청소년을 위한 연세 라이프스타일 프로파일(Yonsei Lifestyle Profile)의 신뢰도 및 타당도 연구 (Verifying the Reliability and Validity of the Yonsei Lifestyle Profile for Adolescents)

  • 목적 : 본 연구는 기존에 성인 대상으로 개발된 연세 라이프스타일 프로파일(YLP)의 청소년 대상 적합성을 신뢰도 및 타당도 검증을 통해 확인하고자 하였다. 연구방법 : 국내 중·고등학생 436명을 대상으로 연세 라이프스타일 프로파일(YLP)의 신뢰도와 타당도를 평가하였다. 내적 일관성 분석은 크론바흐 알파(Cronbach's α)와 스피어만-브라운(Spearman-Brown) 계수를 통해 분석하였다. 동시타당도 분석은 한국청소년 신체활동 설문지(KYPAQ), 한글판 아동·청소년 참여 척도(CASP), 청소년을 위한 영양지수(NQ-A)와의 피어슨 상관계수(Pearson Correlation Coefficient)를 통해 분석하였다. 결과 : 신뢰도의 경우, 연세 라이프스타일 프로파일(YLP)의 문항 전체 내적 일관성은 Cronbach's α = .811, Spearman-Brown 계수 = .779로, 높은 신뢰도를 나타내었다. 타당도의 경우 신체활동(r = .555, p < .01), 활동참여(r = .177, p < .01), 식습관(r = .633, p < .05) 모두 유의미한 양의 상관관계를 보여, 연관성 척도들과의 일관된 상관성을 확인하였다. 결론 : 본 연구는 청소년 대상 연세 라이프스타일 프로파일(YLP)을 분석하여 신체활동, 활동참여, 식습관 영역에서 높은 신뢰도와 유의미한 타당도를 확인하였다. YLP는 청소년의 건강한 라이프스타일을 평가하는 신뢰할 수 있는 도구로 사용될 수 있음을 시사한다.

Factors Related to Dietary Fiber Intake Among Sixth Grade Elementary School Children in Daejon City

  • This study was conducted to investigate variables related to dietary fiber intake among sixth grade children in an elementary school in Daejon city. One hundred and forty-seven children completed a questionnaire for determining their socioeconomic background and their food habits. Anthropometric measurements were taken, and a 24-hour diet recall method was used to collect three-day food intakes. The socioeconomic status of the children's families belonged to the upper middle class; 53.0% of their fathers and 25.8% of their mothers had completed college or higher degrees. 27.9% of the mothers had jobs, including part-time jobs. Approximately 30% of the children skipped breakfast, 66.4% of the children preferred animal foods to plant foods, and 52.4% of the children preferred green vegetables to yellow or pale vegetables. Grilled meat dishes, such as Grilled beef rib with seasoning, Bulgogi, grilled pork belly and beef steaks, were the most popular types of food eaten outside home by the children. Daily dietary fiber intake was 14.5 g in boys and 14.5 g in girls, and these intakes are low compared to the standard guidelines. The average intakes of energy and protein of the children were 84.5% and 114.0% of the Korean Recommended Dietary Allowances (RDAs), respectively. Besides energy, riboflavin, iron and calcium intakes were below the RDAs, and especially calcium intake was only 50% of the RDAs. On the other hand, thiamin, niacin, ascorbic acid, protein and phosphorus intakes exceeded the Korean RDAs. Family income or the children's body mass index (BMI) was not directly related to dietary fiber intakes. However, higher dietary fiber intakes tended to be related to higher intakes of green vegetables and fruits. Children with higher dietary fiber intake tended to prefer plant foods to animal foods. Energy and most nutrients, except heme iron and retinol, showed positive relationships with dietary fiber intake. Especially potassium and plant origin protein and calcium were highly correlated with dietary fiber intakes(r>0.6). From these results, it is concluded that dietary fiber intakes of these sixth grade elementary school children were less than the standard reference and it is anticipated to decrease further in the future with increased incomes. Therefore, increased intakes of dietary fiber by elementary school children should be promoted through nutrition education, together with the development of cooking methods and recipes utilizing green vegetables and fruits.

