Moyamoya 질환에서 1차 통과기법을 이용한 자기공명관류영상의 이해 (Understanding on MR Perfusion Imaging Using First Pass Technique in Moyamoya Diseases)

  • The purpose of this study was to investigated the usefulness of MR perfusion image comparing with SPECT image. A total of pediatric 30 patients(average age : 7.8) with Moyamoya disease were performed MR Perfusion with 32 channel body coil at 3T from March 01, 2010 to June 10, 2010. The MRI sequences and parameters were as followed : gradient Echo-planar imaging(EPI), TR/TE : 2000ms/50ms, FA : $90^{\circ}$, FOV : $240{\times}240$, Matrix : $128{\times}128$, Thickness : 5mm, Gap : 1.5mm. Images were obtained contrast agent administrated at a rate of 1mL/sec after scan start 10s with a total of slice 1000 images(50 phase/1 slice). It was measured with visual color image and digitize data using MRDx software(IDL version 6.2) and also, it was compared of measurement with values of normal and abnormal ratio to analyze hemodynamic change, and a comparison between perfusion MR with technique using Warm Color at SPECT examination. On MR perfusion examination, the color images from abnormal region to the red collar with rCBV(relative cerebral blood volume) and rCBF(relative cerebral blood flow) caused by increase cerebral blood flow with brain vascular occlusion in surrounding collateral circulation advancement, the blood speed relatively was depicted slowly with blue in MTT(Mean Transit Time) and TTP(Time to Peak) images. The region which was visible abnormally from MR perfusion examination visually were detected as comparison with the same SPECT examination region, would be able to confirm the identical results in MMD(Moyamoya disease)judgments. Hymo-dynamic change in MR perfusion examination produced by increase and delay cerebral blood flow. This change with digitize data and being color imaging makes enable to distinguish between normal and abnormal area. Relatively, MR perfusion examination compared with SPECT examination could bring an excellent image with spatial resolution without radiation expose.

흰쥐의 적출된 심장에서 Verapamil이 허혈성 심근에 미치는 효과 (Effects of Verapamil in Cardioplegic Perfusates on the Ischemic Myocardium in Isolated Rat Heart)

  • 저자들은 흰쥐의 적출된 심장을 이용하여 Verapamil이 허혈성 심근에 대하여 심근보호효과가 있는지를 관찰하였다 흰쥐의 적출된 심장은 허혈상태에서 심근온도가 $25^{\circ}C$가 되게 유지하였다. 24마리의 흰쥐로부터 적출된 심장을 Krebs-Henseleit완충액을 사용하여 30분간의 비작업성 역관류로 안정시킨 후 $25^{\circ}C$의 심정지액 (St. Thomas' Hospital Cardioplegic Solution)에 60분 동안 저장하였다. 허혈성 심정지를 유도하기 전에 적출된 심장을 저온의 심정지액으로 처리한 군을 대조군(n=12)으로 하고, Verapamil 이 첨가된 저온의 심정지액으로 처리한 군을 실험군(n=12)으로 하였다. 60분 동안의 허혈성 심정지후 심정지전에 측정했던 혈역학적 및 생화학적 지표인 심박동수, 좌심실압, +dpfdt max, 관상관류량과 CPK치를 재관류후 30분에 재측정하여 심정지전과 비교하여 심기능 회복 정도를 관찰하였다. Verapamil이 첨가된 저온의 심정지액을 사용한 실험군이 심박동수, 좌심실압, +dp/dt max, 관상관류량과 CPK치에서 대조군에 비하여 유의하게 높은 회복율을 나타내었다(p <0.05).

