영양교육에 의한 노인 영양증진전략연구 : 효과분석 (Strategies of Improve Elderly Nutrition through Nutrition Education : Evaluation of the Effectivenss of the Program)

  • The increased number of elderly in Korea presents great challenges for community nutrition programs. Strategies to enhance elderly nutrition are needed, as well as dietary advice that are reality-based and tailored to the need of the elderly. The purpose of this study was to evaluate the overall effectiveness of an elderly nutrition improvement program(ENIP), by assessing changes in nutrition knowledge, attitudes, and behavior after program completion. ENIP was conducted in Suwon for 4 months in 1998 at 5 centers for the elderly. The program provided individual nutrition counseling and brief biweekly group education sessions. To stimulate participation and motivation, the elderly in two centers were served free milk packs(200ml) three time a week during the program(milk+education group), and the elderly in 3 other centers were served yoghurt on the program day(education group). Altogether, 102 subjects finished the program(milk+education group 67 ; education group 35). Data about nutrition knowledge, dietary attitudes, diet records(24 hour recall) and dietary habit was collected before(baseline) and after the program(follow-up). The Drop-out rate for the milk+education group(2.9%) was significantly lower than that for the education group(30.0%)(p<.001). The Nutrition Knowledge fo females milk+education group increased significantly after the program, but no significant differences were observed in other groups. Over 60% of all the elderly showed positive responses to the healthy eating attitudes. Energy, protein, vitamin A, riboflavin, Calcium and Potassium intakes improved in the male milk+education group. The Vitamin C intake improved in the male education group. Energy and vitamin A intakes improved in the female milk+education group and energy, vitamin A, thiamin, riboflavin, niacin, Ca nad P in the female education group. A stepwise multiple regression analysis was performed to examine the effects of food intake changes on overall nutrient quality. For all elderly, the fish intake explained 12.6% of the variance, followed by candy intake and vegetable intake(model $R^2$=19.6%). Dietary habits such as meal time regularity and salty food reduction were markedly improved in both groups. Sixty-one percent of the milk+education subjects stated their participation was voluntary and active, while 51% of education subjects did. Sixty four percent of the subjects stated their interest on diet increased remarkably through program participation. Considering the results, it is conjectured that ENIP had a great impact on nutrient intake, dietary attitudes and habits. Milk supplement showed no direct effects on elderly nutrition improvement, but it encouraged the program by reducing drop-out rates and inducing voluntary participation. Therefore milk supplement could enhance the community nutrition education program. Moreover it is strongly suggested that the ENIP be expanded to other communities.

대학생의 건강교육 경험 및 요구 (Experience of and Needs for Health Education in University Students)

  • This study examined the experience of health education and assessed the needs for health education in the students attending a university located in Inchon. The primary subjects were 749. who were selected from each college by using systematic random sampling. Of these students. 457 subjects responded to the self-reported questionnaire. revealing the response rate of $61.01\%$. As for the analysis of the data. percentages. chi-square test. and correlation and regression analyses were adopted. The experience rate of health education was 39.02% and the rate was higher in women than in men. The students experienced the education about smoking and first-aid most frequently. whereas they experienced stress management much less frequently. Stress management was given the highest priority among the subjects of health education which they proposed to need. Physical health states of the students were better than their mental health states. The women tended to eat meals irregularly compared to men. Those students living not with their family nor with their relatives were more likely to eat meals twice a day compared to those who live at their homes. Those who recognized the need for health education showed the willingness to join health education to be given. The willingness was, also observed in those who had better self-evaluated mental health. regardless of the recognition of the need for health education. These findings revealed both objective and subjective needs for educating management of mental health. Education on healthy eating habit should be given particularly to woman students and those who do not live with family nor with relatives. When educating for the management of mental health. there should be strategies to facilitate the participation of the students who do not have good mental health.