농촌 국민학교 아동의 혈압, 짠맛에 대한 역치, 최적 염미도, 뇨 중 배설성분 및 혈압에 관한 영양지식 (Blood Pressure, Salt Threshold, Salt Preference, Urinary Excretions and Nutrition Knowledge About Blood Pressure of Elementary School Children in Rural Area)

  • The purpose of this study was to observe the relationship of dietary factors to blood pressure in 5th and 6th grade school children. Salt threshold, salt preference and nutrition knowlege about blood pressure were tested. Twenty-four hour urines were collected for the measurements of the volume and concentrations of sodium, potassium, chloride, calcium, phosphorus, creatinine and urea nitrogen. 1) Mean systolic and diastolic blood pressure of elementary school children were 106.8/67.6mmHg in males and 108.7/69.5mmHg in females. Seven children(4%) of total subjects were found to be hypertensive. Their mean blood pressure was 130.0/86.4mmHg. 2) Urinary excretions of creatinine and urea nitrogen during twenty-four hours were 621.1mg and 1524mg, respectively. The mean daily urinary sodium excretion was 2940mg, which is equivalent to 7.37g NaCl. 3) The daily urinary excretions of other minerals were as follows; potassium was 1301mg, chloride, 4991mg, calcium, 55.7mg and phosphorus, 700.03mg. 4) Salt preference of hypertensive children90.473$\pm$0.275) was significantly higher than those of normotensive children(0.473$\pm$0.213%), but salt threshold was lower in hypertensives(0.105$\pm$0.04%) than normotensives(0.081$\pm$0.022%) (p<0.05). INdices for assessing obesity, such as body weight, triceps skinfold, weight for heigth and body surface area, were high in hypertensive children than normotensive children(p<0.05). But there was no significant difference in energy and nutrient intakes between two groups. 5) Various anthropometric measurements had positive correlations with blood pressure, but body muscle mass(%) had a negative correlation with blood pressure. And daily energy and nutrient intakes were not related to blood pressure. 6) Blood pressures-both systolic and diastolic-were significantly correlated with urinary excretions of potassium and chloride. Daily sodium excretion was related to systolic blood pressure, but no related to diastolci blood pressure. There was no relationship of salt threshold to salt preference, and neither threshold nor preference was related to blood pressure. Results of this study suggest that nutrition education program for children including the information about desirable food habits for the prevention of hypertension should be developed.

중년 남성 근로자의 고혈압 발생에 관련된 요인의 검토 (Risk Factors for Hypertension of Middle Aged Male Workers using Data from Health Check-ups)

  • 본 연구는 성인 남성 근로자들의 고혈압 발생에 관련된 요인을 밝히고, 고혈압의 1차 예방대책을 실시하기 위한 과학적 근거를 제공할 목적으로 대전광역시의 모 건강검진센터에서 2011년에 건강검진을 받은 남성 근로자 2,230명을 대상으로 하였다. 조사는 대상자의 연령, 비만도, 내당능, 흡연 및 음주습관, 운동습관과 혈압수준과의 관계를 조사하였다. 연구결과, 전체 조사대상자의 연령계층별 고혈압 발생은 30대 18.8%, 40대 23.8%, 50대 33.0%를 보였으며, 고혈압 발생은 연령이 증가할수록, BMI가 증가할수록 높아지는 경향이었으며, 내당능 이상군, 상습음주군, 운동부족군에서 그렇지 않은 군보다 고혈압 발생이 유의하게 높았다. 고혈압 발생에 대한 관련변수의 위험비를 구한 결과 비만도의 경우 30대, 40대, 50대 연령층 모두 정상군에 비해 과체중군과 비만군에서 고혈압이 발생할 위험비가 1.0 이상 유의하게 상승하였다. 내당능 이상군은 정상군에 비해 40대와 50대 연령층에서, 흡연군은 비흡연군에 비해 30대 연령층에서, 상습음주군은 비상습음주군에 비해 30대 연령층에서, 비운동군은 운동군에 비해 40대와 50대 연령층에서 고혈압이 발생할 위험비가 1.0 이상 유의하게 상승하였다. 위와 같은 결과는 고혈압의 1차 예방대책으로 비만, 알코올섭취, 운동부족이 중요하므로 바람직한 음주습관, 정기적인 운동 및 적정 체중의 유지에 신경을 쓸 필요가 있음을 시사한다.

