Reliability Study of Diagnosis System of Oriental Medicine DSOM(r) D.1.1 (한방진단(韓方診斷)시스템 DSOM(r)D.1.1의 신뢰도연구(信賴度硏究))

  • Objectives : This study examined the reliability of disease mechanism diagnosis, to evaluate items of questionnaires and inquire about the relationships between disease mechanisms and 'diagnosis program' questionnaires used for the objective diagnosis of Oriental medicine in the department of Oriental OB&GYN, Oriental Medical Hospital of Dongeui University. Method : We analyzed the results of questionnaires from 3504 outpatients of OB&GYN disease at the Oriental Medical Hospital of Dongeui University from April 2000 to April 2005. Results & Conclusions : 1. The research questionnaire had 188 questions, the summary questionnaire 137, and the diagnosis questionnaire 80. 2. The reliability of all questionnaires shows above 90% in deficiency of qi, deficiency of Yin, insufficiency of Yang coldness heat syndrome liver and spleen kidney in all, 8 case disease mechanisms. These are higher in the diagnosis questionnaires than in the research questionnaires and the summary questionnaires, except for kidney disease mechanism. 3. Cronbach a of the questionnaires decreased, especially blood deficiency, phlegm, heat syndrome, and insufficiency of Yang; these 4 case disease mechanisms were lower than 0.6. 4. For degree of correspondence of meeting points, both. the diagnosis and the summary questionnaires were above 80% with the exception of the 2 case disease mechanisms heart and blood deficiency. The meeting points of both the diagnosis and research questionnaires were above 80% in the to case disease mechanisms deficiency of qi blood stasis deficiency of Yim insufficiency of Yang damp dryness liver spleen kidney phlegm. 5. The change in the result values of questionnaires was a decreased level of deficiency of qi heat syndrome phlegm damp kidney and raised level of coldness heart disorder of qi dryness 6. The computation degree of disease mechanism in DSOM(r) D.1.1 was much lower on phlegm deficiency of qi heat syndrome disorder of blood, somewhat lower on insufficiency of Yang and higher on coldness than in the two different questionnaires.

  • Objective : To investigate the syndrome differentiation's types and herbal medicine of migraine through Chinese journals review Methods : Journal search was performed using the searching engine of China Academic Journal(CAJ) in China National Knowledge Infrastructure(CNKI) from January 2000 to November 2010. Searching key words were "migraine", "chinese traditional medicine" and "syndrome differentiation". We included all kinds type of journals that explained or referred definite syndrome differentiations. The methods of treatment and Herbal medications by syndrome differentiation in contents of finally selected journals were extracted and summarized. Results : Eighteen chinese journals were selected finally. Fifteen kinds of syndrome differentiations about migraine were investigated, which included blood stasis due to qi stagnation (氣滯血瘀) quoted 15 times, middle obstruction of phlegm-dampness (痰濕中阻) 11 times, liver yang transforming into wind (肝陽化風) 10 times, deficiency of qi and blood (氣血虧虛) 6 times, wind-cold invading 風寒侵襲 淸陽鬱遏 4 times, cold invading reverting yin (寒犯厥阴) 4 times, liver-kidney deficiency (肝腎虧虛) 3 times, liver qi depression and qi stagnation (肝鬱氣滯) 2 times, liver depression transforming into fires (肝鬱化火) 2 times, wind-fire of liver-gallbladder (肝膽風火) 3 times, intense stomach fire and heat (胃火熱盛) 2 times, insufficiency of blood deficiency (血虛不榮) 2 times, insufficiency of qi deficiency (氣虛不充) 2 times, insufficiency of kidney qi and sea of marrow deficiency (腎氣不足, 髓海空虛) 2 times, and qi depression due to wind invading (風邪侵襲, 氣鬱不宣) 2 times. Conclusion : We suggests the first choice of oriental treatment for migraine can be considered among syndrome differentiation's types of blood stasis due to qi stagnation, middle obstruction of phlegm-dampness, liver yang transforming into wind, deficiency of qi and blood, and cold invading reverting yin. further systematic study will be needed.

  • Objectives : The Syndromes of Daecheongryongtang (大靑龍湯證) and Mahwangtang (麻黃湯證) are similar in pathogenesis. However, both syndromes have a different pathogenesis. So we will try to distinguish the difference between the two syndromes. In addition, we'll see about 40 provision. Methods : Examine the claims of the syndrome of Daecheongryongtang in "Sang Han Lon" (傷寒論). The scholar's claims are not all same. Thus, we analysis the claims of many scholars. Results and Conclusions : The Syndromes of Daecheongryongtang and Mahwangtang are similar in pathogenesis. However, the Syndrome of Daecheongryongtang is different from that the sweat purgations in exterior of the body caused by heat in the interior. The pathogenesis of 39 and 40 provisions are different. 40 provision is more severe symptoms than 39 provision in the stagnation. So 40 provision used the term "fazhi (發之)".