국민건강증진을 위한 영양 부문의 2000년 잠정목표와 전략 (Provisional National Nutrition Targets and Strategies for Health Promotion of Koreans)

  • This study attempts to propose a priority of national nutrition targets and strategies for health promotion by the year 2000 in Korea, as a part of the task set for national health promotion objectives and strategies. Among all of the important health issues raised, ten were chosen, nutrition was one priority area. In the first part, the current status of the nutrition-related health problems and risk factors are reviewed, in conjunction with the newly arisen health phenomena, such as changes in prevalence of lifestyle disease and causes of death, changes of food consumption patterns in our country. In the second section this study suggests six feasible national nutrition targets, eight implementing strategies and current major tasks on the basis of the assessment of present status and in consideration of the other health promotion goals and strategies, with reference to that of other developed countries. The main targets and strategies are suggested as follows ; Firstly, the national nutrition monitoring and surveillance system should be established for identifying the nutritional problems for our people, and current National Nutrition Survey is a strong need for improvement to a more comprehensive and reliable one. Secondly, effective administrative mechanism should be operation at national level for the development of nutrition policy. Ministry of Health and Welfare (MOHW) as well as local health department must be remarkably renewed and strengthened the nutrition section. And it is recommended that MOHW organize and operate “The Council of Nutrition”, in which all government authorities related with foodstuffs and nutrition would incorporated. The Council of Nutrition would act as an adjustor as well as a coordinator in nutrition related policy-making. Thirdly, healthy eating pattern will be supported by activities of introducing a nutrition labeling for providing consumers with the necessary information and skills for food selection. Fourthly, nutrition education, and nutrition intervention programs will be carried out in various settings such as health centers, schools, and clinical fields and workplace. Fifthly, the current dietary guidelines shall be continuously improved in detail, and publicly circulated to particular levels of people by age group and by health condition. And finally, researches and epidemiological studies particularly in regard to diet for development of chronic diseases are needed for more investigation and up-to-date national health and nutrition data should be collected with the support and cooperation from the various medical professional teams . 전북지역 청소년 운동선수 대상 하루 필요 식품군 수 맞춤 점심급식을 병행한 영양교육 효과 (Effects of Nutrition Education Providing School Lunch by Personalized Daily Needed Food Exchange Units for Adolescent Athletes in Jeonbuk Province)

  • Objectives: The purpose of this study was to investigate effects of nutrition education providing school lunch by personalized daily needed food exchange units using Food Exchange System for adolescent athletes. Methods: The subjects were 60 sports high school students (educated group, 30 vs. noneducated group 30). Nutrition education was provided for 4 weeks (40 min/lesson/week). In addition, personalized school lunch was served for 4 weeks, nutrition education period. The personalized lunch were provided Food Exchange Units according to personalized daily needed energy. The lessons were '5 Major nutrients, functions and foods', 'My daily needed energy and food exchange units by Food Exchange System', 'My meal plan by food exchange units according to my daily needed energy' and 'Smart choice of healthy snacks and eating outs'. After nutrition education, we examined the differences in anthropometric characteristics, nutrition knowledge, dietary attitude and dietary intake between the educated and the non-educated group. Results: We observed improvements in lean body mass in the educated group. With regard to nutrition knowledge, there were improvements in 'Functions of vitamins', 'Functions of minerals', 'Foods of fat', 'Foods of vitamin', and 'Foods of mineral' in the educated group. In relation to dietary attitude, there were improvements in 'Taking a meal with family and friend', 'Taking a meal at ease', 'Taking a meal with kimchi and vegetables', 'Taking a meal with three kinds of side dishes', 'Priority of choosing snacks' and 'Type of snacks' in the educated group. With regard to dietary intakes according to Dietary Reference Intakes for Koreans, there were improvements in intakes levels of fiber, vitamin A, vitamin C, folate, calcium, iron and zinc. The index of nutrition quality, as indicated by nutrition adequacy ratio also improved in the educated group. Conclusions: These results showed that a nutrition education program providing education lessons and personalized school lunch by food exchange units according to daily needed energy showed positive changes in nutrition knowledge, dietary attitude and dietary intake of adolescent athletes. Nutrition education program providing personalized school lunch by Food Exchange Units may improve dietary behaviors and dietary intakes of adolescents.

