돌발성 난청을 동반한 이명 환자 1례에 대한 증례보고 (A Clinical Case Study on the Tinnitus with Sudden Sensorineural Hearing Loss)

  • Tinnitus is noise from ear or head without any external sound stimulation and can cause hearing difficulties, psychologenic disturbances or many difficulties in everyday life. the etiology has yet been discussed and the management is also very difficult. Sensorineural hearing loss is defined as a sudden hearing impairment which was develope over a period of hours to days. I report one tinnitus case which accompanied with sudden sensorineural hearing loss. In the early and acute stage which judged as Dam-Wha, herbal treatment with Tong-myeong-ri-gi-tang was efficacious on the tinnitus. In the convalescent stage and judged as Sin-heo, herbal treatment with Ja-sin-tong-i-tang was efficacious on the tinnitus that accompanied with sudden sensorineural hearing loss.

Hearing Rehabilitation with Combined Electroacoustic Stimulation and Ossiculoplasty

  • We report a case of hearing rehabilitation following combined cochlear implantation and ossiculoplasty. A 71-year-old patient visited the clinic for right-sided mixed hearing loss. We targeted neural and conductive components, performing two different operations simultaneously. At two months post-operative, the patient showed satisfactory results with respect to hearing threshold and speech comprehension. Our experience suggests that careful evaluation of patients and consideration of the diverse array of available treatment strategies can be used to provide personalized rehabilitation with maximal effectiveness. To the best of our knowledge, this is the first report to take such an integrated approach to treat hearing impairment and is thus likely to have clinical importance for otologists.

A Case of Idiopathic Sequential Profound Bilateral Sudden Sensorineural Hearing Loss Occurring 37 Days After Unilateral Presentation

  • Sudden sensorineural hearing loss (SSNHL) is a common disorder; however, sequential, bilateral presentation of the disease is rarer than unilateral presentation. Clinical otologists usually focus on treating the side with impaired hearing when patients first present with unilateral SSNHL, and therefore, may not warn patients of the possibility of subsequent hearing impairment in the contralateral ear. Furthermore, it is professionally discouraging when a patient presents with profound, sequential SSNHL after initial treatment. This may adversely impact the doctor-patient relationship, even if the patient is offered the best possible care from their first visit. Herein, we report the case of a patient with profound, idiopathic, bilateral SSNHL with a time interval of 37 days between involvement of both ears. Even though high-dose steroids were administered intraorally and intratympanically, the patient's hearing was not restored, and the patient eventually required bilateral cochlear implant surgery. Our report demonstrates that sequential, profound, bilateral SSNHL may manifest without any specific signs.

Association between cadmium exposure and hearing impairment: a population-based study in Korean adults

  • Background: The present study aimed to evaluate the clinical association between cadmium exposure and hearing impairment among the Korean population. Methods: This retrospective cross-sectional study used the data obtained from the Korean National Health and Nutrition Examination Survey were used for our study. Finally, 3,228 participants were included in our study, which were then divided into quartiles based on their blood cadmium levels: first quartile (1Q), second quartile (2Q), third quartile (3Q), and fourth quartile (4Q) groups. The hearing thresholds were measured using an automatic audiometer at 0.5, 1, 2, 3, 4, and 6 kHz. Hearing loss (HL) was defined as >25 dB average hearing threshold (AHT). Results: All the groups had 807 participants each. The area under the receiver operating characteristic curves of cadmium level for HL were 0.634 (95% confidence interval [CI], 0.621-0.646). The participants in the 4Q group had higher Low/Mid-Freq, High-Freq, and AHT values than those in the other groups in the multivariate analysis after adjusting for confounding factors. The logistic regression showed that the OR for HL per $1{\mu}g/L$ increase in cadmium was 1.25 (95% CI, 1.09-1.44; p=0.002) on the multivariate analysis. Moreover, the multivariate logistic regression analyses revealed that the participants in the 4Q group exhibited a 1.59-, 1.38-, and 1.41-fold higher odds for HL than those in the 1Q, 2Q, and 3Q groups, respectively. Conclusion: High cadmium level quartile was associated with increased hearing thresholds and HL among the Korean adult population.