체격지수에 따른 기숙사 여대생의 건강과 관련된 식행동과 영양소 섭취량에 대한 연구 (A study on food behavior to related health and daily food intakes of female dormitory students according to BMI)

  • 본 연구에서는 자취 형식의 기숙사에 살고 있는 여대생들의 건강과 관련된 식행동과 영양소 섭취 실태를 알아보고 이들을 BMI에 따라 저체중군과 정상체중군으로 나누어 비교한 결과를 요약하면 다음과 같다. 1.평균 연령은 18-20세가 66.3%, 21-25세 33%, 26-27세 0.6%순이었다. 한달 평균 용돈은 20-30만원 41.4%, 30-40만원 34.3%, 40만원 이상 14.6%, 20만원 이하가 8.4%였다. 평균 식비는 10-15만원 42.2%, 15-10만원 27.9%였다. 건강상의 문제는 없다 77.3%, 변비 33.9%, 소화불량 32.6%, 빈혈 21.4%, 위장질환 13.2%순이었다. 2. 평균 신장은 162.37$\pm$4.36cm 평균 체중은 52.48$\pm$5.54kg, BMI 19.89$\pm$1.89이었다. 저체중군(BMI 20이하)은 53.1%로, 평균신장 162.61$\pm$14.19, 체중 48.99$\pm$3.60kg, BMI는 18.51$\pm$0.98이었다. 정상체중군(BMI 20-25이하)은 46.9%로, 평균 신장 162.37$\pm$4.36cm 체중 58.43$\pm$4.63kg, BMI는 21.46$\pm$1.38이었다. 3. 건강에 대한 자가 인식은 BMI에 상관없이 대부분이 보통 이상(76.4%)으로 대답하였다 저체중군(28.7%)이 정상체중군(17.9%) 보다 건강이 좋지 않다고 생각하였다(p'||'&'||'lt;0.05). 자기 체형에 대한 이미지는 저체중군의 63.4%가 '보통 체형'이라고 생각하였고, 정상체중군에서는 73.1%가 '살찐 편'이라고 생각하였다.(p'||'&'||'lt;0.001). 체중조절의 경험은 정상체중군이 유의성있게 많았다(p'||'&'||'lt;0.001). 체중조절에 사용된 방법은 음식 덜 먹기, 운동, 한 끼 굻기, 간식 안 먹기 순이었다. 운동은 거의 하지 않았으며, 음주 횟수는 한달에 1-2회가 가장 많았고(45.5%), 흡연은 '안 한다'가 95.1%였다. 영양제는 저체중군이 정상체중군보다 더 많이 복용하였다(p'||'&'||'lt;0.05). 4. 81.4%가 불규칙한 식사를 하고, 거르는 끼니는 아침이 89.6%로 가장 많았고, 거르는 이유는 시간이 없어서(75.4%), 먹을 것이 마땅치 않아서(28.3%) 순이었다. 간식섭취는 하루 2번이 가장 많았고, 저체중군이 정상체중군 보다 '간식을 거의 먹지 않는다'라고 대답한 학생이 유의적으로 많았다(p'||'&'||'lt;0.05). 