슬관절 전치환술 후 발생한 감염에 시행한 관절경적 치료의 임상 결과 (Clinical Results of Arthroscopic Treatment for Infection after Total Knee Arthroplasty)

  • 목적: 슬관절 전치환술 후 발생한 감염에 대해서 시행한 관절경적 치료의 효용성과 치료 결과에 영향을 미치는 인자들에 대하여 알아보고자 하였다. 대상 및 방법: 슬관절 전치환술 후 발생한 감염에 대하여 관절경적 치료를 시행한 17예를 연구 대상으로 하였다. 혈액검사와 관절액 천자를 통해 감염을 확인한 후 관절경을 이용하여 변연 절제술과 활액막 제거술을 시행하고 항생제를 혼합한 생리 식염수를 사용하여 세척술을 시행하였다. 수술 후 정기적인 검사를 통해 치료의 실패 또는 재발 여부를 확인하고 수술 후 2년까지 감염의 재발이 없는 경우를 치료의 성공이라 판단하였다. 결과: 슬관절 전치환술 후 발생한 감염에 대하여 관절경적 수술로 치료받은 17예 중 13예에서는 1회의 관절경적 치료만으로 감염이 치료되었으나 4예는 감염이 지속 또는 재발되어 재수술을 시행하였다. 치료 결과에 영향을 미치는 인자들에 대해 분석해 본 결과 재수술을 시행한 군에서 관절경적 수술만으로 치료된 군보다 감염 증상 발현 후 수술까지의 기간이 더 길었다(p<0.05). 결론: 관절경적 수술을 이용한 슬관절 전치환술 후 발생한 감염의 치료는 적절한 환자의 선택, 철저한 변연 절제술과 세척술 및 적합한 항생제의 사용 등을 통해서 성공적인 결과를 기대할 수 있으나 감염이 의심된다면 가능한 빨리 수술을 시행해야 할 것으로 사료된다.

골다공증성 족관절 골절 (Osteoporotic Ankle Fracture)

  • The incidence of osteoporotic ankle fractures is increasing as the population ages. These fractures are becoming more common in clinical practice and require careful management because of the higher likelihood of developing complications than typical ankle fractures. The introduction of a method for measuring the ankle joint bone mineral density is a valuable tool for assessing the bone quality of the ankle joint. By evaluating the bone mineral density, healthcare professionals can better understand the extent of osteoporosis and the overall strength of the ankle joint. This information is crucial for determining the appropriate treatment for individual fractures. Several factors must be considered when deciding on the treatment for osteoporotic ankle fractures. These include the ankle joint bone mineral density, skin condition, any comorbidities the patient may have, and the patient's functional demands. Taking these factors into consideration allows healthcare providers to tailor the treatment plan to the specific needs and the circumstances of each patient. By applying the appropriate treatment, it is expected that the complications associated with osteoporotic ankle fractures can be minimized, and the prognosis for patients can be improved.

심부전으로 인한 호흡곤란 환자에게 시행한 침도침 시술 1례 보고 (A Case Report of Congestive Heart Failure Treated with Acupotomy Therapy)

  • Objectives : The purpose of this report is to report the effect of acupotomy for patients with congestive heart failure. Methods: We treated 1 patient who has dyspnea due to congestive heart failure with acupotomy. To check the effect and satisfaction of acupotomy we used New York Heart Association class(NYHA class), Modified Borg Scalw Dyspnea Index(Borg Index), Baseline Dyspnea Index(BDI), Five-point Likert scale. Results : After 1 month of treatment, the patient felt much better in breathing, and had better score in NYHA class, Borg Index, BDI. Conclusions : This report demonstrates that acupotomy therapy has useful effect on congestive heart failure, but the more cases and researches are needed.

  • With global aging, the number of patients with heart failure has increased markedly. Heart failure is a complex condition intricately associated with aging, organ damage, frailty, and cognitive decline, resulting in a poor prognosis. The relationship among frailty, sarcopenia, cachexia, malnutrition, and heart failure has recently received considerable attention. Although these conditions are distinct, they often exhibit a remarkably close relationship. Overlapping diagnostic criteria have been observed in the recently proposed guidelines and position statements, suggesting that several of these conditions may coexist in patients with heart failure. Therefore, a comprehensive understanding of these conditions is essential, and interventions must not only target these conditions individually, but also provide comprehensive management strategies. This review article provides an overview of the epidemiology, diagnostic methods, overlap, and prognosis of frailty, sarcopenia, cachexia, and malnutrition in patients with heart failure, incorporating insights from the FRAGILEHF study data. Additionally, based on existing literature, this article discusses the impact of these conditions on the effectiveness of guideline-directed medical therapy for patients with heart failure. While recognizing these conditions early and promptly implementing interventions may be advantageous, further data, particularly from well-powered, large-scale, randomized controlled trials, are necessary to refine personalized treatment strategies for patients with heart failure.

