Agenda and Alternatives of Home Health Care Nursing Policy based on Health Care Law and Regulations (가정간호사업을 위한 의료법 제정의 의미와 과제)

  • This article was trying to suggest an agenda and alternatives of home health care policy for the future home care growth and development and examine the significance of new health care law and regulations. In addition. It was to analyze and drive the home care system problems. of which was made an announcement on the 11th of April. that home health care project must be centered from the nationwide general hospital. As we have learned from the developed countries, the home health care have been closely related health care policy among the field of nursing area. Therefore. we need to understand our national health care policy and need to predict the future direction and goal of our home health care policy in order to enhance the growth and activation of health care system. Additionally. we also need to have a vision and ability to develop under our own health care policy with systematic and rational home care business escaping from past perspective and standpoint. We must create a master plan of home care system to fulfill one part of system of the function and an important role in order to pursue an advanced health care plan under our system. For instance. in the 21th century as we establish a master plan for the growth of our country home care plan with improving plan systematically and also we need to produce many highly qualified researching and good personal who can develop and maintain the system efficiently. Specially. based on the unique characteristics of our health care system and the direction of development of plan, we need to find and correct the problems which we have faced the present time, so that we can provide and reach the goal of advanced health care system which our government want to pursue. Finally. we have to strive our best effort to make our home health care system can be positioned and stand the right direction to have the benefit for every individual citizen in our country.

Factors Related to Family Caregivers' Burden with the Community-Dwelling Disabled Elderly under the Long-Term Care Insurance System (장기요양 재가서비스 이용자를 돌보는 가족주부양자의 부양부담감에 영향을 미치는 요인)

  • Background: Informal care is increasingly recognized as placing a significant burden on the lives of family caregivers. The purpose of this study is to investigate factors related to family caregivers' burden with the community-dwelling disabled elderly under the long-term care insurance system, using the Stress Process Model developed by Pearlin (1990). Methods: Total 1,233 family caregivers with the disabled elderly, using the long-term care services in their home from May to June 2009, completed questionnaires finally. The questionnaire of this study consists of a total of 32 questions, including 11 questions related to background and context, 17 questions related to objective stressors, and 4 questions related to coping resourses. Family caregivers' burden is measured by the Korean Revised Caregiving Appraisal Scale (K-RCAS, Cronbach's alpha=0.86). To investigate factors related to family caregivers' burden, multiple regression analysis was conducted. Results: The average score of caregivers' burden was 22.0 (${\pm}6.12$). In multiple regression analysis, there were statistically significant factors affecting on the family caregivers' burden, that are related to background and context (region, living status, education level, relationship with beneficiary), objective stressors (duration of caregiving), coping resourses (caregiver's health status). Conclusion: This study found that family caregivers experience a considerable burden. The findings suggest that policies must be taken to relieve family caregivers of their duties temporarily, and to support them with counselling and education.

An Evaluation of Accountable Care Organization in USA and Policy Implications for Korean Health Care System (미국의 책임의료조직(Accountable Care Organization) 운영현황 분석과 국내 의료정책에서 정책적 함의 평가)

  • Background: As a reform plan of health care system, Accountable Care Organization (ACO) has became an object of attention in the United States after Patient Protection and Affordable Care Act was enacted. ACO is a group of various health care providers and provide coordinated care to its assigned beneficiaries. If ACOs improve the quality level and reduce the cost of care, they can get financial incentives. Under the discussion for a quite long time and demonstration projects, ACO has been established. We aimed to analysis and discuss the history, policy mechanism, contents, status and outcomes of ACO. Also, we intended to suggest political implication Korean health care system with regard to ACO. Methods: We searched the articles related ACO in PubMed and selected several available papers about ACO. Total 56 studies were reviewed and categorized three parts; demonstration projects for formation of ACO, policy mechanism and agenda, empirical results of ACO performance. Results: As a result, establishment of ACO was successful partly in the US. It seems to be due to various project and pilot test for verification in the long time. The empirical effect of ACO was also identified in a few study but it needs more evidences to judge its positive effect. Conclusion: In Korea, there are arguments for the application of ACO. However it is difficult to implement a ACO by different political conditions between Korean and US. Nevertheless ACO proposed us the necessity of paradigm shift in our health policy and could be significant to national policy orientation in the future.

