구안와사 환자의 12경맥 전위측정 연구 (Differences in Electric Potential of Meridian System -Comparing Electrical Potentials of Patients with Facial Hemiparalysis-)

  • Objective : Assuming that the characteristic of meridian system has been similar to this of electric potentials in human body and that measurements of electric potential at well(井穴) and sea (合穴) points in branches of the twelve meridians(WSBTM) will be representative of measurements of the twelve meridians, to measure the electric potentials of 13 patients with Facial Hemiparalysis (구안와사<口眼蝸斜>, FH), to find out the characteristic of meridian system in patients with AK. Methods : Electric potentials of well and sea points in the meridians in twenty one patients with the pain in the lion diagnosed as FH were repeatedly measured by physiograph(PowerLab). Measurements of those electrical potentials were analyzed by factor analysis. Results : The electric potentials of WSBTM at the left side were divided into five factors. On the other hand those at the right side Were divided into four factors. Conclusions : In conclusion, their electrical potentials at the left and right side were each other five and four factors. Thus electrical potentials of well and sea points might be the representative meridian to show their characteristics.

중풍 후 운동장애에 대한 지황음자(地黃飮子) 투여 2례 (Two Cases of Motor Impairment after Cerebrovascular Disease by Jihwangeumja)

  • Cerebrovascular disease causes various symptoms like hemiparalysis, facial palsy, dysphagia, dysphasia etc. Although patients take lots of time to treatment, there are many cases symptoms could not be cured completely and patients could get sequelae. Jihwangeumja(dihuangyinzi) is used the cerebrovascular disease with dysphasia and motor impairment of the lower extremities due to weakness of kidney. The purpose of this study was to investigate the effect of Jihwangeumja on lower extremity weakness caught by cerebrovascular disease. In this study, we administered Jihwangeumja to two stroke patients with lower extremity weakness and analyzed the clinical progresses. As a result of the treatment, patients showed improved symptoms.

말초성안면신경마비(末梢性顔面神經麻痺) 구치료(灸治療) 및 복합치료(複合治療)가 미치는 영향(影響) (The Influence of Moxibustion and Basic Compound Therapy on Peripheral Facial Paralysis)

  • Thirty cases were observed among the peripheral facial paralysis patients who admitted. the Department of Acupuncture and Moxibustion of Seoul Oriental Hospital between Feb. 2nd, 1995 and Aug. 14th, 2000. These patients were divided into two groups; the first group was treated with indirect facial moxibustion., while the second with basic oriental-westem medicine treatment. Object : To improve the therapeutic rate of peripheral facial paralysis by applying basic oriental-western medicine treatment and indirect facial moxibustion. Method : Inpatients suffering from peripheral paralysis of facial nerve were divided into two groups; both groups recieved basic oriental-western medicine treatment; the experimental group additionally received indirect facial moxibustion onto Tuyu(頭維) and Chich'ang(地倉) once or twice daily. Result : In the therapeutic result from moxibustion treatment group, 3 excellent, 7 good, 3 fair, and 1 poor case were observed. From compound treatment group, 2 excellent, 5 good, 5 fair and 4 poor cases were observed. Among the excellent and good patients of both groups, therapeutic rate of moxibustion treatment group (71.4%) was higher than that of compound treatment group(43.8%). Conclusion : The overall therapeuric rate of the moxibustion treatment group was 92.8%, which was higher than that of the compound treatment group(75.1%).

The Comparison of Muscle Contraction Time and Modulation Ratio during Reaching Tasks in Hemiparalysis, Elderly and Young Adults

  • Objective: The purpose of this study was to evaluate the Electromyography (EMG) of the upper limb during reaching tasks according to two heights in the sitting position. Design: Cross sectional design Methods: Fifteen hemiplegia, fifteen elderly, and fifteen healthy subjects have participated in this study. The targets (90% length of the subject's arm) were located at the two heights (the eye and xiphoid process). We have recorded EMG signals of seven upper limb muscles (anterior deltoid (AD), posterior deltoid (PD), pectoralis major (Pec), infraspinatus (Inf), supraspinatus (Sup), biceps brachii (Bi), triceps brachii (Tri)). The dependent variables were movement time(s), modulation ratio, working ratio, and the co-contraction ratio of the hemiplegia, elderly, and healthy at the reaching task. Two-way repeated-measures ANOVA (2-heights) was analyzed with the LSD post hoc test. Results: The study results were as follows: (1) The movement time to the target during reaching movement was significantly longer for the hemiplegia and elderly groups compared to the healthy group. (2) The modulation rate was significantly higher at eye height than the xiphoid height in AD, PD, Pec, Inf, Bi muscles, and the hemiplegia group and elderly group were significantly lower than the healthy group. Additionally, the modulation ratio showed a significant interaction between heights and groups. Conclusions: It is expected that the variables using the muscle contraction characteristics, the evaluation method of this study, can be used as an electromyography-based feedback method that can be objectively evaluated and quantified in clinical practice.