노인의 감각장애와 기능상태에 관한 연구 (Effects of the Sensory Impairment on Functioning Levels of the Elderly)

  • The purposes of this study were to describe the level of vision and hearing impairments, depression and functional capacity, among Korean institutionalized elderly and to examine the relation-ship between sensory impairments, depression, and functional capacity in these people. The final pupose was to test the cognitive function path model using sensory competencies as predictors. A convenience sample of thirty nine male and 90 female subjects with a mean age of 80.5 were the subjects of this study. The subjects were tested for cognitive function, and vision and hearing impairments. Physical function and social function were measured by observation of designated task performance by the subjects. Their level of de-pression was measured using a Geriatric Depression Scale administered through an interview. Individual subjective ratings of hearing and vision were marked by the subjects, on a ladder scale. The results of the study showed that 48.8% of the subjects had a hearing impairment, 63.5% had a vision impairement, and 36.4% had both a vision and hearing impairement. The four sensory groups (no sensory impairement, hearing impairement, vision impairement, hearing and vision impairement) were tested for differences in depression, physical function, social behavior and cognitive function. The only significant difference that was found was in cognitive function, between the no sensory impairement group and the hearing and vision impairement group(F=3.25, P<.05), Subjective ratings of hearing showed a significant correlation with cognitive function(r=.34, p<.001) and with social behavior(r=.31, p<.001). There was no correlation between subjective vision ratings and cognitive function or social behavior. However there was a significant correlation between vision and hearing(r=.49, p<.001). There was also a significant negative correlation between age and vision(r=-.21, p<.01) and between age and hear-ing(r=-.34, p<.001). There was a significant correlation between depression and physical function (r=-.32, p<.001) but there was no correlation between depression and cognitive function or social behavior. Based on the literature review and the result, this study, a path model of sensory competence-> cognitive function- >social behavior was developed and tested : Perceived vision and perceived hearing were the exogenous variahles and cognitive function and social behavior were the endogeneous variables in the model. The path analysis result demonstrated an accept-able fit (GFI=.997, AGFI=.972, X$^2$=.72 (p=.396), RMSR=.019) between the data and the model. There was a significant direct effect($\beta$=.38) of perceived hearing on cognitive function. There was a significant direct effect ($\beta$=.32) of cognitive function on social behavior. The total effect of hearing on social behavior was $\beta$=.32 including the indirect effect ($\beta$=.12) . However perceived vsion had little effect ($\beta$=-.08) on cognitive function. The result of path analysis confirms that hearing levels influence cognitive function, and both hearing and cognitive function levels influence social behavior. However, vision has little effect on cognitive function or on social behavior. For the next study, a combined model of the pre viously developed environment - >depression- > physical and social function model, and the present cognitive function model, should be tested to further refine the functional capacity model. There also a need for longitudinal study of functional capacity and sencory competence in order to better understand how declining sensory competence influences functional capacity and how it effects in-creasing dependency and nursing needs in the elderly.

일반인의 청력이 삶의 질에 미치는 영향 (The Influence of Hearing Ability of Ordinary People on the Quality of Life)

  • Background: In this research, we conducted an analysis to grasp the relationship between hearing and quality of life and to prepare effective troubleshooting. Methods: In this study, we analyzed using the data of the 6th term of the National Health and Nutrition Survey of the Disease Management Headquarters. And, out of 7,380 people, 3,598 adults over the age of 40 years who participated in the hearing test were selected as final research subjects and analyzed. Results: It was confirmed that the result of the analysis affects the quality of life based on hearing. The quality of life associated with hearing loss was unpleasant, motor ability, anxiety/depression, and everyday life affected in order of self management. And factors having an effect on the quality of life of the study subjects were found to be gender, elderly, unmarried, low hearing, low household income, absence of economic activity, and low teaching standards. Conclusion: The conclusion is that strengthening education for realistic management and prevention rather than treatment for unconditional hearing impairment will overcome low self esteem and loneliness and alleviate any uncomfortable life that people with hearing loss give. It is expected to be possible.

부모 듣기 지도 프로그램이 청각장애아동의 언어 능력과 의사소통 행동에 미치는 영향 (The Effect of Parent Involvement Auditory Training Program on Communication Ability of Children with Hearing Impairments)

  • The purpose of this study is to examine the effects of the parents listening guidance program, which allows the parents to understand their hearing impaired children and how to listen at home, on the communication skills of the hearing impaired children. The research subjects were 3 hearing impaired children who did not accompany with the intellectual, emotional and behabioral problems, and the listening guidance has been performed for their parents for 3 months through the listening guidance program. The changes in the communication skills in the hearing impaired children were observed comparing before and after the education. In the results, first, the receptive language skill of the hearing impaired children was improved after than before the parents listening guidance. Second, the expressive language skill of the hearing impaired children was improved after than before the parents listening guidance, too. Third, in the communication behavior of the hearing impaired children, the phonation and the speech production were increased together with the gesture after the parents listening guidance. In conclusion, it is deemed that the parents listening guidance program would have positive influence on the communication behavior of the hearing impaired children.
