Purpose : Generally dermatitis is treated by corticosteroids, antihistamines ect, but the use of these drugs may affect pregnant women adversely. In this case Oriental medical therapy can be very useful. The purpose of this study is to report clinical management of dermatitis patients during pregnancy by oriental medicine including Hwa-acupuncture and herbal-combined extraction. Methods : We experienced two cases of dermatitis patient during pregnancy. In the beginning of treatment, they had severe itching, burning sensation, bulls, papule. We teated them with Hwa-acupuncture, herbal-combined extraction. Results : After quite short oriental treatment including Hwa-acupuncture and herbal-combined extraction, the symptoms of patients are remarkable alleviated. Conclusion : We think that many case of dermatitis during pregnancy can be managed effectively by Hwa-acupuncture and herbal-combined extraction. If we make more studies about disease under pregnancy condition, it will bring the high satisfaction of patient and the elevation of treatment rate.

Objectives : This study aims to evaluate the add-on effect of herbal bath combined with acitretin in patients with psoriasis. Methods : We searched randomized controlled trials(RCTs) reporting the effect of herbal bath and acitretin combined treatment for psoriasis in 10 electronic databases from their inception to January 2023. Study collection, data extraction, and risk of bias assessment were performed by two independent researchers. Data synthesis and risk of bias evaluation was conducted using Revman 5.4. Results : Ten RCTs with 1,008 subjects were included. Herbal bath and acitretin combined treatment group showed higher average total effective rate than acitretin alone group(RR 1.39; 95% CI: 1.15-1.68, P=0.0006, I2 =0%). Herbal bath and additional treatments such as phototherapy(NB-UVB) and oral herbal medicine, combined with acitretin group also showed higher average total effective rate than acitretin alone group(RR 1.81; 95% CI: 1.47-2.22, P<0.00001, I2 =43%). Herbal bath and phototherapy(NB-UVB) combined with acitretin group also showed higher average total effective rate than acitretin alone group(RR 1.51; 95% CI: 1.18-1.93, P=0.0009, I2 =0%). The overall risk of bias of the included studies was unclear or of high risks. Conclusions : This review showed that the add-on of herbal bath combined with acitretin treatment could be more effective than acitretin alone treatment in patients with psoriasis. However, because of few included studies, heterogeneity between studies, and insufficient quality of included studies, further well-designed RCTs are needed to confirm the add-on effect of herbal bath on psoriasis.