뇌졸중 환자의 균형 증진을 위한 체감형 전자게임 훈련에 적용되는 되먹임 방식에 따른 효과 (The Effect of Feedback on Somesthetic Video Game Training for Improving Balance of Stroke Patients)

  • PURPOSE: The purpose of this study is to assess the difference in the effect of provision of feedback on knowledge of performance and knowledge of result in the training using somesthetic video game aimed at enhancement of balance of hemiparalysis patients due to stroke. METHODS: 20 stroke patients participated in the study. The participants were randomly divided into 2 groups, namely, the knowledge of performance feedback group (KP group, n=10) and the knowledge of result feedback group (KR group, n=10). Both groups received somesthetic video game training 5 times (30 minutes each) a week for total of 4 weeks. The KP group received feedback on the patterns of movement in execution of somesthetic video game. The KR group received feedback on the scores acquired following execution of somesthetic video game. Verification of the significance of the data was performed through paired t-test and independent t-test. RESULTS: Both groups displayed significant reduction in the movement of center of pressure (COP) and Timed up and Go (TUG), and significant increase in the Berg Balance Scale (BBS) following the training. Although the movement of COP was reduced for the KP group in comparison to the KR group, it was not statistically significant, and there was significant reduction in TUG and significant increase in BBS. CONCLUSION: The above results illustrate that provision of feedback on knowledge of performance is more effective than feedback on knowledge of result in somesthetic video game training for the purpose of enhancement of balance in stroke patients. Therefore, provision of feedback on knowledge of performance is necessary in somesthetic video game training for stroke patients.

편마비 환자에게 적용된 경두개직류자극이 하지 근 활성도 및 보행능력에 미치는 영향 (Effect of Trans Cranial Directed Current Stimulus on Lower Extremity Muscle Activation and Walking Capacity for Hemiparalysis Patients)

  • Purpose: The purpose of this study is to investigate the effect of non-invasive transcranial direct current stimulation (tDCS) on muscle activity, including 10 m WT, TUG, and BBS, in hemiplegic stroke patients. Methods: This study was conducted on 42 inpatients diagnosed with hemiplegia due to stroke at hospital B in Daejeon for more than 6 months. Walking training was conducted for six weeks, five times a week for 30 minutes, with a general walking group (14 people), tDCS walking group (14 people), and tDCS (sham) walking group (14 people). Results: As a result of the study, the change in the muscle activity before and after tDCS intervention was significantly increased in the tibialis anterior muscle in the CG group. In the EG group, the erector spine (lumbar), rectus femoris, and tibialis anterior muscles significantly increased. In the SEG group, significant increases were observed in the rectus femoris and tibialis anterior muscles. Significant differences were found in the rectus femoris and tibialis anterior muscles in the comparison between groups after intervention according to tDCS application. Also, 10 m WT, TUG, and BBS were significantly increased in the CG, EG, and SEG groups after intervention, and there were significant differences in 10 m WT, TUG, and BBS in comparison between groups after intervention according to tDCS application. Conclusion: As a result, tDCS is an effective in improving the walking ability of stroke patients, and in particular, it effectively increases the muscle activity of the rectus femoris and tibialis anterior muscles, which act directly on walking, and also improves the speed and stability of walking. It is considered being an effective method to increase the gait of stroke patients by combining it with the existing gait training.

