A literal study on the Gu-Chang (구창의 문헌연구)

  • Gu-Chang is a disorder characterized by recurring ulcers confined to the oral mucosa. Despite much clinical and research attention, the causes remain poorly understood. In this paper, we will compare Gu-Chang with Recurrent Aphthous Stomatitis(RAS) in order to know what is the similiarity between Gu-Chang and RAS. So we will arrange various oriental and western medical literatures which are important. As a result of arrangement of the causes, symptoms and therapys of Gu-Chang, we can conclude through the studies as follows. 1. The etiologies of Gu-chang are following. In the Sthenia syndrome, there are evil heat of external factor, heat of heart and spleen, insomnia, heat of upper warmer, stress and diet, heat of lung and heart, excessive heat of upper warmer, inappropriate food intake, heat conveyance of organ, heat of stomach merdian, moistured heat of spleen and stomach and stasis of liver energy. In the Asthenia syndrome, there are deficiency of stomach energy, deficiency of upper warmer leading to heat, deficiency of middle warmer leading to cold, deficiency of lower warmer leading to heat, deficiency of middle energy, deficiency of blood, decreased fire and deficiency of soil, yin fire of lower warmer, deficiency of heart yin, deficiency of spleen yin and deficiency of qi and blood. 2. In western medicine the causes of RAS is presumed as local, microbial, systemic, nutritional, genetic, immunologic factors. 3. Once Gu-chang is compared with RAS, in the deficiency of yin leading to hyperactivity of fire, deficiency of yin leading to floating of fire and stasis of liver energy, recurring of Gu-chang is similar to RAS. Although recurring of Gu-chang due to tripple warmer of excessive fire has no recurrance, since there are the degree of Pain, site of lesion, dysphagia etc, it is similar to major RAS. It is may be believed that Sthenia Gu-chang is similar to major RAS, shape of recurring, site of lesion, degrree of Pain and white color of Asthenia Gu-chang are similar to minor RAS, but there is no similarity concerning herpes RAS in the literatures that describe the symptoms. 4. Generally, the treatment of Gu-chang is divided into Asthenia and Sthenia Syndrome. The method of cure to Sthenia syndrome is heat cleaning and purge fire, Asthenia syndrome is nourish yin to lower and adverse rising energy and strength the middle warmer and benefit vital energy. 5. Following is the medication for Sthenia syndrome. Heat of heart and spleen is Do Jok San, Yang Gyek San, Juk Yup Suk Go Tang, evil heat of external factor is Yang Gyek San Ga Gam, Stasis of liver energy is Chong Wi Fae Dok Yum, moistured heat of spleen and stomach is Chong Gi Sam Syep Tang. The medication for Asthenia Syndrome is following. Deficiency of upper warmer leading to heat is Bo Jung Ik Gi Tang, deficiency of middle warmer leading to cold is Bu Ja Lee Jung Tang, deficiency of lower warmer leading to heat is Yuk Mi Ji Hwang Tang, deficiency of yin leading to hyperactivity of fire is Ji Baek Ji Hwang Hwan, deficiency of yin leading to floating of fire is Lee Jung Tang Ga Bu Ja Medicine for external use were Yang Suk San, Boo Wyen San, Rok Po San, Yoo Hwa San ate. 6. In western medicine, there is no specific treatment for RAS, and management strategies depend on dinical presentation and symptoms and includes antibiotics, oral rinses, glucocorticoids, immunomodulatory drugs, vitamines, analgesics, laser and antiviral agents.

  • Urinary disturbance is deficit in volitional control of voiding. Stroke can cause temporary or permanent urinary disturbance. The purpose of this study was to confirm the effect of Jasin-hwan on urinary disturbance as heat syndrome. This study was carried out on 3 patients with urinary disturbance caused by stroke. We measured components of IPSS (international prostate symptom score) at pre-treatment and post-treatment. After prescribing medication to the patients, symptoms improved rapidly within three weeks. These results support that Jasin-hwan is effective in reduction of urinary disturbance as heat syndrome.

  • 1. Objectives The purpose of this case study is to evaluate the effects of Sasangconstitutional diagnosis and treatment of chronic idiopathic urticaria patient who was diagnosed Taeeumin Dry-Heat Syndrome(煥熱證). 2. Methods Chronic idiopathic urticaria patient was diagnosed Taeeumin Dry-Heat Syndrome(煥熱證) based on their Nature & Emotion(性情), physical characteristics, symptoms. She was medicated Cheongsimyeonjatang(淸心蓮子湯). 3. Results and Conclusions Chronic idiopathic urticaria patient who was treated with Cheongsimyeonjatang(淸心蓮子湯) showed improvement in urticaria symptom and general condition. This case study describe the effectiveness on Chronic idiopathic urticaria symptom by using Cheongsimyeonjang(淸心蓮子湯).