Background: The study was conducted to elucidate the extraction conditions under which white ginseng has cognition-improving efficacy. Methods and Results: Extracts from white ginseng under different solvent and temperature conditions were analyzed for ginsenoside content and inhibitory effect on N-methyl-D-aspartate (NMDA) receptor and acetylcholinesterase. The total ginsenoside contents and amounts of ginsenoside Rb1 plus ginsenoside Rg1 from the 1st extracts (prepared with EtOH/$H_2O$ as solvent) were higher than those from the 2nd extracts (extracted with $H_2O$ after the 1st EtOH/$H_2O$ extraction). The contents in the 1st and 2nd extracts produced at $80^{\circ}C$ were also higher than those obtained at $50^{\circ}C$. Samples from the 1st extraction at $80^{\circ}C$ indicated higher inhibitory activities on NMDA receptors-whose excessive activation is thought to mediate the calcium-dependent neurotoxicity associated with several neurodegenerative diseases-than those from the 2nd extraction. Among the samples prepared at varying temperatures, the extract prepared at $50^{\circ}C$ showed the highest suppression activity on NMDA receptors. Note, however, that the extracts from the 2nd extraction at $50^{\circ}C$ inhibited acetylcholinesterase-whose inhibition could be a therapeutic strategy for neurodegenerative diseases with cognitive deficits and memory malfunction-more effectively than those from the 1st extraction. Conclusions: To enhance the cognition-improving activity of white ginseng extract, it is suggested that the extracts be utilized after being combined the 1st extracts (made with EtOH/$H_2O$ solvent) and the 2nd extracts (prepared with $H_2O$) at low temperature.
Objectives : When herbal medicines are extracted together, they may interact with each other, leading to change of chemical characteristics. This study aimed to evaluate the influence of Atractylodes rhizomes (Atractylodes japonica, A. macrocephala, and A. chinensis) on the chemical features of the roots and rhizomes of Glycyrrhiza uralensis, which is are commonly combined with herbal medicines in many herbal formulae, when they are co-decocted. Methods : Liquiritin apioside, liquiritin, ononin, and glycyrrhizin levels of G. uralensis in hot-water extracts prepared by the combination of Atractylodes rhizomes with various weight ratios (G. uralensis : Atractylodes rhizomes = 10:0, 10:5, 10:10, and 10:20) and extraction times (60, 90, and 120 min) were quantified using a HPLC-diode array detector and compared by statistical analysis. Results : The concentrations of liquiritin apioside, liquiritin, ononin, and glycyrrhizin from G. uralensis roots and rhizomes mostly reduced when co-extracted with Atractylodes rhizomes, and the addition of A. chinensis most reduced their contents between Atractylodes combination groups. A. japonica and A. macrocephala rhizomes also showed differences of liquiritin and glycyrrhizin levels at 10 g and 20 g groups of Atractylodes rhizomes. Extraction times also affected the concentrations of liquiritin, ononin, and glycyrrhizin mostly during 60 and 90 min. Conclusions : Atractylodes rhizomes might alter the chemical characteristics of G. uralensis when these herbs are co-decocted. This study provides the understanding of the chemical interactions of herbal medicines during the extraction in hot water.
Objectives : The biological activities and compound contents of herbal medicine vary depending on manufacturing processes. In this study, we compared anti-inflammatory effects and compound contents of three kinds of multi-herbal extract HT008 produced by different manufacturing processes in order to determine chemical and biological equivalence. Methods : HT008 was produced by three different manufacturing methods: 1. Freeze dried extract of Eleutherococcus senticosus, Scutellaria baicalensis and Angelica sinensis (HT008 FD), 2. Spray dried extract of E. senticosus and S. baicalensis combined with reflux extract of A. sinensis (HT008 SD), 3. Spray dried extract of E. senticosus and S. baicalensis combined with supercritial fluid extract of A. sinensis (HT008 SF). Anti-inflammatory effects were evaluated using acetic acid induced pain model and ${\lambda}$-carageenan induced paw edema model. Compound contents were evaluated by HPLC quantitative analysis of standard compounds of HT008, eleutheroside E, baicalin, z-ligustilide. Results : HT008 FD, HT008 SD and HT008 SF significantly decreased acetic acid induced pain index and ${\lambda}$-carrageenan induced paw edema volume compared with that of control group. There was no significant difference in efficacy among the HT008 FD, HT008 SD and HT008 SF. Standard compound contents of HT008 FD, HT008 SD and HT008 SF were quantified within the range of Korean pharmacopoeia or other research. Conclusions : Three different manufacturing methods of multi-herbal extracts have been developed without noticeable difference in the efficacy or compound contents. The results might be used to establish manufacturing process and industrialization of herbal extracts.

Objectives : This review was conducted to validate the effectiveness and safety of herbal medicine combined with conventional therapy for rosacea. Methods : Randomized controlled trials(RCTs) reporting the effects of herbal medicine treatment on rosacea were searched through eight electronic databases from 2016 to March 17, 2020. This study collection and data extraction were performed by two independent reviews. The Cochrane risk-of-bias tool was used for the evaluation of the risk of bias in all included RCTs. Mean differences(MD) and Risk ratio(RR) of 95% Confidence intervals(Cls) were calculated and data synthesis was conducted using Review Manager(RevMan, ver.5.4) Results : Eighteen RCTs were included and all trials compared the combined therapy of herbal medicine with conventional western therapy to conventional therapy alone. The effective rate of the combination of herbal medicine with western medicine(RR 1.20, 95% CI : 1.13-1.28, p<0.00001, I2=0%), the effective rate of the combination of herbal medicine with laser-based therapy(RR 1.12, 95% CI : 1.04-1.21, p=0.004, I2=18%) and the effective rate of the combination treatment group using herbal medicine, western medicine and external drugs were all statistically higher that of the control group(RR 1.19, 95% CI : 1.11-1.28, p<0.00001, I2=0%). The score of non transient erythema(MD -0.36, 95% CI : -1.01 0.29, p=0.27, I2=93%), flushing(MD -0.69, 95% CI : -0.97, 0.41, p<0.00001, I2=32%), papules or pustules(MD 0.10, 95% CI : -0.15, 0.35 p=0.44, I2=0%) were also seen in the herbal medicine and western medicine combination group. The overall risk of bias of the included studies was some concerns. No serious adverse effects were observed. Conclusions : This review found the safety and effectiveness of the combined therapy of herbal medicine with conventional western therapy for rosacea.

