Case study of HBV-related Disasters in a High-risk Family

  • Hepatitis B virus (HBV) is one of the most common intracellular parasites, of which 350 million people worldwide are chronic carriers. It also related to a high incidence of hepatocellular carcinoma. In general, it has been well known that HBV is a noncytolytic virus, so not the virus itself but any unfavorable response by host immune cells and inflammatory cytokines mainly result in chronic liver injury. From this viewpoint, we hopefully assume that Oriental therapies based on immunologic strategies may be able to provide a therapeutic alternative for caring for these illnesses. We also need to be thoroughly familiar with information about HBV epidemiology and the pathologic process of chronic HBV carriers. In this study, to clarify the important considerations of HBV infection and the high risk of HBV induced life-threatening diseases, we introduced our pilot practices given to the patients and the possibility of Oriental therapies as a novel strategy for chronic HBV carriers.

A Study on the Positive Rates of HBsAg and Anti-HBs from Old People's Home and Casual Ward in Daegu, Korea

  • Serum hepatitis is still recognized as a major public health problem in many countries. The most common etiologies are acute viral hepatitis A and B, and hepatitis B virus (HBV) was known as the pathogen of serum hepatitis. since the HBV causes chronic hepatitis. HBV infection is hyperendemic in Korea. It is known that 8% and 1% of the Korean population is chronic carriers of HBV. The total number of 487 serum specimen (old people's home 190, causal ward-Daegu hope village 297) were collected at the laboratory of Department of Clinical Pathology of Daegu medical center. The laboratory tested the specimen to detect for hepatitis B surface antigen (HBsAg) and anti-HBs using reverse passive hemag-glutination (RPHA) and passive hemagglutination (PHA) method between April, 2011 and June, 2011. In one-step test, HBsAg and anti-HBs of Humasis were used. The positive rate for HBsAg was 6.80% (33/487), and the rate in male was 6.93% (19/274) higher than that of female 6.57% (14/213). In the positive rate of HBsAg by age group, the highest positive rate group was in the group of 40-49 years in male (12.0%), and 50-59 years in female (17.65 %). The overall positive rate for anti-HBs was 42.70% (208/487), showing the higher positive rate of 43.80% in male (120/274) than that of 41.30% (88/213) in female. The highest positive rate of anti-HBs in age group was the 40-49 years group in male (52.00%) and the group of under 29 in female (66.67%).

B형 간염 바이러스 감염과 연관된 사구체신염의 임상병리학적 특성과 신 조직내에서 HBs Ag 과 HBe Ag의 발현 (Clinicopathologic Features and HBsAg and HBeAg Expressions in Hepatitis B Virus-associated Glomerulopathy)

  • 목적 : HBV 감염과 연관된 신병증의임상 및 검사소견과 병리학적 소견을 분석하여 그 특징을 규명하고, HBeAg과 HBsAg에 대한 면역조직화학염색을 신생검조직에서 시행하여 이들 항원의 사구체내에서의 면역학적 발현을 관찰하므로써 이 질환의 발생 기전에 대한 이해를 돕고자하였다. 방법 : 1990년 4월부터 1997년 2월까지 사구체신염으로 신조직 생검을 시행하여 부산백병원 해부병리과에 병리조직검사가 의뢰된 증례 중 혈청 HBsAg 양성인 28례를 대상으로 임상 및 검사소견을 조사하였고, 신조직의 광학현미경적, 면역형광학적, 전자현미경적 검사와 HBsAg과 HBeAg 항원에 대한 면역조직화학검사를 시행하였다. 결과 : 1. 연령 분포는 6세에서 73세였으며 소아 환자가 8명이었고 성인 환자는 20명 이었다. 평균 연령은 28세였고, 남녀 비는 3:1 이었다. 내원당시 67.9%에서 혈뇨, 75.0%에서 단백뇨를 보였고 57.2%에서 신증후군을 나타내었다. 전 예에서 혈청 HBsAg이 양성이었고, 혈청 HBeAg은 검사가 시행된 9례 중 6례 (66.6%)에서 양성이었다. 2. 간기능검사가 시행된 11례 중 7례 (63.6%)에서 AST와 ALT치가 증가되어 있었으며, 이중 간생검이 시행된 3례 모두 만성 활동성 간염으로 진단되었다. 3. 28례 중 막성 사구체 신염 (membranous glomerulonephritis: MGN)이 10례 (35.7%)로 가장 많았으며, 메산지움중식성 사구체신염 (mesangioproliferative glomerulonephritis: MesGN)이 8례 (28.6%), 막증식성 사구체 신염 (membranoproliferaive glomerulonephritis : MPGN)은 7례 (25.0%) 그리고 미세변화질환 (minimal change disease : MCD)가 3nl (10.7%)순이었다. 4. HBV와 연관된 MGN에서는 혈관간 세포의 증식 및 혈관간 기질의 확장이 자주 관찰되었으며, 사구체간질 내와 내피하 침윤도 동반되어 MPGN을 시사하는 소견이 함께 관찰되는 예가 많았고, HBV 항원 혈증에 동반된 MPGN에서는 내피하 침윤 뿐 아니라 기저막 내 및 상피세포하 침윤도 종종 관찰되었다. 5. 신조직의 면역조직화학검사상 HBsAg은 모두 음성이었으나 HBeAg은 28례 중 8례 (28.6%)에서 사구체 모세혈관벽을 따라 양성 반응을 나타내었고, 이 중 3례는 혈치 HBeAg 검사를 받았는데 모두 양성이었다. 결론 : HBV와 연관된 사구체신염은 다양한 병리학적 양상을 보여주며, 특히 MGN과 MPGN에서는 전자현미경소견상 두 질환의 특징이 중복되어 나타났고, 사구체내에서 HBeAg이 검출되어 HBV의 활동성 정도와 사구체신염의 발생이 연관성이 있을 것으로 사료되나 더 많은 연구보고가 있어야 하겠다.