경 두개 직류자극이 뇌졸중 환자의 시간적, 공간적 보행능력에 미치는 영향 (Effect of Trans cranial Directed Current Stimulus on Temporal and Spatial Walking Capacity for Hemiparalysis Patients)

  • Background: This study was to investigate the effect of non-invasive transcranial direct current stimulation due to hemiplegic patients due to stroke on temporal and spatial gait ability. Design: Randomized sham controlled trial. Methods: For the study method, 42 patients with hemiplegia due to stroke were randomly assigned to 14 patients each, and the general walking group, tDCS walking group, and tDCS (sham) walking group were subjected to 5 times a week, 30 minutes a day, and 6 weeks. In the temporal gait variables of hemiplegic patients due to stroke, the effect of the gait time, gait cycle, single support, double support, swing phase, stance phase, gait speed, cadence were measured. In spatial variables, one step length and one step length were measured. Results: As a result of the study, the EG group significantly increased in the step time, gait velocity, and cadence of the paralysis side in the comparison of temporal walking variables between groups according to the application of tDCS of walking ability in hemiplegic patients due to stroke patients(p<.05). In the change in spatial walking variables between groups according to the application of tDCS, the step length and stride length of the EG group showed a significant increase. Both the comparison of temporal and spatial symmetry walking variables between groups according to tDCS application was not significant(p>.05) Conclusion: As a result, tDCS has an effective effect on the improvement of the gait ability of stroke patients. In particular, it is an effective method of physical therapy that can improve the cadence and speed of gait, which can be combined with the existing gait training to effectively increase the gait of hemiplegia due to stroke patients.

뇌졸중환자의 희망 (The Hope of the Stroke Patients)

  • Cerebrovascular diseases in Korea is an important health problem since mortality and mobidity have been increased rapidly. It marked the 2nd cause of specific death rates in 1993. The stroke causes physical function disorder due to hemiparalysis and emotional disorder, and stroke patients experience helplessness, powerlessness. sense of alienation and loss of hope. These feelings make the rehabilitation difficult because they lose the will of life. The subjects of the study were seven citizens who live in Pusan, are over 50 years old and belong to low income-level. The data were collected from Jan. to Sep. 1995. The researcher as a caregiver and volunteer made confidence of them and asked for their agreement on the purpose of the study. The subjects expressed their experience as openheartedly as possible. The analysis of the data was made through the phenomenological analytic method suggested by Giorgi, which is as follows ; as an unit of description which include the subject' expressions and the researcher's observation, it is examined the theme that express the hope experience with the subject's language(underlining), and the focal meanings are identified. The focal meaning is the crystalization of the theme, which is written in the language of the researcher. After intergrating the focal meaning and make the situated structural description as the meaning of the hope experience identified on each subject's point. After intergrating the situated structural description and make the general structural description as the meaning of the hope experience identified on total subject's point then the systemizing of the structure of the hope experienced phenomena and the flowing of the conciousness was researched. The conclusions of this study was as follows : The ten sources of hope which the subjects experienced were sorted as under 〈mutual relations to others : spouse, children, relatives, fellow believer. health professioner. associate patient group〉, 〈spiritual dependence〉. 〈recovery of physical function〉. 〈rumination of the past life〉, 〈expectation of the future〉. 〈economic power〉, 〈belief〉, 〈ability〉. 〈spontaneous participation〉 and 〈recovery of roles〉. Their hope was spoken out by the following two kinds of linguistics. First. the hope was expressed in the affirmative expression as follows : 〈 to be dependable〉, 〈to make efforts〉, 〈to keep under control〉, 〈to desire〉, 〈to be pleasant〉, 〈to be peaceful〉, 〈to be grateful〉, 〈to give help〉, 〈self-confidence. Courage〉, 〈to be happy〉, 〈to satisfy oneself〉, 〈to share with others〉, 〈to understand〉 and 〈to be affected, be impressed〉 Second, the hope was expressed in the negative on pression as under : 〈to be distressed〉, 〈to be uneasy〉, 〈to be sorry, be unsatisfied〉 〈despair〉, 〈to abandon〉, 〈to be fearful〉, 〈to suffer〉, 〈to bear a burden〉 〈to be confused〉, 〈to be solitary〉, 〈chest trouble〉, 〈to feel heavy〉 〈grief〉, 〈to be daunted〉, 〈to get angry〉, 〈to be uncomfortable〉, 〈to have something regretable〉 and 〈to feel guilty〉. And their hope was expressed by the following four behavioral expressions : 〈physical sphere〉, 〈psychological sphere〉, 〈social support sphere〉 and 〈spiritual sphere〉. The reaction patterns of their hope experience appeared in the following 4 coping method : 〈conquest type〉, 〈dependence type〉, 〈adaptation type〉 and 〈fate type〉. Finally, in the hope structure the sense of certainty don't always coexict with the sense of uncertainty, When the stroke patients try to search for the best quality of life, the senses of certainty and uncertainty make a continual cyclic system in the hope structure.