Sipyukmiryuki-eum (SYMYKE), Danjacheongpi-tang (DJCPT), Jipae-san Ⅰ (JPS Ⅰ), Jipae-san Ⅱ (JPS Ⅱ), and Chungganhaeul-tang (CGHUT) are representative herb-combined remedies used in traditional Korean medicine for treating breast cancer patients, as mentioned in "Dongeuibogam." In this study, we investigated the total phenolic contents (TPCs) and the anti-oxidant and anti-microbial activities of hot water and 70% ethanol extracts of these herbal prescriptions. Among the five herb-combined remedies, the extraction yields of the hot water extracts and 70% ethanol extracts were the highest in JPS Ⅱ (34.30%) and DJCPT (30.50%), respectively. The TPCs of the hot water extracts from the herb medicines were rich in the order of JPS Ⅰ < CGHUT < JPS Ⅱ < SYMYKE < DJCPT. In addition, the 70% ethanol extracts from the herb medicines were rich in the order of JPS Ⅱ < JPS Ⅰ < CGHUT < SYMYKE < DJCPT. Among them, DJCPT and SYMYKE displayed a strong anti-oxidant capability, which was determined using ferric-reducing anti-oxidant power and scavenging of 2,2'-diphenyl-1-picrylhydrazyl and 2,2'-azinobis(3-ethylbenzothiazoline-6-sulfonate) cationic radical activity assays. In addition, anti-microbial activities against Staphylococcus aureus and Escherichia coli were stronger in the 70% ethanol extracts than in the hot water extracts. Together, these findings reveal a positive relationship between TPCs and their anti-oxidant activities.

Here we reported a methodology for identification of triacylglycerols (TAGs) and diacylglycerols (DAGs) in coix seed by preparative thin layer chromatography (prep-TLC) and non-aqueous reversed-phase liquid chromatography (NARP LC)-atmospheric pressure chemical ionization (APCI) tandem mass spectrometry (MS/MS). Lipid components were extracted from coix seed by reflux extraction using n-hexane for 3 hr. TAGs and DAGs in coix seed extract were effectively purified and isolated from matrix interferences by prep-TLC and then analyzed by LC-APCI-MS and MS/MS for identification. TAGs were effectively identified taking into consideration of their LC retention behavior, APCI-MS spectra patterns, and MS/MS spectra of $[DAG]^+$ ions. In MS/MS spectra of TAGs, diacylglycerol-like fragment $[DAG]^+$ ions were useful to identify TAGs with isobaric fragment ions. Based on an established method, 27 TAGs and 8 DAGs were identified in coix seed extract. Among them, 15 TAGs and 8 DAGs were for the first time observed in coix seed. Interestingly, some of TAGs isolated by prep-TLC were partly converted into DAGs through probably photolysis process during storing in room temperature. Thus, degradation phenomenon of TAGs should be considered in the quality evaluation and nutritional property of coix seed. LC-APCI-MS/MS combined with prep-TLC will be practical method for precise TAG and DAG analysis of other herbal plants.
Objectives : This study focused on Schisandrae Fructus (SF) and Perilae Folium (PF), traditional medicine herbs and health functional food in Korea, Japan and China. We investigated various pharmacological activities that include a potential source of free radical scavenging, anti-viral, anti-microbial, anti-allergy and anti-inflammatory activities. Methods : We conducted an investigation of total contents of phenolic and flavonoid compounds in these single herbal extraction with/without combined to mixture. We also measured antioxidant activities such as DPPH free radical scavenging, SOD-like scavenging, nitrite scavenging and hydroxyl radical scavenging activities, xanthine oxidase inhibition, linoleic acid peroxidation inhibition, and reducing power. Results : As the results, contents of total phenoilc compounds and flavonoids were higher in those of PF than those of SF. Those of SF+PF mixture showed the synergy effects compared with those of SF and PF single extractions. Activities of DPPH free radical and SOD-like scavenging in 1 mg/mL concentration increased in dose dependent manners. That of SF increased compared with that of PF. That of SF and PF mixture also increased compared with that of BHA as a positive control. The other antioxident activities also showed similar to patten of activity of DPPH free radical scavenging. When combined to SF and PF extractions, there was showed synergic effect compared with those of BHA, excepted activities of xanthine oxidase inhibition and reducing power. Taken together, SF and PF have high phenolic and flavonoid compounds content furthermore, antioxidant activities in SF and PF mixture showed more synergy effect compared with those of BHA. Conclusions : Therefore, these findings suggest that SF and PF mixture may offer functional materials potential for development of functional beverage. But further studies are needed for the identification of the active compounds.