  • Pollen-food allergy syndrome (PFAS) is an immunoglobulin E-mediated immediate allergic reaction caused by cross-reactivity between pollen and the antigens of foods-such as fruits, vegetables, or nuts-in patients with pollen allergy. A 42.7% prevalence of PFAS in Korean pediatric patients with pollinosis was recently reported. PFAS is often called oral allergy syndrome because of mild symptoms such as itching, urticaria, and edema mainly in the lips, mouth, and pharynx that appear after food ingestion. However, reports of systemic reactions such as anaphylaxis have been increasing recently. This diversity in the degree of symptoms is related to the types of trigger foods and the characteristics of allergens, such as heat stability. When pediatric patients with pollen allergy are treated, attention should be paid to PFAS and an active effort should be made to diagnose it.

Development of a Guideline for the Application of a Diagnostic Tool for Menopausal Syndromes Based on the Use of Systemic Review and Delphi Method (체계적 문헌고찰과 델파이 기법을 활용한 갱년기장애 변증(辨證)진단 도구 개발을 위한 기초 연구)

  • Objectives: This study was conducted towards developing a screening tool for syndrome differentiation in the diagnosis of menopause in menopausal and perimenopausal women. Methods: We conducted a literature review of studies on menopausal diagnosis based on syndrome differentiation, and examined well-founded differentiated syndromes and their respective clinical symptoms. Based on the findings, we created a questionnaire through consultations with Oriental medicine experts in physiology, pathology, and diagnostics. Finally, the research team conducted an expert Delphi study on differentiated syndromes and the associated clinical symptoms. Results: Seven differentiated syndromes were selected, including Liver Depression (肝鬱), Kidney Yin Deficiency (腎陰虛), Kidney Yang Deficiency (腎陽虛), Liver and Kidney Yin Deficiency (肝腎陰虛), Kidney Yin and Yang Deficiency (腎陰陽兩虛), Heart-Kidney Noninteraction (心腎不交), and Dual Deficiency of Heart and Spleen (心脾兩虛); 4 disease locations, including liver (肝), heart (心), spleen (脾), and kidney (腎); and 3 disease natures, including Yin Deficiency (陰虛), Qi Stagnation (氣滯), and Blood Deficiency (血虛). In addition, we added 3 supplemental disease natures, including Yang Deficiency (陽虛), Qi Deficiency (氣虛), and Heat (火熱), in consideration of syndrome differentiation categories that may possibly be added in a follow-up clinical questionnaire. Conclusions: This resulted in a total of 7 differentiated syndromes, 4 disease locations, and 6 disease natures. We translated the clinical symptoms of these 17 categories into Korean Hangeul. After consulting with 5 Oriental medicine experts and a psychology expert, we produced a questionnaire for use in diagnosing menopause based on syndrome differentiation. The calculation of scores for the syndrome differentiation screening tool will be confirmed through clinical research based on the results of a review of existing literature.

  • Delirium is an acute syndrome of disorientaion caused by dysfunction of brain and has many various symptoms. The characteristic symptoms of delirium are conscious disturbance with disorientation, dysarthria and emotional disturbance. We experienced 84-year-old man who had Wi syndrome(痿證) as well as delirium during the treatment of pneumonia, and whose condition was improved through Oriental medical treatment. The patient was diagnosed as Yangmyeong Disease(陽明病) that was caused by heat in the stomach and stool in the colon, and was treated with Daeseungki-tang (Dachengqi-tang, 大承氣湯). We were able to improve the delirium caused by high fever infection through Oriental medical treatment.

  • Accumulation of pathogens in chest refers to a syndrome of fullness, stuffiness and pain in the chest, diaphragm, epigastrium and abdomen, which is ascribable to accumulation of pathogenic heat with stagnancy of fluid or phlegm in the chest. Retching refers to vomiting with sound but without any vomitus. It is usually ascribable to adverse flow of ki due to deficiency of stomach, or by pathogenic heat or cold, and failure in descending of the stomach-ki. It may also be found in Soyang disease. Bloody purulent stool refers to passing stool with blood and pus. It is formed mainly due to invasion of the stomach and the intestines by epidemic pathogenic summer-heat, steaming of stagnateed damp-heat fighting against ki and blood, or improper diet, obstruction of bu-ki stagnation of blood and ki.