적출심장의 장시간 [24시간] 보존에 관한 실험적 연구 -4$^{\circ}C$ 관류 보존법- (Experimental Study of Isolated Canine Heart Preservation for 24 Hours at 4$^{\circ}C$ - A Portable Continuous Hypothermic Perfusion System -)

  • After 24 hours of preservation under 15 mmHg perfusion pressure the recovery rates of isolated canine hearts were determined. Preservation was performed in a cold room maintained at 4*C with 4 different types of perfusates bubbled with a mixture of 95% 0y and 5% CO~ using a modified perfusion unit designed in our institute. The perfusates used were as follows; Group 1: Krebs-Henseleit solution, Group 2: Krebs solution added by albumin and PGE1. Group 3: Modified Wicomb*s solution, Group 4: Modified Collin*s solution. The extent of myocardial recovery was evaluated using a modified isolated carmine perfusion model by measuring heart rate, systolic arterial pressure, left atrial pressure[LAP] and cardiac output. In addition to the above hemodynamic parameters, biochemical and enzymatic assays from perfusates and electron microscopic changes of the myocardium were also studied. The results were as follows; 1] The heart recovery rates were 41.6%, 53.4% and 108.9% in groups 1, 2 and 3, respectively, and group 3 elicited the best result[p< 0.001]. The heart beat was never recovered in group 4. 2] Recovered systolic arterial pressures[mmHg] were 63.3% in group 1, 94.9% in group 2 and 94.3% in group 3. 3] LAPs[mmHg] were 20 in group 1, 13.5 in group 2 and 11.2 in group 3, which suggested that the best myocardial preservation was elicited in group 3[p< 0.05]. 4] Cardiac output, the sum of aortic stroke volume and coronary leakage, were 69.1% in group 2, and 90.7% in group 3, but these were not statistically significant[p=0.24]. No aortic stroke output was measured in group 1 and 4. 5] The degree of myocardial edema increase was 17.5` in group 1, 24.6% in group 2, 20.9% in group 3 and 55.3% in group 4. But there were no statistical differences in each group[p= 0.08]. 6] CPK-MB[U/L] levels were increased 750% and 332%[p< 0.05], glucose levels[mg/dl] 60.5% and 78.2% and SGOT[U/L] levels 523% and 333%, in groups 2 and 3, respectively. Biochemical and enzymatic assays could not be performed in group 1 and group 4, because of poor recovery of heart beat. 7] Electron microscopic findings in the myocardium of most groups revealed slight to moderate muscle cell and mitochondrial edema. But all these findings were within the limits of reversible change. From these above results, it is suggested that modified Wicomb*s solution seems to be the most useful physiologic salt solution for preservation of the heart. We propose that after further study and improvement, our portable continuous hypothermic perfusion system will contribute to the development of a better preservation method for donor hearts for human heart transplantation.

임파유통에 영향을 주는 인자에 관한 실험적 연구 (Various Factors Influencing Thoracic Duct Lymph Flow in the Dogs)

  • Various factors influencing the lymph flow from thoracic duct were investigated in an attempt to evaluate their contributing degree and the mechanisms. Sixteen mongrel dogs weighing between 10 and 16 kg were anesthetized and polyethylene catheters were inserted into the thoracic duct and femoral veins. Arterial blood pressure, heart rate, central venous pressure, lymph pressure and lymph flow were measured under various conditions. Electrical stimulation of left sciatic nerve, stepwise increase of central venous pressure, manual application of rhythmical depressions onto abdomen, injection of hypertonic saline solution and histamine infusion were employed. Measurement of cental venous pressure was performed through the recording catheter inserted into abdominal inferior vena cava. Changes in central venous pressure were made by an air-ballooning catheter located higher than the tip of the recording catheter in the inferior vena cava. Lymph flow from thoracic duct was measured directly with a graduated centrifuge tube allowing the lymph to flow freely outward through the inserted cannula. The average side pressure of thoracic lymph was $1.1\;cmH_2O$ and lymph flow was 0.40 ml/min or 1.9 ml/kg-hr. Hemodynamic parameters including lymph flow were measure immediately before and after (or during) applying a condition. Stimulation of left sciatic nerve with a square wave (5/sec, 2 msec, 10V) caused the lymph flow to increase 1.4 times. The pattern of lymph flow from thoracic duct was not continuous throughout the respiratory cycle, but was continuous only during Inspiration. Slow and deep respiration appeared to increase the lymph flow than a rapid and shallow respiration. Relationship between central venous pressure and the lymph flow revealed a relatively direct proportionality; Regression equation was Lymph Flow (ml/kg-hr)=0.09 CVP$(cmH_2O)$+0.55, r=0.67. Manual depressions onto the abdomen in accordance with the respiratory cycle caused the lymph flow to increase most remarkably, e.g,. 5.5 times. The application of manual depressions showed a fluctuation of central venous pressure superimposed on the respiratory fluctuation. Hypertonic saline solution (2% NaCl) administered Intravenously by the amount of 10 m1/kg increased the lymph flow 4.6 times. The injection also increased arterial blood pressure, especially systolic Pressure, and the central venous pressure. Slow intravenous infusion of histamine with a rate of 14-32 ${\mu}g/min$ resulted in a remarkable increase in the lymph flow (4.7 times), in spite of much decrease in the blood pressure and a slight decrease in the central venous pressure.