Evaluation of Changes in Serum Thyroid Hormone Levels in Patients with Hepatitis B Infection

  • Purpose: We aimed to determine the differences in the levels of serum thyroid hormone (free T4 [FT4]) and thyroid stimulating hormone [TSH]) as biomarkers for hepatitis B virus (HBV) infection status, with respect to age and sex. Methods: We retrospectively analyzed serum samples from 200 patients who underwent HBV testing from August 2022 to September 2022. Serum samples were collected from patients suspected of having HBV infection who visited this hospital. Thyroid hormone levels were measured, and patients were grouped according to age and sex. Results: Differences in TSH and FT4 levels in the serum of patients in the HBV-positive and -negative groups were not significant. Among the HBV-positive patients in the younger age group (<60 years), TSH and FT4 levels were 1.78 ± 0.09 µIU/mL (normal: 0.4-5.0 µIU/mL) and 1.24 ± 0.02 ng/mL (normal: 0.8-1.9 ng/mL), respectively, whereas among the HBV-positive patients in the older age group (≥60 years), TSH and FT4 levels were 2.22 ± 0.17 µIU/mL and 1.24 ± 0.07 ng/mL, respectively. Conclusions: The presence of HBV did not markedly affect serum thyroid hormone levels. Our findings shed light on the conflicting evidence on the association between thyroid hormone levels and HBV infection. 농촌지역의 B형, C형 간염에 관한 혈청역학적 연구 (Seroepidemiologic Study of Hepatitis B Virus (HBV) and Hepatitis C Virus (HCV) Infection among Koreans in Rural Area, Korea)

  • To estimate the prevalence of hepatitis B virus (HBV) and hepatitis C virus (HCV) infection and to determine associated risk factors, a population-based seroepidemiologic study was carried out. In 1993, a health examination survey of the population was carried out in rural area known to have a high incidence of liver cancer. The study population were those who volunteered to participate in a health survey over 10 years of age. Examinees were interviewed by specially trained staffs. Sera from 1,033 study subjects were tested for hepatitis B surface antigen (HBsAg) by .everse passive hemagglutinin (RPHA) estimation and for hepatitis C virus antibody (anti-HCV) by 2nd generation passive hemagglutinin (PHA) estimation. The age and sex standardized prevalence of HBsAg was 6.3% which was similar to national average, but that of anti-HCV was 5.1% which was 4 to 5 times higher than that of blood donors or other health examinees in Korea. In a multivariate analysis, transfusion history, surgical operative history, and acupuncture history were not associated with HBsAg positivity. In contrast, acupuncture history (adjusted odds ratio[OR]=2.2 : 95% Confidence interval[CI] 1.0-4.7) and surgical operative history(adjusted OR=2.0 : 95% CI 1.0-4.1) were associated with anti-HCV positivity. The present study suggest that there is an highly endemic area of HCV infection in Korea and probably this endemicity is associated with a parenteral source of HCV infection other than blood transfusion.

B형 간염 바이러스의 ntC1731T 및 G1806A의 core 프로모터 돌연변이에 의한 HBV 유전자 발현 증가 분석 (Core Promoter Mutation of ntC1731T and G1806A of Hepatitis B Virus Increases HBV Gene Expression)

  • B형간염바이러스(HBV)의 만성 감염은 간경화와 간세포암 발생 빈도를 현저히 높인다. HBV 감염의 임상적 결과는 숙주 유전적 요인과 바이러스의 유전자 변이, 그리고 환경적 요인 등에 결정된다. HBV 복제를 위한 HBV의 pre-genomic RNA 전사는 바이러스의 core promoter 활성화에 의해 조절된다. Core promoter 돌연변이는 급성간 질환과 간세포암 발생에 연관되어 있다. 본 연구팀은 미얀마의 HBV 감염 환자들로부터 바이러스 유전자를 획득하여 core promoter 부위의 유전자 변이들을 파악하였다. Core promoter의 상대적 유전자 활성 차이를 분석하기 위해서 core promoter를 luciferase reporter에 재조합한 시스템을 제작하였다. 분석한 core promoter의 유전자 변이들 중에서 C1731T와 G1806A 돌연변이가 HBV core promoter의 전사 활성화를 증가에 관여하였다. 돌연변이 부위를 중심으로 전사 인자들의 가능한 결합 부위 변화를 컴퓨터 프로그램 분석을 통해 조사한 결과, C/EBPβ와 XBP1 반응 부위가 새롭게 생성되었음을 도출하였다. C/EBPβ의 세포 내 발현은 C1713T 돌연변이를 가진 core promoter의 전사 활성을 증가시켰으며, XBP1 발현은 G1806A 돌연변이를 함유한 M95 promoter를 활성을 증가시켰다. HBV 감염의 치료는 약제 내성과 백신 회피 돌연변이 발생으로 문제점을 가지고 있는 상황에서, 이 연구 결과는 HBV core promoter 돌연변이의 분자생물학적 그리고 임상학적 중요성을 제공한다.