좋아하는 간식은 두 군 다 과자, 과일류 순이었다. 정상체중군이 저체중군보다 빵과 케?을 더 많이 먹었다(p'||'&'||'lt;0.05). 나쁜 식습관으로는 불규칙한 식사(60.5%), 과식(39.2%)순이었다. 정상체중군이 저체중군보다 맵고 짠것을 더 먹는 것으로 나타났고(p'||'&'||'lt;0.05), 저체중군이 편식을 더 많이 하였다(p'||'&'||'lt;0.01). 5. 외식시 맛과 가격을 우선적으로 고려하였고, 하루에 한번 이상은 외식을 하며, 외식장소는 분식점과 학교 식당이며, 선택하는 음식은 한식, 일품요리, 분식 순이었다. Fast food 이용 횟수는 '거의 가지 않는다'가 가장 많았고, 일주일에 3-4번 순이었다. 정상체중군이 저체중군보다 이용횟수가 많았다 (p'||'&'||'lt;0.05) 6. 식품을 주로 구입하는 곳은 동네 슈퍼와 대형 슈퍼였고, 밥은 적어도 하루에 한번은 하고, 반찬은 주로 집에서 부쳐오는 반찬을 먹고 있으며, 하는 요리의 종류는 찌개, 국 순이었다. 7. 우유와 유제품군에서는 우유를 가장 자주 마시고 있으며, 아이스크림, 요구르트 순이었다. 육류군은 계란, 두부와 햄과 소세지의 순으로 자주 먹었으며, 정상체중군이 저체중군보다 소고기, 돼지고기, 닭고기, 햄과 소세지를 덜 자주 먹었다(p'||'&'||'lt;0.05). 곡류는 밥은 하루에 한번은 먹으며, 채소는 김치는 하루에 한 번씩은 먹으나, 담색, 황색, 녹색채소의 섭취 횟수는 낮았다. 과일은 일주일에 1-2회 이상 먹었으며, Fast food중에서는 햄버거를 가장 자주 먹으며, 음료는 커피를 가장 많이 마셨다. 8. BMI에 따른 열량과 영양소에는 차이가 없었다. 평균 열량은 1575.3Kcal로 권장량의 78.7%이었고. 단백질은 권장량의 124.7%, 비타민 A 101.0%, 비타민 B$_1$ 131.0%, 비타민 B$_2$ 82.3%, 나이아신 89.7%, 비타민 C 78.1%, 철분은 80.2%이었다. 칼슘은 권장량의 57.0%로 가장 낮았다. 이상의 결과로 여대생은 체중 과다보다는 저체중의 비율이 높았는데 이들이 가임 연령층이라는 것을 고려하여 본 다면 이에 대한 식생활 대책이 필요하다. 기숙사의 여대생의 건강과 관련된 행동, 식행동과 영양소 섭취는 집에서 있는 여대생과 비교하여 불규칙한 식사의 비율이 높았고, 아침 결식율도 높으며, 채소와 과일의 섭취 빈도가 낮음을 알 수 있었다. BMI에 따라서는, 저체중과 정상체중의 여학생간에는 식행동과 영양소 섭취에 큰 차이가 없음을 알 수 있었다. 기숙사생들이 바른 식습관을 가지기 위하여 기숙사 자체와 학교의 영양교육을 통하여 간편한 아침식사, 간단히 할 수 있는 요리 등을 강좌를 통하여 가르치고, 학생들이 가장 많이 이용하는 학교식당이 식단의 다양함과 질을 높일 수 있도록 식단 개발이 필요하다고 생각된다.