  • The result were achived from 29 kinds of the medical literature of many generation is like this. 1. Oriental medical expressions of Xerophthalmia are "Baksab(白澁)", "Kunsabhonhwa(乾澁昏花)", "Sinsoojanggo(神水將枯)", "Donginkunkyul(瞳人乾缺)", "Taljung(奪精)", etc. "Baksab(白澁)" is very close to Xerophthalmia. 2. The cause of Xerophthalmia is Wind, Wind-heat Pathogen, Damp-heat of spleen and lung, DefIciency of body fluid, Yin-deficiency of liver and kidney, Liver deficiency syndrome, Deficiency of blood, Fire, Fire of deficiency type, Liver heat, etc. 3. In the frequency of prescription used Xerophthalmia are "Sangbakpi-Tang(桑白皮湯)", "Eunkyosan(銀翹散)", etc as Excess type, "Kikookjihwang-Tang(杞菊地黃湯)", "Samooloja hwan(四物五子丸)", etc as Deficiency type. 4. In the frequency of medical herbs of Xerophthalmia use much Rehmannia root nourishing Yin and clearing away heat and Divaricate Saposhnikovia root(expelling pathogenic wind.

A study on Applications of prescriptions including Fructus Schisandra as a main component in Donguibogam (동의보감(東醫寶鑑) 중(中) 오미자(五味子)가 주약(主藥)으로 배오(配伍)된 방제(方劑)의 활용(活用)에 대한 고찰(考察))

  • This report describes 47 studies related to the use of Fructus Schisandra main blended prescriptions from Donguibogam. The following conclusions were reached through investigations on the prescriptions that use Fructus Schisandra as a key ingredient. 1.34% of a cough, 10.6% of a consumptive disease, recorded the largest number of clinical frequency of the prescriptions in therapeutic use when Fructus Schisandra was taken as a monarch drug in prescriptions 2. Prescriptions that utilize Fructus Schisandra as the main ingredient are used in the treatmeant of a cough, a consumptive disease, an exogenous febrile disease, a carbuncle, and cellulitis, and they are also used for treating 11 different types of diseases. 3. The prescriptions are compounded with Fructus Schisandra as a monarch drug can apply to a deficiency syndrome of the lung a deficiency syndrome of both the lung and the stomach, a deficiency syndrome both the spleen and the lung a deficiency syn-drome of the kidney, a hypofunction of the bladder with cold syndrome, a cold of insufficiency type, a deficiency syndrome of the heart, a heat syndrome of the stomach, an affective by cold, an invasion by wind, a consumptive disease. 4. The dosage of Fructus Schisandra is 5pun(about 1.88g) to 5jeon(about 18.75g), however 1jeon(about 3.75g) has been taken the most for clinical application. 5. When Fructus Schisandra is combined with base prescriptions such as Ijintang Chungliongsan, Saengmaksan, it applies symtoms of cough. In addition, when Fructus Schisandra is combined with base prescriptions such as Liukmizihwangtang, Ssangbohwan, Sipjeondaebotang, it utilizes a consumtive disease.

The Literature Review of FibroMyalgia Syndrome (섬유근통 증후군에 대한 문헌고찰)

  • Fibromyalgia syndrome(FMS) is a chronic pain disorder of unknown etiology characterized by widespread musculoskeletal aches and pains, stiffness, and general fatigue, disturbed sleep and sleepiness. Frequently misdiagnosed, FMS is often confused with myofascial pain syndrome, polymyalgia rheumatica, polymyositis, hypothyroidism, metastatic carcinoma, rheumatoid arthritis (RA), juvenile rheumatoid arthritis, chronic fatigue syndrome, or systemic lupus erythematosus, any of which may occur concomitantly with FMS. The management of FMS often begins with a thorough examination and a diagnosis from a physician who is formally trained in tender-point/trigger-point recognition. An initial diagnosis provides reassurance to the patient and often reduces the anxiety and depression patterns associated with FMS. The most common goals in the management of FMS are (1) to break the pain cycle, (2) to restore sleep patterns, and (3) to increase functional activity levels. Because FMS is a multifactorial syndrome, it is likely that the best treatment will encompass multiple strategies. Medication with analgesics and antidepressants and also physiotherapy, are often prescribed and give some relief. The other most effective intervention for long-term management of FS to date is physical exercise. Physical therapists can instruct patients in the use of heat at home (moist hot packs, heating pads, whirlpools, warm showers or baths, and hot pads) to increase local blood flow and to decrease muscle spasm and tension. Also instruct patients in the proper use of cold modalities (ice packs, ice massage, and cool baths) to anesthetize localized areas of pain (tender points) and break the pain cycle. Massage and tender-point massage also may promote muscle relaxation. To date, the two most important interventions for the long-term management of FS are patient education and physical exercise. Lately, is handling FMS and Chronic Fatigue syndrome(CFS) together, becuase FMS and CFS are poorly understood disorders that share similar demographic and clinical characteristics. Because of the clinical similarities between both disorders it was suggested that they share a common pathophysiological mechanism, namely, central nervous system dysfunction.