The Literatural Study on Arthralgia Syndrome(痺病) (비병(痺病)의 문헌적(文獻的) 연구(硏究))

  • I would like to state my own opinion on arthralgia syndrome(痺病) through the literatural studies. First of all, arthralgia symdrome(痺病) must be classified into six type basically, which are migratory arthralgia(痺病(行痺)), arthritis of heat type(濕痺), arthritis due to blood stasis(瘀血痺) and deficient rheumatism(虛痺), and then could be considered to try the compound names of arthralgia syndrome. These can come from according to the rise and decline of causes in wind(風), cold(寒), damp(濕), heat(熱), blood stasis(瘀血) and qi-blood(氣血). For example, it would be possible to apply the wind-dampness rheymatism(風濕痺) of damp-heat rheumatism(濕熱痺) in terminology of arthralgia syndrome(痺病). As rheumatoid arthritis(歷節風), rheumatoid arthritis like white tiger bite (白虎歷節風) and gout (痛風) not to mean the gout in western medicine have been announced a kind of arthralgia syndromes(痺病) by many doctors since Ming dynasty(明代) and proved it to be true, it is reasonabie not to try it any longer. And tingling and deficiency of sensation(廢木 不仁) is a symptome showing the decline of muscle power including mainly the abnormal sensation of skin, it would be recommended to be classified into fliaccidity syndrome(?痺). And then the names rheumatism invoiving lendon and ligament(筋痺), rheumatism involving blood vessels(脈痺), rheumatism involving muscle(肌痺), numbness of skin (皮痺) and rheumatism involving bone(骨痺), which have been used as the classification title with the season be received bad-qi(邪氣), must be classlfied to the location appearing aymptomes. Though obstruction of the liver-qi(肝痺), obstruction of the heart-qi(心痺), stagnation of the spleen-qi(脾痺), stagnation of the lung-qi(肺痺), stagnation of the kidney-qi(腎痺) and dysfunction of the bladder(胞痺) that used visceral and bladder name, that stated a kind of arthralgia syndrome(痺病), but it must be classified into a different diseases from arthragia syndrome.

  • Thoracic outlet syndrome's chief symptom has numbness and tingling sensation of tharm, hand and fingers. In the morning, patient complain of pins and needles of the hands and weakness. TOS classified three categories : Anterior scalene syndrome, Claviculocostal syndrome, Pectoralis minor syndrome Physical therapy of the TOS is heat, massage for soft tissue, stretching exercise for scalene muscles and pectoralis minor muscles, and strengthening exercise for upper trapezius and levator scapular and neck muscles. A main problem of soft tissue is mechanical causes, so physical therapists have to solve that problem by mechanical manual methods.

  • Lee Dong-won based on the machanism of internal disease founded on Umhwa and Wonki in discriminating internal injury from external. His general idea of discrimination of internal injury from external could be thought as an reinterpretation of the concept from "Neijing", to Triple heater Wonki and ascending and descending of stomach qi. He distinguished between Yin disease and Yang disease and classified into internal and external injury. "Insufficiency of yang brings about cold syndrome." and "An excess of yang brings about heat syndrome." are considered as external infection, and "Insufficiency of yin brings about heat syndrome." and "An excess of yin brings about cold syndrome." as internal injury.

  • This thesis is focused to get treatment on climacteric syndrome through literary study. The results are followed as below. 1. The climacteric syndrome is beginning with lowering of secretion of female hormone. It's not understood as abnormal disease but as normal phenomenon. 2. The causes of climacteric syndrome are defined as deficiency of the Kidney, stagnation of Liver, disharmony between Heart and Kidney, insufficiency of both the Heart and the Spleen, blood stagnation. 3. The causes of climacteric bleeding are continuous with overstrain, injury of the five emotions, blood heat, deteriorating blood. 4. The treatment of climacteric syndrome are mainly nutrition of Kidney and Liver, that of Kidney heat, descending Yang of Liver, nutrition of blood of heart, having a comunication with Kidney and Heart, nutrition of Spleen and Stomarch. 5. For the prescriptions on climacteric syndrome, the treatments such as Jaguium(左歸飮), Wooguium(右歸飮), Soyosan(逍遙散加減), Jibakjihwangtang(知柏地黃湯加減), Esuntang(二仙湯), Ejihwan(二至丸加味), Sihogayonggolmoryutang(柴胡加龍骨牡蠣湯加減) are used. And the acupuncture points such as Conception Channel, the Spleen Channel, the Urinary Bladder Channel, the Kidney Channel which are related to the lower belly of woman.