다문화가정 결혼이주여성의 출신 국가에 따른 식생활행동 조사 (Dietary Behavior of Marriage Migrant Women according to Their Nationality in Multicultural Families)

  • Objectives: Nowadays, the multicultural families make up significant portion of Korean population and communities. Successful re-settling in a new country can be difficult, particularly when there are disparities in dietary behavior compared to home country. The objective of the study was to investigate the dietary behavior of marriage migrant women according to their nationality in multicultural families. Methods: The primary research was conducted targeting 94 marriage migrant women who came from China (40.4%), Vietnam (26.6%), and other countries except for Japan (33.0%). We investigated their dietary behavior, such as eating attitude and food choice behavior for Korea acculturation. We also studied dietary behavior among 14 selected subjects who had high level of integration and assimilation acculturation patterns by administering the Focus Group Interview (FGI). Results: The multicultural families had more integration acculturation patterns, which could have been influenced by their nationality. Vietnamese origin has the highest cultural adaptation as marginalization pattern. The common types of Korea acculturation were integration ($3.03{\pm}1.08$), separation ($3.10{\pm}0.59$), marginalization ($3.10{\pm}0.58$), followed by assimilation ($2.84{\pm}0.51$). There were significant differences in the four types of acculturation by marriage immigrant women's country of origin (p<0.05). According to dietary behavior, 'eliminating hunger' was the most important value in a meal. Chinese marriage migrant women, who had higher level of food intake attitude significantly, also considered 'being healthy' an important value. Regarding food choice behavior, Vietnamese had lowest frequency of homeland food intake. Most of marriage immigrant women were satisfied with the Korean food, and need for education was very high with interest for cooking, good nutrition, and managing their children's dietary life. Conclusions: Coping with a change in dietary behavior is one of the biggest transitional difficulties, and family members may need support to find their familiar food items and to continue their cultural food choice behavior in the local areas. Further researches with quantitative and qualitative analysis are needed to understand the effect of dietary behavior for acculturation in multicultural families.

모바일 개인건강기록(Personal Health Records: PHR) 어플리케이션의 이용이 소비자 건강행태에 미치는 영향 (The Effects of a Mobile Personal Health Records (PHR) Application on Consumer Health Behavior)

  • 본 연구는 모바일 개인건강기록(Personal health record: PHR) 어플리케이션의 장단점을 분석하고 그 어플리케이션의 사용이 소비자 건강정보행태에 미치는 영향을 파악하고자 하였다. 본 연구를 위해 총 27명의 대학생들이 3개월 동안 PHR 어플리케이션을 사용하였으며, 그 경험을 바탕으로 서면인터뷰를 실시하였다. 내용분석 결과, 시각적 인터페이스를 통한 예방 차원의 건강관리 및 건강 향상을 위한 동기부여와 구체적 가이드라인의 제공, 가족과의 데이터 공유 및 간병인(caregiver)의 환자 건강관리지원, 무엇보다도 환자와 의료전문가와의 상호작용 강화 등이 PHR의 혜택으로 부각되었다. 반면 근력운동과 같은 콘텐츠의 미비 내지는 타 기기와의 호환성 미흡 등이 문제점으로 지적되었다. PHR의 도움으로 건강을 향상시키고자 하는 소비자들은 감정적 지원을 받는가 하면, 수면장애극복, 금주, 금연, 그리고 체중감량 및 식습관의 변화 등 건강행태를 변화시키고자 하는 동기부여가 생겼으며, 실제로 건강행태의 변화를 성취함으로써 자기효능이 증가하기도 하였다. 본 연구는 국내 연구에서는 도입단계인 PHR에 대한 연구를 시도했다는 것에 학문적 의의가 있으며, 헬스케어분야의 패러다임 변화에 대응하기 위한 건강 및 의료정보서비스를 모색하는데 통찰력 있는 시사점을 제공한다.

