistical analysis of data. Results: Job stress measured with Korean Occupational Stress Scale was $56.16{\pm}5.88$. Emotional and instrumental support from supervisors were $37.05{\pm}7.30$ and $32.39{\pm}7.34$, respectively. After controlling for general characteristics, job stress was significantly related to depression (OR: 1.07, 95% CI: 1.01~1.14). However, job stress became non-significant in the model that included instrumental support from supervisors. Not emotional but instrumental support from supervisors was significantly related to depression of care givers (OR: 0.93, 95% CI: 0.88~0.99). Conclusion: Results of this study suggest that job stress has negative effects and instrumental support from supervisors has protective effects on depression of care givers in elderly care facilities. Therefore, measures to increase instrumental support by supervisors should be developed to improve psychological health of care givers in elderly care facilities.

Suicide Attempt Behavior among Secondary School Students in Peru through PRECEDE Model (PRECEDE 모형을 통한 페루 중・고등학교 학생들의 자살시도 경험요인)

  • Objectives: There is higher rates of experience of suicide ideation and suicide attempt of adolescents in the Lima and Callao region compared with the results of the epidemiological study (2013) conducted by Peru National Institute of Mental Health and the Global School-based Student Health Survey (2010) which is was conducted among adolescents across all regions of Peru. Based on the PRECEDE model, this study analyzed the prevalence and the factor associated with the suicide attempt among the adolescent in a poor urban area in Peru. Methods: A stratified random sampling was used for the survey to select study participants from the 6 secondary schools based in Comas or Callao. The survey was conducted November 25th and December 4th in 2015 and a total of 738 individuals included in the analysis. In order to analyze factors influencing suicide attempt, comparison of predisposing factor, reinforcing factor, enabling factor, behavioral factor, environmental factor, psychological factors by suicide attempt and suicide risk group were identified through the chi-square test and hierarchical logistic regression. Results: Results showed that those who reported having less parental understanding, less time spent with parents, and 'almost none/none' for parental affection had more experience of suicide attempt and were more likely to sort into the high suicide risk group. Also, a greater proportion of those with the experience of suicide attempt had more experience of smoking and alcohol consumption, and experience of physical abuse and feeling insulted and depression in comparison to that of those without experience of suicide attempt. Variables that had a significant effect on suicide attempt included depression, subjective happiness, smoking experience, sexual intercourse, involvement in fight, parental affection, and gender (male). Conclusions: The results of the current study can serve as grounds for the necessity of acknowledging that adolescent suicide does not simply depend on a couple of factors, but arises from situations in which individual, home, school, social factors influence one another, and therefore adolescent suicide should be prevented and addressed through a multi-dimensional and integrated approach.

Association between Perceived Oral Health and Quality of Life in Korea Older Population (한국 노인의 주관적 구강건강평가와 삶의 질 관련성 연구)

  • The aim of this study was to investigate the association between self-rated oral health, self-rated health, and quality of life (QOL) among older population from the 7th Korea National Health and Nutrition Examination Survey (KNHANES VII-1, 2016). Data of 1,866 older adults (Mean age 69.5 years) were analyzed with hierarchical logistic regression analyses with IBM SPSS 23.0. Over 87.7% of the subjects rated their oral health as fair or poor. Women and less educated participants more likely to report their perceived oral health as poor (p<.05). Older participants who rated their general health positively were more likely to rate their oral health as good (F=19.04, p<.001). Elders who had bad perceived health (OR: 2.86, 95% CI: 1.5~5.5), had carries in permanent teeth (OR: 2.68, 95% CI: 1.67~4.32) and anxiety or depression (OR: 1.45, 95% CI: 1.42~2.57) had negative oral health perception after controlling for covariates. In conclusion, perceived oral health and QOL were associated with each other in Korean older adults. Therefore, it is recommended to approach holistic strategy for improve health and quality of life in the elderly population.