  • The study was done to evaluate the aetiology, symptoms, and treatment with acupuncture about climacteric syndrome on literature. The results were obtained as follows. 1. The aetiologies of the climacteric syndrome are insufficiency of Chung-Im, outside evil, injury of the five emotions, Labor, exceeding of lust, blood heat, insufficiency of Yin(陰), gathering of phlegm. 2. The climacteric syndrome is to connected with conception vessel, the pulse of Spleen, and Liver. 3. The treatments of climacteric syndrome are nutrition of Kidney and Liver, that of Kidney heat, descending Yang(陽) of Liver, nutrition of blood of heart, having a comunication with Kidney and heart, nutrition of Spleen and Stomarch. 4. For treatment with acupuncture, they have been used the conception vessel, the pulse of Spleen, Bladder, and they use acupuncture points for nutrition of Yin(陰), paece of soul, nurition of blood of Spleen. 5. The acupuncture point in ear for treatment of climacteric syndrome are Pi-Jil-Ha(皮質下), Kyo-Gam(交感), Nae-Bun-Bi-Jeom(內分泌點). These acupuncture points are to connected with estrogen and Kidney functure.

A Study on the Relation Between Lung Atrophy Syndrome and Upper Wasting Thirst - Focusing on Huangdineijing and Jinguiyaolue - (폐위(肺痿)와 상소(上消)의 관계에 대한 소고(小考) - 『황제내경(黃帝內經)』과 『금궤요약(金匱要略)』을 중심으로 -)

  • Objectives : In this paper, lung atrophy syndrome[肺痿] in 『Huangdineijin(黃帝內經)』 and 『Jinguiyaolue(金匱要略)』 were compared, followed by examining its relation with upper wasting thirst[上消]. Also, ways in which psychological factors that contribute to lung atrophy syndrome could cause upper wasting thirst were studied. Methods : Verses from 『金匱要略·肺痿肺癰咳嗽上氣病脈證治』 and 『素問·痿論』 were analyzed based on various annotators's opinions to determine the cause and mechanism of lung atrophy syndrome and its relationship with upper wasting thirst. Results : In 『Jinguiyaolue(金匱要略)』, lung atrophy syndrome is described as the heat of the upper body entering the lungs to dry it out. The description in 『Suwen(素問)』 differs in that it accompanies atrophy symptoms, but the mechanism is the same. Lung atrophy syndrome in 『Jinguiyaolue』 could come from wasting thirst, while wasting thirst can be accompanied in deficiency caused by chronic lung atrophy syndrome. Heat in the lungs is caused by psychological factors where the person has lost its subject of possession or was unable to attain what was desired. When expanded to include heart atrophy syndrome[心痿] and lung atrophy syndrome[肝痿], the reason for upper wasting thirst could include immense sadness or excessive indulgence in pleasure due to unmet desires. Conclusions : Although diabetes and wasting thirst are not identical, application of wasting thirst pattern differentiation to diabetes treatment and management could lead to tailored treatment of each patient. Moreover, the five zhang pattern differentiation from the 『Suwen(素問)』 could increase treatment efficacy when applied to conditions caused by stress and emotional disorder, which are increasingly playing larger roles in causing wasting thirst, or diabetes.

Understanding the Phenomenon of "Clear Qi Below, Turbid Qi Above" with Reference to Symptom Patterns of the Sanghallon (Treatise on Cold Damage 傷寒論) ("청기재하(淸氣在下), 탁기재상(濁氣在上)"에 대한 고찰(考察) - 『상한론(傷寒論)』 병증(病症)과의 비교 -)

  • Objective : Describe the phenomenon of "clear qi below, turbid qi above" as found in the Somun Eumyangeungsangdaelon (Major Essay on Yinyang Resonances and Appearances 素問 陰陽應象大論) and compare this pattern with water-grain dysentery and flatulence symptom patterns in the Sanghallon (Treatise on Cold Damage). Method : Study the annotation of the Hwangjenaegyeong (Yellow Emperor's Internal Classic 黃帝內經) and compare the results with the Sanghallon's water-grain dysentery and flatulence. Conclusions and Results : The causes of water-grain dysentery and flatulence are associated with the ascending and descending properties of Yin and Yang. Additionally, these symptoms can also be caused by pathogenic heat, turbid pathogenic factors, and interruption of the movement of clear and turbid qi. Aspects of water-grain dysentery resemble several patterns found in the Sanghallon. If caused by a weakness of yang qi, it resembles Sayeoktang (四逆湯) syndrome. Weakness of spleen qi resembles Ijungtang (理中湯) syndrome. Flatulence is similar to fullness in the chest syndrome, which in the Sanghallon is caused by an obstruction of cold qi. If there is excessive cold, water-grain dysentery is similar to the syndrome of Gyeolhyung (結胸). If the qi is not scattered, deficiency syndrome is similar to Gyejigejagyaktang (桂枝去芍藥湯) syndrome and excess syndrome is similar to Mahwangtang (麻黃湯) syndrome. When flatulence is caused by fever in chest, it is similar to Chijasitang (梔子?湯) syndrome. When caused by heat and phlegm build up in chest, it is similar to Sipjotang (十棗湯) syndrome.