근로자의 대사증후군 예방을 위한 웹기반 비만 관리 프로그램 개발의 필요성 (Necessity of the Development of a Web-based Obesity Management Program to Prevent Metabolic Syndrome of the Workers)

  • 본 연구는 19-34세 근로자 1,152명의 건강검진 자료를 분석하여 비만에 따른 대사증후군 유병률, 구성요소 및 생활습관과의 차이를 비교함으로써 사업장에서 근로자의 대사증후군 예방 및 관리를 위한 기초자료를 제공하고자 실시되었다. 연구 결과 대사증후군 유병률은 남성과 여성에서 각각 8.9%, 1.5%이었다. 비만에 따른 대사증후군 유병률은 남성에서 비만이 없는 경우 1.6%, 비만이 있는 경우 21.7%로 차이가 있었으며 여성에서도 비만이 없는 경우 0%, 비만이 있는 경우 12.5%로 차이를 나타내었다. 비만에 따른 대사증후군 구성요소를 살펴보면 여성에서는 중성지방 상승을 제외한 4가지 요소, 남성에서는 모든 요소가 비만과 유의하게 관련이 있는 것으로 나타났다. 이상의 결과로 비만은 40세 미만 근로자에서 대사증후군 유병률 및 구성요소에 영향을 미치는 위험 인자임이 확인되었다. 따라서 보건관리자는 직장에서의 대사증후군 관리 및 예방을 위하여 비만 관리를 지속적으로 실시하고, 시공간적 제약을 고려한 웹기반 비만 관리 프로그램의 개발 및 활용을 통하여 근로자의 참여를 높이고 효율적으로 대사증후군을 관리해 나가도록 제언한다.

The Effect of Eating Behavior on Being Overweight or Obese During Preadolescence

  • Objectives: Being overweight or obese is central to metabolic syndrome, and these characteristics constitute dominant risk factors for chronic diseases. Although behavioral factors, including eating habits and sedentary lifestyles, are considered to be determinants of obesity, the specific childhood factors that contribute to this condition have not been clearly defined. Methods: The subjects consisted of 261 children aged 7- 9 years who were recruited from an elementary school during October 2003. Information was obtained from their parents using a questionnaire focused on eating behaviors and lifestyle factors, additional data were also collected via anthropometric measurements and biochemical examinations, including blood tests. Results: A total of 48 (18.4%) of the 261 children were overweight, and 34 (70.8%) had at least one other component of metabolic syndrome. Eating behaviors emerged as significant lifestyle-related risk factors for being overweight or obese. Those who engaged in overeating more than twice per week had three times the risk of being overweight (odds ratio [OR], 3.10, 95% confidence interval [CI], 1.39 to 6.92), and those who ate rapidly had three times the risk of being overweight (OR, 3.28; 95% CI, 1.68 to 6.41). Those who had fewer family meals (fewer than 2?3/month) had a nine times higher risk of being overweight than those who had family meals more frequently (at least 1/day) (OR, 9.51; 95% CI, 1.21 to 74.82). Conclusions: This study showed that being overweight or obese during preadolescence is associated with a higher risk of metabolic syndrome and is related to unhealthy eating behaviors. Thus, weight-control strategies and healthy eating behaviors should be developed early in life to reduce the risk for metabolic syndrome.

근로자(勤勞者)의 양생수준(養生水準)과 비만(肥滿)과의 관계(關係) (A Study on the Relations between Yangseng Level and Obesity in Industrial Workers)