건강한 청년 남자 14 례에서 Dipyridamole 부하 $^{99m}Tc$-MIBI 심근 SPECT 극성지도의 분석 (Analysis of Polar Maps of Dipyridamole Stress/Rest $^{99m}Tc$-MIBI Myocardial SPECT in 14 Healthy Young Men)

  • 저자들은 14명의 관상동맥질환이 의심되지 않는 건강한 젊은 남자를 대상으로 dipyridamole 부하 $^{99m}Tc$-MIBI 심근 SPECT를 시행하였다. 이를 이용하여 Cedars-Sinai방법으로 극성지도를 구하고 전체 심근을 9부분으로 나누어 각 부위의 상대적인 방사능 세기를 조사하고 분석하였다. 이와 함께 dipyridamole 부하 전후의 혈역동학적인 변화와 부작용을 알아보았다. 극성지도에서 부하기와 휴식기 모두 방사능 계수치가 가장 높은 부위는 Latero-anterior wall이었고, 가장 낮은 부위는 Infero-septal wall이었다. 부하기와 휴식기사이의 각 부위의 방사능세기는 서로간에 유의한 차이가 없었다. dipyridamole 투여후 심박수와 이완기 혈압 및 Double product는 유의한 변화를 보였다. dipyridamole에 의한 부작용은 14명 중 12명에서 나타나 비교적 높은 비율을 보였으나 중한 부작용은 관찰되지 않았다. 이상의 결과로 $^{99m}Tc$-MIBI 심근 SPECT에서 구한 극성지도에서 관상동맥질환을 판정할 때는 부위별로 혈류 저하를 판정하는 기준이 달라야만 할 것으로 생각된다.

폐장의 허혈-재관류 손상과 세포간부착물질-1 의 발현 (Expression of Intercellular Adhesion Molecule- 1 after Ischemia Reperfusion Injury of the Canine Lung)