작업기억의 향상을 위한 tDCS 자극이 뇌졸중 환자의 뇌파변화에 미치는 영향 (Effect of tDCS Stimulation for Improving Working Memory on Stroke Patients' EEG Variation)

  • 본 연구는 작업기억 향상을 위한 직류두개자극(tDCS)이 뇌졸중 환자의 뇌파에 어떠한 변화를 가져오는지 알아보기 위해 시행되었다. MMSE와 숫자 외우기 검사(DST)를 통해 선별된 만성 뇌졸중 환자 20명을 전산화 인지 훈련(CCT)만 실시한 I군(10명)과 tDCS와 CCT를 병행한 II군(10명)으로 무작위 배치하여 주 3회, 1일 30분, 4주간 중재하였다. 뇌파 변화를 알아보기 위해 중재 전, 2주 후, 4주 후 시점에서 뇌전도로 단어, 사진, 암산과제 시 세가지 밴드(${\theta}$; 4~8 Hz, lower ${\alpha}$; 8~10.5 Hz, upper ${\alpha}$;10.5~13 Hz)로 절대 스펙트럼 파워를 산출하여, 기준뇌파에 대한 증감률(%)을 구하였다. 연구 결과, 첫째, 측정시점에 따라서, ${\theta}$ 파워는 단어기억과제와 사진기억과제에서 유의한 차이를 보였다. lower ${\alpha}$ 파워는 모든 기억과제에서 유의한 차이를 보였다. upper ${\alpha}$ 파워는 암산과제에서만 유의한 차이를 보였다. 둘째, 4주째에서 모든 과제의 lower ${\alpha}$ 파워에서만 두 군간에 유의한 차이를 보였다. 따라서 본 연구의 과제에 따른 세 가지 밴드의 차이를 통해 작업기억의 정도를 정량화 하여 비교가 가능하며, 뇌손상환자에게 tDCS의 병행이 인지재활에 보조적인 도움을 줄 수 있을 것으로 생각된다.

뇌졸중 환자가 지각하는 사회적 지지와 희망과의 관계 (The Correlation between Perceived Social Support and Hope of Stroke Survivors)

  • A Cerebrovascular accident(CVA), or Stroke is a medical emergency that occurred when the blood supply to the brain is interrupted or blocked. The stroke causes physical function disorder due to hemiparalysis and emotional disorder. Also the stroke patients experience helplessness, powerlessness, sense of alienation and loss of hope. These feelings make the rehabilitation difficult because they lose the will of life. The purpose of this study is to identify the correlation between perceived social support and hope of stroke survivors. The subjects for this study were 100 out-patients with stroke in one general hospital and oriental medicine hospital located in Mokpo. The data were analysed by frequency, t-test, ANOVA. Duncan test, Pearson's correlation, using the SPSS WIN 9.0 program. Data were collected from July 11 to September 9, 2000, using a structured questionnaire. The instruments used for this study : The social support scale developed by Park, Ji-won(1985) and the hope scale developed by Miller(1988). The results were as follows. 1. It was found that the higher the degree of perceived social support, the higher the degree of hope(r=.726, p=.000). Therefore hypothesis was supported. 2. The mean score of perceived social support was 77.8(SD=21.0) with a score range from 27.0 to 104.0. 3. The mean score of perceived hope was 117.0(SD=25.7) with a score range from 57.0 to 160.0. 4. The level of social support depending on general characteristics were significantly different in variables such as marital status(t=3.131, p=.010). degree of income satisfaction(F=16.027, p=.000). 5. The level of hope depending on general characteristics were significantly different in variables such as marital status(t=2.681, p=.040). current job(t=-2.055, p=.043) degree of income, satisfaction(F=11.363, p=.000). For these subjects, there was a significant relationship between social support and hope. The stroke survivors need social support to inspire their hope. Nurses should plan interventions to enhance social support for patients with stroke. The above results may be used as the basic data to seek more efficient way of elevating nursing practice and rehabilitation for the patients with stroke.