유아의 점심 식사장소에 따른 영양소 및 식품 섭취량의 비교 : 제5기 국민건강영양조사 자료를 이용하여 (Comparison of nutrients and food intakes of young children according to lunch places: based on the fifth Korea National Health and Nutrition Examination Survey)

  • 본 연구는 유아의 점심 식사장소에 따라 영양소와 식품섭취량에 차이가 있는지를 조사하였다. 제5기 (2010 ~ 2012년) 국민건강영양조사 원시자료를 이용하여 개인별 24시간 회상에 의한 식품섭취조사 항목 중에서 점심 식사장소가 유아원/유치원일 경우 보육기관 점심식사군으로, 점심 식사장소가 가정과 이웃집/친척집일 경우 가정 점심식사군으로 나누어 복합표본설계 요소를 반영한 통계분석을 실시하였다. 대상자수는 1~2세에서 보육기관 점심식사군 179명, 가정 점심식사군 367명이었고, 3 ~ 5세에서 보육기관 점심식사군 549명, 가정 점심식사군 225명이었다. 연령군별 하루 총에너지섭취량, 점심과 저녁 식사로부터 에너지 섭취량은 두 집단 간 차이가 없었으나, 아침 에너지섭취비율이 보육기관 점심식사군에서 낮은 반면에 간식 에너지섭취비율은 더 높았다. 3 ~ 5세 유아에서는 나트륨, 칼륨 및 비타민 C 섭취량이 보육기관 점심식사군에서 많았으며, 리보플라빈 섭취량은 가정 점심식사군에서 유의하게 많았다. 점심으로부터 섭취한 식품군을 비교한 결과 1~2세 보육기관 점심식사군에서 채소류 섭취량이 유의하게 많은 반면에 과일류, 난류, 우유류 섭취량은 가정 점심식사군에서 많았다. 3 ~ 5세 보육기관 점심식사군의 점심으로부터 채소류, 두류, 어패류 섭취량이 유의하게 많은 반면에 우유류 섭취량은 가정 점심식사군에서 많았다. 3~5세 유아들이 보육기관 점심으로부터 채소류, 두류, 어패류를 더 많이 섭취하고 칼륨과 비타민 C를 많이 섭취하여 보육기관 점심이 영양소 균형과 식품 다양성 측면에서 가정 점심보다 나은 것으로 판단된다. 결론적으로 유아의 성장 발육을 위한 적절한 영양 공급은 물론, 올바른 식습관 형성을 위해서 보육기관 급식은 물론, 가정 식사에서도 채소류, 생선류 등 다양한 식재료를 활용하는 노력이 요구된다.

Ca과 Vitamin D 보충이 폐경 이후 여성의 뼈대사에 미치는 영향 (The Effect of Ca and Vitamin D Supplementation on Bone Metabolism in Postmenopausal Women)

  • This study was designed to investigate the effects of Ca and/or vitamin D supplementation for 53 weeks on bone metabolism in postmenopausal women. The subjects were healthy 18 women aged from 59 to 69 years old. They were divided into three groups : placebo, Ca(1000mg/day) supplementation and Ca(1000mg/day) with vitamin D(12.5$\mu\textrm{g}$/day) supplementation. During the experimental periods except for metabolic studies, the subjects ate their usual diets and the use of drugs as well as excessive exercise was prohibited. Metabolic studies were conducted in the 1st week and in the 53rd week of the experimental periods. The subjects ate experimental diets which consisted of 1787.3kcal, 69.6g of protein, 561.5mg of Ca and 1078.6mg of P daily during both of the metabolic study periods. The results were summarized as follows; 1) Bone density of the second lumbar spine and trochanter measured after treatment decreased significantly in control group as compared with pre-experimental level(p<0.05). On the contrary, bone density of femoral neck and Ward's triangle in Ca group and the second lumbar spine in Ca.Vit D group increased significantly after treatment. 2) Serum PTH and calcitonin levels did not show any significant differences among groups before and after treatment. But serum PTH level increased significantly in all groups after treatment(P<0.05). 3) Serum Ca and P levels did not show any significant differences among groups before and after treatment. But serum Ca level increased significantly in all groups after treatment (P<0.05) and serum P level decreased significantly in Ca.Vit D group after treatment(P<0.05). 4) Mean 24-hours fecal Ca excretion of Ca group was the highest in the 1st week of treatment(P<0.01), and that of control group was the lowest in the 53rd week of treatment(P<0.01). Fecal Ca excretion increased significantly in control and Ca.Vit D group in the 53rd week of treatment(P<0.05). Urinary Ca excretion did not show any significant differences among groups in the 1st and 53rd week of treatment, but that of Ca.Vit D group was the highest the 1st week of treatment(P<0.01). In the 53rd week of treatment Ca and Ca.Vit D group showed positive Ca balance, but control group showed negative Ca balance. The above results showed that it will be difficult to prevent degenerative bone loss without Ca and/or vitamin D supplementation in postmenopausal women eating Korean usual diets.