A Study on the Evaluation of School Health Policy Performance Process of Korea -Focused on the Secondary School in Pusan City- (한국의 학교보건정책집행과정평가 연구 -부산직할시를 중심으로-)

  • This study intended to grasp the results of performance of school health policy and to provide information for the future policy performance in Korea. As the objects of the study we chose the secondary school(142 middle schools, 111 high schools) in Pusan, 1993. The contents of the study were about school health service and school health education. For the study we collected datum and took statistics through the existing theses, books, various statistics, and interviews with the persons concerned. Therefore the method of the study was basically emperical and demonstrative. The results of the study can be presented as follows : 1. the results of performance of health service proved to be 5.40% for vaccination against hepatitis B, 5.26% for typhoid fever, 6.65% for EH fever, and 9.84% for influenza in middle schools, In high schools' 5.76% for vaccination against hepatitis B, 0.03% for typhoid fever, 0.25 for EH fever, and 0.86% for influenza. 2. The results of policy performance of health education were found out 64.08% in middle schools, and 17.12% in high schools. 3. The factors which affected the results of performance of health service turned out 1) the improper standard of policy performace that applicants should pay their own expenses. 2) the lack of performer's will to carry out the policy owing to the improper standard. 3) the shortage of interorganizational enforcement activities and communications which influence the will of performance. 4. The affecting factors to the will of performance of health education proved to be 1) the improper standard that nurse-teachers can teach students in the classroom in case of need. 2) the lack of nurse-teachers due to the above inappropriate standard. 3) the lack of enforcement activities of supervising organs which employ and direct nurse-teachers. 4) the lack of activities of organs which should communicate one another and allot nurse-teachers of public schools without fail.

  • This research analyzed. knowledge structure and its effect factor by analysis of co-author and keyword network in Korea's health policy and administration sector. The data was extracted from 339 articles listed in the Korean Journal of Health Policy and Administration, and was transformed into a co-author and keyword matrix. In this matrix the existence of a link was defined by impact factors which were calculated by the weight value of what the role was and the rate of how many authors contributed. We demonstrated that the research achievement was dependent on the author's status and network index. Analysis methods were neighborhood degree, correspondence analysis, multiple regression and the difference of weight distribution by research fields. Co-author networks were developed as closeness centrality as well as degree centrality by a few high productivity researchers. In particular, power law distribution was discovered in impact factor and research productivity. The effect of the author's role was significant in both the impact factor calculated by the participatory rate and the number of listed articles. Especially, this journal shared its major researchers who had a licensed physician with the Journal of Preventive Medicine and Public Health. Therefore, social scientists were likely to be small co-author network differently from natural scientists. It was so called 'two cultures' phenomenon. This study showed how can we verified academic research structure existed in the unit of journal like as citation networks. The co-author networks in the field of health policy and administration had more differentiated and clustered than preventive medicine and epidemiology fields.

Development and Adjustment of Indicators for Underserved Area (분야별 의료 취약지 선정지표 개발 및 적용)

  • Underserved area is a region that has a lack of healthcare resources. In the context of Korea, however, there are not enough detailed criteria for underserved areas. In this study, we aimed to develop indicators for underserved area through Delphi technique. We systematically reviewed the existing measure of underserved area. Sixty indicators were extracted as candidates across four domains in secondary medical care. Four domains are demand, medical resource, quality of care, and health outcome. To develop indicator, two round Delphi survey was conducted among 15 professional experts such as professionals and public administrators. In conclusion, 2 final indicators (accessibility, medical utilization) was determined as an appropriate measure in order to designate underserved area for secondary medical services. Using our criteria from Delphi technique, 36 areas were found as underserved areas for the secondary medical care.