  • In this study, the researcher tried to present the groundwork to prepare the oriental medical yangseng plan for the future obesity with the comparison between yangseng level and the obesity of workers. The researcher made up the questionnaire which asks the general character, health-related character and yangseng level, projected among 560 people. All collected material was analyzed by SPSS and tested by T-test and ANOVA. 1. The general yangseng level average is 3.27, morality yangseng 3.90, sleep yangseng 3.39, mind yangseng 3.31, sex life yangseng 3.30, exercise yangseng 3.15, activities and rest yangseng 3.08, diet yangseng 2.94, seasonal yangseng 2.84. The highest is morality yangseng and seasonal yangseng is the lowest. 2. In the aspect of yangseng level: Having a spouse, Non-smoking, Regular exercising, Sufficient sleeping have higher yangseng levels. 3. Obesity related index is changing into the higher level when he/she is older, more paid, more job experience, more education background but less metabolic calory. And drinking and regular exercising have relativity with body composition analysis. 4. After comparing yangseng level with body composition analysis, we can easily find that the more visceral fat, the higher WHR has a high yangseng level. The more mineral also has a high mind yangseng and a low diet yangseng. The visceral fat level has a high yangseng level when higher morality yangseng and mind yangseng are getting higher and higher. And sex life yangseng shows that the highest yangseng level is from 9-10 visceral fat and the lowest yangseng level is from below 4 visceral fat. The higher WHR, the higher morality, mind and sleep yangseng. The heavier, the lower diet yangseng. The mind yangseng was very high when body fat rate was higher. The more body fat, the higher morality yangseng. The higher yangseng, when we have more muscle. Yangseng level and obesity of laborer has a close relationship with individual character and daily habits. Also, relevance can be easily found between yangseng level and obesity. Now the researcher came into the conclusion that we need to control over laborers' health and prevention of their obesity.

Evaluation of a Specially Designed Tobacco Control Program to Reduce Tobacco Use among School Children in Kerala

  • Background: Smoking and smokeless tobacco use are almost always initiated and established during adolescence. More than 80% of adult smokers begin smoking before 18 years of age. The main objective of the present study is to assess the feasibility of preventing adolescent tobacco use with the help of a specially designed tobacco control program. Materials and Methods: A cross sectional survey on tobacco use and related health effects was conducted using a structured questionnaire in 13 randomly selected schools in Kannur district of Kerala. These students were followed for a period of one academic year with multiple spaced interventions such as anti-tobacco awareness classes, formation of anti-tobacco task forces, inter-school competitions, supplying IEC (information, education and communication) materials and providing a handbook on tobacco control for school personnel. Final evaluation was at the end of one year. Results: There were 4,144 school children who participated in the first phase of the study. The prevalence of tobacco smoking and chewing habits were 9.85% and 2.24% respectively. Ninety-one percent had parental advice against tobacco use and only 3.79% expressed desire for future tobacco use. The final evaluation witnessed a sharp decline in the current tobacco use as 4.68%. We observed a statistically significant difference towards the future use of tobacco (p<0.001) and awareness about the ill effects of passive smoking (p<0.001) among boys and girls. Further a significantly increased knowledge was observed among boys compared to girls about tobacco and oral cancer (p=0.046). Conclusions: The comprehensive school based tobacco control project significantly reduced the tobacco use pattern in the target population. School tobacco projects incorporating frequent follow ups and multiple interventions appear more effective than projects with single intervention.

Human immunodeficiency virus 감염과 치주 질환의 상관관계, 진단 및 처치에 관한 문헌 고찰 (Relationship between human immunodeficiency virus infection and periodontal disease; diagnosis and management strategy)

  • Purpose: Infection with HIV-1 virus has become a critical worldwide public health problem. The oral complications of HIV infection with its progression of impairment of the host response to combat infection present unique challenges to the periodontists. Material and Methods : Medline research was carried out to find relationship of the progression of HIV infection to the occurrence of oral lesions including the HIV-related periodontal diseases. Results: The linear gingival erythema, necrotizing ulcerative periodontitis, necrotizing ulcerative gingivitis and oral candidiasis are common lesions in HIV-infected individuals. The linear gingival erythema and necrotizing ulcerative periodontitis lesions in HIV-infected subjects were found to have a similar microbiological profile. There are several general considerations in the periodontal management of the HIV-infected patient with or without periodontal disease. The altered immunity and host response in patients with HIV infection may also affect the incidence and severity of other common forms of periodontal disease not associated with HIV infection. Conclusion: Periodontal diseases in HIV-infected individuals present unique challenges in diagnosis, monitoring, treatment and maintenance. Therefore exact HIV staging, geographic location, antiviral and antimicrobial therapies and oral habits should be taken into consideration when treating HIV-infected patients.