Skipping breakfast is associated with diet quality and metabolic syndrome risk factors of adults

  • The aim of the present study was to assess the effects of skipping breakfast on diet quality and metabolic disease risk factors in healthy Korean adults. Subjects included 415 employees (118 men, 297 women; 30-50 years old) of Jaesang Hospital in Korea and their acquaintances. Data collected from each subject included anthropometric measurements, 3-day dietary intake, blood pressure, and blood analyses. The subjects were classified into three groups based on the number of days they skipped breakfast: 'Regular breakfast eater', 'Often breakfast eater', or 'Rare breakfast eater'. Participants in the 'Rare breakfast eater' group consumed less rice, potatoes, kimchi, vegetables, fish and shellfish, milk and dairy products, and sweets than did participants in the other two groups (P for trend <0.05) and ate more cookies, cakes, and meat for dinner (P for trend <0.05). Participants in the 'Rare breakfast eater' group consumed less daily energy, fat, dietary fiber, calcium, and potassium than did participants in the other groups (P for trend <0.05). The percent energy from carbohydrates was lower and fat intake was higher in the 'Rare breakfast eater' group than in the other groups (P for trend <0.01). When diets were compared using the Acceptable Macronutrient Distribution Range for Koreans, 59.1% of subjects in the 'Rare breakfast eater' group consumed more energy from fat compared with the other two groups (P <0.005). According to the Estimated Average Requirements for Koreans, intake of selected nutrients was lower in the 'Rare breakfast eater' group than in the other two groups (P <0.05). The risk of elevated serum triglycerides was decreased in the 'Rare breakfast eater' group (OR, 0.3 [0.1-1.0], P for trend=0.0232). We conclude that eating breakfast regularly enhances diet quality, but may increase the risk of elevated serum triglycerides.

청소년의 우유섭취 증진방안 연구(II) -중.고등학생의 식태도, 간식빈도, 신체활동 및 교내판매시설과 우유 섭취 빈도와의 상관관계- (A Study on the Promotion of Adolescent's Milk Consumption (II) -Relationships of Adolescent's Milk Intake Frequency with Food Attitude, Snacking Frequency, Physical Activity and School Vending Facilities-)

  • In order to investigate the ecological factors affecting milk intake frequency of adolescents, the questionnaire survey was conducted with 929 middle and high school students living in Chungnam urban area through October and November 2004. The subjects consumed milk 8.6$\pm$6.7 times per week, other beverages 4.4$\pm$4.1 times per week and drinking water 3.7$\pm$2.1 cups per day. Of the students 77.3% took balanced meals, 72.8% ate regularly and 36.2% ate adequate amount. About 61% had breakfast everyday and nearly a half students snacked once a day. Nutritional knowledge scores about milk was 7.2$\pm$1.7 and milk attitude scores was 28.1$\pm$6.5. The subjects spent daily 1.8$\pm$1.1, 1.9$\pm$1.1, and 1.0$\pm$0.5 hours for computer use, TV watching and exercise, respectively. Milk intake frequencies were positively correlated with excercise, snack frequency, meal balance and regularity, breakfast frequency, food attitude score and milk preference, while showed negative correlationships with TV watching and computer use. Intake frequency of fruit-tasted and chocolate milk showed inverse correlation with nutritional knowledge. Subjects without either school store or vending machine took milk more frequently than those with one or both did. Of the correlated variables, milk preference was the most important influencing factor to milk intake frequency according to the stepwise linear regression analysis, which presented other 5 important influencing factors as food attitude, school vending facilities, excercise, snacking frequency and watching TV. In conclusion, the improvement of milk preference is the most important and effective way to promote milk consumption in adolescents. The favorite ways of drinking milk, nutritional benefit of milk, healthy beverage and good snacking should be taught in nutrition education. Also physical activities should be recommended to students rather than watching TV, computer use and vending facilities selling soft drinks should be limited to be established inside school.