Overexpression and Purification of PreS Region of Hepatitis B Virus Antigenic Surface Protein adr Subtype in Escherichia coli

  • PreS domain of Hepatitis B virus (HBV) surface antigen is a good candidate for an effective vaccine as it activates both B and T cells besides binding to hepatocytes. This report deals with overexpression and purification of adr subtype of surface antigen that is more prevalent in Pakistan. PreS region, comprising 119 aa preS1 region plus a 55 aa preS2 region plus 11 aa from the N-terminal S region, was inserted in pET21a+ vector, cloned in E. coli $DH5\alpha$ cells and expressed in E. coli BL21 codon+ cells. The conditions for over expression were optimized using different concentrations of IPTG (0.01-5 mM), and incubating the cells at different temperatures (23-$41^{\circ}C$) for different durations (0-6 h). The cells were grown under the given optimized conditions (0.5 mM IPTG concentration at $37^{\circ}C$ for 4 h), lysed by sonication and the protein was purified by ion exchange chromatography. On the average, 24.5 mg of recombinant protein was purified per liter of culture. The purified protein was later lyophilized and stored at $-80^{\circ}C$.

만성 바이러스성 B형 간염 환자에서 HBeAg이 혈청전환된 1례 (A Case Report of HBeAg Seroconversion in Chronic Viral Hepatitis B)

  • Objectives: The purpose of this case study is to report the efficacy and safety of treatment with Korean medicine of a patient with HBeAg-positive chronic viral hepatitis B.Methods: The patient took Korean medicine (mainly Injinchunggan-tang-gamibang) from July 20th, 2010, to March 14th, 2016, without any antiviral or interferon therapy. Changes to laboratory records, abdomen ultrasonography, and clinical symptoms were reviewed.Results: The laboratory records showed that AST, ALT, and HBV DNA had decreased to normal ranges, and HBeAg showed seroconversion. Clinical symptoms also improved after taking Korean medicine.Conclusion: The results suggest that treatment with Korean medicine and without antiviral or interferon therapy could be effective for HBeAg-positive chronic hepatitis B.

가족집적성을 보이는 B형간염 바이러스 만성보유자에서 바이러스 유전자의 돌연변이와 주조직접합체 양상 - 질병발현 형태와의 관련성을 중심으로 - (Hepatitis B Virus DNA Mutation, Pattern of Major Histocompatibility Class-I among Familial Clustered HBV Carriers in Relation to Disease Progression)

  • Objectives : Chronic HBsAg carriers are the principal source of infection for other susceptible people, and are themselves at high risk of developing serious liver diseases. In Korea, it has been estimated that 65-75% of the HBsAg positives remained as persistent carriers. Additionally, familial clustering of MBV infection has frequently been observed among carriers. Some would become progressive, chronic hepatitis patients, and others would not. The aim of this study was to evaluate the association between various factors, such as the duration of infection, type of virus, mutation of precore/core region in HBV, major histocompatibility class-I, and developing chronic liver diseases among familial HBV carriers. Methods : Chronic carrier status was identified by repeated serological tests for HBsAg at intervals of six months or more. A familial chronic carrier was defined when the disease was observed in a family member over two generations. Two families were recruited, among which a total of 20 chronic HBsAg carriers(11 carriers in No.1, and 9 in No.2 family) were identified. Data on the general characteristics and liver disease status were collected. Identification of the HBV-DNA was successful only for 13 subjects among the 20 carriers. Analysis of viral DNA in terms of subtype, pre-core and core region mutations was carried out. The type of major histocompatibility class-1 for the 13 subjects was also analysed. Results & Conclusions : Seven of 10 chronic HBV carriers of the 1st generation and one of 10 of the 2nd generation were clinical patients with chronic hepatitis, the others, three of the 1 st and nine of the 2nd generation, were asymptomatic carriers. This data indicates that the duration of HBV carriage is one of the major factors for disease severity. The subtype of HBsAg analysed using MBV-DNA identified in 13 carriers were adr, and the pattern of precore nonsense mutation in HBV-DNA was identical among family members, which meads that the same virus strains were transmitted between the family members. The association between the precore or core mutations in HBV-DNA and the disease severity was not observed. While it was suggested that a specific type of MHC class-I may be related to disease progression.