  • 배경: 폐장 이식 수술 시 필연적으로 1~4 시간 이내에 나타나는 급성 허혈-재관류 손상 기 전에 세포간부착물질의 활성화가 중요한 역할을 하리라는 가설을 세우고 동물실험을 통해 폐장에서 허혈-재관류를 유도한 후 세포간부착물질의 활성화 정도와 장기의 기능을 동시에 평가하여 실제로 세포간부착물질의 증가에 의한 염증반응과 장기 기능 저하 현상 사이에 밀접한 관계가 있는지를 증명하고자 하였다. 대상 및 방법: 체중 15~20kg의 잡견 7마리를 사용하여 좌측 폐문부를 차단하여 좌측 폐 온혈 허혈을 100분간 유발시킨 후 재관류를 시켰다. 재관류 후 4시간 동안 우측 폐문부를 간헐적으로 차단하면서 허혈시킨 좌측 폐 단독으로의 가스교환능, 혈역학적 변수, 호흡 역학적 변수를 측정하고, 각 시간대별로 폐정맥혈 내 TNF-$\alpha$및 cICAM-1 농도를 측정하였다. 실험 종료 후 폐 조직 수분 함량, 조직 내 지방 산화물과 (malonedialdehide; MDA) ATP양을 측정하고, 광학 현미경 소견 및 면역 형광염색법으로 혈관내피세포의 ICAM-1 발현 양상을 관찰하였다. 결과: 동맥혈 산소 분압, 폐혈관저항 및 조직검사, 조직 내 MDA 농도와 ATP 농도는 폐손상 소견을 나타내었다. TNF-$\alpha$는 재관류 1시간에 8.76$\pm$2.37 ng/ml으로 최고치를 보인 후 빠르게 감소하였으나 cICAM-1은 변화가 없었다, 면역현광염색법을 이용한 혈관내피세로의 ICAM-1은 변화가 없었다. 면역형광염색법을 이용한 혈관내피세포의 ICAM-1염색은 2마리에서는 허혈 전, 후의 차이가 발견되지 않았으나, 3마리에서는 허혈-재관류의 손상 후 ICAM-1의 발현이 관찰되었고, 2마리에서는 그 발현 양상이 허혈 전에 비해 매우 뚜렷하였다. 결론: 이상의 결과에서 폐장의 허혈-재관류 손상 시 싸이토카인 과 세포부착물질이 증가하나 그 증가 시기에 차이가 있음을 밝혔다.

  • 제 I 형 당원병에서 폐고혈압은 드문 합병증으로 외국 문헌에서 6예만이 보고되었으나 국내에는 아직 보고된 바가 없다. 저자들은 최근 제 Ia형 당원병으로 진단 받았던 31세 남자 환자에서 폐고혈압이 동반되었던 예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

개심술중 심폐우회 충전용객속에서의 10% 펜타스타치와 20% 알부민의 임상 비교 (The Clinical Comparisons of 10% Pentastarch and 20% Albumin in Priming Solutions for Cardiopulmonary Bypass in Cardiac Operations)

  • Pentastarch is a new synthetic hydroxyethyl starch similar to hetastarch. We report on the clinical comparisons the clinical efficacy and safety of 10% pentastarch in prime solutions for CPB in cardiac operations with that of 20% serum albumin. During CPB, group P [n = 20] received 500ml of 10% pentastarch and group A [n = 20] received 100ml of 20% albumin in prime solutions The postoperative time of ICU stay in group P and the day and amount of chest drain, hospital stay in group A were longer [p<0.05]. Fresh whole blood and PRBC were added only in group A and a higher amount of hartman solution was added in group A during CPB [p<0.05]. Prothrombin time was prolonged preoperatively and 2 days postoperatively in group A and 7 days postoperatively in group P [p<0.05] but there were no significant differences in bleeding time or fibrinogen level. Platelet count was higher immediately postoperatively in group A and preoperatively and 1, 2, and 7 days postoperatively in group P [p<0.05].Total protein and albumin level were higher 1 day postoperatively in group A and 2 and 7 days postoperatively in group P [p<0.05]. BUN was increased 2 days postoperatively in group A and Cr was increased 1 day postoperatively in group P [p<0.05]. CPK was higher preoperatively and 1, 2, and 7 days postoperatively in group A and plasma hemoglobin level was also higher 2 and 7 days postoperatively in group A [p<0.05]. There were no significant differences in arterial blood gas analysis but higher pO2 and lower pCO2 levels were maintained in group P and ejection fraction was higher 7 days postoperatively in group P [p<0.05]. Both groups were improved postoperatively in NYHA class and the hemodynamic parameters such as MAP, CO, CI, SV, LVSWI were well maintained in group P [p<0.05]. The amount of blood products used was higher in group A and urine output was higher immediately postoperatively in group A and 1, 2 days postoperatively in group P and the chest output was higher in group A. The complications were developed in 7 patients in group A and 5 patients in group P and mortality was not present in both groups.In conclusion, 10% pentastarch is as safe and effective as 20% albumin in prime solutions for cardiopulmonary bypass in cardiac operations.