Succinylcholine Chloride로 근이완된 견에 있어서 Doxapram Hydrochloride에 의한 회복효과 (Analeptic Effects of Doxapram after Succinylcholine Treatment in Dog)

  • This study was performed to evaluate the effects of doxapram after succinylcholine treatment. Succinylcholine was administered intravenously at a dose rate of 0.07 mg per kg of body weight and then ten minutes after the injection of succinylcholine doxapram was administered intravenously at a dose rate of 2 mg per kg of body weight. The results obtained were as follows : 1. Recovery time in dog given doxapram after succinylcholine treatment was shortened comparing with control group. 2. The changes in respiratory rate revealed a maximal increase immediately after the injection of doxapram. Thereafter respiratory rate gradually decreased, and revealed normal levels 20 minutes after the injection of doxapram. 3. The changes in heart rate revealed a maximal increase immediately after the injection of doxapram. Thereafter heart rate gradually decreased, but remained above the levels of control group. 4. Although arrhysthmias were observed after treatment of succinylcholine, these were disappeared after doxapram treatment. And there was no another change on electrocardiograms.

Successful Surgical Treatment of Cardiac Complication of Graves Disease

  • Cardiac complications such as arrhythmia and heart failure are common in Graves disease. Early detection and proper treatment of hyperthyroidism are important because cardiac complications are reported to be reversible if the thyroid function is normalized by medical treatment. We report here a case of cardiac complication of Graves disease that was too late to reverse with medical treatment and required surgical treatment.

  • The goal of this treatment is the surgical-orthodontic eruption of impacted permanent teeth associated with a large dentigerous cyst in a preadolescent patient. Although enucleation of the entire cyst and the extraction of impacted teeth are common treatments, missing permanent teeth cause several problems in young patients. In this report, an 11-year-old female visited with the chief complaint of a large radiolucent lesion from the mandibular anterior area to the left mandibular posterior area. The permanent left canine and premolars were displaced toward the mandibular inferior border area. The extraction of infected deciduous teeth and marsupialization were performed. After 4 months, orthodontic buttons for forced eruption were applied to the impacted permanent teeth. The teeth emerged into the oral cavity 3 months after the orthodontic treatment. Although the root form was abnormal, there were no other pathogenic signs. The alveolar bone had a normal trabecular pattern and the teeth appeared to be well maintained at postoperative 24 months.

감염성 심내막염의 외과적 치료 (Surgical Treatment of Native Valve Endocarditis)

  • This paper reports 15 native valve endocarditis cases had surgical operation in the past 10 years at the department of Cardiovascular and Thoracic Surgery, Chonbuk National University Hospital. In this study, 10 cases out of 15 were in class I or II by the New York Heart Association functional classification. None of the cases had a history of taking addictive drugs. Five cases were congenital heart disease, three cases were rheumatic heart disease and two cases were degenerative heart disease. Thus 10 cases had the underlying disease. All cases had antibiotics treatment for 3 to 6 weeks before operation. In the culture test, only four cases were positive in the blood culture and one case was positive in the excised valve culture. Organisms on blood and valve culture were Streptococcus epidermis, Streptococcus viridans, Staphylococcus aureus and Staphylococcus epidermidis. In the 10 cases without ventricular septal defect, the aortic valve was involved in four, mitral in four, both in two and involved valves in the 5 cases with ventricular septal defect were tricuspid in three, pulmonic in two. Eight cases had operation because they showed moderate congestive heart failure due to valvular insufficiency and vegetation with or without embolism. Seven cases had operation because they showed persistent or progressive congestive heart failure and/or uncontrolled infection. Five cases with ventricular septal defect underwent the closure of ventricular septal defect, vegetectomy and leaflet excision of the affected valves without valve replacement. In the cases without ventricular septal defect, the affected valves were replaced with St. Jude mechanical prosthesis. Postoperative complications were recurrent endocarditis in two, embolism in one, allergic vasculitis in two, spleen rupture in one and postpericardiotomy syndrome in one. At the first postoperative day, one case died of cerebral embolism. At the 11th postoperative month, one case died of recurrent endocarditis and paravalvular leakage in spite of a couple of aortic valve replacement. In the survived cases[13 cases in this study , all cases but one became class I or II by the New York Heart Association functional classification.

Chemical Pleurodesis Using Tetracycline for the Management of Postoperative Pneumothorax Recurrence

  • Background: Although there is no obvious recommendation for the management of pneumothorax recurrence after surgery, chemical pleurodesis with tetracycline has been applied as a significant treatment approach. The objective of this study was to evaluate the effectiveness of chemical pleurodesis with tetracycline for the management of postoperative recurrence of primary spontaneous pneumothorax (PSP). Methods: We retrospectively analyzed patients who underwent video-assisted thoracic surgery (VATS) as therapy for PSP at Hallym University Sacred Heart Hospital from January 2010 to December 2016. Patients who had ipsilateral recurrence after surgery were included in this study. Patients who underwent pleural drainage with chemical pleurodesis were compared with patients who only underwent pleural drainage. Results: In total, 932 patients who underwent VATS for PSP were analyzed, and ipsilateral recurrence after surgery occurred in 67 patients (7.1%). The treatment modalities for recurrence after surgery were observation (n=12), pleural drainage alone (n=16), pleural drainage with chemical pleurodesis (n=34), and repeated VATS (n=5). Eight of the 16 patients (50 %) treated with pleural drainage alone had recurrence again, while 15 of the 34 patients (44.1%) treated with pleural drainage and chemical pleurodesis experienced further recurrence. Chemical pleurodesis with tetracycline did not show a meaningful difference in the re-recurrence rate in comparison with pleural drainage alone (p=0.332). Conclusion: Chemical pleurodesis with tetracycline for the management of postoperative recurrence of PSP was not effective. Further research is required to identify alternative drugs that can significantly decrease the re-recurrence rate.