Factors of Welfare Recognition toward Health Insurance and Health Care: Using 2013 Korea Welfare Panel Study (건강보험 및 보건의료에 대한 복지인식에 영향을 주는 요인: 2013년 한국복지패널 자료를 이용하여)

  • Objectives : This research was performed to investigate the characteristics and determination factors of health care policy satisfaction and welfare recognition for health insurance & health care financing. Methods : The utilized data were 4,174 cases who responded to a welfare recognition survey in the 8th wave of the Korea Welfare Panel Study (2013). The statistical methodology used in this study is the multiple regression model. Results : The significant affecting factors of health care policy satisfaction were age, education, household income, welfare attitudes, and health status. Medical utilization & private medical insurance were not related to health care policy satisfaction. The affecting factors of health insurance reinforcement were age, health status, welfare attitudes. The affecting factors of health care financing expansion were age, economic activity type, medical utilization, welfare attitudes. The affecting factors of welfare attitudes were age, economic activity type, household income, health insurance, and health status. Conclusions : Health care policy satisfaction, health insurance reinforcement, and health care financing expansion were all affected by age and welfare attitude; but this was not the case for private health insurance. This study recommended that the Korean government provide active planning for reinforcement of health insurance and publicity of the health care system in order to accord with the prospects of people.

Evaluation of Tobacco Control Policy in Korea: Development and Application of the Korean Tobacco Index for Policy Monitoring and Evaluation (한국 담배규제 정책의 평가: 담배규제정책 전문가 의견 조사를 토대로)

  • Background: Due to lack of regular and systematic evaluation tool, Korea's tobacco control policy has not been examined its overall process of implementation including efficiency and adequacy of the policies. This study developed policy monitoring and evaluation model to assess policy implementation and effectiveness of tobacco control in Korea. Methods: Based on World Health Organization operational manual for assessment, MPOWER (monitor tobacco use and prevention policies, protect from tobacco smoke, offer help to quit tobacco use, warn about the dangers of tobacco, enforce bans on tobacco advertising promotion and sponsorship, and raise taxes on tobacco) related policies were reviewed by rating policy efforts, programme management, people (human resources and their development), provision of organization, provision of fund and partnerships (range, 0 to 5). Results: As a result of the experts' assessment, overall Korean tobacco control policies scored 2.61 points, which is poor. In relation to each 'MPOWER' policies, 'W' scored the highest points (2.93), followed by 'O' (2.91), 'M' (2.87), 'P' (2.86), and 'E' (2.23). 'R' scored the lowest points of 1.87, meaning government efforts in tobacco price policy is insufficient. Conclusion: This study concludes that Korean tobacco control policy should strengthen tax and price measures, while programme infrastructure, people, and funds for policy enforcement should be secured. Furthermore, rather than focusing on one specific measure, a balanced approach reflecting various aspects of tobacco controls should be considered in order to decrease smoking rates and prevent smoking initiation.

A Study on the Policy Decision Making Process of Seoul-Type Paid Sick Leave: Applying Kingdon's Multiple Streams Model (다중흐름모형을 적용한 서울형 유급병가 정책 도입과정에 관한 연구)

  • In 2019, the Seoul metropolitan government established its own 'Seoul-type paid sick leave project'. Although the central government had to introduce such a system, which is also called sickness benefits, it was not implemented. In order to understand the process by which the Seoul government has implemented such a policy, this study used Kingdon's multiple streams framework. As a result, in the problem stream, it was found that the economic burden of sickness has been considered only in terms of medical expenses in the past of Korea. Then Songpa's three women and Middle East respiratory syndrome incidents raised awareness of the necessity of the sickness benefit system in 2014 and 2015. In the political stream, several social affairs such as national health insurance huge surpluses and the 2017 presidential election opened policy window. At that time, Seoul Mayor actively promoted sickness benefits as a policy entrepreneur. In the policy stream, the sickness benefit system has gained new attention through political events. To summary, these three streams flowed separately and then they assembled around huge political affairs. As a result, it was confirmed that Kingdon's model is the most effective theory than any other models in analyzing the health care policy decision process in Korea.