Improved postoperative recovery profile in pediatric oral rehabilitation with low-dose dexmedetomidine as an opioid substitute for general anesthesia: a randomized double-blind clinical trial

  • Background: Low-dose dexmedetomidine may be a suitable alternative to opioids for pediatric ambulatory procedures under general anesthesia (GA). However, the recovery profile remains unclear. Herein, we aimed to evaluate the effects of low-dose dexmedetomidine on the recovery profile of children. Methods: Seventy-two children undergoing ambulatory oral rehabilitation under GA were randomly and equally distributed into two groups (D and F). Group D received an infusion of dexmedetomidine 0.25 ㎍/kg for 4 min for induction, followed by maintenance of 0.4 ㎍/kg/h. Group F received an infusion of fentanyl 1 ㎍/kg over 4 min for induction, followed by maintenance at 1 ㎍/kg/h. The primary outcome was the extubation time. The secondary outcomes were awakening time, end-tidal sevoflurane (ET-Sevo) requirement, change in hemodynamic parameters, Richmond Agitation-Sedation Scale (RASS), Children's Hospital of Eastern Ontario pain scale (CHEOPS) score, length of PACU stay, and incidence of adverse events. Results: Statistically significant differences were observed in the recovery profile between the groups: the median time for extubation was 3.65 (3.44-6.2) vs. 6.25 (4.21-7) minutes in groups D vs. F (P=0.001), respectively, while the corresponding awakening times were 19 (18.75-21) and 22.5 (22-24) minutes, respectively (P < 0.001). The mean ET-Sevo was low in group D (1.1 vs. 1.2; P < 0.001). The heart rate was significantly low across all time points in group D, without resulting in bradycardia. The median RASS and CHEOPS scores were also significantly lower in group D. No significant differences were observed in the mean arterial pressure, incidence of adverse events, or length of PACU stay. Conclusion: Low-dose dexmedetomidine was more effective than fentanyl as an opioid substitute at providing a better recovery profile in pediatric ambulatory oral rehabilitation under GA. Dexmedetomidine also significantly reduced sevoflurane consumption without causing adverse events or prolonging hospital stay.

Effect of aortic smooth muscle BK channels on mediating chronic intermittent hypoxia-induced vascular dysfunction

  • Chronic intermittent hypoxia (CIH) can lead to vascular dysfunction and increase the risk of cardiovascular diseases, cerebrovascular diseases, and arterial diseases. Nevertheless, mechanisms underlying CIH-induced vascular dysfunction remain unclear. Herein, this study analyzed the role of aortic smooth muscle calcium-activated potassium (BK) channels in CIH-induced vascular dysfunction. CIH models were established in rats and rat aortic smooth muscle cells (RASMCs). Hemodynamic parameters such as mean blood pressure (MBP), diastolic blood pressure (DBP), and systolic blood pressure (SBP) were measured in rats, along with an assessment of vascular tone. NO and ET-1 levels were detected in rat serum, and the levels of ET-1, NO, eNOS, p-eNOS, oxidative stress markers (ROS and MDA), and inflammatory factors (IL-6 and TNF-α) were tested in aortic tissues. The Ca2+ concentration in RASMCs was investigated. The activity of BK channels (BKα and BKβ) was evaluated in aortic tissues and RASMCs. SBP, DBP, and MBP were elevated in CIH-treated rats, along with endothelial dysfunction, cellular edema and partial detachment of endothelial cells. BK channel activity was decreased in CIH-treated rats and RASMCs. BK channel activation increased eNOS, p-eNOS, and NO levels while lowering ET-1, ROS, MDA, IL-6, and TNF-α levels in CIH-treated rats. Ca2+ concentration increased in RASMCs following CIH modeling, which was reversed by BK channel activation. BK channel inhibitor (Iberiotoxin) exacerbated CIH-induced vascular disorders and endothelial dysfunction. BK channel activation promoted vasorelaxation while suppressing vascular endothelial dysfunction, inflammation, and oxidative stress, thereby indirectly improving CIH-induced vascular dysfunction.
